의학교육 평가인증, 기회인가 부담인가? (Is the Accreditation in Medical Education an Opportunity or a Burden?)

  • The accreditation process (AccP) is both an opportunity and a burden for medical schools-which one it becomes depends on how medical schools recognize and utilize the AccP. In other words, if a medical school recognizes the AccP only as a formal procedure or as a means for continuing medical education, it will be a burden for the medical school. However, if a medical school recognizes the real and positive value of the AccP, it can be both an opportunity and a tool for developing medical education. The educational value of the AccP is to improve the quality, equity, and efficiency of medical education, along with increasing the options of choice. In order for the AccP to contribute to the development of medical education, accrediting agencies and medical schools must first be recognized as part of an "educational alliance" working together towards common goals. Secondly, clear guidelines on the accreditation standards should be periodically reviewed and shared. Finally, a formative evaluation using self-evaluation as a system that can utilize the AccP as an opportunity to develop medical education must be introduced. This type of evaluation system could be developed through collaboration among medical schools, academic societies for medical education, and the accrediting authority.

침샘 선양낭성암종의 세포학적, 분자생물학적 특성에 관한 연구 (CELLULAR AND MOLECULAR CHARACTERIZATION OF ADENOID CYSTIC CARCINOMA OF THE SALIVARY GLANDS)

  • Adenoid cystic carcinoma (ACC) of salivary glands has a protracted clinical course with perineural invasion, delayed onset of hematogenous metastasis, and poor responses to classical cytotoxic chemotherapic agents. Most deaths from salivary ACC are caused by lung metastases that are resistant to conventional therapy. Therefore, knowledge of cellular properties and tumor-host interactions that influence the dissemination of metastatic cells is important for the design of more effective therapy of salivary cancer. I determined in vitro expression of epidermal growth factor receptor (EGFR) and its downstream effectors and vascular endothelial growth factor receptor (VEGFR)-2 on a human salivary ACC cell line (ACC2). I also evaluated the expression of EGF and VEGF signaling molecules and metastasis-related proteins on human salivary ACC cells orthotopically growing in nude mice. In Western blot and immunohistochemical analyses, EGFR and VEGFR-2 were presented and phosphorylated in ACC2 cells. In human parotid cancer xenografts in nude mice, EGF and VEGF signaling molecules, IL-8, and MMP-9 were expressed at markedly higher levels than in normal parotid tissues. Moreover, tumor-associated endothelial cells of this orthotopic parotid tumor expressed phosphorylated VEGFR-2 and phosphorylated Akt, which is a cell-survival protein. These data show that those biomarkers can be molecular targets for therapy of salivary ACC, which has a propensity for delayed lung metastasis.

  • 현재 전세계적으로 170kV급 이하의 초고압 지중케이블 접속함으로 기존의 현장몰드방식의 TMJ(Tape Molded Joint)와 부품조립방식의 PJ(Prefabricated Joint)보다 조립시간이 짧고 작업자의 숙련도가 높지 않아 경제적이며, 유지보수에 장점을 가진 고무몰드 접속함(PMJ)이 채용되고 있으며, 이러한 추세는 적용 전압급이 점차 상승하면서 230kV급까지 그 비중이 늘어나고 있는 경향을 보이고 있다. 당사에서는 이러한 추세에 발맞추어 154kV급 고무몰드접속함의 개발 당시의 Silicone의 기초물성시험 DATA 획득, 고무몰드 UNIT 및 금형 설계 기술 확립과 접속 공법 개발의 경험을 바탕으로 230kV급 고무몰드접속함의 개발에 성공하였다.

적응형 정속 주행 시스템에 대한 운전자 신뢰와 도로 혼잡도가 작업부하 및 상황인식에 미치는 효과 (The Effects of Driver's Trust in Adaptive Cruise Control and Traffic Density on Workload and Situation Awareness)

  • 본 연구에서는 운전 시뮬레이션을 이용하여 적응형 정속 주행(adaptive cruise control: ACC) 시스템에 대한 운전자의 신뢰 및 도로 혼잡도가 운전자의 작업부하와 상황인식에 미치는 효과를 알아보았다. ACC 시스템에 대한 운전자의 신뢰는 ACC 시스템이 정상 작동하는 조건과 시스템이 오작동하는 조건을 통해 신뢰상승 집단과 신뢰감소 집단으로 구분하였다. 도로 혼잡도는 운전자 차량 주변의 차량 수로 수준을 조작하였다. ACC 시스템에 대한 신뢰와 도로 혼잡도를 달리한 네 가지의 실험 조건 각각에 대해 운전자들의 작업부하와 상황인식을 측정하였다. 본 연구의 결과를 요약하면 다음과 같다. 먼저 ACC 시스템에 대한 신뢰감소 집단은 신뢰상승 집단에 비해 이 시스템의 사용으로 인한 운전부담 경감이나 안전운전 확보 등을 포함한 측정 항목 모두에서 시스템에 대한 신뢰 점수가 유의하게 더 낮았다. 둘째, ACC 시스템에 대한 신뢰감소 집단은 신뢰상승 집단에 비해 이차과제에서 더 느린 반응시간을 보였고, 시스템 사용에서의 주관적인 작업부하 수준도 더 높게 평정하였다. 셋째, 이와는 대조적으로 운전자들의 운전상황에 대한 상황인식은 ACC 시스템 신뢰감소 집단이 신뢰상승 집단보다 유의하게 더 우수하였다. 본 연구의 결과들은 ACC 시스템에 대한 신뢰가 운전 중에 수행하는 다양한 정보처리에 영향을 미칠 수 있음을 보였는데, 이것은 자동화된 운전보조 시스템의 설계에서 사용자의 시스템에 대한 신뢰가 중요한 변인으로 고려되어야 한다는 것을 시사한다.

Biotinoyl Domain of Human Acetyl-CoA Carboxylase;Structural Insights into the Carboxyl Transfer Mechanism

  • Acetyl-CoA carboxylase (ACC) catalyzes the first step in fatty acid biosynthesis: the synthesis of malonyl-CoA from acetyl-CoA. As essential regulators of fatty acid biosynthesis and metabolism, ACCs are regarded as therapeutic targets for the treatment of metabolic diseases such as obesity, In ACC, the biotinoyl domain performs a critical function by transferring an activated carboxyl group from the biotin carboxylase domain to the carboxyl transferase domain, followed by carboxyl transfer to malonyl-CoA. Despite the intensive research on this enzyme, only the bacterial and yeast ACC structures are currently available, To explore the mechanism of ACC holoenzyme function, we determined the structure of the biotinoyl domain of human ACC2 and analyze its characteristics using NMR spectroscopy. The 3D structure of the hACC2 biotinoyl domain has a similar folding topology to the previously determined domains from E. coli and P. Shermanii, however, the 'thumb' structure is absent in the hACC2 biotinoyl domain. Observations of the NMR signals upon the biotinylation indicate that the biotin group of hACC2 does not affect the structure of the biotinoyl domain, while the biotin group for E. coli ACC interacts directly with the thumb residues that are not present in the hACC2 structure. These results imply that, in the E. coli ACC reaction, the biotin moiety carrying the carboxyl group from BC to CT can pause at the thumb of the BCCP domain. The human biotinoyl domain, however, lacks the thumb structure and does not have additional non-covalent interactions with the biotin moiety; thus, the flexible motion of the biotinylated lysine residue must underlie the "swinging arm" motion. This study provides insight into the mechanism of ACC holoenzyme function and supports the "swinging arm" model in human ACCs.

A New Alkalophilic Bacterium Producing Ethylene

  • A new isolate, Bacillus sp. ALK-7 can synthesize ethylene from l-aminocyclopropane-l-carboxylic acid (ACC) as well as from methionine. The ACC has only been recognized as a key intermediate found in the metabolic pathway leading to ethylene formation in various plants. The efficiency of ethylene formation from the ACC by Bacillus sp. ALK-7 was about 2 times as high as that from the methionine. The reaction from ACC to ethylene formation was also shown to be mediated by the cell-free extracts of Bacillus sp. ALK-7.

Arabidopsis 피토크롬 돌연변이체(phyAB)의 뿌리 굴중성 반응 (Root Gravitropic Response of Phytochrome Mutant (phyAB) in Arabidopsis)

  • Arabidopsis의 피토크롬 2중 돌연변이형 (phrAB)은 야생형 (WT)과 비교하여 뿌리의 굴중성 반응이 지연되었다. 중력 자극을 받은 지 8시간 후에 돌연변이체의 굴중성 반응은 야생형의 48%를 나타내었다 지연된 반응은 중력 자극을 준 후 1.5 시간 뒤에 나타났다. 12시간 동안 야생형과 돌연변이형의 뿌리 절편에서 에틸렌 생성을 측정하였다. 돌연변이형의 에틸렌 생성은 12시간이 경과한 후에 야생형의 40% 정도로 감소되었다. 이러한 결과는 피토크롬이 에틸렌 생성과 연관되어 있음을 제시하고 있다. 일반적으로 에틸렌은 식물의 뿌리나 줄기를 억제한다. 본 연구에서는 에틸렌 전구체인 1-aminocycloprpane-1-carboxylic acid (ACC)를 처리하여 뿌리의 생장을 측정하였다. 야생형은 ACC 존재하에 뿌리 생장이 억제되었으나, 돌연변이형은 야생형만큼 억제를 나타내지 않았다. 이 결과를 확인하기 위하여 ACC 존재하에 굴중성 반응을 측정한 결과, 야생형은 ACC가 없는 경우와 비교하여 37.4%의 억제를 나타냈으나, 돌연변이형은 ACC가 없는 경우와 비교하여 6.6%만을 억제하였다. 이 결과는 피토크롬이 에틸렌 생성을 통하여 뿌리 굴중성 반응을 조절한다는 것을 제시한다.

Acoustic Change Complex에 기반한 와우소실영역 검사의 객관적인 방법 제시를 위한 예비 연구 (The Objective Test of Cochlear Dead Region Using Acoustic Change Complex: A Preliminary Report)

  • Background and Objectives Cochlear dead region (CDR) is a region in the cochlear where hearing loss has occurred due to damage to the inner hair cells and/or neurons. Recently, a subjective test involving a pure-tone test in the presence of threshold-equalizing noise (TEN) was introduced to identify CDR. However, for uncooperative patients, such a subjective method would be unsuitable and objective methods would be needed instead to detect CDR. The acoustic change complex (ACC) is an evoked potential elicited by changes in the ongoing sound. In this study, we developed an objective method of identifying CDR by combining ACC response with a TEN test, namely the TEN-ACC test, and investigated its feasibility in normal-hearing listeners. Subjects and Method Ten normal-hearing subjects participated in this study. All subjects underwent both behavioral TEN test and electrophysiological TEN-ACC test. The stimuli for the TEN-ACC test consisted of TEN and embedded pure tones with different frequencies/signals to noise ratios (SNRs). To identify the thresholds, the range SNR of stimulation was varied from 0 to 20 dB, in stages of 4 dB. Results The ACC responses of all subjects who participated in this study were well elicited by stimuli developed for the TEN-ACC test. We confirm that the pure-tones embedded in TEN elicited the objective ACC response. Conclusion The results of this study suggest that the novel TEN-ACC test can be applied to evoke ACC in normal-hearing listeners. Future research should incorporate hearing-impaired listeners to determine the feasibility of the TEN-ACC test as an objective method to identify CDR.

국내도로 환경을 고려한 ACC 시험평가 방법에 관한 연구 (A Study on Evaluation Method of ACC Test Considering Domestic Road Environment)

  • With automobiles sharply increasing worldwide, we are faced with serious social problems such as traffic accidents, traffic jams, environmental pollution, and economic inefficiency. In response, research on ITS is promoted mainly by regions with advanced automotive industry such as the US, Europe, and Japan. While Korea is working to get ahead to take global market through developing and turning into global standards systems related to ASV(Advanced Safety Vehicle), the country is not excellently prepared for such projects. The purpose of ACC is to control the vehicle's longitudinal speed and distance and minimize driver workload. Such a system should be useful in preventing accidents, as it reduces driver workload in the 21st-century world of telematics created by the development of automobile culture industry. In this light, the thesis presents a method to test and evaluate ACC system and a mathematical method to assess distance. Furthermore, for the proposed test and evaluation, theoretical values are tested with vehicle test and a database is acquired, by using vehicles equipped with an ACC system. We proposed the scenarios suitable for the domestic environment and conducted the actual road test. Theoretical evaluation criteria for developing ACC system may be employed and scenario-specific evaluation methods may find their useful application through testing the formula proposed by comparing the database and the mathematical method. and, many companies are expected to utilize the scenario in the developing stage of ACC, and be able to employ as a verification method by harnessing theoretical formula before a road test.
