관절경하 전방십자인대 재건술 후 경골 터널 크기 변화 (Tibial Tunnel Enlargement following Arthroscopic ACL Reconstruction)

  • We retrospectively evaluated the changes in the diameter of the tibial tunnel over time following the reconstruction of the anterior cruciate ligament with a bone-patella tendon-bone(BPTB) autograft(25 cases) and quadruple semitendinosus(ST) graft(27 cases) in 52 patients at one year postoperatively. The changes in the geometry of the bony tunnel were measured with radiography. The demensions at final follow up were correated with the clinical results. An increased width of the tibial tunnel was noted in all cases. On the femoral side, however, no tunnel expansion was noted. In AP view, the average tibial tunnel enlargement in ST and BPTB graft groups were 1.30mm(13%) and 1.82mm(17%), respectively. In lateral view, the average tibial tunnel enlargement in ST and BPTB graft group was 1.30mm(13%) and 2.04mm(19%). The differences between two groups were not statistically significant, however, there was evidence of a borderline significance(P=0.0502). Although the tunnel enlargement does not appear to adversely affect the clinical outcome in the short term, the exact mechanism which are involved should be demonstrated. Furthermore histologic study is needed to evaluate graft replacements with emphasis on the graft-tunnel interface.

한국 보병에서 발생한 중족골 행군골절 양상의 예비적 보고 (Preliminary report of March Fractures in Infantry Soldiers of Korea - About 15 (19cases) march fracture patients -)

  • Stress or march fractures among military personnel, especially recruits, has been appreciated for many years. According to the classical references, the second metatarsal was one of the first sites identified as a focus for march fractures and radiological evidence of fracture appeared as late as several weeks. The purpose of this study was to document the clinical feature of march fractures in Korean infantry soldiers. From 1997 to 1998, at one infantry medical company of OO infantry corps in Korea, 15 (19cases) patients with march fracture were detected among infantry soldiers. There were some different finding in the fracture site and its clinical features from the previous foreign reports. 1. There were pain and local swelling in all cases as clinical manifestation. By physical examination, direct point tenderness on the location of the fractured metatarsal shaft was characteristic. 2. On roentgenographic examination, cortical fissuring or break was seen one week after onset of symptoms and external callus was seen from two weeks or at the least four weeks. Oblique view was more useful than AP view in the diagnosis of march fractures. 3. The third metatarsal was the most frequently involved site(7 cases, 48%). and the second metatarsal was Jess frequent(3 cases, 20%). This difference of frequent site with previous reports might be attributed to the relatively long shaft of the third metatarsal, but should be analyzed in further study. 4. The incidence of the development of march fracture was 1 per 104 infantry soldiers.

어깨관절의 접시오목을 나타내는 Grashey법에 대한 연구 (The Study of Grashey Method Viewing the Glenohumeral(shoulder) Joint)

  • Glenohumeral joint's를 나타내는 단순방사선 촬영법으로 전후사방향 자세에서 촬영하는 Grashey법이 있다. 이 촬영법 자세는 선 자세에서 검사 측을 들어 올리고 촬영한다. 하지만 촬영 시 환자를 몸통을 들어 올리고 회전하는 정도에 따라 관찰범위가 변화한다. Glenohumeral joint's를 가장 잘 나타내기 위한 몸통 회전 각도를 알아보기 위하여 총 20명(남:13명, 여:7명)을 대상으로 촬영을 실시하였으며, 정확한 영상의 판단을 위하여 정형외과 전문의 2명, 방사선사 2명(경력 15년 이상자)이 관찰 평가하였다. 평가의 척도는 Glenohumeral joint's 분리 정도의 점수를 0점은 '분리 안 됨', 1점은 '불량', 2점은 '보통', 3점은 '양호'로 점수를 부여하였다. 몸통 회전 각도를 $30^{\circ}$ 인 영상은 Humerus head와 Glenoid fossa가 겹쳐서 나타났고, $35^{\circ}$ 인 영상에서는 Humerus head와 Glenoid fossa가 약간 겹쳐서 나타났고, $40^{\circ}$ 인 영상에서는 Humerus head와 Glenoid fossa의 관절강이 가장 겹침이 적게 나타났고, $45^{\circ}$ 인 영상에서도 겹침 정도가 많았다. 각 영상의 평가 점수는 $30^{\circ}$$0.40{\pm}0.499$, $35^{\circ}$$1.34{\pm}0.657$, $40^{\circ}$$1.84{\pm}0.573$, $45^{\circ}$$0.76{\pm}0.649$로 나타났고 통계적으로 유의한 차이를 나타냈다.(p < .05). 선 자세에서 Glenohumeral joint's를 나타내기 위한 자세에서 환자 몸통을 $40^{\circ}$ 검사 반대 측을 들어 올리고 촬영하였을 경우 우리는 가장 양호한 접시오목(Glenoid fossa)를 나타낼 수 있었다.

선천성 만곡족의 장기 수술적 치료 결과 (Long-Term Results of Surgical Treatment for the Idiopathic Clubfoot)

  • 목적: 도수조작 및 석고 교정을 통해 보존적 치료를 시행하였으나 잔여 혹은 재발변형으로 수술적 치료를 받았던 특발성 만곡족 환아의 장기 치료 결과를 분석하였다. 대상 및 방법: 방사선상 골 성숙이 완료된 32명의 환자 총 51예를 대상으로 후향적인 연구를 시행하였다. 최종 추시 시 평균 연령은 18.7세였다. 수술은 선택적 혹은 광범위 연부조직 유리술과 힘줄 연장, 힘줄 이전, 다양한 절골술 등을 시행하였다. 방사선적 분석은 최종 촬영한 체중부하 전후면 사진에서 거종골 각(talocalcaneal angle), 거골-제1 중족골 각(talo-first metatarsal angle), 측면 사진에서 측정한 거종골 각, 종골 경사각(calcaneal pitch)을 측정하여 성인정상 측정값과 비교하였다. 임상적 평가는 American Orthopaedic Foot and Ankle Society (AOFAS)의 ankle-hindfoot score와 midfoot score를 이용하여 우수(>85), 양호(71-85), 보통(56-70), 불량(<56)으로 평가하였다. 결과: 최종 방사선 계측치는 전후면 거종골 각에서 각각 41.2%, 전후면 거골과 제1 중족골 간의 각에서 90.2%, 측면 거종골 각에서 84.3%, 측면 종골 경사각에서 각각 61%가 정상범위에 포함되었다. AOFAS 평균은 ankle-hindfoot score에서 88.1±10.7점 midfoot score에서 86.7±11.5점이었다. 결론: 수술적 치료를 받았던 특발성 만곡족 환아의 장기 치료 결과에서 정상범위에 속하는 방사선 지표 비율은 43%-90%로 확인되었으며 임상적으로 AOFAS 평균값은 우수에 속하였다. 따라서 비수술적 교정 방법에 실패한 경우라도 변형의 요소를 보다 더 정확히 분석한 후 선택적 수술 방법을 통할 경우 임상적으로 만족스러운 결과를 가져올 수 있다.

Multi-Dimensional Reinforcement Learning Using a Vector Q-Net - Application to Mobile Robots

  • Reinforcement learning is considered as an important tool for robotic learning in unknown/uncertain environments. In this paper, we propose an evaluation function expressed in a vector form to realize multi-dimensional reinforcement learning. The novel feature of the proposed method is that learning one behavior induces parallel learning of other behaviors though the objectives of each behavior are different. In brief, all behaviors watch other behaviors from a critical point of view. Therefore, in the proposed method, there is cross-criticism and parallel learning that make the multi-dimensional learning process more efficient. By ap-plying the proposed learning method, we carried out multi-dimensional evaluation (reward) and multi-dimensional learning simultaneously in one trial. A special neural network (Q-net), in which the weights and the output are represented by vectors, is proposed to realize a critic net-work for Q-learning. The proposed learning method is applied for behavior planning of mobile robots.

청소년기의 구조적 척추측만증 환자 치험 1례에 대한 보고 (A Clinical Case Study on Treatment of Adolescent Structural)

  • Objectives : This study was performed to evaluate the effect of oriental medical treatment on adolescent structural scoliosis. Methods : A patient who has scoliosis was treated with chuna and acupuncture treatment. After the treatment, we evaluated the effects of treatment with radiological measurement by calculating the scoliosis correction angle from the initial and follow-up spine AP view. Result : After the treatment, the cobb's angle of T-spine was changed $23^{\circ}$ to $15^{\circ}$, that of L-spine was changed $25^{\circ}$ to $10^{\circ}$. Conclusions : In this study, Chuna and acupuncture treatment was efficacious against scoliosis. And it is necessary for adolescent scoliosis patient to be more early detection, earily treated and continuous observed.

척추측만증 환자에 관한 임상적 고찰 (A Clinical Study on Patients of Scoliosis)

  • Objective : This study is designed to provide clinical data about patients of scoliosis for clinicians. Methods : We investigated 112 patients who have signs and symptoms of scoliosis. visited Vertebral -Joint Center of Conmaul Oriental Medical Hospital from January 2003 to July 2004. We measured Cobb's angle in the 112 patients' X-ray of full spine AP and lateral view and also analyzed age, sex, chief complaints. Results : 1. We investigated 72 female and 40 male patients. The most common distribution of age was 20's in both sex. 2. In the scoliosis angle : The average scoliosis angle was $9.27^{\circ}{\pm}6.06^{\circ}$ in 112 patients. The average scoliosis angle of 40 male patients was$8.9^{\circ}{\pm}6.77^{\circ}$. The average scoliosis angle of 72 female patients was $9.48^{\circ}{\pm}5.67^{\circ}$. 3. The most common region of scoliosis was thoracic spine with 37 patients. 4. The most common chief complaint was low back pain with 69 patients. In the 48 patients. self awareness of scoliosis was included in the chief complaint.

Thermal-Hydraulic Aspects of an Advanced Reactor Core with Triangular Lattice Fuel Assemblies

  • Thermal-hydraulic performance has been analyzed for an advanced reactor core loaded with hexagonal fuel assemblies. Currently available CHF prediction models and data base for triangular lattice bundles have been thoroughly reviewed, and as a result the KfK-3 CHF correlation with limit CHFR of 1.235 has been determined to be most appropriate. The pressure drop model in COBRA-IV-I code has been modified for the analysis of triangular lattice rod bundles. In view of maximizing the thermal margin, the geometry of a hexagonal fuel assembly, such as rod diameter and rod pitch, has been optimized with a fixed fuel assembly cross sectional area The optimum value of the moderator-to-fuel volume ratio is estimated to lie between 0.65 to 1 with 9.5 mm rod diameter. The thermal margin of these hexagonal fuel assemblies in the AP600 core has been evaluated and compared with that of square lattice fuel assemblies such as VANTAGE-5H and KOFA. The analysis result shows that the performances of hexagonal fuel assemblies are more favorable than the square fuel assemblies in the aspect of steady-state overpower margin.

  • 안경은 단지 안경을 창조한 디자이너의 기술에 의해서만 결정되지 않는다. 안경을 잘 만들기 위해서는 항상 장용자의 견해를 고려해야 한다. 왜냐하면 안경은 무엇보다도 시력을 교정하는 의료용구이기 때문에 어떠한 경우에도 패션 디자이너의 절대권을 감수해서는 안된다. 그러므로 안경 디자이너의 역할은 민감한 감수성, 영감에 의한 착상, 독창성, 기능과 기술을 발전시키기 위한 노하우 및 조화성 디자인의 제작과 인체공학적 개념을 가지고 design하는 것이다. 현재 컴퓨터 그래픽과 cad/cam과 같은 컴퓨터 처리기술의 도움으로 기술적인 디자인과 신속한 견본제작(RP)을 할 수 있는 큰 변화가 진척되고 있다.

  • The efficient and well organized database is essential for the management of information in every industrial field. In this study, a practical and effective database which can handle 3-D information of steel bridges is built on the basis of a STEP-based data model. The data model of steel bridge information is classified into geometric and non-geometric part and the design information is represented by linking geometric information and life cycle supported non-geometric information. Especially, the shape information is represented by boundary representation method, which is one of the representative methods of solid model information. In this study, ISO/STEP(STandard for the Exchange of product model data) AP203(configuration controlled design) EXPRESS schema is used to represent the shape information of steel bridge. The syntax of EXPRESS schema of developed data model is verified by NIST Expresso - is a tool for parsing and compiling EXPRESS schema. Also, this study verifies the conformance of the data model by applying to the real data of Hannam bridge. Therefore, the constructed database using STEP-based data model of steel bridges can be used effectively in the concurrent engineering point of view with transferring and sharing steel bridge information.
