Change of Smoking Behavior by Male White-collar Workers after a Tobacco Price Increase (담배가격 인상에 따른 사무직남성근로자들의 흡연행태 변화)

  • Objectives: The purpose of this study is to examine the effect of the raise of cigarette prices by KRW 2,000 at the beginning of 2015 on the change in smoking behavior among male office workers, and to analyze the correlation of various factors including their work behaviors and socio-economic factors with their smoking rate. Methods: In this research, a follow-up observation panel was constituted with 420 smokers as targets from among male office workers at a bank located in Daegu, South Korea. A cross-analysis and ANOVA analysis were carried out in order to examine whether changes in smoking status, amount of smoking, stop-smoking motivation, and reasons for smoking cessation failure after the passage of time since the cigarette price hike were statistically significant. The level of statistical significance was P < 0.05. Results: After the cigarette price hike, among the 420 smokers who were the target of the panel the rate of smoking cessation declined at the time-point of the survey to 15.5%, 12.4%, 8.5%, and 5.7% after one month, three months, six months, and 12 months, respectively. As a result of a follow-up observation of 65 smokers who stopped smoking immediately after the price hike, the actual non-smoking rate declined to 15.5%, 8.3%, 4.4%, and 3.1% after one month, three months, six months, and 12 months, respectively. One (1) year after the cigarette price hike, the non-smoking rate among the 420 smokers reached as low as 3.1% (13 persons). The most important reason for the failure of the attempts to quit smoking was stress for more than 60% of the smokers who attempted to stop. Conclusions: It seems that a powerful anti-smoking policy by the state targeting the nation's workers is necessary. For companies, mediation for workers' job stress can become a strategy for the success of non-smoking attempts. The government seems to require a practical policy to reduce the smoking rate by actively carrying out social, economic, and scientific research to come up with a reduction method for the cigarette hazard, an effective price hike policy, and other non-price policies.

Factors Affecting Re-smoking in Male Workers (남성 근로자의 재흡연에 관련된 요인)

  • Objectives: This study was performed to examine the factors affecting re-smoking in male workers. Methods: A self-administrated questionnaire survey was conducted during April 2003 to examine the smoking state of 1,154 employees of a company that launched a smoking cessation campaign in1998. Five hundred and eighty seven persons, who had stopped smoking for at least one week, were selected as the final study subjects. This study collected data on smoking cessation success or failure for 6 months, and looked at the factors having an effect on re-smoking within this period. This study employed the Health Belief Model as its theoretical basis. Results: The re-smoking rate of the 587 study subjects who had stopped smoking for at least one week was 44.8% within the 6 month period. In a simple analysis, the re-smoking rates were higher in workers with a low age, on day and night shifts, blue collar, of a low rank, where this was their second attempt at smoking cessation and for those with a shorter job duration (p<0.05). Of the cues to action variables in the Heath Belief Model, re-smoking was significantly related with the perceived susceptibility factor, economic advantages of smoking cessation among the perceived benefits factor, the degree of cessation trial's barrier of the perceived barriers factor, smoking symptom experience, recognition of the degree of harmfulness of environmental tobacco smoke and the existence of chronic disease due to smoking (p<0.05). In the multiple logistic regression analysis for re-smoking, the significant variables were age, perceived susceptibility for disease, economic advantages due to smoking cessation, the perceived barrier for smoking cessation, recognition on the degree of harmfulness of environmental tobacco smoke, the existence of chronic disease due to smoking and the number of attempts at smoking cessation (p<0.05). Conclusion: From the result of this study, for an effective smoking ban policy within the work place, health education that improves the knowledge of the adverse health effects of smoking and the harmfulness of environmental tobacco smoke will be required, as well as counter plans to reduce the barriers for smoking cessation.

The Effect of Smoking Status upon Occurrence of Impaired Fasting Glucose or Type 2 Diabetes in Korean Men (우리나라 성인 남성에서 흡연량과 공복혈당장애 또는 제2형 당뇨병 발생과의 연관성)

  • Objectives: To investigate whether smoking and the smoking status are predictors of the incident impaired fasting glucose (IFG) or type 2 diabetes in Korean men. Methods: A cohort of 1,717 Korean men without IFG or diabetes, who underwent annual periodic health examinations for 4 years (2002-2006), were retrospectively investigated. IFG and diabetes were defined as a serum fasting glucose concentration of 100-125 mg/dL and more than 126 mg/dL, respectively. Cox's proportional hazards model was used to evaluate the association between smoking and development of IFG or type 2 diabetes. Results: A total of 558 cases (32.5%) of incident IFG and 50 cases (2.9%) of diabetes occurred. After controlling for the potential predictors of diabetes, the relative risk for IFG, compared with the never smokers, was 1.02 (95% CI=0.88 to 1.19) for the ever-smokers, 0.96 (95% CI=0.79-1.16) for those who smoked 1-9 cigarettes/d, 1.15 (95% CI=1.01 to 1.30) for those who smoked 10-19 cigarettes/d, and 1.31 (95% CI=1.10 to 1.57) for those who smoked 20 or more cigarettes/d (the P value for the current smokers was only p<0.002). The respective multivariate adjusted relative risks for type 2 diabetes, compared with the never-smokers, were 1.07 (95% CI=0.64 to 1.92), 1.47 (95% CI=0.71 to 3.04), 1.84 (95% CI=0.92-3.04), and 1.87 (95% CI=1.13-3.67), respectively (the P value for the current smokers was only p=0.004). Conclusions: The smoking status and the number of cigarettes smoked daily are associated with an increased risk for developing IFG or type 2 diabetes in Korean men.

  • Using the AUDIT-K (Korean versions of alcohol use disorders identification test), it was able to select a large number of potential danger drinkers who routinely live. There were no statistically significant differences in age, religion, marital status, occupation, family history of alcohol drinking, age at onset of alcohol use, smoking, and BMI (body mass index). However, only education and drinking periods were statistically significant. These results are consistent with the previous study, which suggests that alcohol consumption is higher among men who are educated. The level of education also suggests that there is a need to look at the differences depending on what you learned and how. Alcohol consumption by education also shows different results. Also, drinking at a young age will lead to a longer drinking period, which is consistent with previous studies on the age at onset of drinking. In this study, drinking period was the most important factor of alcohol use disorder and it is considered that measures and education of national government system which can delay drinking age as much as possible is needed as a method to shorten drinking period.

Association of Current and Past Smoking with Metabolic Syndrome in Men (성인남성에서 현재 및 과거 흡연과 대사증후군의 관련성)

  • Objectives : The objective of this study was to determine the relationship between past smoking and the risk factors for metabolic syndrome. Methods : From January 2007 to December 2007, a total of 3,916 over thirty years old male health screen examinees were divided into the nonsmoking, smoking, ex-smoking groups. The diagnosis of metabolic syndrome was based on the criteria of the NCEP ATP III(Executive Summary of The Third Report of The National Cholesterol Education Program). Metabolic syndrome was defined as the presence of three or more of the following: a blood pressure $\geq$ 130/85 mmHg, a fasting glucose level $\geq$ 110 mg/dL, a HDL-C (High Density Lipoprotein Cholesterol) level < 40 mg/dL, a triglyceride level $\geq$ 150 mg/dL and, a waist circumference men $\geq$ 102 cm, but a waist to hip ratio > 0.90 was used as a surrogate for the waist circumference. Results : After adjustment for age, alcohol consumption and, exercise in the smokers, for the ex-smokers compared with the nonsmokers, the odds ratio (OR) of a lower HDL cholesterol level (<40 mg/dL) was 1.29 (95% CI=1.03-1.61) in the smokers, the ORs of a higher triglyceride level were 1.35 (95% CI=1.09-1.66) in the ex-smokers and, 2.12 (95% CI=1.75-2.57) in the smokers, and the OR of a waist to hip ratio was 1.25 (95% CI=1.03-1.52) in the ex-smokers. When there were over three components of metabolic syndrome in the ex-smokers and smokers as compared with the nonsmokers, the odds ratio against the risk of metabolic syndrome were 2.39 (95% CI=1.00-6.63) and 2.37 (95% CI=1.02-6.46), respectively. Conclusions : The present study suggested that there is an association of smoking with metabolic syndrome in men.

EEG Study for the Effects of Mouth Breathing on Brain Functions (구강 호흡이 뇌기능에 미치는 영향에 관한 EEG 연구)

  • In this study, we investigated the effects of mouth breathing on brain activity through electroencephalogram (EEG). EEG was performed on 12 healthy volunteers of age ranging from 21 to 27 years (male: female = 6:6, non-smoker). Brain waves on resting state (Rest_N/Rest_M) and auditory-language stimuli state (Eng_N/Eng_M) were recorded during mouth and nose breathing. Four different regions (R1~R4) were classified based on the brain functionality. And each channel (e.g., Pf1 and Pf2) and frequency (${\alpha}$, ${\beta}$, ${\gamma}$, and ${\theta}$) were analyzed using their absolute power ratios of fast Fourier transform (FFT). The results showed that there was no significant difference between Rest_N and Rest_M. Eng_N had significantly higher brain activity than Rest_N; on the other hand, there was no significant difference between Rest_M and Eng_M. These results demonstrate that mouth-breathing on resting state does not induce any significant effects on brain activity and/or functionality, even though it causes subtle temporary inconvenience. In addition to the uncomfort, the brain activity can be adversely influenced by mouth-breathing, which could lower the cognitive skills under certain circumstances.

Effect of Complex Extract Including Cornus officinalis on the Cimetidine/Ethanol-induced Erectile Dysfunction Model in Rats (산수유를 포함한 복합추출물이 cimetidine으로 발기부전을 유도한 동물모델에서 성기능 개선 효과)

  • Erectile dysfunction (ED), also known as impotence, is the inability to attain and sustain an erection firm enough to have sexual intercourse. Frequent ED may be a symptom of health problems including heart disease, obesity, alcoholism, stress, smoking, and depression, that need treatment. This study aimed to effect of complex extract (CPL) including Cornus officinalis on sexual function factor in the erectile dysfunction rat model. The erectile dysfuction rat model was induced by cimetidine (500 mg/kg in 5% ethanol, oral injection 2 weeks). Rats were oral administered with different concentration of CPL in rat erectile dysfunction model. As a results, sexual function factors (NO, cGMP) significantly improved in CPL treated groups (CPL-300, 600, 900 mg/kg) compared to CON group. Serum testosterone was increased in a dose-dependent manner after CPL treatment. Furthermore, administrations of CPL restored lumen areas of the prostate in the erectile dysfunction rat model. These results indicated that CPL alleviated erectile dysfunction by increasing sexual function factor and testosterone in rat model. CPL could be used to natural treatement for erectile dysfunction. However, further study is required to identify active ingredient and its mechanism of erectile dysfunction.

Association of Periodontal Status with Health Lifestyle in Adults (성인의 건강관련 라이프스타일과 치주상태와의 관련성 연구)

  • To investigate the necessity of periodontal treatments, and the association of the health lifestyle with periodontal treatments, the data of 11,488 adults targeted in the 5th (2010, 2012) National Health and Nutrition Examination Survey were used in this study. Chi-squared test was conducted to compare the subjects' general features with the health lifestyle of the patients who needed periodontal treatments. Logistic regression analyses were conducted to confirm the variables that could affect the necessity of periodontal treatments. As a result of verifying the necessity of periodontal treatments according to the general features and health lifestyle, the necessity decreased with the higher income level and education level, and in the cases of nonsmokers, frequent toothbrushing, and using oral hygiene products. When the subjects who did not had oral examinations, their necessity for periodontal treatments significantly increased (p<0.01). The variables that affected the necessity of periodontal treatments included residence, gender, education level, obesity, and oral examinations (p<0.01). Therefore, health lifestyle was thought an important variable that had affected the oral health status. Professional dental hygienists must provide people with action programs of health lifestyle to promote the national oral health status.

Factors Affecting Dental Service Utilization of Adult: An Application of the Andersen Model (앤더슨 모형을 적용한 일부 성인의 치과 의료이용 행태에 영향을 미치는 요인)

  • This study aims to determine the factors affecting the dental service utilization of adults. The subjects in this study were 455 adults, whom a survey was conducted form May 7 to June 7, 2013. Statistical verification conducted through PASW Statistics was 18.0. The difference in the distribution of independent variables related with the dental service utilization was verified with chi-square test. Relevant factors were determined using Hierarchical logistic regression analysis. Model I is predisposing factor, Model II is predisposing factor and enabling factor. Model III is predisposing, enabling, need factor. Andersen model factor which infuences dental service utilization of adults resulted that use dental floss (OR, 2.32; CI, 1.39~3.86), use electric toothbrush (OR, 2.98; CI, 1.0~8.89), use interdental brush (OR, 2.55; CI, 1.36~4.78), self-efficacy (OR, 0.68; CI, 0.48~0.96), barriers (OR, 1.45; CI, 1.04~2.04). Predisposing factor and need factor were found to be determinants for dental service utilization in adult.

  • 미국 캘리포니아 주의 건강문제로는 흡연, HIV/AIDS, 천식, 정신질환, 아동비만, 유방암, API, 건강한 생활양식 형성, 인종간의 건강격차 등을 들 수 있다. 이러한 건강문제를 해결하기 위해 캘리포니아주에서 진행되는 지역사회 중재전략은 지역사회 단체와의 협력이라는 특성을 지니고 있으며, 도시지역의 노인이나 다양한 인종들을 대상으로 하는 프로그램들의 경우는 교회를 통한 접근방법이 빈번하게 활용된다. 중재 프로그램들은 지역사회 조직모형과 역량강화의 개념을 활용하며, 문화적으로 다른 배경을 가진 인구집단을 대상으로는 이들의 요구에 맞는 프로그램을 진행하는 형태로 기존의 프로그램을 채택하는 것이 아닌 이들의 다양한 요구에 맞도록 수정된 프로그램을 제공하고 있다. 개인 간 의사소통을 활용하여 사회적 관계망의 활성화를 도모하며, 일반인이 건강지도자가 되는 교육모형도 활용하고 있다. 한국교민사회 자문위원회(Korean Community Advisory Board, KCAB)는 CDCPRC(CFCH)의 기금으로 진행되는 연구 프로젝트를 수행할 목적으로 1993년 설립되었다. 장기적인 지역사회 건강증진을 위하여 전문가와 일반인 지도자들이 지역사회에서 협력하는 방법을 활용하며 요구도 진단, 수행 평가, 건강옹호와 같은 다양한 사업을 진행한다. 회원들은 핵심 회원과 일반회원으로 구분되고 이들은 사업의 수행과 관련된 의사결정에 도움을 주게 된다. 한국교민사회 자문위원회가 수행한 프로그램은 'Health is Strength', 'Virtual Village Program'이 있으며, 현재는 'Quitting is Winning'이라는 미국거주 한국 남성을 대상으로 하는 인터넷 금연 프로그램을 수행하고 있다. 또한 캘리포니아 주 거주 한국인의 건강상태에 대한 조사를 진행하고 그 결과를 보고하는 프로젝트를 진행하고 있다. 이들이 하는 주요 업무는 지역에서 발생되는 건강문제에 대한 우선순위를 파악할 수 있도록 하여 한국교민들의 건강요구를 파악하고 사업의 우선순위를 설정하는데 도움을 주고 있다. 센터에서 진행되는 한국교민 대상 연구나 사업에 가장 효과적일 수 있는 방법에 대한 지침이나 정보를 제공한다. 또한 문화적으로 민감한 사항이나 적절성 여부에 대한 확인을 하며 연구결과를 보고하고 확산하는데 도움을 주고 있다. 캘리포니아 주는 다양한 인종들이 거주하는 지역이다. 따라서 문화적 민감성이나 문화적 대응전략, 문화적 적절성은 건강증진사업을 수행할 경우에 매우 중요한 이슈가 된다. 또한 이들은 소수민족인 경우가 많아 언어장벽으로 인하여 의료서비스 이용에 제한을 받게 된다. 따라서 보건의료전문가들은 이러한 문화적 요소들을 능숙하게 다루는 기술이 필요한데, 이러한 기술이란 문화가 다른 사람들 간에는 갈등이 존재한다는 점을 인식하고, 건강관련 믿음과 건강행동에 영향을 미치는 사회문화적 요인들을 파악하고, 대안적 해결책을 제시하며, 결과를 평가할 수 있는 능력을 의미한다. 이러한 영역에서 health care interpreter는 소수인종의 보건의료 서비스의 원활한 이용을 위하여 매우 활발한 역할을 수행하고 있다.
