Imported Intraocular Gnathostomiasis with Subretinal Tracks Confirmed by Western Blot Assay

  • We report a case of intraocular gnathostomiasis diagnosed by western blot assay in a patient with subretinal tracks. A 15-year-old male patient complained of blurred vision in the right eye, lasting for 2 weeks. Eight months earlier, he had traveled to Vietnam for 1 week and ate raw wild boar meat and lobster. His best-corrected visual acuity was 20/20 in both eyes and anterior chamber examination revealed no abnormalities. Fundus examination showed subretinal tracks in the right eye. Fluorescein angiography and indocyanine green angiography showed linear hyperfluorescence of the subretinal lesion observed on fundus in the right eye. Ultrasound examination revealed no abnormalities. Blood tests indicated mild eosinophilia (7.5%), and there was no abnormality found by systemic examinations. Two years later, the patient visited our department again for ophthalmologic evaluation. Visual acuity remained 20/20 in both eyes and the subretinal tracks in the right eye had not changed since the previous examination. Serologic examination was performed to provide a more accurate diagnosis, and the patient's serum reacted strongly to the $Gnathostoma$ $nipponicum$ antigen by western blot assay, which led to a diagnosis of intraocular gnathostomiasis. This is the first reported case of intraocular gnathostomiasis with subretinal tracks confirmed serologically using western blot in Korea.

CLINICAL STUDY OF SURGICALLY ASSISTED RAPID MAXILLARY EXPANSION (외과적 술식을 동반한 급속 상악 확장술의 임상 연구)

  • Orthopedic rapid maxillary expansion(RME) has been a common treatment modality used to widen narrow maxillae in young children. However, since more skeletally matured adolescents or adults has closed midpalatal suture, the result of RME was undesirable because of dental tipping with little or no basal skeletal movement and resulted to many other complications. After such treatment, complications often occurred such as alveolar bending, compression of periodontal ligament, extrusion, buccal tipping, and severe relapse. Thus, surgically assisted rapid maxillary expansion(SA-RME) is required, especially for patients over 14 years old, to skeletally release maxillary expansion. We used two methods of maxillary expansion surgery. Surgically assisted rapid maxillary expansion(SA-RME) & surgically assisted posterior segmental expansion(SA-PSE) were used for narrow maxilla. The study was divided into two groups(SA-RME group and SA-PSE group). SA-RME group was consisted of 2 males and 4 females, and the ages of materials ranged from 15 years to 25 years with a mean of 20.2 years. SA-PSE group was consisted of 1 male and 5 females, and the ages ranged from 13 years to 23 years with a mean of 18.7 years. Dental study models were fabricated before starting the expansion and immediately after the expansion was completed. It was fabricated again 1 month later, 3 months later when the expansion device was removed, and 6 months later after the expansion was completed. A repeated measures analysis of variance(ANOVA) test was applied to assess changes between each groups over time. The amount of expansion and the amount of tipping movement each in both groups were compared by using paired t-test and it was also compared between each subjects within the group by using independent t-test. Both SA-RME and SA-PSE group showed stable results, but SA-PSE group showed statical significance in tipping movement of second premolar. We compared 6 patients who recieved SA-RME with 6 patients who received SA-PSE, and appraised the clinical usefulness.

Effect of Geographic Area on Dietary Quality across Different Age Groups in Korea (연령별 식사의 질에 미치는 지역의 영향)

  • Objectives: The objective of this study was to examine whether dietary quality varies among different age groups and geographic areas, and whether the difference between geographic areas varies across several age groups in Korea. Methods: The subjects were 14,170 subjects who participated in the 2013-2015 Korea National Health and Nutrition Examination Survey. The dietary quality was assessed using the Korean Health Eating Index (KHEI). Age groups were categorized into six groupings, and areas were categorized into urban and rural according to their administrative districts. The effect of area on the KHEI score was analyzed by multiple linear regression analysis. Results: The KHEI was the lowest in the 20-30s group (57.7 ± 0.4 score for 20s and 61.2 ± 0.3 score for 30s) and increased with age (p<0.001), showing the highest score in the 60s (67.9 ± 0.3 score), and then decreased again in the 70s and older (64.6 ± 0.3 score). As a result of comparing the KHEI score by area, the urban areas had higher KHEI scores than did the rural areas (63.5 ± 0.2 score for urban area and 62.2 ± 0.4 score for rural area, p=0.002). The difference between areas was dependent on the age group, showing a significant difference for subjects who were aged from 50s and older (p=0.002 for 50s, p<0.001 for 60s and p<0.001 for 70s and older). After adjusting for confounding factors, the effect of area on the KHEI score was only shown for those subjects in the over 60 years old group (p=0.035 for 60s and p<0.001 for 70s and older). Conclusions: The dietary quality differed according to the age group and geographic area. The dietary quality was lower for younger people than that for older people, and in rural areas compared to that in urban areas, and especially for older adults. The area factor was a very important factor for the dietary quality.

IgG4-related Ophthalmic Disease Associated with Adult Xanthogranulomatous Disease (황색육아종과 동반된 면역글로불린G4관련안질환 1예)

  • Purpose: To report a case of immunoglobulin G4 (IgG4)-related ophthalmic disease associated with adult xanthogranulomatous disease. Case summary: A 38-year-old male with a history of cholecystectomy visited our clinic for bilateral periorbital swelling. Histopathology of the orbital biopsy showed diffuse infiltration of foamy histiocytes with Touton giant cells and lymphoid follicles, with a diagnosis of adult-onset xanthogranuloma. After excisional biopsy, he was treated with azathioprine and prednisolone. Four years after treatment, he again visited the clinic due to bilateral, yellowish eyelid masses. Serological examinations were all nonspecific findings, except for elevation of IgG and IgG4 levels. Magnetic resonance imaging showed bilateral symmetric soft tissue enlargement with slightly heterogeneous T1/T2 isosignal intensity, with contrast enhancement at the superolateral aspect of extraconal spaces. Excisional biopsy and blepharoplasty were performed. Immunohistochemical sections showed that the IgG4+/IgG plasma cell ratio was 10-20% and the IgG4 plasma cell count was 22/high power field (HPF). His past sections of 2013 from the pathology department were again stained and showed that the IgG4+/IgG plasma cell ratio was 40-50% and the IgG4 plasma cell count was 59/HPF. Thus, he was definitely diagnosed with IgG4-related ophthalmic disease. Conclusions: If there is recurrent eyelid swelling, IgG4-related ophthalmic disease should be considered as a differential diagnosis. And the patient with adult xanthogranulomatous disease can be diagnosed with IgG4-related ophthalmic disease.

The Effects of Music Therapy on Vital Sign Changes of operational Patients (음악요법이 수술환자의 활력징후 변화에 미치는 영향)

  • The purpose of this study was to determine the effects of music therapy on changes in the vital signs of patients about to undergo an operation. The patients listened to the music at a time when they were feeling preoperative anxiety up until the preanesthesia was given in the operating room. The subjects for this study were selected from sixty patients to undergo operations, who were hospitalized at Dong Eui hospital in Pusan city. They were assigned to two groups, thirty to the experimental group and thirty to the control group. The subjects were from 20 years old to 69 years old, and had no other problem except the one requiring the operation, and no premedication. The data were collected during the period from July 1 to September 30, 1993. The method used in this study was to measure state-anxiety on the ward in the morning of the operation, and vital signs immediately before leaving for the operating room. Vital signs were measured immediately before the anesthesia was given and after the experimental group had listened to the music during the ten minutes needed to prepare the operation setting. The control group just waited during ten mimutes. Vital signs were check again before the anesthesia was given. The data were analyzed by descriptive statistics, mean±SD, p-value, and t-test using the SPSS progrom. The results of this study are : 1. Systolic blood pressure taken in the operating room was elevated, over the level measured on the ward, by 5.00 ± 15.26㎜Hg in experimental group and 18.67±14.56㎜Hg in control group. (t=-3.5496, p=.0008) 2. Diastolic blood pressure was elevated by 6.67±12.95㎜Hg in experimental group and 18.67±12. 79㎜Hg in control group. (t=-3.6100, p=.0006) 3. Pulse was elevated by 2.931±9.44 / min in experimental group and 8.03±8.37 /min in control group. (t=-2.2144, p=.0307) 4. Respiration was elevated by 0.60±1.35 /min in experimental group and 1.57±1.48 /min in control group. (t=-2.6409, p=.0106) 5. Body temperature was down by 0.13±1.91'c in experimental group and elevated by 1.13±1.11'c in control group. (t=-3.1471, p=.0026) Thus, in this study there was a statistically significant difference in the change in the vital signs between the experimental group treated with music therapy and the control group which received no treatment. Because music therapy is valuable to decrease the anxiety of patients facing operations, the result of this study support its effect in relieving anxiety as a valuable nursing intervention. From this study, the following recommendations can be made : First, it is necessary to further study music therapy to develope a better system and determine optimal time. Second, it is necessary that more detailed re-search on measurement of changes in vital signs be done to determine changes over time intervals.

The effect of pregnancy and parity on bone marrow density using Dual Energy X-ray Absorptiometry (DXA) after childbirth (두 차례의 출산 직후 골밀도 측정을 통한 임신 및 출산이 골밀도에 미치는 영향 고찰)

  • Purpose : This study was conducted to investigate the effect of pregnancy and parity on bone marrow density using Dual Energy X-ray Absorptiometry (DXA) after parturition Methods : The observation cases who was over 20 and under 35 years old just after childbirth were admitted twice to woosuk university hospital from Aug 2000 to July 2005. During the first admission period, we measured the bone marrow density(BMD) using DXA in 13th day, and when the patient came to the hospital just after childbirth again, we followed up the BMD in 13th day. The evaluation index of this report was comparison of the T-score which was about the lumbar spine(L1-L4) BMD and femur neck BMD. Results : The continuous parturition was increased the lumbar spine BMD(P<<0.05), and decreased the femur neck BMD but it had no meaning. In the cases of the parturition interval under 24 months regarded as siblings born within a year of each other, the analysis results of BMD showed increase in lumbar spine BMD. In the cases of the parturition interval over 24 months, there was increased in lumbar BMD, and decreased in femur neck BMD. To the analysis of the weight variation, the increased BMI group has a significant increase in lumbar spine BMD, and the decreased BMI group also increase in lumbar spine BMD but there was no meaning about that. Conclusion : The continuous parturition was increased the lumbar spine BMD.

Effects of Irrigation and Ginseng Root Residue on Root Rot Disease of 2-Years-Old Ginseng and Soil Microbial Community in the Continuous Cropping Soil of Ginseng (인삼 연작토양에서 관수 및 인삼뿌리 잔사물이 토양 미생물상 및 뿌리썩음병 발생에 미치는 영향)

  • Background: Some phenolics detected in the soil may inhibit the seed germination and seedling growth of ginseng (Panax ginseng). This study investigated the effect of irrigation and ginseng root residue addition on the soil microbial community and root rot disease in 2-year-old ginseng. Methods and Results: Each $20{\ell}$ pot was filled with soil infected with ginseng root rot pathogens, and irrigated daily with $2{\ell}$ of water for one month. After the irrigation treatment, ginseng fine root powder was mixed with the irrigated soil at a rate of 20 g per pot. In descending order, ${NO_3}^-$, electric conductivity (EC), exchangeable Na (Ex. Na) and K (Ex. K) decreased due to irrigation. In descending order, ${NO_3}^-$, EC, Ex. K, and available $P_2O_5$ increased with the additon of ginseng powder to the soil. The abundance of Trichoderma crassum decreased with irrigation, but increased again with the incorporation of ginseng powder. The abundance of Haematonectria haematococca increased with irrigation, but decreased with the incorporation of ginseng powder. The abundance of Cylindrocarpon spp. and Fusarium spp., which cause ginseng root rot, increased with the incorporation of ginseng powder. The abundance of Arthrobacter oryzae and Streptomyces lavendulae increased with irrigation. The abundance of Streptomyces lavendulae decreased, and that of Arthrobacter spp. increased, with the incorporation of ginseng powder. Aerial growth of ginseng was promoted by irrigation, and ginseng root rot increased with the incorporation of ginseng powder. Conclusions: Ginseng root residues in the soil affected soil nutrients and microorganisms, and promoted ginseng root rot, but did not affect the aerial growth of ginseng.

Estimation of Glomerular Filtration Rate from Plasma Creatinine and Height in Children (소아에서 신장과 혈장 Creatinine 농도를 이용한 사구체여관율 측정)

  • In clinical practice, creatinine clearance(Ccr) remains the most commonly used laboratory assessment of glomerular function despite methodological and technical problems of urine collection. Schwartz et al. in 1976, reported that an accurate estimate of glomerular filtration rate(GFR) could be obtained from the simple determinations of plasma creatinine(Pcr) and body length(L) : GFR($m{\ell}/min/1.73m^2$=k L(cm)/Pcr(mg/$100m{\ell}$), (k=constant). The subject of this study were ill children admitted to our pediatric department from July, 1985 to June, 1987 and they were divided into three groups; group I, from 1 to 5 years old, group II, from 6 to 10 years old, group III. from 11 to 15 years old. The results were as following ; 1) Measured creatinine clearance($Ccr_M$, $m{\ell}/min/1.73m^2$) were $109.73{\pm}9.97$ in group I, $108.26{\pm}9.02$ in group II, $96.20{\pm}4.72$ in group III and $105.48{\pm}5.23$ in all age group. 2) Measured k($k_M$) obtained from $Ccr_M=k$ Ht/Pcr were $0.49{\pm}0.03$ in group I, $0.48{\pm}0.02$ in group II, $0.43{\pm}0.02$ in group III, and $0.47{\pm}0.02$ in all age group.(Ht ; height) 3) Linear equations and correlation coefficients between Ht/Pcr(x) and Ccr(y) were y=0.822x-65.63(r=0.99) in group I, y=0.61x-23.46(r=0.72) in group II, y=0.18x+54.44(r=0.54) in group III and y=0.58x-22.13(r=0.81) in all age group. 4) $Ccr_E$ was again estiamted from linear equations between Ht/Pcr and $Ccr_M$ and $k_E$ was calculated with Ht/Pcr and $Ccr_E$ were $0.48{\pm}0.01$ in group I, $0.49{\pm}0.01$in group II, $0.43{\pm}0.01$ in group III and $0.47{\pm}0.00$ in all age group. 5) Consistant values of $k_E$ and $k_M$ were highly significant as 95~97.5% in group I and II, 90~95% in group III and 97.5~99% in all age group. In summary, we could estimate GFR with height, plasma creatinine and measured k($k_M$) according to the age in easy and rapid way.

Perceived Weight and Health Promoting Behavior - Normal and Overweight Adults - (정상체중과 과다체중 성인의 체중, 건강상태, 건강개념 지각과 건강증진 행위에 관한 연구)

  • The objective of this study was to clarify whether there are any differences between normal and over-weight adults in their perceived weight, health status, health conception and health promoting behavior. The sample consisted of 238 normal weight and 106 over-weight(11% above on the Body Index Scale) adults, more than 20 years-old, who live in Seoul metropolitan. One participant per household was selected for conveneience. The findings from this study are summarized below. 1) Among 106 overweight adults, 30 were above 20% on the Body Index Scale and 11 were above 30%. Twenty-one(19.8%) of the overweight group and 34(14.4%) of the normal weight group had one disease, and there were 30(28.3%) in the overweight group and 46(19.6%) in the normal weight group where one of the family members had a disease, but these differences were not statistically significant. The average monthly family income for the overweight group was \2,220,000 compared to \2,070,000 for the normal weight group, and this difference was statistically significant. The age range for the whole group was between 20 and 74(mean=35.6 for total, 39.4 for overweight and 34.0 for normal weight group). Again significant difference was found. Occupations were salaryman(57.6%), teacher(7.4%), student(5.4%) and others(27.3%). Fifty-six salaryman(70.0%) from the overweight group and 92(52.0%) from the normal group did not consitute a statistically significant different. For the educational status, 90(87.5%) of the overweight adults and 222(93.7%) of the normal weight group finished high school or more educational courses, and there was significant statistical difference. Ninety-two(86.8%) of the overweights and 156(65.5%) of the normal weight group were married, and again significant statistical difference was found. 2) A test for difference in health characteristics between the two weight groups indicated that two groups did not show statistical differences in their perceived health status, health conception or health promoting behavior. That is, the overweight group also perceive their health status as good as the normal group, and regard 'Health' as a state that enables them to carry out social roles and functions rather than as the traditional concept of health as no disease or no symptoms. Both group showed slightly high level of health promoting behavior. To determine if no statistical difference might be related to the overweight group's failure of perceive themselves as overweight, the perceived and objective overweight status were compared by the Pearson Correlation Analysis, and a strong corelationship was found(r=.76, p=.000). That is, if participants perceived themselves as overweighted, they thought and replied to be got more weight comparing to the other person who are in same age and sex. However, 43(18.1%) of the normal group perceived themselves as being overweight and 28(26.4%) of the overweight group thought they were of normal weight. Even though the overweight group employed in this study perceived themselves as being overweight, they regarded themselves as healthy as those in the normal weight group. It was shown that there was no statistical difference between the two groups in health conception, health status and health promoting behavior. 3) Perceived health conception was shown to be significantly related to health promoting behavior(r=.20, p=.004 for whole group ; r=.27, p=.009 for overweight group ; and r=.21, p=.001 for normal group). It means that in both group the higher perceived health conception level, the more frequent health promoting behavior. And, perceived health status was also shown to be significantly related to health promoting behavior(r=.16, p=.000) as a whole and especially for overweight group(r=.24, p=.018), but no significant relationship for normal group(r=.08, p=.620). 4) By means of multiple regression analysis, health conception, perceived health status, age, sex and marital status provided predicted 15.18% on health promoting behavior.

Isolation and Identification of Influenza Viruses from Busan, during 2000-2001 (2000-2001년 부산지역 호흡기 바이러스 발생 양상 비교)

  • Respiratory viruses were isolated from patients with acute respiratory infections in Busan during 2000-2001 and characterized for their antigenic properties. In 2000, 39 out of 43 isolated viruses were identified as influenza viruses and the others were adenoviruses. Among the isolated influenza viruses,23 were type A influenza viruses and 16 were type B influenza viruses. As a result of antigenic characterization, the influenza viruses were determined to A/Sydney/05/97(H3N2)-like, A/Beijing/262/95(H1N1)-like, and B/Harbin07/94-like viruses and serotypes of the isolated adenoviruses were type 1, 2, and 5. In 2001, 56 viruses were isolated and all of the viruses were identified as influenza viruses. They were A/panama/253/99(H3N2)-like and A/Newcaledonia/2007/99(H1Nl)-like viruses when determined by their antigenic properties. The sex distribution of the patients is as follows, 14 males (32.56%),23 females (67.44%) in 2000, and 23 males (41.07%), 33 females (58.93%) in 2001. Occurrence rate was found to be higher in female patients in both years. Age distribution of patients, in 2000, 48.84% of infection occurred in 0 to 1 year old while in 2002, 33.93% occurred among 11-20 year olds. In 2000, occurrence rate was found to be high in January and again in April and various types of viruses were isolated. These results may be useful for vaccine development and establishment of reliable epidemic data.
