본 연구의 목적은 119구급대원을 대상으로 COVID-19에 대한 지식 및 교육 요구도를 확인하여 신종감염병 대비 시뮬레이션 교육 프로그램 개발에 기초자료를 제공하고자 시도되었다. 186명의 119구급대원을 대상으로 2020년 11월 15일~11월 30일까지 구조화된 설문지를 통해 자료를 수집하였다. 연구 결과 COVID-19 감염관리 교육을 받지 않은 경우가 98명(52.7%)이었고 COVID-19감염관리 지식은 25점 만점에 18.21±1.98점이었으며 환경관리 영역에 대한 정답률이 가장 낮았다. COVID-19 감염관리 교육 요구도는 총 평균은 3.81±.28(4점 척도)점으로 나타났으며 전파경로 및 확산 예방과 직원 안전관리 영역에 대한 교육요구도가 가장 높았다. 일반적 특성에 따른 지식은 성별(t=-1.999, p=.047)에 따라 통계적으로 유의한 차이가 있었으며 교육 요구도는 경력(t=-1.999, p=.047), 교육수준(t=2.336, p=.021)에 따라 유의한 차이가 있었다. 이에 따라 119구급대원을 대상으로 COVID 감염관리 교육 프로그램 개발 시전파경로 및 확산 예방, 직원안전, 환경관리를 포함하는 것이 필요하다.
Purpose: This is a descriptive research study that aims to identify factors affecting fatigue by examining the degree of emotional labor, job stress, and fatigue of 119 emergency medical technicians (119 EMTs). The correlation between emotional labor, job stress, and fatigue was also confirmed. Methods: A total of 139 EMTs working in the Daejeon area participated in this study. Data were collected from July 6-15, 2021. Results: When the number of dispatches was high and the job was dissatisfying, the degree of emotional labor was high. In addition, the shorter the work experience and the lower the job satisfaction are, the higher the job stress is. The degree of fatigue was significantly higher in female than male 119 EMTs, and when they had short work experience and were dissatisfied with their job, the degree of fatigue was higher. Moreover, there was a significant correlation between emotional labor, job stress, and fatigue of the 119 EMTs. Conclusion: To reduce emotional labor, job stress, and fatigue of 119 EMTs, appropriate interventions for each factor and institutional improvements are required.
이 연구는 수난 구조 활동에서의 급상승이 활성산소 증가에 미치는 영향을 분석한 것이다. 119구조대원을 상대로 급상승에 따른 활성산소의 변화율을 실험한 연구는 없는 실정으로, 구조대원 인체에 나타나는 증상을 확인하고자 한다. 실험대상자는 5명으로 B, C, E는 잠수 전·후 활성산소 상승 수치가 각각 0.41µmol/L, 0.11µmol/L, 0.87µmol/L로 비슷한 수치를 나타내었다. 그런데, 실험대상자 D는 잠수 전·후 활성산소 상승 수치가 1.41µmol/L로 유의하게 높게 나타났는데, 수중 시계의 불량에서 오는 불안감 증가와 수중구조 활동 후에 급상승 시 피로도 상승에서 기인한 것으로 보인다. 실험대상자 A는 잠수 전·후 활성산소 상승 수치가 0.07 µmol/L로 유의하게 낮게 나타났다. 그 이유는A의 연령이 54세로 실험대상자 중 가장 잠수 경험이 많고, 119구조대원으로서의 풍부한 구조 활동 경험과 능숙한 수중활동이 불량한 시계에서 스트레스를 적게 받은 결과로 보인다. 피로도와 불안감은 4로 둘 다 높은 수치를 나타내었다. 수중활동 시의 심리적 긴장도가 피로도를 높였고, 수중 시야의 혼탁함이 불안감을 올린 것으로 사료된다.
Purpose: This study was conducted to investigate the disaster response competencies of 119 emergency medical technicians (EMTs). Methods: The subjects of this study were 226 119 EMT's at the fire station located in G province. The questionnaire consisted of 5 sub-factors and 29 items for measuring disaster response competencies and the collected data were analyzed using SPSS program. Results: Disaster response competency comprising immediate response, patient triage, patient treatment, patient transport, and collaboration support were 3.48, 3.64, 3.52, 3.64, and 3.16, respectively. Advanced EMTs scored significantly higher in patient triage (p<.001), treatment (p<.001), and transport (p=.022) competency than basic EMTs. Conclusion: In order to improve disaster response competency, it is necessary not only to enhance individual ability but also to continue disaster management training with related organizations.
Purpose : The goal of the present study is to provide the basic information to medical control which is the most important improving factor of pre-hospital medical treatment. Method : A total of 749 records of direct medical control were collected from 119 EMTs in emergency medical information center of Daejeon, Chungcheongnam-do and Chungcheongbuk-do from March 1, 2010 to February 28, 2011. Results : The 119 EMTs should record the level of qualification of EMT and general patient history taking precisely when they receive direct medical controls. The doctors should take medical controls within the task range of qualification of EMTs. Conclusion : It is necessary to establish the guideline of medical direction and protocol of prehospital emergency care. The quality improvement of pre-hospital emergency services will be possible by the guideline and protocol.
Purpose: This study aimed to provide baseline data for implementing advanced life-support for 119EMTs. This is done with an in-depth examination of the emergency care experiences of the 119EMTs on-site and during transportation. Methods: Interviews were conducted with twelve 119EMTs, and the data were analyzed using the phenomenological method. Results: The 34 topics had 19 sub-components and were analyzed according to seven categories: 'A work environment where you cannot focus only on emergency care on-site and during transportation', '119EMTs struggling with manpower shortage', 'Current status of education blocking the upward standardization of 119EMT', 'Implementation of professional emergency care was held back due to the limited legal scope of 119EMT', 'Medical oversight makes it difficult to implement emergency care due to low flexibility', '119EMTs who cannot choose hospital selection and find it difficult to receive help', 'An evaluation system not reflecting reality at all that only leaves high scores'. Conclusion: This study was undertaken to implement specialized emergency care for 119EMTs; not only the necessity of reviewing the expansion of the work scope of 119EMTs but also the necessity of effective 'Medical Oversight system' reorganization raised as well.
Purpose: This study attempts to improve the status of emergency care for major trauma patients transferred by 119 paramedics by analyzing the status of emergency care and the obstacles to the spinal motion restriction (SMR) for major trauma patients. Methods: A total of 600 rescue logs were collected from major trauma patients transported by 119 paramedics in the C fire department from Jan. 1, 2015, to Dec. 31, 2017. And then, 280 questionnaires were collected from the 119 paramedics in C fire department from May 3 to Jun. 3, 2019. Data were analyzed using SPSS 24.0 version. Results: Among 499 spinal motion restriction adaptive patients, the spinal motion restriction rate was 51.1% (255 individuals). Lack of human resources and quality control problems were among the obstacles to spinal motion restriction. Conclusion: The 119 paramedics should improve their activeness and skills in performing emergency care, and since training and experience are of crucial importance, they should expand various education systematized according to demand.
본 연구에서는 우리나라의 응급의료 이송정책을 구성하고 있는 병원 전 단계인 응급환자 발생에서 현장의 응급처치 단계, 환자의 이송단계, 통신망 구축단계 등의 상호 유기적인 협조체계의 기틀 속에서 119구급대원을 대상으로 직무와 관련된 일반적인 내용을 중심으로 설문조사를 실시하여 분석한 결과, 발생된 문제점으로는 구급대원들이 의료분쟁에 휘말리지 않도록 제도적 장치를 마련하여야하고, 그리고 응급 구호장비의 현대화, 구급전문 요원의 확보, 적절한 인력배치가 이루어져야 되는 것으로 조사되었다. 따라서 응급의료 이송체계를 현실적 환경에 적합한 모형으로 개선하기 위하여 구조와 기능적인 측면에 초점을 맞추어 그 정책 방안을 제안하고자 한다.
본 연구는 여성119구급대원이 증가하고 있는 추세에 따라 소방 조직 내에서 여성119구급대원의 이미지를 향상시키고 그들을 효과적으로 활용할 수 있는 기초자료로 활용하고자 시행된 연구이다. 연구는 2013년 5월 1일부터 6월 30일까지 Q표본을 선정하여 구급차동승 현장실습을 경험한 응급구조(학)과 학생 총 31명을 대상으로 QUANL program을 이용한 Q방법론을 사용하여 분석하였다. 연구결과 '슈퍼우먼형', '체력한계 히어로형', '섬세한 수호천사형', '능숙한 데스크잡형'의 총 4가지 유형으로 분류되었으며 각각의 설명력은 제I유형 42.2%, 제II유형 4.8%, 제III유형 4%, 제IV유형은 3.6%로 전체변량의 54.7%를 설명하는 것으로 나타났다. 전체적으로 여성119구급대원을 전문가로 인식하고 있지만 체력적인면과 출산 및 육아에 대하여 부담을 가지고 있는 것으로 나타나 조직 구성원의 인식 변화와 제도적인 장치가 마련되어야 할 것이다.
간호사는 의료법 상 의료인으로서 의료기관에서 진료의 보조 등의 업무를 수행한다. 의료인인 간호사는 119 구조·구급에 관한 법률에 근거 구급대원으로서 병원 전 단계 응급의료체계에서 응급환자를 대상으로 응급의료를 제공한다. 병원 전 응급의료체계에서 업무를 수행하는 간호사의 업무범위는 의료법 상 포괄적으로 규정되어 있는 업무를 판례 등을 통해 구체화되어 왔다. 반대로 응급구조사의 업무범위는 응급의료법에 의해 구체적 행위를 열거하는 방식으로 규정되어 있다. 병원 전 단계 응급의료체계에서 응급의료종사자로서 업무를 수행하는 간호사의 업무범위는 대상 환자의 긴급성, 응급의료기관으로의 이송 중이라는 점, 통신 상의 의료지도라는 특수성을 고려하여 간호사의 업무범위를 의료기관 내에서와 달리 적용하여 해석되어야 한다. 따라서 상대적으로 병원 전단계에서 간호사가 응급구조사보다 넓은 업무범위가 인정되고 이것이 정책목표에도 부합된다.
