A nationwide study of children and adolescents with pneumonia who visited Emergency De­partment in South Korea in 2012

  • Purpose: Acute respiratory infection, particularly pneumonia, is the most common cause of hospitalization and death among children in developing nations. This study aimed to investigate the characteristics of children and adolescents with pneumonia who visited Emergency Department (ED) in South Korea in 2012. Methods: We analyzed National Emergency Department Information System (NEDIS) records from 146 EDs in South Korea for all pediatric patients aged ${\leq}18years$ who were diagnosed with pneumonia between January and December 2012. Results: Among 38,415 subjects, the male-to-female ratio was 1:0.8. Patients aged <12 months comprised 18.0% of the study population; those aged 1 to 3 years, 54.4%; those aged 4 to 6 years, 16.8%; those aged 7 to 12 years, 7.4%; and those aged 13 to 18 years, 3.4%. Presentation rates were highest in April, followed by January, March, and May. The hospital admission rate was 43.5%, of which 2.6% were in intensive care units. The mortality rate was 0.02%. Based on the International Classification of Diseases, 10th Revision, diagnostic codes, the types of pneumonia according to cause were viral pneumonia (29.0%), bacterial pneumonia (5.3%), Mycoplasmal pneumonia (4.5%), aspiration pneumonia (1.3%), and pneumonia of unknown origin (59.3%). Conclusion: Despite the limited data due to the ED data from the NEDIS lacking laboratory results and treatment information, this study reflects well the outbreak patterns among children and adolescents with pneumonia. Our results provide a basis for future studies regarding ED treatment for children and adolescents with pneumonia.

  • 2007 Renewed Korea Elementary Mathematics Curriculum introduce algebra 6th grade. According to many studies about introducing algebra, it is desirable to teach 6th graders algebra through generalization of patterns. In this study, 6th graders' understanding processes and difficulties in pattern generalization were analyzed and possiblities of introducing algebra to 6th graders through pattern generalization were examined.

Quality Characteristics of White Pan Bread with Ingredients of Sagoonja-Tang (사군자탕(四君子湯) 재료를 첨가한 식빵의 품질특성)

  • This study was carried out to determine the functional works and basic ingredients of Sagoonja-Tang on white pan bread. The -experimental groups consisted of 6% mixtures of five(Ed- confirm the number) kinds powders, : B1 for Sagoonja-Tang powder, B2 for Panax ginseng, B3 for Poria cocos, Koidz, B4 for Atractylodes macrocephala, and B5 for Glycyrrhiza uralensis Fisch. The volume of white pan bread somewhat decreased after adding the ingredients, and the volume differences in order from highest to lowest, were white pan bread control>B2>B3>B4>B1>B5. For white pan bread, pH of control, B3, and B5 decreased on the 2nd day during storage, but increased on the 4th and 7th days. In the texture analyses of white pan bread, B4 and B5 showed a higher degree of hardness than that of control, while B2, and B3 were the lowest. In sensory test, the overall acceptability of regular white pan bread in order from highest to lowest was control>B3>B2>B1 ${\cdot}$ B5>B4.

A Study of Revision of the History Class(900) for the KDC 6th Edition (한국십진분류법 역사(900) 분야 개정에 대한 연구)

  • The purpose of this study is to investigate and analyse the revised contents of the history class in the Korean Decimal Classification(KDC), 5th edition, and then identify problems and propose the revised contents for the KDC, 6the edition. Major analysed areas are divided into four. First, geographic area table is discussed. It includes extension of the geographic area table, emphasis of hierarchical structure in the geographical area, revision of North Korean geographical names, extension of subgeographical structure of major nations in the world, and revision of nations in the central and west Asia. Second, Korean time period is extended. Third, the notes of entries of the Chinese and Japanese history areas are shorten. Fourth, the geographical and personal names are changed their native pronunciation, specially Chinese and Japanese. For the revision of the KDC, 6th edition, four areas are discussed: first, Korean geographic areas would be categorized by broaden area, second, the areas are arranged from the capital of the nation to others, third, foreign geographical names would be used their native names, and last, time period would be categorized by years.

Erythromycin Withdrawal Time in Olive Flounder (Paralichthys olivaceus) after Oral Administration (양식 넙치(Paralichthys olivaceus)에 대한 Erythromycin 휴약기간 설정)

  • The withdrawal time of erythromycin in cultured olive flounder was investigated to ensure the food safety of the fish treated with erythromycin. The oral administration of erythromycin was carried out using the experimental diet containing erythromycin (200 mg/kg) daily dose of 40 mg/kg body weight. The 45 day experimental period was broken into 7 days of habituation, 8 days of medication and 30 days of additional feeding without antibiotics. The erythromycin concentration in the flounder muscle had been increased gradually with medication. After 5 days of medication, the concentration increased to its maximum level of 6.05 mg/kg. After discontinuing the antibiotic, the erythromycin concentration decreased drastically and day 9 was below 0.1 mg/kg. The erythromycin concentration had slowly declined from the 6th to the 20th day after medication and disappeared completely after 25 days. From these results, the time needed to reduce the erythromycin level to the 0.2 mg/kg limit adopted by the ED and Japan was suspected to be 4-6 days. Therefore, a reasonable withdrawal time following ED and Japanese regulatory guidelines for erythromycin in the cultured flounder could be estimated to be 10 days.

Administrative codes may have limited utility in diagnosing biliary colic in emergency department visits: A validation study

  • Backgrounds/Aims: Biliary colic is a common cause of emergency department (ED) visits; however, the natural history of the disease and thus the indications for urgent or scheduled surgery remain unclear. Limitations of previous attempts to elucidate this natural history at a population level are based on the reliance on the identification of biliary colic via administrative codes in isolation. The purpose of our study was to validate the use of International Statistical Classification of Diseases and Related Health Problems codes, 10th Revision, Canadian modification (ICD-10-CA) from ED visits in adequately differentiating patients with biliary colic from those with other biliary diagnoses such as cholecystitis or common bile duct stones. Methods: We performed a retrospective validation study using administrative data from two large academic hospitals in Toronto. We assessed all the patients presenting to the ED between January 1, 2012 and December 31, 2018, assigned ICD-10-CA codes in keeping with uncomplicated biliary colic. The codes were compared to the individually abstracted charts to assess diagnostic agreement. Results: Among the 991 patient charts abstracted, 26.5% were misclassified, corresponding to a positive predictive value of 73% (95% confidence interval 73%-74%). The most frequent reasons for inaccurate diagnoses were a lack of gallstones (49.8%) and acute cholecystitis (27.8%). Conclusions: Our findings suggest that the use of ICD-10 codes as the sole means of identifying biliary colic to the exclusion of other biliary pathologies is prone to moderate inaccuracy. Previous investigations of biliary colic utilizing administrative codes for diagnosis may therefore be prone to unforeseen bias.

  • There occurred a moderate size earthquake of M=5 at Whagae-Myun Hadong-Gun Kyongsangnam-Do of Korea, It caused severe damage to the buildings and other structures is Sang-Gye-Sa a famous and beautiful Buddhist Temple, The 5-story stone pagoda was standing in front of Keumdang. The top component tipped over and fell to the ground during the earthquake. In order to have accurate and quantitative estimate of the intensity of earthquake a full-scale replica is made through rigorous verification process, The completed model was mounted on the shaking table and subjected to th seismic tests. It was observed that the top component overturned at 0.16 G of EPGA when the NS component of the 1940 el Centro earthquake records was used as the input motion. A brief history of this project is presented and important test results are report6ed and their implication is discussed.

Changes of relative impulse of foot on carrying 5 types of schoolbag during level walking (가방 휴대 방법에 따른 보행 시 발의 비례추진력 변화)

  • Purpose: The purpose of this study was to analyze the changes of the relative impulse of the foot when carrying 5 types of schoolbag during level walking. Methods: Forty-four normal subjects were divided into 5 groups according to the method of carrying a bag: carrying no bag (condition 1), a backpack (condition 2), a shoulder bag (condition 3), a cross bag (condition 4) and a one-hand bag (condition 5). To evaluate the partial relative impulse, 7 areas were measured on the sole of a foot: the hallux, toe, 1st metatarsal head (Met1) and the 2nd & 3rd metatarsal heads (Met2/3), the 4th & 5th metatarsal heads (Met4/5), mid foot and heel. Repeated one-way analysis of variance (ANOVA) and independent t-tests were used to analyze the statistical data. Results: In the right foot, the relative impulses in every areas revealed no significant difference (p>0.05), however, the relative impulses at Met1, Met2/3, Met4/5 and heel were significantly different between the different conditions (p<0.05). There were significant differences on comparing the relative impulse between left and right foot within one condition, the mid foot in condition 2, as well as the hallux and Met1 in condition 3 (p<0.05). In condition 4, the toe, Met1, Met4/5 and mid foot were significantly different (p<0.05). The values at the hallux, toe and Met1 in condition 5 also were significantly different (p<0.05). Conclusion: These findings showed that the symmetrical relative impulse of the foot was present in the case of carrying a backpack, but there was an extra load on specific regions and asymmetrical relative impulses in the case of carrying a shoulder bag, a across bag and a one-hand bag. New Food Code Numbering for Calculation of Nutritive Value (영양가 계산을 위한 새로운 식품코드화)

  • New food item numbers for each food included in Food Composition Table in Korea (4th ed) and other Food Composition Table. New Food item numbers classified to base 5-basic food groups and its classification was as follows. 1 As for the 1569 food items , they were classified as 20 food sub-groups(82 food sorts) for 5-basic food groups. 2. As for the 82 food sorts, they were individually classified with raw prepared , fat sugar content and arranged in order, ㄱ , ㄴ and ㄷ and made the item number. 3. The data set of nutritive value of food with new item numbers was accessed on computer files. 4. The Food & Description Table was drafted as 1572 food items were arranged in order, ㄱ, ㄴand ㄷ. 5. The Food Table arranged in the order or each nutrient content (energy, carbohydrate, protein , etc....) was drafted. Clipper program for computing nutritive values and tabulation of nutrients of daily diet were coded by applying new food item numbers. It is expected that should utilized as a basic data of computer program for calculating the nutritive value of diet, evaluating the nutrition and counseling the nutrition.

  • Effects of alumlnlum(AA), soil pH and calcium(Cal on growth response and heavy metal accumulation and regulation of nitrogen fixation In Melilotus suaveolens seedlings, a biennial legume plant dominating in the disturbed area, were quantitatively analyzed during the growing periods. Accumulation of metals In each organ of M. suaveojens was Increased UC the lowering of pH. Al contents In leaf and root treat- ed with 30ppm Al at pH 4.2 on the 28th day after treatment were 8 and 11 folds higher than those of control, while the contents with 100ppm AA were 21 and 24 folds as compared to control. The significent inhibition in Al contents was induced by 100ppm AA and Ca at pH 6.5. Increased metals inhibited higher at pH 4.2 and the growth inhibition due to Al was reversed by the addition of Ca, suggesting that growth correlates to the pH value. Chlorophyll contents in leaves increased during growth stave were inhibited by Al treatments. The biomass was decreased UD the lowering of pH and the increase of concentrations. 100ppm Ca treatment resulted in 5.1-5.9% increase of the biomass as compared to that of 100ppm Al. Specific nitrogen fuation activities In nodules In the 100ppm Al at pH 4.2 and pH 6.5 were reduced to 35. 2% and 52.2% of control, on the 28th day after treatment, while the combination of Al and Ca induced reduced by 10% as compared to control with the treatment of 100ppm AA at pH 4.2 due to the inhibition bination of 100ppm Al and Ca and lowering of pH.
