Tai Chi Exercise on MDA, SOD and Physical Fitness in Breast Cancer Patients (규칙적인 태극권의 참여가 유방암 수술환자의 산화-항산화계에 미치는 영향)

  • Treatment for breast cancer produces side effects that diminish functional capacity and quality of life (QOL) among survivors. Tai Chi is a moderate form of exercise that may improve functional capacity, physical activity and oxidative stress. The purpose of this study was to evaluate the effects of regular Tai Chi exercise on malondialdehyde (MDA), SOD and physical fitness (muscle strength, flexibility, flexion, extension, adduction, and abduction). Forty obese women were recruited from a public health center and divided into control (CON: n=20) and trained (EXP: n=20) groups. The Tai Chi exercise group participated in a 12-week (4 times/week) training program. Data were analyzed with T-test. MDA, SOD and physical fitness (muscle strength, flexibility, flexion, extension, adduction, and abduction) were evaluated before and after the Tai Chi program in both groups. There were significant improvements in shoulder flexibility, flexion, extension, abduction, and adduction. However, there was no improvement in muscle strength. There were also significant improvements in MDA and SOD. Based on these results, Tai Chi exercise has been shown to stimulate endogenous antioxidant enzymes and reduce oxidative damage markers. and also be effective in improving physical fitness and QOL. Further study is needed in this area.

  • Purpose : The purpose of this study was carried out to review the correlation between foot shape(supination foot, pronation foot) and low back pain, hip abduction muscle and ankle lateral sprain. Methods : By using internet, we research the PubMed, Science Direct, KISS, DBpia We selected the article between 1990 and 2007. Key words were supination foot, pronation foot, balance. Results : Normal control balance of human body needs a optimal anatomical alignment and function of musculoskeletal and central nerve system that control continuously to integrate. Especially ankle and foot complex play an important role in postural control because it is located distal part in human body. Supination foot brings to chronic ankle sprain or chronic ankle instability and range of motion limitation due to the weakness of lateral ankle muscle. Pronation foot brings to knee injury because of lower leg internal rotation force. Conclusion : Excessive supination and pronation foot happen to muscle imbalance. Especially weakness of hip abduction or injury of ankle lateral muscle or low back pain are due to abnormal balance and anatomical alignment.

Effects of Isometric Upper Limb Contraction on Trunk and Leg Muscles During Sit-to-stand Activity in Healthy Elderly Females

  • PURPOSE: The purpose of this study was to investigate the effects of isometric upper limb contraction on the trunk and lower extremity muscles during the sit-to-stand activity in elderly females. METHODS: Eighteen healthy elderly females performed three directional isometric upper extremity contractions (flexion, extension, and horizontal abduction movements) using an elastic band during sit-to-stand activity. Electromyography signals were collected from the internal oblique, erector spinae, rectus femoris, and biceps femoris muscles. RESULTS: Internal oblique activity was greater in bilateral shoulder flexion and bilateral shoulder horizontal abduction than in neutral position (p<.05). Erector spinae and rectus femoris muscle activities in bilateral shoulder flexion was greater than in neutral position and bilateral shoulder extension (p<.05). Biceps femoris activity was significantly greater in bilateral shoulder flexion than in bilateral shoulder extension and horizontal abduction, and in neutral position compared to bilateral shoulder extension (p<.05). CONCLUSION: These results suggest that incorporating isometric upper limb contraction may be beneficial for enhancing the contribution of trunk and lower extremity muscle activities to trunk stabilization during sit-to-stand activity. Therefore, isometric upper limb contraction during sit-to-stand tasks, especially in flexion, may be used to elicit contraction of the lumbopelvic region muscles within a tolerable range, for developing endurance and strength in the elderly.

The Comparative Study on Effect of Jungsongouhyul Phamacopuncture and Electroacupuncture in Patients with Acute Traumatic Shoulder Pain (급성 외상성 어깨 통증 환자에 대한 중성어혈약침과 전침의 효과에 관한 비교 연구)

  • Objectives : The purpose of this study is to report the effect of Jungsongouhyul phamacopuncture and electroacupuncture on patients with acute traumatic shoulder pain. Methods : The patients with acute traumatic shoulder pain were diagnosed by blood stasis and divided two groups. One group was treated byJungsongouhyul phamacopuncture. The other group was treated by electroacupuncture. Each group was treated 3 times a week for 2 weeks. The results were measured by visual analog scale(VAS), range of abduction and internal rotation. Results : Two groups had no significant differences before treatment. Jungsongouhyul phamacopuncture group had effects on VAS, range of abduction and internal rotation significantly. Electroacupuncture group also had effects on VAS, range of abduction and internal rotation significantly. But there was no significant differences between two groups. Conclusions : Jungsongouhyul phamacopuncture and electroacupuncture have effect on patients with acute traumatic shoulder pain significantly.

  • This research described dynamic scapulohumeral rhythm though the measurement of inter relationship between humorous and scapular movement during elevation of the arm in the scapular plane. In the previous studies static plane film X-ray has usually been used, but the limitation of such method was that it did not show the real· movement. In this study, we investigated real movement of humorous and scapular bones by Digital fluoroscopy system. 1. In resting position, humorous was taken to lie at the average $5.97^{\circ}$ in abduction, and scapular at the average $3.66^{\circ}$ in internal rotation. 2. At the maximal abduction, humorous was abducted to the average of 181.25", and scapular was rotated externally to $51.43^{\circ}$. 3. This research has shown that there is a variety of scapulohumeral rhythm from resting position to $60^{\circ}$ abduction in elevation of humorous. This case study demonstrated that the ratio n of scapulohumeral rhythm is 3.46 to 1, but there was individual difference.

The Effect of Dong-gi Acupuncture(DGA) on Rehabilitation after Stroke (중풍(中風) 편마비(偏痲痺) 환자(患者)의 기능적 회복에 있어 침시술(鍼施術) 시(時) 동기요법(動氣療法)의 효과)

  • Objectives : The purpose of this study is evaluate of the effect of Dong-gi Acupuncture(DGA) on rehabilitation after stroke. Methods : 32 patients who had a stroke of paralysis were randomized into two groups. 17 patients were treated with DGA on sound side combined active or passive exercise on affected side. The other 15 patients were exclusive DGA group and the method retaining needles on acupoints of affected side for about 20 minutes were applied. We evaluated by Manual muscle test, Range Of Motion(ROM), Visual Analog Scale(VAS) and Modified Barthel index(MBI) before treatment, after 10 days and after 20 days. Results : In terms of power of lower limb, active ROM of shoulder abduction and MBI, the sample group showed statistically significant improve. In terms of power of lower limb ROM of hip joint abduction and affected side pain, significant difference between two groups was not shown. Conclusions : In this clinical study, Dong-gi Acupuncture therapy can be used for improving the power of upper limb, active ROM of shoulder abduction and MBI score after hemiplegic stroke.

Influences of Shoe Heel Height on Isometric Shoulder Abductor Strength and EMG Activities of Selected Shoulder Muscles (신발 뒤굽 높이가 등척성 어깨 외전 근력과 근전도 활동에 미치는 영향)

  • PURPOSE: The objective of this study was to identify the effects of shoulder abduction strength and EMG activities of the selected scapular and shoulder muscles during isometric shoulder abduction. METHODS: Thirty-four healthy young females were recruited for this study. Surface EMG equipment with inline force sensor was used to determine the shoulder abductor strength and the activity of the serratus anterior (SA), upper trapezius (UT), lower trapezius (LT), and middle deltoid (MD) during three shoe heel height conditions: (1) barefoot, (2) 3-cm shoe heel height, and (3) 7-cm shoe heel height. RESULTS: Isometric shoulder strength showed statistically significant difference among the conditions (p<0.05), and post-hoc test showed lower strength during the 7-cm condition ($49.98{\pm}17.56kg$) than during the barefoot ($44.97{\pm}20.15kg$) and 3-cm conditions ($36.59{\pm}17.07kg$). Furthermore, EMG activities of the SA, UT, and MD appeared to be statistically significantly different among the conditions, with lower values in the 7-cm condition compared to the barefoot condition (p<0.05). EMG ratios (MD/UT and SA/UT) were lower during the 7-cm condition than during the barefoot condition (p<0.05). CONCLUSION: These findings suggest that isometric shoulder abduction strength and EMG activities of scapular and shoulder muscles may be adversely changed with increasing shoe heel height.

Can Suboccipital Release Followed by Cranio-Cervical Flexion Exercise Improve Shoulder Range of Motion, Pain, and Muscle Activity of Scapular Upward Rotators in Subjects With Forward Head Posture?

  • Background: For the treatment of forward head posture (FHP) and forward shoulder posture, methods for strengthening scapular retractors and deep cervical flexors and stretching pectoralis and upper cervical extensors are generally used. No study has yet assessed whether suboccipital release (SR) followed by cranio-cervical flexion exercise (CCFE) (SR-CCFE) will result in a positive change in the shoulders and neck, showing a "downstream" effect. Objects: The purpose of this study was to investigate the immediate effects of SR-CCFE on craniovertebral angle (CVA), shoulder abduction range of motion (ROM), shoulder pain, and muscle activities of upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA) and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction in subjects with FHP. Methods: In total, 19 subjects (7 males, 12 females) with FHP were recruited. The subject performed the fifth phase of CCFE immediately after receiving SR. CVA, shoulder abduction ROM, shoulder pain, muscle activities of UT, LT, and SA, and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction were measured immediately after SR-CCFE. A paired t-test and Wilcoxon signed-rank test were used to determine the significance of differences in scores between pre- and post-intervention in the same group. Results: The CVA (p<.001) and shoulder abduction ROM (p<.001) were increased significantly post-versus pre-intervention. Shoulder pain was decreased significantly (p<.001), and LT (p<.05) and SA (p<.05) muscle activities were increased significantly post- versus pre-intervention. The LT/UT muscle activity ratio was increased significantly post- versus pre-intervention (p<.05). However, there was no significant change in UT muscle activity and SA/UT muscle activity ratio between pre- and post-intervention (p>.05). Conclusion: SR-CCFE was an effective intervention to improve FHP and induce downstream effect from the neck to the trunk and shoulders in subjects with FHP.

Agreement of Manual Muscle Testing and Test-Retest Reliability of Hand Held Dynamometer for the Posterior Gluteus Medius Muscle for Patients With Low Back Pain (요통 환자를 대상으로 후중둔근 도수근력검사의 일치도 및 휴대용 근력계 측정 방법의 신뢰도 검사)

  • The purpose of this study was to assess the agreement of manual muscle testing (MMT) and test-retest reliability of a hand held dynamometer for the posterior gluteus medius muscle, with and without lumbar stabilization, using a pressure biofeedback unit for patients with low back pain. The pressure biofeedback unit was used to minimize the substitute motion of the lumbopelvic region during hip abduction in patients lying on their side. Fifteen patients with low back pain participated in this study. A tester determined the MMT grades of the posterior gluteus medius with and without the pressure biofeedback unit. Active hip abduction range of motion with an inclinometer and the strength of their posterior gluteus medius using a hand held dynamometer were measured with and without the pressure biofeedback unit in the MMT position. The agreement of the grade of muscle strength in the MMT, and intra-rater reliability of both the active hip abduction range of motion and the strength of posterior gluteus medius were analyzed using the weighted kappa and intraclass correlation coefficient (ICC), respectively. The agreement of MMT with the pressure biofeedback unit (weighted kappa=.92) was higher than the MMT (weighted kappa=.34)(p<.05). The inclinometer with pressure biofeedback unit measurement of the active hip abduction range of motion had an excellent intra-rater reliability (ICC=.90). Also, the hand held dynamometer with pressure biofeedback unit measure of strength of the posterior gluteus medius had a good intra-rater reliability (ICC=.85). Therefore, the test for muscle strength with pressure biofeedback unit will be a reliable method for the determination of the MMT grades or amount of posterior gluteus medius muscle strength and the measurement of the range of motion for hip abduction in patients with low back pain.

A Study on Shoulder Joint Motions in the Caudal Gliding of Kaltenborn-Evjenth Concept (칼텐본-에비엔즈컨셉의 어깨관절 아래쪽미끄러뜨림 적용시 관절의 이동성 연구)

  • This study aimed at identifying changes in the acromiohumeral distance (AHD) and arm when the caudal gliding gradeII and III of Kaltenborn-Evjenth Concept$^{(R)}$ are applied to the right glenohumeral joint. The humeral head moved down about 5mm from the initial position when the gliding gradeII was applied, and about 8mm from the initial position when the gliding gradeIII was applied. Although men showed a higher acromiohumeral distance per grade than women in comparisons by gender for the acromiohumeral distance, there was no significant difference in statistics. The Abduction angle improved about $10^{\circ}$ from the initial angle when the gliding gradeII was applied, and about $12^{\circ}$ from the initial angle when the gliding gradeIII was applied. Although women showed the abduction angle greater than men for every grade in comparisons by gender for the abduction angle, there was no significant difference in statistics. Based on the aforesaid findings, the extent of kinematic changes in the humeral head could be identified when the gliding grades were applied. Accordingly, it is considered that more scientific evidence based treatments could be expected if influences on the surrounding structures by these changes could be learned through more studies in the future.
