Influence of Professional Self-concept, Ego-resilience, and Job Stress on Job Performance of Nurses who are Reemployed after Career Interruption (경력단절 후 재취업한 간호사의 전문직 자아개념, 자아탄력성, 직무 스트레스가 업무수행능력에 미치는 영향)

  • Purpose: This study was conducted to investigate the influence of professional self-concept, ego-resilience, and job stress on job performance in reemployed nurses after career interruption. Methods: The participants were 207 nurses who worked at hospitals with more than 100 beds located in Seoul, Gyeonggi and Gangwon provinces. The nurses had experienced a career interruption of at least one year, and had less than five years of experience at the current workplace. The collected data were analyzed utilizing SPSS 24.0 program and using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The participants' job performance, professional self-concepts, ego-resilience, and job stress showed significant positive correlations. It was found that the professional self-concept, ego-resilience, and job stress had an effect on the participant's job performance in order, and explained 51.3% of variance in job performance. Conclusion: To improve the job performance of career disconnected nurses, it is necessary to reinforce job stress control, professional self-concept, and level of ego-resilience.

The Relationship among Social Support, Work-Family Conflict and Work-Family Enrichment of Married Nurses (기혼간호사의 사회적 지지와 일-가정 갈등, 일-가정 향상과의 관계)

  • Purpose: This study is to identify the relationship among married nurses' social support, work-family conflict and work-family enrichment and to examine the influence of social support on work-family conflict and work-family enrichment. Methods: Data were collected from married nurses working at three hospitals with more than 100 beds in J Province and analyzed by descriptive statistics, t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis H test, Pearson's correlation analysis and hierarchical regression analysis using SPSS 21 program. Findings: In the correlation analysis only social support and work-family enrichment showed positive correlation. As a result of hierarchical multiple regression analysis, social support still acted as a significant influence factor on work-family conflict and work-family enrichment even in the state of considering the effect of control variables. Conclusion: In order to lower work-family conflict and to enhance work-family enrichment of married nurses, it is necessary to introduce and implement welfare policies for work-family reconciliation at the workplace level and at home level it is necessary to provide concrete measures so that married nurses can find the value of life as workers while house working and nurturing.

Validity and Reliability Analysis of Korean Version of Hospital Safety Organization Scale (한국어판 병원 안전조직화 척도의 타당도와 신뢰도 분석)

  • In this study, we explored the psychometric properties of the Korean version of the Safety Organization Scale (K-SOS). The SOS is based on the principles of a high-reliability organization and is an effective measure of the self-reported behaviors underlying the safety culture of an organization. We conducted a methodological study to examine the validity and reliability of the 9-item SOS translated into Korean. The data of 108 nurses working in three general hospitals with more than 100 beds located in D City were included. Content validity, construct validity, criterion validity, and reliability were evaluated. The results confirmed that the K-SOS is a unidimensional structure scale. Higher K-SOS scores exhibited a significant correlation with Hospital Survey On Patient Safety Culture, thus indicating the validity of the criterion. The internal consistency of the K-SOS was acceptable. Thus, the K-SOS is a reliable self-report scale for directly measuring the safety behaviors of an organization. The K-SOS can be used to improve patient safety in Korean hospital settings.

Influences of Coping Strategies and Vocational Self-Efficacy on Burnout among Social Workers in Long-Term Care Hospitals (노인요양병원 사회복지사의 대처전략과 직무효능감이 직무소진에 미치는 영향)

  • The purpose of this study is to examine the influences of coping strategies and vocational self-efficacy on burnout among social workers in long-term care hospitals. Using a convenience sample, researchers collected responses from 138 participants in those long-term care hospitals that have at least 100 sick beds among 528 hospitals(as of September 2015) registered as members of the Korean Association of Geriatric Hospitals. Multiple regression analyses revealed that working hours, externalizing strategies, vocational self-efficacy and group efficacy were significantly correlated with burnout of social workers. In other words, longer working hours led to higher degrees of burnout and higher levels of externalizing strategies, job efficacy, and group efficacy led to lower degrees of burnout in job performance. Based on the study results, it was discussed to relieve the extent of burnout among social workers in long-term care hospitals such as reinforcement of systematic supervision, development of burnout prevention program and on-the job training.

Influences of Antibiotic Components Produced by Trichoderma spp. to Oyster Mushroom (Trichoderma속(屬)이 생산(生産)하는 항생물질(抗生物質)이 느타리버섯균(菌)에 미치는 영향(影響))

  • Isolates of the different species groups of Trichoderma from the mushroom culture beds were identified according to Rifai's classification and influence of antibiotics produced by them against the oyster mushroom was examined. Trichoderma islolates were identified as Trichoderma hamatum, Trichoderma viride and Trichoderma koningii. Among the Trichoderma isolates, fungistatic action of Trichoderma viride was found to be most remarkable. Pleurotus ostreatus and Pleurotus sajor-caju were the most susceptable of the edible mushrooms tested, followed by Lentinus edodes, Flammulina velutipes and Auricularia auricula. A needle-shaped crystal gained from the chloroform extract of the culture filtrate of Trichoderma viride repressed distinctively the mycelial growth of the oyster mushroom. The grade of repression of the crystal at 500ppm and 1/10 aequ­ous solution of the chloroform extract against the oyster mushroom, seemed equal to that of cycloheximide at $100{\sim}200ppm$.

Variations and Factors Associated with the Supply and Utilization of Nursing Home Services in Japan and South Korea (한국과 일본 장기요양시설 공급과 이용의 지역 간 변이)

  • Background: Few studies have examined the performance of the public long-term care insurance (LTCI) from the perspective of geographic equity. This study investigated regional variations and associated factors in the supply and utilization of nursing home care within and also between Japan and Korea. Methods: A comparative dataset was developed by extracting data from 2013-2015 LTCI statistics yearbooks and Organization for Economic Cooperation and Development regional statistics, as well as other comparable data in Japan and Korea. The unit of analysis was the prefecture in Japan and the province in Korea. We computed variation indices and conducted regression analyses for regional variations within each country and decomposition analyses to examine the variations between the countries. Results: The overall regional supply and use of nursing home care were higher in Japan, but the regional variations in Korea were larger than in Japan. In both countries, the nursing home supply was negatively associated with the proportion of older people with independent living. Nursing home use was also negatively associated with the supply of hospital beds and home care agencies in Korea; the relationship was the opposite in Japan, however. The country-based differences were more likely to be explained by differences in the distributions of the variables included in the analytical model than country-specific characteristics. Conclusion: Regional-level nursing home supply and use were unequal in both countries, and the contributing factors were not the same. Policy efforts are needed to advance regional equality in long-term care (LTC) and collaboration between health and LTC institutions for frail older people, especially in Korea.

External Validation of Carbapenem-Resistant Enterobacteriaceae Acquisition Risk Prediction Model in a Medium Sized Hospital (중규모 종합병원 대상 카바페넴 내성 장내세균속균종(Carbapenem-resistant Enterobacteriaceae) 획득위험 예측모형의 외적타당도 평가)

  • Purpose: This study was aimed to evaluate the external validity of a carbapenem-resistant Enterobacteriaceae (CRE) acquisition risk prediction model (the CREP-model) in a medium-sized hospital. Methods: This retrospective cohort study included 613 patients (CRE group: 69, no-CRE group: 544) admitted to the intensive care units of a 453-beds secondary referral general hospital from March 1, 2017 to September 30, 2019 in South Korea. The performance of the CREP-model was analyzed with calibration, discrimination, and clinical usefulness. Results: The results showed that those higher in age had lower presence of multidrug resistant organisms (MDROs), cephalosporin use ≥ 15 days, Acute Physiology and Chronic Health Evaluation II (APACHE II) score ≥ 21 points, and lower CRE acquisition rates than those of CREP-model development subjects. The calibration-in-the-large was 0.12 (95% CI: - 0.16~0.39), while the calibration slope was 0.87 (95% CI: 0.63~1.12), and the concordance statistic was .71 (95% CI: .63~.78). At the predicted risk of .10, the sensitivity, specificity, and correct classification rates were 43.5%, 84.2%, and 79.6%, respectively. The net true positive according to the CREP-model were 3 per 100 subjects. After adjusting the predictors' cutting points, the concordance statistic increased to .84 (95% CI: .79~.89), and the sensitivity and net true positive was improved to 75.4%. and 6 per 100 subjects, respectively. Conclusion: The CREP-model's discrimination and clinical usefulness are low in a medium sized general hospital but are improved after adjusting for the predictors. Therefore, we suggest that institutions should only use the CREP-model after assessing the distribution of the predictors and adjusting their cutting points.

Thermal and light impacts on the early growth stages of the kelp Saccharina angustissima (Laminariales, Phaeophyceae)

  • Anthropogenic disturbances, including coastal habitat modification and climate change are threatening the stability of kelp beds, one of the most diverse and productive marine ecosystems. To test the effect of temperature and irradiance on the microscopic gametophyte and juvenile sporophyte stages of the rare kelp, Saccharina angustissima, from Casco Bay, Maine, USA, we carried out two sets of experiments using a temperature gradient table. The first set of experiments combined temperatures between $7-18^{\circ}C$ with irradiance at 20, 40, and $80{\mu}mol\;photons\;m^{-2}\;s^{-1}$. The second set combined temperatures of $3-13^{\circ}C$ with irradiance of 10, 100, and $200{\mu}mol\;photons\;m^{-2}\;s^{-1}$. Over two separate 4-week trials, in 2014 and again in 2015, we monitored gametogenesis, the early growth stages of the gametophytes, and early sporophyte development of this kelp. Gametophytes grew best at temperatures of $8-13^{\circ}C$ at the lowest irradiance of $10-{\mu}mol\;photons\;m^{-2}\;s^{-1}$. Light had a significant effect on both male and female gametophyte growth only at the higher temperatures. Temperatures of $8-15^{\circ}C$ and irradiance levels of $10-100{\mu}mol\;photons\;m^{-2}\;s^{-1}$ were conditions for the highest sporophyte growth. Sporophyte and male gametophyte growth was reduced at both temperature extremes-the hottest and coldest temperatures tested. S. angustissima is a unique kelp species known only from a very narrow geographic region along the coast of Maine, USA. The coupling of global warming with high light intensity effects might pose stress on the early life-history stages of this kelp, although, as an intertidal species, it could also be better adapted to temperature and light extremes than its subtidal counterpart, Saccharina latissima.

The factors affecting Turnover Intention of General Hospital Nurses (종합병원 간호사의 이직의도에 영향을 미치는 요인)

  • This study was to identify the influence of organizational commitment, nursing work environment, emotional labor on turnover intention of general hospital nurses. The participants were 129 nurses in general hospitals with 100~300 beds in G metropolitan city. Data was collected with questionnaire, during Aug, 1 to 20, 2020. The result of this study, organizational commitment 3.19, nursing work environment was 2.75, emotional labor was 3.19, turnover intention was 3.16. The factors affecting turnover intention are organizational commitment and emotional labor, these variables explained 32.0% of the variance. The results of this study, it can be used as basic data for developing intervention programs to reduce turnover intention of general hospital nurses.

Applicability of Appropriateness Evaluation Protocol and Delay Tool (적절성 평가지침과 이유목록의 적용 가능성 평가)

  • Background: An appropriate use of hospital beds can improve productivity of hospital significantly. The authors' previous study revealed that approximately one third of Korean hospital bed days and one sixth of admissions were inappropriately used, when it was measured by Appropriateness Evaluation Protocol(AEP) and Delay Tool modified into Korean situation by the authors. This study aims to evaluate applicability of the instruments in a new hospital. More specifically the study aims to measure appropriateness of the instruments used by newly trained nurse reviewers at a new hospital setting. Methods: In order to evaluate applicability of these instruments, agreement rates of the scores recorded by newly trained nurse reviewers with by skilled nurse reviewer and also compared with the scores recorded by physician's implicit decision were assessed. Agreement rates were derived from concurrent application of AEP and Delay Tool to 52 admissions and 104 patient days from internal medicine, pediatrics, and general surgery of one university hospital. Overall agreement rate, specific nonacute agreement rate, and kappa statistics were used to indicate level of agreement. Results: Overall agreement rates on appropriateness between newly trained nurse reviewers and skilled nurse reviewer were 100% in admission and 98% in bed days. Overall agreement rates on reason for inappropriateness between newly trained nurse reviewers and skilled nurse reviewer were 96% in admission and 91% in bed days. Overall agreement rates between newly trained nurse reviewers and physician reviewer were 86% in admission and 87% in bed days. Conclusion: Results indicated that AEP and Delay Tool were applicable to a new hospital in detecting inappropriate utilization of beds and reasoning of the inappropriateness. These instruments could contribute to enhance efficiency of hospital use, through continuous monitoring of level of inappropriate hospital use at national or individual hospital level.
