A Study on the IADL Affecting Subjective Health Index of the Aged in Some Area (일부지역(一部地域) 노인(老人)들의 주체적(主體的) 건강수준(健康水準)에 영향을 미치는 IADL에 관한 연구(硏究))

  • This research have been made to define how the IADL (Instrumental Activity of Daily Living) performance and the subjective health index of the Aged are affected by their residential circumstance, gender and age, and how deeply these two factors are related and interact. For the period of June 1 to July 31, 2000, we had conducted a questionnaire and direct interview with 693 persons over age sixty-five (65) in Daejon and in other adjacent area, grouping into three different residential types The Aged living at home, The Aged living at welfare facilities and The Aged living alone, and studied on how the IADL performance and the subjective health index of the Aged are influenced and interact as per their characteristics, daily activity and mentality. We had analyzed all the data obtained through this research by the method of : - analysis of frequency as per specific factors by SPSS 1O.0/PC+, - $x^2$ test, - t-test, - ANOVA, - multiple regression analysis by factors. The research concludes followings : a. It appears that the three (3) factors such as gender, age and residential circumstance of the Aged deeply affect the IADL performance and subjective health index of the Aged. (p<0.01) b. With regard to IADL performance of the Aged by the gender, it was analyzed that the female-Aged gains 23.8 point on average, which shows the performance of the female-Aged is less independent. (p<0.01). In addition, it was also found that the IADL performance is becoming less and less independent following their age increasing. In analyzing IADL performance by the residential type, it appears that the Aged living at welfare facilities gains the lowest 21.5 points and is least independent. It was also found that the Aged living at welfare facilities need some assistances from others for their performing IADL. (20-24 point) (p<0.001) c. With regard to the subjective health index of the all-Aged participated in this research, the analysis indicates 8.8 point and this is considered as a point of general standard (7-10 point). In analyzing this index by the gender, the female-Aged gains only 8.6 point which explains a lot of female-Aged consider they are not really healthy. (p<0.001) In analyzing this index by the residential type, the Aged living at welfare facilities and the Aged living alone gain the comparatively lower point, - respectively 8.4 point for the Aged living at welfare facilities and 8.8 point for the Aged living alone. The Aged living at these two residential types express they are obviously in a bad situation of health. (p<0.001) d. With regard to the factors affecting the IADL performance and the subjective health index of the Aged, it was analyzed that the IADL performance can largely be affected by the factors such as depression, frequency of going-out and age rising, and that the subject health index can also deeply be affected by depression, pain and by how much they are satisfied with their current living conditions. e. It was analyzed that the interacting between the IADL performance and the subjective health index is not that strong but even weak. As a result, we were able to conclude that the IADL performance is less independent in case of the female-Aged, the Aged living at welfare facilities, and following the age rising. As for the subjective health state the Aged themselves are aware of, we concluded that the female-Aged, the Aged living at welfare facilities and the Aged living alone, are more critical about their health. From this research, we were able to realize that, when the OMT (Orthopedic Manual Therapy) needed, the physical therapists are really required to have a correct and cautious understanding of the situation in which the aged persons are, and take care of them with more concerns and more improved treatment.

Chromium Distribution in Korean Soils: A Review (우리나라 토양의 크롬 분포특성에 관한 고찰)

  • Chromium as a constituent of rocks occurs naturally in the environment in varying concentrations. However, the human activity has changed the geochemical cycle of chromium in the environment and has caused the chromium accumulation in soils. Korean soils revealed a wide range of chromium contents depending on parent material and land use. The total chromium contents of volcanic ash soils in Jeju, which were determined using $HNO_3$ + $HClO_4$ + HF, ranged from 434 to 1,164 mg $kg^{-1}$. The 'ecological' total chromium contents extracted using conc. HCl + conc. $HNO_3$ (aqua regia) in the same soils varied in a lower range of 50-189 mg $kg^{-1}$ (averaged percentage of aqua regia contents in $HNO_3$ + $HClO_4$ + HF contents: 14.9%). Serpentine soils in Andong showed a 'ecological' total chromium content of 309 mg $kg^{-1}$ and against it granitic soils in Andong only 20 mg $kg^{-1}$. In uncontaminated forest soils of Korea, the 'ecological' total chromium contents varied from 4.89 to 106 mg $kg^{-1}$ and the soluble chromium contents determined using 0.1 M HCl ranged from 0.01 to 0.64 mg $kg^{-1}$ (averaged percentage of 0.1 M HCl contents in aqua regia contents: 0.4%). Arable lands contained more soluble chromium than reported in forest soils (averaged soluble chromium: 0.36 and 0.09 mg $kg^{-1}$, respectively). In particular, the soluble chromium contents in greenhouse, orchard and upland soils were higher than in contaminated soils near mine and industrial site (maximum contents: greenhouse 15.3 mg $kg^{-1}$; upland 12.1 mg $kg^{-1}$; orchard 8.29 mg $kg^{-1}$; mine site 4.76 mg $kg^{-1}$; industrial site 2.80 mg $kg^{-1}$). On the basis of these results a accumulation of chromium in some specific arable lands can be assumed, probably by long-continued applications of fertilizers or soil amendments containing chromium. In Korean Enforcement Decree of the Soil Environment Conservation Act soil standards for total chromium do not exist yet.

Effect of Chlorella vulgaris on Immune-enhancement and Cytokine Production in vivo and in vitro

  • The object of this study was to investigate the immune-enhancing effects of Chlorella vulgaris (CV) on a deteriorated immune function by a protein-energy malnutrition (PEM) diet. Unicellular algae, CV were used as a biological response modifier. Male C57BL/6J mice were fed for 15 days with standard diet or a PEM diet, which is associated with decreased host immune defense. After 8 days, mice in the PEM diet group were orally administered by 0.05, 0.1, and 0.15 g/kg body weight of CV or distilled water. Nutritional parameters, and interferon (IFN)-$\gamma$ levels were significantly increased in the blood serum of the CV (0.15 g/kg)-treated group (29.6$\pm$2.8 pg/mL) compared to the non-treated PEM group (4.1$\pm$0.4 pg/mL, p<0.05). In addition, cell proliferation and production of cytokines were investigated via a CV (0.01, 0.1, and 1 mg/mL) treatment using a human T cell line MOLT-4 cell. The CV treatment (1 mg/mL) significantly increased the production of both IFN-$\gamma$ and interleukin (IL)-2 (51.3$\pm$3.4 and 285.9$\pm$18.8 pg/mL, respectively) compared to the control (51.3$\pm$3.4 and 442.6$\pm$14.3 pg/mL, respectively), but did not affect the production of IL-4. These results suggest that CV may be useful in improving the immune function.

A Study on the Gas Chromatographic Analysis of Alcohols and Organic Acids during Takju Fermentation (가스 크로마토그래피에 의한 재래주 발효중 알코올과 유기산 분석)

  • Takju, a Korean traditional wine, was prepared by using nuruk and Koji which were inoculated with Aspergillus kawachii and Aspergillus shirousamii. Those chemical composition such as alcohols and organic acids were determined with gas chromatography to investigate the variation of its content by the fermentation. Alcohol such as methyl, ethyl, n-propyl, i-butyl, i-amyl alcohol were detected in the most takju mash samples. Alcohol contents in the groups fermented with KNR and SNR were slightly higher than KKR, SKR groups. Lactic acid were concentrated and organic acids such as pyruvic, oxalic, malonic, succinic, maleic, malic, ${\alpha}-keto$ glutaric acid were also detected in the most samples. The pH was lowest in the KKR group. The total acid content was slightly decreased at the later fermentation and was highest in KKR. The content of minerals were highest in the WNR and BNR groups. Throughout fermentation the content of potassium and magnesium varied greatly with the tested groups.

An Analysis of Teachers' Self-evaluation on Health Teaching Behaviors in Elementary School (초등 학교 교사의 보건수업 행동 평가 분석)

  • The purpose of this study was to analyze the results of teachers' self-evaluation on their health teaching behaviors, then to furnish the basic data to be able to improve teachers' health teaching activities and the educational issues on the education of teachers. To put above aims into practice, these were required: 1. Are there any differences in the results of self-evaluation on health teaching behaviors factors by teachers? 2. Are there any differences in the results of health teaching self-evaluation whether he/she take P.E as major of study in-serviece training period? 3. Does it have any influence on the results of health teaching self-evaluation whether he or she completed on the job training for the school health? 4. Are there any differences in the results of health teaching self-evaluation by sex and career? To carry out a research for this purpose, the factors of health teaching self-evaluation were divided into the clearness of the procedure, the active interaction, the variety of the ways showing the contents, and the individualization of the procedure. Then a questionnaire form, consisting of 28 specific inquires to evaluate health teaching behaviors, was delivered and conducted by 450 teacher of the elementary school in Kyungki-do. The analysis of data was done by SPSS; producing mean and standard deviation and they were inspected statistically to compare the evaluation levels and find out the differences by teachers' personal variables. The conclusion were as follows: 1. In the self-evaluation level of teachers' health teaching behaviors, teachers showed 68.23 point as are percentile distribution. And it was in order of a school-nurse(71.68), an athletic teacher(67.29), and a class-room teacher (65.66). Score obtained by teacher was statistically significant difference (p〈.001) 2. In the factors affecting to teachers' health instruction, “active interaction” showed the highest score(18.55), “variety of ways showing the contents”(17.38), “clearness of the procedure” (16.70), and “individualization of the procedure” (15.59). In the analysis of the differences by teachers, according to factors, there were significant differences in “active interaction”, “variety of the ways showing contents”, “clearness of the procedure”(p〈.001). 3. Self-evaluation score for graduates from Dept. of P. E in Teachers' collage was not significant difference compared with other majors(p〉.05). 4. Teachers receiving health education was significantly higher self-evaluation score than that of teachers not-receiving health education (P〈.01). 5. Self-evaluation score of female teacher was significant difference compared with that of male teacher (p〈.001). 6. Career (working duration) did not influenced to self-evaluation score on health teaching behaviors (P〉 .05). On the basis of the conclusion of this study, the next are suggested: First, the further studies to make use of the results of health teaching behaviors and to examine the effect are needed. Second, the further studies to examine the relations between academic achievement and teachers' major(a school-nurse, an athletic teacher, and a class-room teacher) are needed. Third, the following studies to improve health teaching by both teachers' self-evaluation on health teaching behaviors and students' evaluation of teachers, and to find out more effective health teaching, are needed. Fourth, Health education for pre-service training course and On-the-Job training program are need the effective factors on the teachers' Health teaching obtained from this study.

  • This investigation studied the relationship between Body Mass Index(BMI) and dietary intake, levels of serum lipid, lipoprotein(a) and plasminogen activator inhibitor-1(PAI-1) of 28.449 Korean adults(16.937 men, 11.542 women) from 1995 to 1999. The dietary assessment was conducted using 24-hour dietary records and food frequency questionnaires. During this five year study, the BMI normal-weight group, as a percentage of the annual test population, decreased from 68.0% to 60.2%, while the BMI overweight and obese groups collectively increased from 25.0% to 29.7%. The levels of serum total cholesterol (TC), LDL-cholesterol(LDL-C), Triacylglycerol(TG) and PAI-1 increased as the values of BMI increased, while the levels of HDL-cholesterol(HDL-C) and lipoprotein(a)(men only) appeared to decrease as values of BMI increased. The levels of daily energy intake also increased as values of BMI increased in both men and women. The obese group had significantly higher levels of carbohydrate, protein, fat(men only), and cholesterol intake than those of the normal-weight, underweight, and overweight groups. In men, BMI positively correlated with the levels of macronutrients and cholesterol intake(p<0.001), % energy of protein, fat, and alcohol intake(p<0.001), and the levels of serum TC, LDL-C, TG and PAI-1(p<0.001), while BMI negatively correlated with % energy of carbohydrate intake, and the levels of Lp(a) and HDL-C(p<0.001). In women, BMI negatively correlated with level of cholesterol(p<0.01), fat(p<0.001), alcohol intake(p<0.05),% energy of fat (p<0.001), % energy of alcohol intake and level of and HDL-C(p<0.001). Subjects who had serum TC, LDL-C, HDL-C, and TG levels greater than the standard reference values(TC>240mg/㎗, LDL-C>130mg/㎗, HDL-C>35, TG>200mg/㎗) exhibited a higher intake of the three macronutrients, iron, calcium, meat, milk and fatty foods than those subjects who had serum lipid concentrations less-than-or-equal-to the standard reference values. Overall, there was positive correlation between the high risk factors of vascular disease variables, dietary intake, and BMI. Prevalence of hypertension and high blood sugar were increased as BMI increased, but the prevalence of hypertension is decreased as the year goes by. These findings showed that dietary intake, level of serum lipids and other vascular disease risk factors increased as BIM increased. Therefore, middle or upper class Korean adults who have high BMI should improve their eating habits. This involve reducing alcohol, animal fat, high carbohydrate foods, and overall food intake, and balancing intake in order to lower vascular disease risk factors, including obesity.

Conjugated Linoleic Acid in Rumen Fluid and Milk Fat, and Methane Emission of Lactating Goats Fed a Soybean Oil-based Diet Supplemented with Sodium Bicarbonate and Monensin

  • A metabolic study was conducted with four ruminally-cannulated lactating goats (Saanen, 29 weeks lactation, 65${\pm}$5 kg) in a 4${\times}$4 Latin square design with 4 dietary treatments. The goats were fed a basal mixed diet consisting of 80% concentrate and 20% chopped rye grass hay (DM basis, CON). The goats were also fed the CON diet supplemented with soybean oil at a 5% level of the concentrate (SO), the SO diet supplemented with 0.5% of sodium bicarbonate (SO-B) or the SO-B diet supplemented with 30 ppm monensin (SO-BM). The goats were housed in individual pen and the study was conducted for 8 weeks. An increased molar proportion of propionate (C3) was observed at 1 h (p<0.003) and 6 h (p<0.029) post-feeding from all the supplemented diets. Calculated methane emission was markedly decreased prior to morning feeding (p<0.01), and at 1 h (p<0.05) and 6 h post-feeding (p<0.05) in goats fed the supplemented diets. All the supplements increased (p<0.0001) cis9, trans11-CLA content in rumen fluid. Concentrations of both cis9, trans11-CLA (p<0.0001) and trans10, cis12-CLA (p<0.026) were also increased in the milk fat of lactating goats fed the supplemented diets. The SO-B and SO-BM diets further increased CLA content in goat milk compared to the SO diet. All supplements increased unsaturated (UFA, p<0.002), monounsaturated (MUFA, p<0.002) and polyunsaturated fatty acids (p<0.014) and reduced SFA to UFA ratio (p<0.023). The concentration of MUFA was even greater (p<0.002) for SO-BM than for the SO-B diet. In conclusion, feeding soybean oil (5% of concentrate) to lactating goats was a useful way to improve milk fat and to improve fatty acid profile in the milk by increasing potentially healthy fatty acids such as CLA. Supplementation of sodium bicarbonate or sodium bicarbonate with monensin to the soybean oil-based diet increased CLA content further in goat milk. Supplementation of soybean oil may be an effective method to reduce methane emission in lactating goats.

The Nutritional Status by Stress on Freshmen of University (대학 신입생의 스트레스 민감 여부에 따른 영양상태)

  • This study was made to find out how stress affect on nutrition status of the college freshmen who were experiencing physical growth and development as well as drastic emotional change. 400 male and female freshmen in 4 year colleges were surveyed respectively through the health check-up procedure for college entrance in February, In order to find out the stress in each group frustration, deprivation, lack of self efficacy, type A behavior and anxiety response were surveyed through 10 questions with total 40 points by assigning 4 points for each question. Diet Status was expressed by DDS (Dietary Diversity Score by 5 food groups) and DVS(Dietary Variety Score). 24-hrs recall method was used to find out the quantity of daily nutrient of EAR(estimated adquacy ratio) by KDRIs(Korean Dietary Recommended Intakes). Nutrition level was analyzed by Can-Pro for professionals (Korea Nutrition Association). And for the quality intake, percentage was calculated and MAR(Mean Adequacy Ratio) were produced. Highest point was obtained in the stress of anxiety with the total 40 score of 30.20, and the scores were 29.79, 28.67, and 28.39 for deprivation, type A behavior and frustration respectively. There was no difference of blood components in accordance with stress type. Stress type was divided into less sensitive group and highly sensitive one and the relationship with the blood nutrient status was observed. The difference of blood component and blood pressure in sensitive and highly sensitive groups was observed in deprivation and anxiety. The index of blood pressure(p<0.05), hemoglobin(p<0.01), HDL-cholesterol(p<0.05), and Fe(p<0.05) was high in the deprivation of sensitive group. Blood pressure and hemoglobin was high in type A of sensitive group(p<0.05). And the contents of blood triglyceride was high in the anxiety of sensitive group(p<0.001) The result of nutrition intake analysis according to stress type showed that there was low intake for energy, riboflavin, and niacin. When the degree of deprivation was high there was a lack of riboflavin intake and there was no significant difference of nutrition intake in lack of self efficacy, type A behavior and anxiety response. Thus, it is necessary for colleges to educate the students to maintain mental stability through various programs and activities after catching a kind and extent of the stress college students we meeting with like the confusion of value system, open heterosexual relationship, and the employment difficulties linked with political uncertainty and economic recession.

Interleukin-12 and Interleukin-6 Gene Polymorphisms and Risk of Bladder Cancer in the Iranian Population

  • Interleukin-12 (IL-12) as an antitumor and interleukin-6 (IL-6) as an inflammatory cytokine, are immunomodulatory products that play important roles in responses in cancers and inflammation. We tested the association between two polymorphisms of IL-12(1188A>C; rs3212227) and IL-6 (-174 C>G) and the risk of bladder cancer in 261 patients and 251 healthy individuals. We also investigated the possible association of these SNPs in patients with high-risk jobs and smoking habits with the incidence of bladder cancer. The genotype distributions of IL-6 (-174 C/G) genotype were similar between the cases and the control groups; however, among patients with smoking habits, the association between IL-6 gene polymorphism and incidence of bladder cancer was significant. After a control adjustment for age and sex, the following results were recorded: CC genotype (OR= 2.11, 95%CI=1.56-2.87, p=0.007), GC genotype (OR=2.18, 95%CI=1.16-4.12, p=0.014) and GC+CC (OR=2.6, 95%CI=1.43-4.47, p=0.011). A significant risk of bladder cancer was observed for the heterozygous genotype (AC) of IL-12 (OR=1.47, 95%CI=1.01-2.14, p=0.045) in all cases, and among smokers (AC) (OR=3.13, 95%CI=1.82-5.37, p=0.00014), combined AC+CC (OR=3.05, 95%CI=1.8-5.18, p=0.000015). Moreover among high risk job patients, there was more than a 3-fold increased risk of cancer in the carriers of IL-12 beta heterozygous (OR=3.7, 95%CI=2.04-6.57, p=0.000056) and combined AC+CC(OR=3.29, 95%CI=1.58-5.86, p=0.00002) genotypes as compared with the AA genotype with low-risk jobs. As a conclusion, this study suggests that IL-12(3'UTR A>C) and IL-6 (-174 C>G) genotypes are significantly associated with an increased risk of bladder cancer in the Iranian population with smoking habits and/or performing high-risk jobs.

Comparison of Three Methods Assessing the Ergonomic Risks of Manual Lifting Tasks at Ship Engine Manufacturing Facilities (선박용 엔진 제조업 들기작업의 인간공학적 위험 평가를 위한 세 가지 방법 비교)

  • A variety of ergonomic assessment methods of lifting tasks known as a major cause of work-related lower back pain have been used. But there is a limited information in choosing the most appropriate assessment method for a particular job and in finding out strengths and weakness of the methods. The purpose of this study was to assess and compare the ergonomic risks of lifting tasks in a marine diesel engine production industry by three lifting ergonomic assessment tools widely used: the National Institute for Occupational Safety and Health(NIOSH) Revised Lifting Equation(NLE), the Washington Administrative Code 296-62-0517(WAC), and the Snook Tables. Lifting index(weight of load/Recommended Weight Limit) of NLE($LI_{NLE}$) was above 1 at 34 tasks(75.6%) of a total number of 45 lifting tasks. LI of WAC($LI_{WAC}$) was above 1 at 11 tasks(24.4 %). LI of Snook Table($LI_{Snook}$) was above 1 at 29 tasks(64.4%). Thus, LI was high in orders of $LI_{NLE}$ > $LI_{Snook}$ > $LI_{WAC}$. There were significantly high correlations among three Lls(p<0.01). The correlation coefficients between $LI_{NLE}$and the other three Lls($LI_{WAC}$ and $LI_{Snook}$) were r=0.93 and r=0.88, respectively. The linear regression equations were y = 0.444x + 0.11(r=0.93) between $LI_{NLE}$ and $LI_{WAC}$, y = 0.93x + 0.008(r=0.88) between LI(NLE) and $LI_{Snook}$. The LI values by WAC was significantly lower than those by the other tools. The compared features, strength and limitation among these tools were described in this paper.
