Use of Reference Ear Plug to improve accuracy of lateral cephalograms generated from cone-beam computed tomography scans

  • Objective: The purpose of this study was to evaluate the effectiveness of the use of Reference Ear Plug (REP) during cone-beam computed tomography (CBCT) scan for the generation of lateral cephalograms from CBCT scan data. Methods: Two CBCT scans were obtained from 33 adults. One CBCT scan was acquired using conventional methods, and the other scan was acquired with the use of REP. Virtual lateral cephalograms created from each CBCT image were traced and compared with tracings of the real cephalograms obtained from the same subject. Results: CBCT scan with REP resulted in a smaller discrepancy between real and virtual cephalograms. In comparing the real and virtual cephalograms, no measurements significantly differed from real cephalogram values in case of CBCT scan with REP, whereas many measurements significantly differed in the case of CBCT scan without REP. Conclusion: Measurements from CBCT-generated cephalograms are more similar to those from real cephalograms when REP are used during CBCT scan. Thus, the use of REP is suggested during CBCT scan to generate accurate virtual cephalograms from CBCT scan data.

  • Statement of problem : Tooth wear is physiological phenomenon. Ninety-seven percent of normal people have tooth wear and about 7% has pathologic teeth wear. If we know the amount of tooth surface loss caused by pathologic tooth wear, we may restore it ideally Purpose : Recently measurement of tooth wear by using 3D scan has been increasing. Therefore, we need to know how accurate 3D scan is. Past accuracy test on 3D scan was about linear change, but as we know that tooth wear is volume change. Thus, the purpose of this study is to know how accurate 3D scan is. Material and Methods : For accuracy test of 3D scanner volume values measured by 3D scanner and micro-balance were compared. For test I, preliminary, 3 ball samples and 3 circular cones were made with pattern resin. For test II, 10 teeth shape rubber samples were used. Results and Conclusion : 1. The result of the accuracy test on 3D scan with 3 ball samples and 3 circular cones made of pattern resin has no significant difference(p<0.05). 2. The result of the accuracy test on 3D scan with 10 samples of tooth shape rubber has no significant difference (p<0.05). As a result, we may concluded the analysis of quantifying tooth wear used by 3D scan is useful in the clinic.

Determination of Optimal Scan Time for the Measurement of Downstream Metabolites in Hyperpolarized 13C MRSI

  • Purpose: For a single time-point hyperpolarized $^{13}C$ magnetic resonance spectroscopy imaging (MRSI) of animal models, scan-time window after injecting substrates is critical in terms of signal-to-noise ratio (SNR) of downstream metabolites. Pre-scans of time-resolved magnetic resonance spectroscopy (MRS) can be performed to determine the scan-time window. In this study, based on two-site exchange model, protocol-specific simulation approaches were developed for $^{13}C$ MRSI and the optimal scan-time window was determined to maximize the SNR of downstream metabolites. Materials and Methods: The arterial input function and conversion rate constant from injected substrates (pyruvate) to downstream metabolite (lactate) were precalibrated, based on pre-scans of time-resolved MRS. MRSI was simulated using two-site exchange model with considerations of scan parameters of MRSI. Optimal scan-time window for mapping lactate was chosen from simulated lactate intensity maps. The performance was validated by multiple in vivo experiments of BALB/C nude mice with MDA-MB-231 breast tumor cells. As a comparison, MRSI were performed with other scan-time windows simply chosen from the lactate signal intensities of pre-scan time-resolved MRS. Results: The optimal scan timing for our animal models was determined by simulation, and was found to be 15 s after injection of the pyruvate. Compared to the simple approach, we observed that the lactate peak signal to noise ratio (PSNR) was increased by 230%. Conclusion: Optimal scan timing to measure downstream metabolites using hyperpolarized $^{13}C$ MRSI can be determined by the proposed protocol-specific simulation approaches.

  • 본 연구에서는 이중 시간 PET/CT 검사에서 지연검사 시간이 다른 두 그룹의 40명의 췌장암 환자에 대한 SUV 데이터의 통계 분석을 통해 최적의 지연검사 시간을 제시하였다. 췌장암의 유무를 확인해야 될 이중 시간 PET/CT 검사에서 새로운 수학적 예측 모델을 만들어 정확도 높은 최적의 지연검사 시간을 제시하였다. 시간에 따른 포도당 대사를 적용한 방사능 붕괴식을 예측 모델로 제시하였다. 지연검사 시간이 다른 두 그룹의 데이터를 가지고 시뮬레이션한 결과 PET/CT에서 췌장암 판별을 위한 최적의 지연검사 시간이 131.5분으로 제시하였다. 제안된 지연검사 시간은 췌장암 결정에 적용될 수 있는 통계적 유의 수준을 보였다(p<0.05). 제안된 지연 검사 시간 131.5분을 결정하여 검사를 진행할 때, 췌장암 조기 판별에 도움을 줄 수 있을 것으로 사료된다.

A Renal Size Discrepancy among the Findings of Renal Sonogram in Children with Their First Episode of Pyelonephritis is One of the Useful Parameters to Predict the Presence of Cortical Defects on the Acute DMSA Renal Scan

  • Purpose: We investigated whether a renal size discrepancy on a renal sonogram (US) in children with febrile urinary tract infection (UTI) was correlated with the presence of cortical defects on their dimercaptosuccinic acid (DMSA) renal scan. Methods: We examined 911 children who were admitted consecutively to our hospital with their first episode of febrile UTI from March 2001 to September 2014. All enrolled children underwent a US and DMSA scan during admission. According to the US findings, including the renal size discrepancy, data were compared between children with positive and negative DMSA scan results. A positive DMSA scan result was defined as reduced or absent tracer localization and indistinct margins that did not deform the renal contour. Results: Mean renal lengths of the right and left kidneys were larger in children with positive DMSA scan results than in children with negative DMSA scan results ($63.2{\pm}11.3mm$ vs. $58.4{\pm}7.8mm$, P<0.001; $64.9{\pm}11.2mm$ vs. $59.9{\pm}7.9mm$, P<0.001; respectively). A significant difference was observed in both renal lengths between children with positive and negative DMSA scan results ($4.6{\pm}3.8mm$ vs. $3.3{\pm}2.6mm$, P<0.001). A multiple logistic regression analysis, revealed that a small kidney, cortical thinning, and a renal length discrepancy on US findings were significant factors for predicting the presence of cortical defects on an acute DMSA scan [P=0.028, 95% confidence interval (CI) 1.054-2.547; P= 0.004, 95% CI 1.354- 4.810; P<0.001, 95% CI 1.077-1.190, respectively]. Conclusion: In conclusion, a renal size discrepancy on US findings in children with their first episode of febrile UTI was a helpful tool for predicting the presence of cortical defects on an acute DMSA scan.

갑상선암 환자에서 방사성 옥소로 오염된 목도리에 의한 위양성 소견 (False-positive I-131 Scan by Contaminated Muffler in a Patient with Thyroid Carcinoma)

  • A 39-year-old female patient who had undergone a total thyroidectomy for a papillary thyroid carcinoma underwent a whole body scan with I-131. The I-131 scan was performed 72 hours after administering 185 MBq (5 mCi) of an I-131 solution. The anterior image of the head, neck, and upper chest showed multiple areas of increased uptake in the mediastinal area considering of functional metastasis. However, radioactivity was not evident in the image taken after removing her clothes and muffler. The image obtained after placing the muffler on the pallet showed that the radioactivity was still present. It is well known that artifacts on an I-131 scan can be produced by styling hair sputum, drooling during sleep, chewing gum, and paper or a cloth handkerchief that is contaminated with the radioactive iodine from either perspiration or saliva. This activity might be mistaken for a functional metastasis. Therefore, it is essential that an image be obtained after removing the patient's clothes. In this study, artifacts due to a contaminated muffler on the I-131 scan were found. These mimicked a functional metastasis of the mediastinal area in a patient with a papillary thyroid carcinoma.

Intraoral scanning of the edentulous jaw without additional markers: An in vivo validation study on scanning precision and registration of an intraoral scan with a cone-beam computed tomography scan

  • Purpose: A fully digital approach to oral prosthodontic rehabilitation requires the possibility of combining (i.e., registering) digital documentation from different sources. This becomes more complex in an edentulous jaw, as fixed dental markers to perform reliable registration are lacking. This validation study aimed to evaluate the reproducibility of 1) intraoral scanning and 2) soft tissue-based registration of an intraoral scan with a cone-beam computed tomography (CBCT) scan for a fully edentulous upper jaw. Materials and Methods: Two observers independently performed intraoral scans of the upper jaw in 14 fully edentulous patients. The palatal vault of both surface models was aligned, and the inter-observer variability was assessed by calculating the mean inter-surface distance at the level of the alveolar crest. Additionally, a CBCT scan of all patients was obtained and a soft tissue surface model was generated using patient-specific gray values. This CBCT soft tissue model was registered with the intraoral scans of both observers, and the intraclass correlation coefficient(ICC) was calculated to evaluate the reproducibility of the registration method. Results: The mean inter-observer deviation when performing an intraoral scan of the fully edentulous upper jaw was 0.10±0.09 mm. The inter-observer agreement for the soft tissue-based registration method was excellent(ICC=0.94; 95% confidence interval, 0.81-0.98). Conclusion: Even when teeth are lacking, intraoral scanning of the jaw and soft tissue-based registration of an intraoral scan with a CBCT scan can be performed with a high degree of precision.

  • A side scan sonar system uses the towfish installed sonars, It is an equipment that search images of the bottom surface of the sea in real time. It is a typical equipment that is related to a sea investigation such as a geological survey, seabed communication cable and power line cable placing repair investigation, fish breeding ground investigation, sea purification, relic and mineral investigation, and mine and submarine search. It used to fined objects and investigate on the seabed surface. But, recently, it is used to sea purification and geological survey that require information of the correct surface of the seabed. So, it needs various filtering technique and image processing techniques development to acquire high resolution image. therefore, this research develops a side scan sonar using multi-beam sensors that supply various information with the fast scan speed and correct high resolution that is not a simple underwater investigation equipment.

유속신호증강효과의 자기공명혈관조영술을 이용한 뇌혈관검사에서 Half Scan Factor 적용한 영상 평가 (Evaluation of TOF MR Angiography and Imaging for the Half Scan Factor of Cerebral Artery)

  • 신호증강효과기법을 이용한 자기공명혈관술에서 뇌동맥을 half scan factor에 따른 절반스캔과 완전스캔의 영상을 평가하는데 목적으로 한다. 뇌혈관성 질환이 없는 환자(n = 30)를 대상으로 절반스캔과 완전스캔 하였고, 뇌동맥의 관심영역을 세 영역(C1, C2, C3)에서 7~8 mm의 범위로 설정하였다. MIP로 재구성한 영상으로 신호강도를 SNR(signal to noise ration), PSNR(peak signal noise to ratio), RMSE(root mean square error), MAE(mean absolute error)을 산출하고 paired t-test를 이용하여 통계분석 하였다. 스캔시간은 절반스캔(4분 53초), 완전스캔(6분 04초)이었다. 뇌혈관의 모든 ROI의 평균 측정 범위(7.21 mm)이었고, 첫번째 C1의 SNR은 완전스캔(58.66 dB), 절반스캔(62.10 dB)이었고, 양의 상관관계($r^2=0.503$)이고, 두 번째 C2의 SNR은 완전스캔(70.30 dB), 절반스캔(74.67 dB)이고 양의 상관관계($r^2=0.575$)이었다. 세 번째 C3의 완전스캔 SNR(70.33 dB), 절반스캔 SNR (74.64 dB)로 양의 상관관계를 ($r^2=0.523$)로 분석되었다. 절반스캔과 완전스캔의 비교에서 SNR($4.75{\pm}0.26dB$), PSNR($21.87{\pm}0.28dB$), RMSE($48.88{\pm}1.61$)이었고 MAE($25.56{\pm}2.2$)로 산출되었다. SNR은 두 검사 스캔에서 통계학적으로 유의하지 않았고 (p-value > .05) 영상의 질에서는 많은 차이가 없어 완전스캔을 사용하였을 때보다 적은 시간이 소요되는 절반스캔을 적용하여 검사하여도 된다.
