자폐스펙트럼장애 아동의 재활을 위한 로봇 관련 문헌분석 (Literature Review of Robots Used for the Rehabilitation of Children with Autistic Spectrum Disorder)

  • 자폐스펙트럼장애 아동은 시각적 처리, 체계화와 같은 강점을 지니며, 사물 및 기계에 대한 흥미와 관심을 나타낸다. 이 같은 맥락에서 로봇이 자폐스펙트럼장애 아동의 재활에 유용한 것으로 제안되고 있다. 즉 로봇은 자폐스펙트럼장애 아동의 관심을 유발하고, 주의를 집중하게 하고, 단순화된 사회적 자극을 제시하며, 반복적용이 가능하고, 대상에 따른 프로그래밍이 가능하다. 이에 본 연구에서는 자폐스펙트럼장애 아동의 재활을 위한 국내 로봇 관련 연구를 살펴보고자 하였다. 이를 위해 자폐, 로봇을 키워드로 검색하여 11편의 국내 문헌을 최종 선정하고, 활용 로봇, 로봇 제시자극, 실험절차 및 종속변인에 따라 문헌을 내용 분석하였다.

장벽게임을 사용한 명료화 중재가 자폐범주성장애 아동의 대화단절 및 발화수정 전략에 미치는 효과 (The Effects of Clarification Training in Barrier Game on Conversational Breakdowns and Repair Strategies of Children with ASD)

  • 연구목적: 본 연구는 초등 저학년 자폐범주성장애 아동3명을 대상으로 장벽게임을 사용한 명료화 중재가 이 아동들의 대화단절과 수정전략 산출에 미치는 효과를 알아보았다. 연구방법: 중재는 장벽게임 상황에서 체계적인 전략을 사용한 '명료화 훈련'을 사용하였다. 중재효과 분석을 위하여 대화단절과 발화 수정전략의 산출 변화를 분석하였다. 결과: 중재 이후 모든 대상 아동들이 대화단절의 총 산출률과 모든 유형의 산출점수에서 감소를 나타냈다. 반면, 수정전략의 전체 및 유형별 산출률과 성공률은 증가하였다. 논의 및 결론: 본 연구에서 사용한 '명료화 훈련'이 임상현장에서 구어 산출이 가능한 자폐범주성장애 아동들의 대화단절 감소와 적절한 수정전략 사용 능력을 향상시키는데 효과적인 방법이 될 수 있을 것이다.

소아 자폐스펙트럼 장애의 추나 치료에 대한 중의학 임상 연구 고찰 (Review of Traditional Chinese Medicine Clinical Studies on Tuina Treatment of Children's Autism Spectrum Disorder)

  • Objectives The purpose of this study is to investigate the effect of Tuina treatment for children with Autism Spectrum Disorder (ASD), and to propose guidance to future studies or application in clinical setting. Methods The articles published from year 2000 to May 2021 by China National Knowledge Infrastructure (CNKI) were searched by setting up field as '中醫學', '中藥', '中西醫結合' and by setting up keyword as '小儿自閉症', '儿童自閉症', '自閉症譜系障碍', 'Autism', 'Autism spectrum disorder' that corresponds to participant and '推拿', '導引', '按摩', 'Tuina', 'massage'. Results Nine selected articles were analyzed and summarized by authors and year published, characteristics, diagnostic criteria, treatment methods and contents, treatment duration, evaluation criteria and results of the study. The articles were also reviewed by reported side effects, and evaluated the quality of the study according to Cochrane Handbook's Risk of Bias 2 (RoB 2) assessment of the Randomized Control Trial (RCT) articles. All studies used Tuina treatment to achieve effective results for pediatric autism spectrum disorder. Conclusion Pediatric Tuina treatment (TM) is a non-invasive treatment that does not stimulate patients with unstable ASD but increase patient adherence, and can be safely and simply operated. Therefore, pediatric Tuina treatment is worth utilizing as a clinical basis for further development of ASD treatment.

자폐스펙트럼장애아동을 대상으로 한 국내 로봇활용 융합연구동향 (Research Trends and Considerations in The Clinical Use of Robots for Children with Autism Spectrum Disorders)

  • 본 연구의 목적은 국내에서 이루어진 자폐스펙트럼장애아동을 대상으로 한 로봇 활용 융합연구동향을 살펴보는 것이다. 이를 위하여 최근에 장애 아동의 교육적, 치료적 중재에 연구되고 있는 로봇연구 동향을 살펴보고자 하였다. 연구방법으로, 자폐를 키워드로 2009년 이후에 발표된 논문을 검색하여 총 17편의 연구 논문을 최종 선정하였고, 활용로봇, 독립변인, 종속변인 유형, 연구방법을 바탕으로 하여 주제별 범주로 묶어 내용 분석하였다. 연구결과, 그 중 자폐아동을 위한 로봇개발은 발전이 빠른 분야 중 하나이며, 자폐스펙트럼장애아동의 중재에 유용한 것으로 나타났다. 연구를 통해 향후 로봇활용의 일반화를 촉진할 수 있는 적용 절차 및 단계 등에 대한 세분화된 접근이 필요함을 알 수 있었다.

섭식장애가 있는 아동의 연하재활치료 : 체계적 고찰 (Dysphagia Rehabilitation Treatment for Children With Feeding Disorder : A Systemic Review)

  • 목적 : 체계적 문헌고찰을 통해 섭식장애가 있는 아동의 질환에 따른 연하장애를 증상별로 세분화하고 증상에 따른 치료방법을 알아보고자 한다. 연구방법 : 데이터베이스는 PubMed, Medline, CINAHL을 통해 검색하였다. PRISMA, PICOS를 이용하여 연구를 선정하고 체계적 고찰을 시행하였다. 최종적으로 연구에 사용한 문헌은 총 13개로 연구의 질적수준, 질환군, 평가도구, 중재방법, 중재 후 효과에 대해서 분석하였다. 결과 : 총 13편의 연구를 분석한 결과 섭식장애가 있는 아동의 질환군은 자폐스펙트럼장애(Autism Spectrum Disorder; ASD)는 6편(46.15%), 뇌성마비(Cerebral Palsy; CP)는 7편(53.85%)으로 나타났다. 섭식장애 중재를 받은 아동의 연령대는 ASD는 2-8세, CP는 12개월-18세까지로 확인하였다. 평가영역에서 ASD는 행동평가, CP는 구강과 삼킴기능 평가영역이 가장 많았다. 중재방법에서 ASD는 행동수정이 가장 많았고, CP는 구강감각운동, 음식질감조절, 전기자극치료가 각각 동일한 빈도로 적용된 것으로 나타났다. 중재 후 치료효과에서는 모두 효과가 있었다. 결론 : 본 연구에서 섭식장애가 있는 아동의 질환군, 적용되는 평가도구, 중재방법을 고찰을 통해 알아보았다. 본 연구가 질환에 따른 평가와 중재 선택 시 임상가들이 시행착오를 거치지 않고 효율적인 섭식장애 중재를 적용할 수 있는 프로토콜 개발을 하는데 기초자료로 활용될 수 있을 것으로 기대한다.

  • Pbjective : A significant proportion of children with autism spectrum disorders (ASD) have regression characterized by loss of previously acquired skills. The purpose of this study was to compare demographic, clinical characteristics and autism-related symptomatology of the children who have regression with children who don't have regression. Methods : The subjects with ASD and their unaffected siblings (SIB) were recruited from the Korean Autism Genetic Study Consortium. Typically developing children (TC) were volunteered from community. The subjects were administered the Korean version of Autism Diagnostic Interview-Revised (K-ADI-R) and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS) to diagnose or exclude ASD. Regression was defined on the basis of K-ADI-R data. The Korean version of Vineland Adaptive Behavior Scale (K-VABS), Aberrant Behavior Checklist (K-ABC) and Social Responsiveness Scale (K-SRS) were obtained from their parents. Results : Regression occurred in 8.33% (n=14) of children with ASD (n=168). Any SIB (n=166) and TC (n=53) did not experience regression. Regression was associated with lower IQ and lower score of K-VABS. There was no difference in autism symptom severity and K-ABC, K-SRS scores, between children with ASD who experienced regression and who did not. Conclusion : Regression seems to be a distinctive feature of ASD. Regression is associated with cognitive and more general functions, rather than symptoms specific to autism.

Patterns of Restricted and Repetitive Behaviors in Toddlers and Young Children with Autism Spectrum Disorder

  • Objectives: Restricted and repetitive behaviors (RRBs) are a core symptom in the diagnosis of autism spectrum disorder (ASD). The complexity of behavioral patterns has called for the creation of phenotypically homogeneous subgroups among individuals with ASD. The purpose of this study was 1) to investigate the different types of RRBs and 2) to explore whether subgroups created by RRBs would show unique levels of functioning in toddlers and young children with ASD. Methods: A total of 313 children with ASD, aged 12-42 months were included in the analysis. The Autism Diagnostic Interview-Revised was used to obtain information on the different types of RRBs by grouping 15 items into six categories. The Vineland Adaptive Behaviors Scale, a parent-reported questionnaire, was used to measure adaptive functioning. A portion of the children were analyzed separately for verbal-related RRBs based on their expressive language level. Two-step cluster analysis using RRB groups as features was used to create subgroups. Analysis of covariance while covarying for age and language was performed to explore the clinical characteristics of each cluster group. Results: Sensory-related RRBs were the most prevalent, followed by circumscribed interests, interest in objects, resistance to change, and repetitive body movements. A subset of the children was analyzed separately to explore verbal-related RRBs. Four cluster groups were created based on reported RRBs, with multiple RRBs demonstrating significant delays in adaptive functioning. Conclusion: Heterogeneity of RRBs emerges at a young age. The different patterns of RRBs can be used as valuable information to determine developmental trajectories with better implications for treatment approaches.

Association between neonatal jaundice and autism spectrum disorders among children: a meta-analysis

  • Autism spectrum disorder is a common neurodevelopmental disorder with an unknown etiology. The correlation between neonatal jaundice and the risk of developing autism spectrum disorder was investigated previously. Some studies showed significant associations, whereas others demonstrated no association. In this meta-analysis, we pooled the results of observational studies to examine the association between neonatal jaundice and the risk of autism spectrum disorder among children. We identified all studies published through April 2018 by conducting a literature search using Web of Science, PubMed, and Scopus databases as well as the reference lists of the retrieved studies. The pooled odds ratios (ORs), rate ratio (RR), and their 95% confidence intervals (CIs) were calculated as random effect estimates of association among studies. We conducted a subgroup analysis to explore any potential sources of intergroup heterogeneity. The pooled estimates of OR and RR showed a considerable correlation between neonatal jaundice and ASD among children (OR, 1.35; 95% CI, 1.02-1.68) and (RR, 1.39; 95% CI, 1.05-1.74). A larger effect size was shown in the pooled estimated crude OR than in the adjusted OR (1.75 [0.96-2.54] vs. 1.19 [1.07-1.30]). This study showed that neonatal jaundice may be associated with ASD and may increase the risk of ASD among children.

Characteristics of late-onset epilepsy and EEG findings in children with autism spectrum disorders

  • Purpose: To investigate the clinical characteristics of late-onset epilepsy combined with autism spectrum disorder (ASD), and the relationship between certain types of electroencephalography (EEG) abnormalities in ASD and associated neuropsychological problems. Methods: Thirty patients diagnosed with ASD in early childhood and later developed clinical seizures were reviewed retrospectively. First, the clinical characteristics, language and behavioral regression, and EEG findings of these late-onset epilepsy patients with ASD were investigated. The patients were then classified into 2 groups according to the severity of the EEG abnormalities in the background rhythm and paroxysmal discharges. In the severe group, EEG showed persistent asymmetry, slow and disorganized background rhythms, and continuous sharp and slow waves during slow sleep (CSWS). Results: Between the two groups, there was no statistically significant difference in mean age (P=0.259), age of epilepsy diagnosis (P=0.237), associated family history (P=0.074), and positive abnormal magnetic resonance image (MRI) findings (P=0.084). The severe EEG group tended to have more neuropsychological problems (P=0.074). The severe group statistically showed more electrographic seizures in EEG (P=0.000). Rett syndrome was correlated with more severe EEG abnormalities (P=0.002). Although formal cognitive function tests were not performed, the parents reported an improvement in neuropsychological function on the follow up checkup according to a parent's questionnaire. Conclusion: Although some ASD patients with late-onset epilepsy showed severe EEG abnormalities, including CSWS, they generally showed an improvement in EEG and clinical symptoms in the longterm follow up. In addition, severe EEG abnormalities tended to be related to the neuropsychological function.

공격적 문제행동을 보이는 자폐스펙트럼장애 아동청소년의 임상적 특징: 예비연구 (Clinical Characteristics of Aggressive Behavior Problems in Children with Autism Spectrum Disorder: Preliminary Study)

  • Objectives: The aim of this study was to investigate the clinical characteristics of aggressive behavior problems in children and adolescents with autism spectrum disorder (ASD). Methods: Children and adolescents with ASD, aged 6-18 years, were included in this study. We divided them into two groups according to the presence or absence of aggressive behavior. They were assessed using three scales, the Korean-Child Behavior Checklist (K-CBCL), Behavior Problem Inventory, and Social Communication Questionnaire, which were completed by their parents, who conducted a continuous performance test to evaluate their attention function. The statistical analysis was done using the Mann-Whitney U-test between the two groups at a p-value of 0.05. Results: A total of 17 children participated in this study. Ten children (7 boys, $12.4{\pm}4.27years$) were included in the group with aggressive behavior problems and 7 children (6 boys, $13.8{\pm}3.53years$) in the group without aggressive behavior problems. There were no significant differences in the age, gender or intelligence quotient of the two groups. The children with aggressive behavior problems showed a trend of higher T-scores in the attention problems and anxiety/depressed subscale of the CBLC (p<0.1). Conclusion: The results suggest that aggressive behavior problems in children with ASD may be related to their attention problems, anxiety and depression.
