흉부(胸部)의 X선촬영(線撮影) 및 촬영장치(撮影裝置) 현황(現況)에 관(關)한 조사(調査) (A Study on Radiographic Equipments and Situation for the Chest)

  • From the study on the chest radiologic equipments and situations of university hospitals, general hospitals, and radiologic clinics located in Seoul area and Choong-cheong province, the author got conclusions as follows : 1. The rectfication methods of the equipments were employing mainly single or three-phase full wave, and 78% of the examined were using single phase full wave. 2. The focal sizes in case of small focus were less than 1.0mm in chest-only use, and more than 1.0mm in 81% of combined use. The focal sizes in case of large focus were more than 1.6mm in 78% of the total. 3. Maximum ratings of x-ray tubes were 47% at 125 kVp maximum tube voltage and 41% at 150kVp. The maximum tube currentes higher than 500mA were shown in 72% of all the examined. And the maximum exposure time was the range of 5-8sec. 4. Grid was not used in the 66% of all the hospitals. Equipment for chest-only was employing 10:1 grid ratio and the 2.0-2.9mmAl of total filtration formed 38% of all the examined. 5. In the processing time of exposed film, 68% of all was 90sec, and only 20% of all the examined used optimum developing temperature according to this condition. 6. In the radiographic conditions, more than 50% used low tube voltages lower than 70 kVp and only 3% of all the equipments used high tube voltages higher than 100 kVp. The 46.9% of all the equipments 300mA, and 53% of all used shorter exposure time less than 1/20(0.05) sec. 7. In the monthly average number of radiographs, 44% of all was x-ray the radiographs.

소아에서의 제한적 심실중격결손의 외과적 치료 (Surgical treatment of restrictive venticular septal defect in pediatric patients)

  • From January 1989 to December 1993, cardiac catheterization and open heart surgery for ventricular septal defect closure were performed in 115 pediatric patients who were selected as meeting the criteria for elective closure of restrictive ventricular septal defect. These criteria included age greater than 1 year and less than 15 years, no evidence of congestive heart failure, Qp/Qs 2.0, pulmonary artery systolic pressure 35mmHg, and no associated cardiac anomalies. Mean age of patients was 5.25$\pm$ 3.53, and 72 patients were male, 43 patients were female[male:female=1.9:1 . Mean systolic pulmonary artery pressure was 19.66$\pm$4.79mmHg, and mean pulmonary to systemic flow ratio was 1.27$\pm$ 0.28. Aortic cusp prolapse was present in 30 patients [26% , aortic insufficiency was present in 1 paient, and 1 patient had prior bacterial endocarditis. There were no instances of complete atrioventricular dissociation, reoperations for bleeding, or reoperations for recurrent ventricular septal defect, but wound infection was present in 1 patient, and there were 7 patients who had the hemodynamically insignificant remnant shunt. There were no early or late deaths or major morbidity.

뇌동맥류에 대한 색전술 시 체적에 따른 코일 특성 (Characteristic of Coils According to Volume in Case of Embolization of Cerebral Aneurysm)

  • 본 연구는 2010년 3월부터 2011년 9월까지 뇌동맥류로 H병원에 내원한 환자 중 혈관조영술을 시행하여 코일 색전술을 받은 53명의 환자를 대상으로 뇌동맥류의 체적에 따른 코일의 길이 특성을 평가하였다. 뇌동맥류에 대한 색전술을 시행한 환자의 뇌동맥류에 대하여 볼륨렌더링기법으로 체적을 구하였으며, 코일색전술을 실시한 후 코일의 체적 및 길이를 구하여 체적율을 계산하였다. 뇌동맥류의 크기에 따른 색전체적율은 6mm 이하에서는 $43.11{\pm}3.11%$, 6~10mm에서는 $36.07{\pm}2.03%$, 10~15mm 이상은 40.91%, 20mm 이상에서는 38.25%를 나타내어 권고된 체적율과 유사한 수치를 나타내었다. 뇌동맥류의 형태에 구분 없이 체적에 따른 코일 길이는 직경 0.25mm인 한 가지 코일을 사용한 경우 20~$100mm^3$$1mm^3$ 당 0.65cm로 조사되었다. saccular type의 aneurysm volume에 사용한 경우 20~$150mm^3$$1mm^3$ 당 0.62cm, multi lobulated type의 aneurysm volume에 사용한 경우 20~$90mm^3$$1mm^3$ 당 0.60cm로 조사되었다.

초음파 영상용 플라스틱 기반의 팬텀제작 및 특성 분석 (Analysis of Properties and Phantom Design Based on Plastic Hardener and Softener for Ultrasonic Imaging)

  • Plastic hardener and softener based ultrasound phantoms were made in various constitutions and their acoustic properties were measured. Speed of sound is approximately $1.4\;mm/{\mu}sec$ in all the phantoms, which is about 7% less than that of in soft tissue. Attenuation coefficient is strongly dependent on the ratio between hardener and softener. In order to achieve the tissue level attenuation (0.5 dB/cm/MHz), 60% of hardener or less is required. The synthesized phantoms can be preserved for more than 6 months without structural degradation.

Measurement of Thermal Conductivity of Foods in Liquid and Solid Phase Using a Thermal Probe

  • An instrument using thermal probe method was designed to measure thermal conductivity of liquid and solid foods. Thermal conductivity probe was designed with diameter to length ratio of 100 and diameter of 0.51 mm to minimize axial flow effect on thermal conductivity measurement. Thermal conductivities of distilled/deionized water, glycerin, and beef frankfurter meat were measured at $20-80^{\circ}C$. Mean thermal conductivity values of water showed less than 2.0% difference from several reference values without using time correction factor or probe calibration constant. For glycerin, difference was less than 0.7% from reference values at $20-50^{\circ}C$. Mean values of thermal conductivity for beef frankfurter meat ranged from 0.389 to $0.350\;W/m{\cdot}K$ at $20-80^{\circ}C$.

Comparative evaluation of molar distalization therapy using pendulum and distal screw appliances

  • Objective: To compare dentoalveolar and skeletal changes produced by the pendulum appliance (PA) and the distal screw appliance (DS) in Class II patients. Methods: Forty-three patients (19 men, 24 women) with Class II malocclusion were retrospectively selected for the study. Twenty-four patients (mean age, $12.2{\pm}1.5years$) were treated with the PA, and 19 patients (mean age, $11.3{\pm}1.9years$) were treated with the DS. The mean distalization time was 7 months for the PA group and 9 months for the DS group. Lateral cephalograms were obtained at T1, before treatment, and at T2, the end of distalization. A Mann-Whitney U test was used for statistical comparisons of the two groups between T1 and T2. Results: PA and DS were equally effective in distalizing maxillary molars (4.7 mm and 4.2 mm, respectively) between T1 and T2; however, the maxillary first molars showed less distal tipping in the DS group than in the PA group ($3.2^{\circ}$ vs. $9.0^{\circ}$, respectively). Moreover, significant premolar anchorage loss (2.7 mm) and incisor proclination ($5.0^{\circ}$) were noted in the PA group, whereas premolar distal movement (1.9 mm) and no significant changes at the incisor ($0.1^{\circ}$) were observed in the DS group. No significant sagittal or vertical skeletal changes were detected between the two groups during the distalization phase. Conclusions: PA and DS seem to be equally effective in distalizing maxillary molars; however, greater distal molar tipping and premolar anchorage loss can be expected using PA.

전신방사선조사시 균등한 선량분포를 이루기 위한 조직보상체의 이용 (Utilization of Tissue Compensator for Uniform Dose Distribution in Total Body Irradiation)

  • 연구목적 : 전신방사선조사시 인체내에 균등한 선량분포를 얻기 위하여 조직보상체를 제작하고 그에따른 선량 분포를 확인하고자 하였다. 대상 및 방법 : 0.8mm두께의 납판을 이용한 조직보상체와, 1mm 및 5mm 두에의 알루미늄판을 이용한 조직보상체를 두경부와 하지 두 부분에 대하여 각각 제작하였다. 좌우 대향전신조사시 각 신체부위에 따른 선량분포의 측정을 위하여 파라핀으로 성인 크기의 인체 모형을 실제 치료시의 체위와 비슷하게 만들어 사용하였다. 방사선은 10MV X-ray(CLIAC 1800. Varian Co., USA)를, 측정기구는 exposure/exposure rate meter(model 192, Capintec, Inc., USA)with ionization chamber(PR 05)를 이용하였고 SAD 360cm에 파라핀 팬텀의 정중선을 맞추고 기하학적 방사선조사야는 $144{\times}144cm^2$으로하여 전신이 포함되도록 하였으며 head, mouth, mid-neck, sternal notch. mid-mediastinum, xiphoid, umbilicus. pelvis. thigh. knee 및 ankle부위에서 midline absorbed dose를 각각 측정하였다. 흉부의 선량 측정에는 조직등가물질로 제작된 상업용 humanoid 팬텀에서 $1{\times}1{\times}6mm^3$부피의 TLD rod(LiF, Harshaw Co.. Netherland)를 이용하였다. 결과 : Umbilicus를 기준으로 하였을때 조직보상체를 적용하지 않은 경우 흡수선량은 $-11.8\%$에서 $21.1\%$까지의 차이를 보였다. 어깨가 포함되는 sternal notch에서의 선량은 $11.8\%$감소하였다. 조직보상체를 적용한 경우의 흡수선량은 0.8mm 납보상체의 경우 mid-neck에서 $-7.9\%$, 그외 다른 부위는 $+1.3\%$에서 $-5.3\%$가지 였다. 그리고 1mm 및 5mm 알루미늄 보상체를 적용한 경우 ankle부위에서 $5.3\%$. 그외 다른 부위는 $-2.6\%$에서 $2.6\%$가지의 흡수선량 차이를 보였다. 결론 : 납과 알루미늄으로 제작한 전신방사선조사용 조직보상체는 선량기준점인 umbilicus선량에 비교하였을 때 두경부와 하지에 있어서 비교적 만족할만한 보상효과를 나타내었다. 어깨가 있는 상흉부에서는 선량이 $11.8\%$ 정도 감소하므로 환자의 lateral thickness 차이에 따라 선량기준점을 sternal notch로 선택하거나 boost irradiation이 고려되어야함을 알 수 있었다.

안골격형과 교합과의 상호관계에 대한 연구 (A STUDY ON RELATONS BETWEEN FACIAL SKELETAL PATTERNS AND DENTAL OCCLUSION)

  • This study was undertaken to document relations between facial skeletal pattern and dental occlusion. The data in .this study were collected from pretreatment cephalometric radiographs and study models of patients' records present in the files of Orthodontic Department, Seoul National University Hospital. Patients were selected on the basis of a mandibular plane-sella nasion angle equal to or greater than $38^{\circ}$ (high SN-MP angle) or equal to or less than $26^{\circ}$ (low SN-MP angle). Patients in the mixed dentition and with missing permanent teeth were excluded for ease of assessing tooth size / arch circumference relationships and then 30 high SN-MP and 11 low SN-MP patients were selected among them. The mean age of these two groups of patients was high SN-MP, $12.8{\pm}1.23$ years and low SN-MP, $13.0{\pm}1.48$ years. The following conclusions were obtained. 1. In the maxilla and mandible the mean tooth size of high SN-MP patients was nearlly identical to the low SN-MP patients. 2. The mean maxillary arch circumference was increased in low SN-MP group compared with high SN-MP group and a smilar, but smaller, mean increase was present in mandible. 3. The difference between the mean maxillary circumference required and the mean maillary circumference present ranged from -4.8mm in the high SN-MP group to -1.3mm in the low SN-MP group. A small range of means occurred in the mandible (high SN-MP: -4.0mm to low SN-MP: -1.8mm). 4. In the maxilla and mandible the mean arch length was nearly identical in the high and low SN-MP groups. 5. The mean incisor inclination was increased as the SN-MP angle decreased in the maxilla and mandible. 6. The men distance of the maxillary first molar from anterior border of the pterygomaxillary fissure was nearly similar between high and low groups. 7. The mean mandibular intermolar width was increased from high SN-MP to low SN-MP patients.

감(대봉시(大奉枾))의 냉장(冷藏)에 있어서 포장효과(包裝效果)에 관(關)한 연구(硏究) (A Study on the Effect of Some Packing Materials in Chilling of Persimmon (Teabong- Si))

  • The sample persimmons were obtained from the of Yung-am Kun, Korea on October 24, and November 9, 1973. These persimmons were kept at room temperature overnight, then packed and stored on the following day at $0^{\circ}C$ and $85{\sim}100%$ humidity. Seven experimental items were studied : 1. control, 2. packing with excelsior in wooden boxes, 3. packing with rice hull in wooden boxes, 4. packing in bags of 0.03 mm polyethylene membrane, 5. packing in bags of 0.06 mm polyethylene membrane, 6. packing in bags of 0.08 mm polyethylene membrane, 7. packing in sealed desiccators. The dimension of polyethylene bags were $14{\times}28cm$ and three persimmons were packing in each bag. The results are as follows: 1. The persimmons both in polyethylene bags and in desiccators lost only 1 % or less of weight, while in control (1) packing with excelsior (2) or with rice hull (3) the loss was 6-7 % in weight after $2{\sim}3$ months. The persimmons became visibly shriveled and spongy-like in it's texture when it lost 5 % of weight. 2. Carbon dioxide content within the polyethylene bags and desiccators rapidly increased during the early storage period, and reached 5 % (in 0.03 mm polyethylene bags) to 50 % or more (in desiccator) after 1 month. In the polyethylene bags of 0.08 mm and desiccator, the carbon dioxide accumulated to a dangerous level and the persimmon showed physiological damage in a longer storage period. 3. The advance of after-ripening and deterioration of persimmon sealed in the polyethylene bags was slower than that in open bags when moved out to room temperature from the chilling and the polyethylene bags of 0.06 mm retarded the after-ripening most effectively. 4. On the basis of these results, a temperature of $0^{\circ}C$, relative humidity of almost $85{\sim}100%$ and combinations of 5 % oxygen and $5{\sim}8%$ carbon dioxide seem to be the optimum condition for the storage of persimmon (Teabong-Si). The most successful method of storing was to pack them in polyethylene bags of 0.06 mm and then to store them at $0^{\circ}C$.

만성 치주염에서 비외과적 또는 외과적 치주치료 후 유지관리기 동안 임상적 변화의 비교 (The comparison of clinical changes during maintenance phase after non-surgical or surgical therapy of chronic periodontitis)

  • Reports on the comparison of clinical effect between non-surgical and surgical therapy, and the change of the clinical parameters during maintenance phase have been rarely presented in Korea. This study was to observe the clinical changes during maintenance phase of 6 months in patients with chronic periodontitis treated by non-surgical or surgical therapy in Department of Periodontics, Chonnam National University Hospital. Among the systemically healthy and non-smoking patients with moderate to severe chronic periodontitis, twenty eight patients (mean age: 47.5 years) treated by non-surgical therapy (scaling and root planning) and nineteen patients (mean age: 47.3 years) treated by surgical therapy (flap surgery) were included in this study. The periodontal supportive therapy including recall check and oral hygiene reinforcement was started as maintenance phase since 1 month of healing after treatment. Probing depth, gingival recession. clinical attachment level and tooth mobility were recorded at initial, baseline and 1, 2, 3 and 6 month of maintenance phase. The clinical parameters were compared between the non-surgical and surgical therapies using Student t-test and repeated measure ANOVA by initial probing depth and surfaces. Surgical therapy resulted in greater change in clinical parameters than non-surgical therapy. During the maintenance phase of 6 months, the clinical effects after treatment had been changed in different pattern according to initial probing depth and tooth surface. During maintenance phase, probing depth increased more and gingival recession increased less after surgical therapy, compared to non-surgical therapy. The sites of initial probing depth less than 3 mm lost more clinical attachment level, and the sites of initial probing depth more than 7 mm gained clinical attachment level during maintenance phase after non-surgical therapy, compared to surgical therapy. Non-surgical therapy resulted in greater reduction of tooth mobility than surgical therapy during maintenance phase. These results indicate that the clinical effects of non-surgical or surgical therapy may be different and may change during the maintenance phase.
