  • Objectives : The purpose of this study is to analyze outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. Methods : We retrospectively analyzed outpatients who visited for treatment for sudden sensorineural hearing loss in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean medical hospital from January 1st, 2013 to December 31th, 2021. according to year, season, gender, age, clinical symptoms, number of visits and initial degree of hearing loss. The statistical analysis performed using IBM SPSS 25.0 for Windows. Results : The results of analyzing the medical records of 194 patients are as follows. 1. The number of patients showed a gradual increase from 2014 to 2021 and spring was highest in season. Acc ording to gender, women visited more than men and the age group that visited the most was middle-aged, including 40s and 50s. 2. Patients experienced 13 symptoms and the average number was 4.06. In the analysis of the major symptoms according to gender, hyperacusis and headache were found to be more common in female. 3. The average of initial hearing loss was 52.54dB and was higher in male. The loss was relatively higher in 60s or older than in the younger age group. Conclusions : If treatment and management methods that reflect these symptoms are developed, it is thought that it will greatly helpful to improve the quality of life of patients with sudden hearing loss.

클로로필 제거 구기엽 추출물의 비알코올성 지방간 보호 효과 (Protective effect of chlorophyll-removed ethanol extract of Lycium barbarum leaves against non-alcoholic fatty liver disease)

  • 구기엽 추출물 (LLE)과 클로로필을 제거한 구기엽 추출물 (LLE(Ch-))이 MCD diet로 비알코올성 지방간을 유도한 C57BL/6 mouse와 팔미트산으로 지방 축적을 유도한 HepG2 세포에서 지방축적의 억제에 미치는 영향을 검토하였다. 대조군 대비 LLE(Ch-)는 혈장 TG 농도와 혈장 AST와 ALT의 활성을 유의하게 감소시켰으며 LLE군에서는 혈장 ALT 활성이 유의하게 감소하였다. 두 군 모두 간조직의 TG와 cholesterol 함량을 유의하게 낮추었으며 간조직의 병리학적 변화 결과에서 LLE군에 비해 LLE(Ch-)군에서 지방축적의 억제효과가 더 크게 나타났다. 팔미트산 0.5 mM을 처리한 HepG2 세포에서 LLE와 LLE(Ch-)가 1,000 ㎍/mL 농도까지 독성이 없었으며 대조군에 비하여 각각 200 ㎍/mL과 500 ㎍/mL 농도부터 세포 내 지방 축적량을 유의하게 감소시켰고 pAMPK와 pACC 발현이 농도 의존적으로 증가하였으며 FAS발현은 농도 의존적으로 억제되었다. 결과적으로 구기엽 100% 에탄올 추출물들은 간조직의 지방 축적을 억제할 수 있으며 그 효과는 클로로필 제거 구기엽의 활성이 좀 더 큰 것으로 나타났다. 따라서 이 두 소재 모두 항 지방간 효능이 있는 것으로 판단되며 기능성 소재로서의 개발 가능성을 확인하였다.

Probiotics Increase Intramuscular Fat and Improve the Composition of Fatty Acids in Sunit Sheep through the Adenosine 5'-Monophosphate-Activated Protein Kinase (AMPK) Signaling Pathway

  • This experiment aims to investigate the impact of probiotic feed on growth performance, carcass traits, plasma lipid biochemical parameters, intramuscular fat and triglyceride content, fatty acid composition, mRNA expression levels of genes related to lipid metabolism, and the activity of the enzyme in Sunit sheep. In this experiment, 12 of 96 randomly selected Sunit sheep were assigned to receive the basic diet or the basic diet supplemented with probiotics. The results showed that supplementation with probiotics significantly increased the loin eye area, and decreased plasma triglycerides and free fatty acids, increasing the content of intramuscular fat and triglycerides in the muscle and improving the composition of the fatty acids. The inclusion of probiotics in the diet reduced the expression of adenosine 5'-monophosphate-activated protein kinase alpha 2 (AMPKα2) mRNA and carnitine palmitoyltransferase 1B (CPT1B) mRNA, while increasing the expression of acetyl-CoA carboxylase alpha (ACCα) mRNA, sterol regulatory element-binding protein-1c (SREBP-1c) mRNA, fatty acid synthase mRNA, and stearoyl-CoA desaturase 1 mRNA. The results of this study indicate that supplementation with probiotics can regulate fat deposition and improves the composition of fatty acids in Sunit sheep through the signaling pathways AMPK-ACC-CPT1B and AMPK-SREBP-1c. This regulatory mechanism leads to an increase in intramuscular fat content, a restructuring of muscle composition of the fatty acids, and an enhancement of the nutritional value of meat. These findings contribute to a better understanding of the food science of animal resources and provide valuable references for the production of meat of higher nutritional value.

Optimization of mix design of micro-concrete for shaking table test

  • Considering their similar mass densities, an attempt was made to optimize the mix design of micro-concrete that used barite sand as an aggregate by substituting marble powder (5%, 10%, 20%, 30%, 40%, 50%, 70%), clay brick powder (30%, 50%, 70%), and fly ash (30%, 50%, 70%) for the concrete (by mass) to form specimens for shaking table tests. The test results showed that for these three groups of materials, the substitutions had little effect on the density. The barite sand played a decisive role in the density, and the overall density of the specimens reached approximately 2.9 g/cm3. The compressive strength and elastic modulus decreased with an increase in the substitution rates for the three types of materials. Among them, the 28 day compressive strength values of the 40% and 50% marble powder groups were 11.73 MPa and 8.33 MPa, respectively, which were 58.7% and 70.7% lower than the control group, respectively. Their elastic modulus values were 1.33×104 MPa and 1.42×104 MPa, respectively, which were 39.1% and 35% lower than those of the control group, respectively. The 28 day compressive strength values of the 50% and 70% clay brick powder groups were 13.13 MPa and 5.8 MPa, respectively, which were 53.8% and 79.6% lower than the control group, respectively. Their elastic modulus values were 1.54×104 MPa and 1.19×104 MPa, respectively, which were 29.7% and 45.4% lower than those of the control group, respectively. The 28 day compressive strength values of the 50% and 70% fly ash groups were 13.5 MPa and 7.1 MPa, respectively, which were 52.5% and 75% lower than those of the control group, respectively. Their elastic modulus values were 1.36×104 MPa and 0.95×104 MPa, respectively, which were 37.9% and 56.6% lower than those of the control group, respectively. There was a linear relationship between the 28 day compressive strength and elastic modulus, with the correlation coefficient reaching a value higher than 0.88. The test results showed that the model materials met the high density, low compressive strength, and low elastic modulus requirements for shaking table tests, and the test data of the three groups of different alternative materials were compared and analyzed to provide references and assistance for relevant model testers.

  • Objectives : Although the anti-obesity effect of Schizandrae Fructus water extract has been demonstrated, its underlying mechanism is still unclear. Therefore, we aimed to evaluate the anti-obesity effect of Schizandrae Fructus water extract through the p-AMP-activated protein kinase (p-AMPK), sirtuin1 (Sirt1), and peroxisome proliferator-activated receptor-gamma coactivator 1α (PGC-1α) signaling in 60% high-fat diet (HFD)-induced obese mouse model. Methods : Male C57BL/6 mice were divided into four groups. The Normal group was fed a normal diet and the obese groups were fed 60% HFD. Except for the Control group, the GG group was supplemented with 0.5% Garcinia gummigutta and the SCW group was supplemented with 0.5% Schizandrae Fructus water extract. After 6 weeks, obesity-related biomarkers in serum were measured and the expressions of protein for lipid-related factors in liver tissue were analyzed by western blot. Results : Treatment with SCW significantly down-regulated body weight compared to the Control group. SCW down-regulated levels of triglyceride and total cholesterol in serum and significantly increased p-AMPK, Sirt1, and PGC-1α in liver tissue. In addition, the expressions of fatty acid oxidation-related proteins such as peroxisome proliferator-activated receptor α (PPARα), carnitine palmitoyltransferase 1A (CPT-1A), uncoupling protein 1 (UCP1), and uncoupling protein 3 (UCP3) were significantly up-regulated. However, fatty acid synthesis-related proteins including sterol regulatory element-binding protein-1 (SREBP-1), phospho-Acetyl-CoA Carboxylase (p-ACC), and fatty acid synthase (FAS) were significantly down-regulated. Conclusions : Taken together, SCW treatment showed anti-obesity effect by regulating both fatty acid oxidation-related and fatty acid synthesis-related proteins through AMPK/Sirt1/PGC-1α signaling in 60% HFD-induced obese mice.

연교의 항산화 효과 연구 (Anti-oxidant effect of forsythia suspensa on cellular damage in the chronic disease)

  • Objectives : This study induced oxidative stress in HepG2 cells by treating them with AA+iron and investigated the effects of forsythia suspensa extract on this stress, as well as elucidated the molecular mechanisms underlying its hepatoprotective effects. Methods : To confirm the antioxidative effects of FSE, HepG2 cells were induced with AA+iron to induce oxidative stress, followed by MTT assay. Additionally, the effect of FSE in reducing the increased ROS levels and mitochondrial damage induced by AA+iron in HepG2 cells was confirmed using FACS. Furthermore, western blot analysis were conducted to investigate the molecular mechanisms underlying the hepatoprotective effects of FSE. Results : FSE increased the decreased cell viability induced by AA+iron. Additionally, FSE normalized the expression of apoptosis-related proteins induced by AA+iron. The elevated ROS levels in HepG2 cells induced by AA+iron were reduced by FSE, and the increase in Rh123-negative cells induced by AA+iron was attenuated by FSE. Moreover, FSE activated the protein expression of AMPK and its related phosphorylating enzymes, LKB1 and ACC. Furthermore, FSE activated YAP and its upstream phosphorylating enzyme, LATS1. Conclusions : These results demonstrate that FSE has an inhibitory effect on oxidative stress induced by AA+iron and may have potential hepatoprotective effects.

Chamaeneron angustifolia 추출물이 3T3-L1 Preadipocyte의 지방대사 및 분화에 미치는 영향 (Effects of Chamaeneron angustifolia Extract on Lipid Metabolism and Differentiation of 3T3-L1 Preadipocyte)

  • Objectives : Obesity is an imbalance between energy intake and consumption due to overeating and lack of exercise, and if it persists, it increases non-infectious diseases such as high blood pressure, diabetes, and hyperlipidemia. In this study, we tried to investigate the possibility of using Chamaeneron angustifolia (CA) as a material for anti-obesity by confirming the effect of inhibiting lipid differentiation. Methods : We measured the effects of CA extract on oil-red-o staining, cell cytotoxicity evaluation activity using 3T3-L1 cells. Additionally, we assessed fat decomposition and metabolism-related protein expression through Western blot analysis. Results : In this study, the anti-obesity effects of CA extract were experimentally assessed. Results showed significant inhibition of adipocyte differentiation and accumulation at concentrations of 0.05, 0.1, and 0.2 mg/ml of oil-red-o staining, with reductions of 80% or more. CA notably increased the phosphorylation of AMPK protein expression compared to the control group across all concentrations. Additionally, phosphorylation of ACC significantly increased at a concentration of 0.2 mg/ml compared to the control. PPAR-γ, which regulates adipogenesis, exhibited a significant decrease compared to the control, while protein expression of CPT-1, involved in fatty acid oxidation, showed a concentration-dependent increase across all groups. Therefore, CA extract demonstrates potential as a functional material for anti-obesity by increasing the expression of proteins related to fat decomposition and synthesis while decreasing others. Conclusions : These results suggest that CA may also be useful as an anti-obesity functional substance.

  • Dans cet article, nous discutons de certains $probl{\grave{e}}mes$ syntaxiques en ayant recours $\grave{a}$ la $th{\acute{e}}orie$ linguistique de Milner. Nous remettons en question $l'ind{\acute{e}}pendance$ et $l'identit{\acute{e}}$ de la structure syntaxique, la relation entre le plan syntaxique et le plan lexical, le $caract{\grave{e}}re$ de la $g{\acute{e}}om{\acute{e}}trie$ de la syntaxe, etc.. La discussion est non seulement linguistique mais aussi interdisciplinaire et ${\acute{e}}pist{\acute{e}}mologique$, dans la mesure $o{\grave{u}}$ nous examinons la nature de $l'entit{\acute{e}}$ syntaxique et la $m{\acute{e}}thode$ "scientifique" de la syntaxe qui donne $acc{\grave{e}}s$ $\grave{a}$ $l'entit{\acute{e}}$ syntaxique. Selon Milner, il faut distinguer la place du terme lexical avec la position syntaxique qui est $l'entit{\acute{e}}$ syntaxique. La $premi{\grave{e}}re$ n'est pas syntaxique $\grave{a}$ strictement parler, mais elle, observable contrairement $\grave{a}$ la seconde, sert $\grave{a}$ conjecturer la dimension syntaxique, $c'est-\grave{a}-dire$ le $syst{\grave{e}}me$ positionnel. Le dispositif $th{\acute{e}}orique$ dans la $th{\acute{e}}orie$ linguistique de Milner n'est rien d'autre que l'ensemble des propositions qui permet, en absence d'observatoire, de conjecturer le $syst{\grave{e}}me$ positionnel sur la base du $syst{\grave{e}}me$ des places. Dire $l'ind{\acute{e}}pendance$ de la structure syntaxique revient $\grave{a}$ dire qu'il y a une coupure entre le $syst{\grave{e}}me$ positionnel et le $syst{\grave{e}}me$ des places. Autrement dit, sans cette coupure, on ne peut parler de $l'ind{\acute{e}}pendance$ de la structure syntaxique. Ainsi $distingu{\acute{e}}s$, les deux $syst{\grave{e}}mes$ en cause se mettent en relation soit naturels soit non naturels ou par distorsion $d'apr{\grave{e}}s$ Milner. La relation naturelle est une relation lexico-syntaxique $n{\acute{e}}e$ au moment $o{\grave{u}}$ un terme lexical occupe une position syntaxique dont la $cat{\acute{e}}gorie$ est identique $\grave{a}$ celle de son occupant. A la $diff{\acute{e}}rence$ de cette relation d'occupation naturelle $suppos{\acute{e}}e$ chez Milner comme une tendance du langage naturel, la relation d'occupation non naturelle est "paradoxale" dans le sens $o{\grave{u}}$ elle est produite dans la rencontre plus ou moins "anomale" entre l'occupant lexical et $l'occup{\acute{e}}$ syntaxique. Le $degr{\acute{e}}$ de l'anomalie qu'une langue autorise peut ${\hat{e}}tre$ $mesur{\acute{e}}$ empiriquement et doit ${\hat{e}}tre$ $vari{\acute{e}}$ en fonction de la langue $concern{\acute{e}}e$. Le $caract{\grave{e}re$ $g{\acute{e}}om{\acute{e}}trique$ de la syntaxe $am{\grave{e}}ne$ ${\grave{a}}$ remettre en cause, entre autres, $l'empiricit{\acute{e}}$ et la $mat{\acute{e}}rialit{\acute{e}}$ de la $g{\acute{e}}om{\acute{e}}trie$ syntaxique. En ce qui concerne ces sujets, nos $th{\grave{e}}ses$ sont les suivantes : la nature de la $g{\acute{e}}om{\acute{e}}trie$ syntaxique n'est pas a priori mais empirique ; la $g{\acute{e}}om{\acute{e}}trie$ de la syntaxe peut et doit ${\hat{e}}tre$ construite $\grave{a}$ l'aide de la logique "empirique".

AMP-activated protein kinase가 셀레늄으로 처리된 3T3-L1 지방세포의 분화과정 억제에 관한 연구 (AMP-activated Kinase Regulates Adipocyte Differentiation Process in 3T3-L1 Adipocytes Treated with Selenium)

  • 셀레늄은 필수미네랄로 항산화작용 등의 체내 주요 작용을 수행한다고 알려져 있다. 만성질환 등의 예방과 치료 등의 바람직한 효능을 기대하여 섭취가 늘고 있어서 주목을 받고 있다. 셀레늄은 암의 억제나 혈중포도당조절 및 지방억제 등의 목적으로도 이용되고 있다. 이러한 다양한 연구에도 불구하고 지방세포를 이용한 지방세포분화나 지방세포사멸 촉진에 미치는 영향은 아직 연구되어 있지 않다. 셀레늄의 지방세포조절을 통한 분자기전에 대한 연구는 늘어가고 있는 비만이나 비만관련대사질환의 조절에 학문적 근거를 제공할 수 있다. 본 연구에서는 셀레늄이 AMP-activated protein kinase (AMPK)를 중심으로 지방 미분화세포인 3T3-L1 지방세포의 분화억제 및 분화된 세포의 사멸작용의 기전을 밝히고자 하였다. AMPK는 세포 내 에너지상태의 센서로 작용하는데 실제적으로 운동의 지방대사 촉진기능이 이 단백질에 기인한다는 사실이 밝혀지면서 각광을 받고 있다. 또한 시상하부에서 AMPK는 호르몬이나 영양소 등의 생리적인 인자들을 통합하고 식욕이나 신체에너지 대사를 총괄하는 물질로 대두되고 있다. 셀레늄을 지방세포의 분화과정에 처리하였을 때에 현저한 지방세포로의 분화가 억제되었고 이미 다 분화된 성숙된 지방세포에 처리하였을 때는 세포사멸(apoptosis)을 유도하는 것을 발견하였으며 이와 동시에 AMPK의 활성화 촉진 및 AMPK의 지질인 acetyl-CoA carboxylase의 $serine^{79}$를 인산화하는 것으로 나타났다. 또한 이러한 셀레늄의 지방세포분화 억제 능력은 AMPK의 활성화 합성물질인 AICAR나 포도의 폴리페놀인 레스베라트롤의 능력과 같은 정도의 억제능을 보여주었다. 또한 AMPK의 항 지방축적작용에서의 필수성을 알아보기 위하여 지방세포에 AMPK 활성화 물질인 AICAR와 억제물질인 Compound C를 사용하여 AMPK활성화를 측정하여 본 결과 지방세포분화와 지방축적의 억제과정에 있어서 AMPK가 필수적임을 밝혀내었다.

감염성 심내막염의 임상적 고찰 (Clinical Analysis of Infective Endocarditis)

  • 배경: 감염성 심내막염의 수술 적응증 및 시기 등은 논란의 대상이다. 저자들은 본 교실에서 최근 10년간 연속적으로 수술받은 감염성 성내막염 환자들에 대한 수술 결과를 분석하여 적절한 수술 적응증 및 시기 등에 대해 문헌 고찰과 함께 보고하고자 한다. 대상 및 방법: 2000년 1월부터 2009년 12월까지 본원에서 연속적으로 수술한 감염성 심내막염 환자 23예를 후향적으로 분석하였다. 진단은 modified Duke criteria에 준하였고 남녀 비율 2.8:1, 평균연령 $46.78{\pm}16.19$ (24~76)세, NYHA 기능 분류상 class I 1예(4.3%), class II 4예 (17.4%), class III 10예(43.5%), class IV 8예(34.8%)였다. 수술 전 혈액내 균 배양 검사에서 8예 (34.8%)가 원인균이 동정되었고 수술 전 평균 항생제 사용기간은 $20.78{\pm}16.00$ (1~56)일이었다. ACC/AHA 심장판막질환 지침중 감염성 심내막염의 수술 적응증에 근거하여 수술을 진행하였으며 12예(52.2%)에서 긴급 수술이 필요하였다. 23명의 환자 모두 외래 추적 관찰이 가능하였으며 평균 외래 추적 관찰 기간은 $49.26{\pm}33.21$ (11~128)개월이었다. 결과: 수술 소견상 우종은 20예에서 발견되었고 대동맥판막 치환술 9예, 승모판막 치환술 8예, 승모판막 성형술 1예, 대동맥판막 치환술과 승모판막 치환술을 함께 시행한 3예가 있었다. 승모판막 성형술 l예를 제외한 20예의 판막치환술 환자 중 19예에서 기계판막을 사용하였다. 감염된 점액종의 제거술이 1예, 감염된 심박동기 lead의 제거술 및 감염조직 절제술이 1예였다. 수술 후 평균 항생제 투여 기간은 $24.39{\pm}15.98$ (l2~85) 일이었다. 수술 후 합병증은 출혈로 재수술한 2예, 심낭막절개술 후 증후군으로 배액술이 필요했던 l예, 수술장에서 발생한 저심박출증 l예, 수술 후 일과성 뇌 허혈증이 있었던 l예 등이었다. 수술 전후로 비교한 NYHA 기능분류는 통계적으로 유의한 개선이 있었고(p<0.001) 평균 좌심실 구현률은 유의하지 않았으며(p=0.468) 좌심실 이완기말/수축기말 크기, 좌심방 크기는 유의한 개선이 있었다(p<0.05). 결론: 감염성 심내막염 환자에서 적절한 수술 적응증을 적용하여 조기수술을 필요로 하는 환자에서는 적시에 수술을 시행하였으며 혈류역학적으로 안정된 경우는 충분한 항생제 투여로 전신성 염증의 소설을 유도한 후 수술을 시행하여 전 예에서 수술 사망이나 감염의 재발없이 좋은 단중기 수술 성적을 얻을 수 있었다.
