Evaluation of Image Quality for Compressed SENSE(CS) Method in Cerebrovascular MRI: Comparison with SENSE Method (뇌혈관자기공영영상에서 Compressed SENSE(CS) 기법에 대한 영상의 질 평가: SENSE 기법과 비교)

  • The object of this research is CS, which increases resolution while shortening inspection time, is applied to MRA to compare the quality of images for SENSE and CS techniques and to evaluate SNR and CNR to find out the optimal techniques and to provide them as clinical basic data based on this information. Data were analyzed on 32 patients who performed TOF MRA tests at a university hospital in Chung cheong-do (15 males, 17 females), ICA stenosis:10, M1 Aneurysm:10, and average age 53 ± 4.15). In the inspection, the inspection equipment was Ingenia CX 3.0T, Archieva 3.0T, and 32 channel head coil and 3D gradient echo as a method for equipment data. SNR and CNR of each image were measured by quantitative analysis, and the quality of the image was evaluated by dividing the observer's observation into 5 grades for qualitative evaluation. Imaging evaluation is described as being significant when the p-value is 0.05 or less when the paired T-test and Wilcoxon test are performed. Quantitative analysis of SNR and CNR in TOF MRA images Compared to the SENSE method, the CS method is a method measurement method (p <0.05). As an observer's evaluation, the sharpness of blood vessels: CS (4.45 ± 0.41), overall image quality: CS (4.77 ± 0.18), background suppression of images: CS (4.57 ± 0.18) all resulted in high CS technique (p = 0.000). In conclusion, the Compressed SENSE TOF MRA technique shows superior results when comparing and evaluating the SENSE and Compressed SENSE techniques in increased flow rate magnetic resonance angiography. The results are thought to be the clinical basis material in the 3D TOF MRA examination for brain disease.

The Research to Correct Overestimation in TOF-MRA for Severity of Cerebrovascular Stenosis (3D-SPACE T2 기법에 의한 TOF-MRA검사 시 발생하는 혈관 내 협착 정도의 측정 오류 개선에 관한 연구)

  • It is very important accurate diagnosis and quick treatment in cerebrovascular disease, i.e. stenosis or occlusion that could be caused by risk factors such as poor dietary habits, insufficient exercise, and obesity. Time-of-flight magnetic resonance angiography (TOF-MRA), it is well known as diagnostic method without using contrast agent for cerebrovascular disease, is the most representative and reliable technique. Nevertheless, it still has measurement errors (also known as overestimation) for length of stenosis and area of occlusion in celebral infarction that is built by accumulation and rupture of plaques generated by hemodynamic turbulence. The purpose of this study is to show clinical trial feasibility for 3D-SPACE T2, which is improved by using signal attenuation effects of fluid velocity, in diagnosis of cerebrovascular disease. To model angiostenosis, strictures of different proportions (40%, 50%, 60%, and 70%) and virtual blood stream (normal saline) of different velocities (0.19 ml/sec, 1.5 ml/sec, 2.1 ml/sec, and 2.6 ml/sec) by using dialysis were made. Cross-examinations were performed for 3D-SPACE T2 and TOF-MRA (16 times each). The accuracy of measurement for length of stenosis was compared in all experimental conditions. 3D-SPACE 2T has superiority in terms of accuracy for measurements of the length of stenosis, compared with TOF-MRA. Also, it is robust in fast blood stream and large stenosis than TOF-MRA. 3D-SPACE 2T will be promising technique to increase diagnosis accuracy in narrow complex lesions as like two cerebral small vessels with stenosis, created by hemodynamic turbulence.

Experimental Study on the Effect of Hyperbaric Oxygen Therapy on the Ischemic Wound Healing of Rats. (고압산소요법이 백서 구강내외 허혈성 연조직 창상치유에 미치는 영향에 관한 실험적 연구)

  • Recently, frequently using hyperbaric oxygen therapy is known for its effectiveness on the healing of problem wounds such as osteomyelitis, osteoradionecrosis and gas gangrenous wound etc. The main objectives of this study was to determine the best protocol of its application of hyperbaric oxygen therapy. Author used 238 rats (Sprague-Dawley strain) deviding into 2 Groups, experimental I group for skin and experimental II group for palatal mucosal ischemic wounds, and observed its effects by microscopically. The obtained results summarized as follows; 1. Severe infiltration of inflammatory cells was observed in initial stages of both control and experimental I group. The infiltration showed decreasing tendency at 5th day of experimental D, E, F group while at 8th day in control group In d, f of experimental II group showed decreasing tendency at 8th experimental day while at 12th day in control group. 2. Macrophages appeared at 2nd day in D of experimental I group while at 6th day in control group. In d, f of experimental II group appeared at 6th day while at 10th day in control group. 3. As to the proliferation of capillary blood vessels showed at 3-4 day in most of experimental I group severely while at 8th day in control group. In experimental II group, it was at 8th day and 12th day respectively. 4. The proliferation of fibroblasts showed rather rapider in experimental I group, at 4-6th day, while at 8th day in control group. In experimental II group, it was at 8th day and 12th day respectively. 5. As to the collagen formations, it was at 4th day in experimental I group while at 8th day in control group. In experimental II group, it was at 6th day and 10th day respectively. 6. 5 rats (2.0 %) in E group and 8 rats (3.4 %) in f group showed oxygen toxic reaction, such as unstable attitude and tremor, during the experiments. This hyperbaric oxygen animal experiments disclosed excellent effects on the ischemic wound healing and it is thought to be the best protocol of its application was on D group (2.5 ATM. and 2 hrs, exposure).

Clinical Utility of Turbo Contrase-Enhanced MR Angiography for the Major Branches of the Aortic Arch (대동맥궁 주요 분지들의 고속 조영증강 자기공명혈관조영술의 임상적 유용성)

  • Purpose : To assess the clinical utility of turbo contrast-enhanced magnetic resonance angiography(CE MRA) in the evaluation of the aortic arch and its major branches and to compare the image quality of CE MRA among different coils used. Materials and Methods : Turbo three-phase dynamic CE MRA encompassing aortic arch and its major branches was prospectively performed after manual bolus IV injection of contrast material in 29 patients with suspected cerebrovascular diseases at 1.0T MR unit. the raw data were obtained with 3-D FISH sequence (TR 5.4ms, TE 2.3ms, flip angle 30, slab thickness 80nm, effective slice thickness 4.0mm, matrix size $100{\times}256$, FOV 280mm). Total data acquisition time was 4. to 60 seconds. We subjectively evaluated the imge quality with three-rating scheme : "good" for unequivocal normal finding, "fair" for relatively satisfactory quality to diagnose 'normal' despite intravascular low signal, and "poor" for equivocal diagnosis or non-visualization of the origin or segment of the vessels due to low signal or artifacts which needs catheter angiography. At the level of the carotid bifurcation, it was compared with conventional 2D-TOF MRA image. Overall image quality was also compared visually and quantitatively by measuring signal-to-noise ratios (SNRs) of the ascending aorta, the innominate artery and both common carotid arteries among the three different coils used(CP body array(n=12), CP neck array(n=9), and head-and-neck(n=8). Results : Demonstration of the aortic arch and its major branches was rated as "good" in 55% (16/29) and "fair" in 34%(10/29). At the level of the carotid bifurcation, image quality of turbo CE MRA was same as or better than conventional 2D-TOF MRA in 65% (17/26). Overall image quality and SNR were significantlygreater with CP body array coil than with CP neck array or head-and-neck coil. Conclusions : Turbo CE MRA can be used as a screening exam in the evaluation of the major branches of the aortic arch from their origin to the skull base. Overall imagequality appears to be better with CP body array coil than with CP neck array coil or head-and-neck coil.

Effect of Acute Heat Stress on Heat Shock Protein 70 and Its Corresponding mRNA Expression in the Heart, Liver, and Kidney of Broilers

  • The objective of this study was to investigate the expression and localization of heat shock protein 70 (Hsp70) and its mRNA in the heart, liver, and kidney of acutely heat-stressed broilers at various stressing times. Male AA broilers (n = 100) were randomly divided into 5 groups of 20 birds per group. After 30 d of adaptive feeding at ambient temperature, 80 experimental broilers were suddenly heat stressed by increasing the environmental temperature from $22{\pm}1^{\circ}C$ to $37{\pm}1^{\circ}C$. The 4 groups were heat stressed for 2, 3, 5, and 10 h, respectively. The localizations of Hsp70 protein and mRNA, determined by immunohistochemical staining and in situ hybridization, respectively, were demonstrated to be tissue dependent, implying that different tissues have differential sensibilities to heat stress. Intense Hsp70 staining was identified in the vascular endothelial cell of heart, liver and kidney, suggesting an association between expression of Hsp70 in vascular endothelial cell and functional recovery of blood vessels after heat shock treatment. Ante-mortem heat stress had a significant effect on the expression of Hsp70 protein and mRNA. The quantitation of Hsp70 protein and mRNA were both time and tissue dependent. During the exposure to heat stress, the heart, liver and kidney of broiler chickens exhibited increased amounts of Hsp70 protein and mRNA. The expression of hsp70 mRNA in the heart, liver and kidney of heat-stressed broilers increased significantly and attained the highest level after a 2-h exposure to elevated temperatures. However, significant elevations in Hsp70 protein occurred after 2, 5, and 3 h of heat stressing, respectively, indicating that the stress-induced responses vary among different tissues.

The Effects of Superior Cervical Sympathetic Ganglion Block on the Acute Phase Injury and Long Term Protection against Focal Cerebral Ischemia/Reperfusion Injury in Rats (백서의 국소 뇌허혈/재관류로 인한 신경손상에서 상경부 교감 신경절 블록의 급성기 및 장기 보호효과)

  • Background: Cerebral blood vessels are innervated by sympathetic nerves from the superior cervical ganglia (SCG), and these nerves may influence the cerebral blood flow. The purpose of the present study was to evaluate the neuroprotective effect of superior cervical sympathetic ganglion block in rats that were subjected to focal cerebral ischemia/reperfusion injury. Methods: Eighty male Sprague-Dawley rats (270-320 g) were randomly assigned to one of two groups (the ropivacaine group and a control group). In all the animals, brain injury was induced by middle cerebral artery (MCA) reperfusion that followed MCA occlusion for 2 hours. The animals of the ropivacaine group received $30{\mu}l$ of 0.75% ropivacaine, and their SCG. Neurologic score was assessed at 1, 3, 7 and 14 days after brain injury. Brain tissue samples were then collected. The infarct ratio was measured by 2.3.5-triphenyltetrazolium chloride staining. The terminal deoxynucleotidyl transferase mediated dUTP-biotin nick-end labeled (TUNEL) reactive cells and the cells showing caspase-3 activity were counted as markers of apoptosis at the caudoputamen and frontoparietal cortex. Results: The death rate, the neurologic score and the infarction ratio were significantly less in the ropivacaine group 24 hr after ischemia/reperfusion injury. The number of TUNEL positive cells in the ropivacaine group was significantly lower than those values of the control group in the frontoparietal cortex at 3 days after injury, but the caspase-3 activity was higher in the ropivacaine group than that in the control group at 1 day after injury. Conclusions: The study data indicated that a superior cervical sympathetic ganglion block may reduce the neuronal injury caused by focal cerebral ischemia/reperfusion, but it may not prevent the delayed damage.

Blood Flow Simulation in Bifurcated Geometry of Abdominal and Iliac Arteries Based on CT Images (CT영상에 기반한 복부대동맥과 장골동맥 분기관 모델의 혈류유동 해석)

  • Numerical simulation of blood flow has been conducted based on real vessel geometries generated front DICOM medical images of abdominal and iliac bifurcated arteries of a healthy man. A program was developed to read cross sectional images of the three dimensional arteries and smoothly extract boundary coordinates of vessels. Commercial programs were employed for mesh generation and flow simulation. Pressures, velocities, and flow distributions were found to lie within normal physiological ranges. Peak velocity measured in the iliac artery by ultrasound was 20% smaller than that obtained by simulation. The trend of velocity variation in a cardiac cycle was fairly similar between the simulation and the ultrasonic measurements. Simulation based on real vessel geometry of individual patient provides information on pressure, velocity, and its distribution in the diseased arteries or arteries to be surgically treated. The results of simulation may help surgeons to better understand hemodynamic status and surgical need of the patient by revealing variation of the hemodynamic parameters. Futhermore, they may serve as basic data for surgical treatment of arteries. This research is expected to develop to a program in the future that early diagnose atherosclerosis by showing distribution of a hemodynamic index closely related to atherosclerosis in arteries.

A Study on the Optimum Amount of Contrast Media in Brain Angiography (뇌 혈관검사 시 적정 조영제량에 관한 연구)

  • Recently, the use of contrast agents has been increasing as a broader range of tests and dynamic tests have become common due to the development of equipment and imaging techniques such as Multi-Detector CT. However, the side effects of using contrast agents have been reduced by the development of non-ionic contrast agents, but they are still occurring often. The purpose of this study was to propose a method to minimize the side effect of contrast agent by using the amount of contrast agent injected to the brain angiography test to suppress excessive use of contrast agent and analyze the amount of contrast agent. Patients who were prescribed Brain Angiography due to cerebrovascular disease, According to the results of the comparison of the results obtained by dividing into 4 groups of 10ml each according to the amount of contrast medium injected with contrast agent according to the BMI of the patient, BA and SNR were not different between groups, and even if the amount of contrast injection was reduced, there was no problem in the evaluation of CT angiography through 3D reconstruction. This result shows that even if the contrast medium is injected into the blood vessels of the patient first and then the contrast medium is used as the physiological saline solution, the contrast medium is reduced by 40% it can be expected to minimize.

Contrast-Enhanced High-Resolution Intracranial Vessel Wall MRI with Compressed Sensing: Comparison with Conventional T1 Volumetric Isotropic Turbo Spin Echo Acquisition Sequence

  • Objective: Compressed sensing (CS) has gained wide interest since it accelerates MRI acquisition. We aimed to compare the 3D post-contrast T1-weighted volumetric isotropic turbo spin echo acquisition (VISTA) with CS (VISTA-CS) and without CS (VISTA-nonCS) in intracranial vessel wall MRIs (VW-MRI). Materials and Methods: From April 2017 to July 2018, 72 patients who underwent VW-MRI, including both VISTA-CS and VISTA-nonCS, were retrospectively enrolled. Wall and lumen volumes, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) were measured from normal and lesion sites. Two neuroradiologists independently evaluated overall image quality and degree of normal and lesion wall delineation with a four-point scale (scores ≥ 3 defined as acceptable). Results: Scan coverage was increased in VISTA-CS to cover both anterior and posterior circulations with a slightly shorter scan time compared to VISTA-nonCS (approximately 7 minutes vs. 8 minutes). Wall and lumen volumes were not significantly different with VISTA-CS or VISTA-nonCS (interclass correlation coefficient = 0.964-0.997). SNR was or trended towards significantly higher values in VISTA-CS than in VISTA-nonCS. At normal sites, CNR was not significantly different between two sequences (p = 0.907), whereas VISTA-CS provided lower CNR in lesion sites compared with VISTA-nonCS (p = 0.003). Subjective wall delineation was superior with VISTA-nonCS than with VISTA-CS (p = 0.019), although overall image quality did not differ (p = 0.297). The proportions of images with acceptable quality were not significantly different between VISTA-CS (83.3-97.8%) and VISTA-nonCS (75-100%). Conclusion: CS may be useful for intracranial VW-MRI as it allows for larger scan coverage with slightly shorter scan time without compromising image quality.

Usefulness Evaluation of HRCT using Reconstruction in Chest CT (흉부CT 검사 시 HRCT 영상 재구성의 유용성)

  • Purpose : Skip the repetitive HRCT axial scan in order to reduce the exposure of patients during chest HRCT scan, Helical Scan Data into a reconstructed image, and exposure of the patient change and visually evaluate the usefulness of the HRCT images. Materials and method : Patients were enrolled in the survey are 50 people who underwent chest CT scans of patients who presented to the hospital from January 2015 to March 2015. 50 people surveyed 22 people men and 28 people women people showed an average distribution of 30 to 80 years age was 48 years. 50 patients to Somatom Sensation 64 ch (Siemens) model with 120 kVp tube voltage to a reference mAs tube current to mAs (Care dose, Siemens) as a whole, including the lungs and the chest CT scan was performed. Scan upon each patient CARE dose 4D (Automatic exposure control, Siemens Medical Solution Erlangen, Germany) was to maintain the proper radiation dose scan every cross-section through a device that automatically adjusts the tube current of. CT scan is the rotation time of the Tube slice collimation, slice width 0.6 mm, pitch factor was made under the terms of 1.4. CT scan obtained after the raw data (raw data) to the upper surface of the axial images and coronal images for each slice thickness 1 mm, 5 mm intervals in the high spatial frequency calculation method (hight spatial resolution algorithm, B60 sharp) was the use of the lung window center -500 HU, windows were reconstructed into images in the interval -1000 HU to see. Result : 1. Measure the total value of DLP 50 patients who proceed to chest CT group A (Helical Scan after scan performed with HRCT) and group B (Helical Scan after the HR image reconstruction to the original data) compared with the group divided, analysis As a result of the age, but show little difference for each age group it had a decreased average dose of about 9%. 2. A Radiation read the results of the two Radiologist and a doctor upper lobe and middle lobe of the lung takes effect the visual evaluation is not a big difference between the two images both, depending on the age of the patient, especially if the blood vessels of the lower lobe (A: 3.4, B: 4.6) and bronchi(A: 3.8, B4.7) image shake caused by breathing in anxiety (blurring lead) to the original data (raw data) showed that the reconstructed image is been more useful in diagnostic terms. Conclusion : Scan was confirmed a continuous, rapid motion video to get Helical scan is much lower lobe lung reduction in visual blurring, Helical scan data to not repeat the examination by obtaining HRCT images reorganization reduced the exposure of the patient.
