In this study, the effective dose for frequently general radiography among the diagnostic reference level (DRL) for examinations provided by the government in Korea was evaluated using the Monte Carlo N-Particle eXtended (MCNPX) simulation tool. We were selected to evaluate for a total of 5 examination sites which included head anterior-posterior, chest (posterior-anterior, lateral), abdomen anterior-posterior and pelvis anterior-posterior. Physical conditions such as tube voltage and tube current used in MCNPX simulation were used in domestic conditions of the Korea Disease Control and Prevention Agency (KDCA). To evaluate domestic medical radiation exposure, we used the HDRK-Man computerized human phantom manufactured based on the international standard ICRP 103 that was applied to the MCNPX simulation. The phantom could represent the standard body shape of Koreans. As a results, the effective dose corresponding to the DRL based on adult males of head anterior-posterior position was 0.086 mSv, chest posterior-anterior position was 0.05 mSv, chest lateral was 0.354 mSv, abdomen anterior-posterior position was 0.548 mSv, and pelvis anterior-posterior position was 0.451 mSv.
This study was conducted to improve the problems of exposure dose and image reading applied to patients due to the incorrect use of AEC during chest radiography. Images were acquired by dividing the case where AEC was used as the test condition and the case where AEC was not used. As a result of the study, the dose was reduced by 1.17% in 110 kVp without AEC than with AEC, 17.2% decrease at 100 kVp, 30.19% decrease at 90 kVp, and 46.45% decrease at 80 kVp. There was a significant difference in the statistical values according to the tube voltage change in the lung, trachea, and heart SNR average values with AEC and without AEC 110 kVp, but the difference in image quality was insignificant in actual images. When AEC was not applied at the same tube voltage, the dose could be reduced by 17.2% while maintaining the image quality similar to that of with AEC at 100 kVp without AEC. Therefore, rather than relying on AE conditions during chest radiographic examination, it is considered that the conditions should be considered for the examination while lowering the dose by selecting an appropriate tube voltage.
With the development of technology, efforts to reduce the exposure dose received by patients in CT scans are continuing with the development of new reconstruction techniques. Recently, deep learning reconstruction techniques have been developed to overcome the limitations of repetitive reconstruction techniques. This study aims to evaluate the usefulness of images according to reconstruction techniques in pediatric chest CT images. Patient study conducted a study on 85 pediatric patients who underwent chest CT scan at P-Hospital in Gyeongsangnam-do from January 1, 2021 to December 31, 2022. The phantom used in the Phantom Study is the Pediatrics Whole Body Phantom PBU-70. After the test, the images were reconstructed with FBP, ASIR-V (50%) and DLIR (TF-Medium, High), and the images were evaluated by obtaining SNR and CNR values by setting ROI of the same size. As a result, TF-H of deep learning reconstruction techniques had the lowest noise value compared to ASIR-V (50%) and TF-M in all experiments, and SNR and CNR had the highest values. In pediatric chest CT scans, TF images with deep learning reconstruction techniques were less noisy than ASiR-V images with adaptive statistical iterative reconstruction techniques, CNR and SNR were higher, and the quality of images was improved compared to conventional reconstruction techniques.
Background: Blunt chest trauma accounts for 90% of all chest traumas in Europe and the United States and this causes 20% of all trauma-related deaths. The major cause of morbidity and mortality after blunt chest trauma is undetected injuries. For this reason, chest computerized tomography has gained popularity for the evaluation of trauma, but it is expensive and it exposes patients to radiation. This study identified the clinical features associated with the diagnosic information obtained on a CT chest scan, as compared with a standard chest X-ray, for patients who sustained blunt trauma to the chest. This study also evaluated the role of a routine computed tomographic (CT) scan for these patients. The patients who had chest computed tomography done after the initial chest x-ray were analyzed separately for the presence of occult injuries. Material and Method: We studied 100 consecutive patients from November 2006 to July 2007: 74 patients after motor vehicle crashes and 26 patients after a fall from a height >2m. Simultaneous with the initial clinical evaluation, an anteroposterior chest radiograph and a helical chest CT scan were obtained for all the patients. The data extracted from the medical record included the vital signs, the interventions and the type and severity of injury (RTS). Result: Among the 100 cases, 79 patients showed at least more than one pathologic sign on their chest radiograph, and 21 patients had a normal chest radiograph. For 17 of the patients who had a normal chest X ray, the CT scan showed multiple injuries, which were pneumothorax, hemothorax, lung contusion, sternal fracture etc. This represents that a CT scan is statistically superior to a chest radiograph to diagnose the pathologic signs. But on the other hand, as for treatment, only 31 patients were diagnosed by CT scan and they were treated with chest tube insertion ect. 42 patients needed ony conservative management without invasive thoracosurgical treatment such as chest tube insertion or open thoracotomy. 27 patients were treated based on the diagnosis made by the chest radiograph and physical examination. Conclusion: Chest computerized tomography was significantly more effective than routine chest X-ray for detecting lung contusion, pneumothorax and mediastinal hematoma, as well as fractured ribs, scapula and, sternum. Although the occult findings increased, the number of patients who needed treatment was small. Therefore, we suggest making selective use of a CT scan to avoid its overuse in ERs.
Purpose: This study was designed to analyze the outcome and toxicity of thoracic radiation therapy (TRT) and chemotherapy for patients who suffer with limited-stage small-cell lung cancer (LS-SCLC). Materials and Methods: We retrospectively studied 35 patients with LS-SCLC. TRT was administered once daily (1.8 to 2 Gy per fraction) and it was directed to the primary tumor for a total 50 to 66 Gy in 6 to 7 weeks. The patients received four cycles of etoposide plus cisplatin. TRT was begun on day 1 of the first cycle of chemotherapy in the concurrent arm and after the fourth cycle in the sequential arm. Results: The median progression-free survival time was 16.5 months (95% confidence interval [CI], 9.0 to 24.1 months) for the sequential arm, and 26.3 months (95% CI, 16.6 to 35.9 months) for the concurrent arm. The 2-year progression-free survival rate was 16.0 percent for the sequential arm and 50.0 percent for the concurrent arm (p=0.0950 by log-rank test). Leukopenia was more severe and more frequent in the concurrent arm than in the sequential arm. However, severe esophagitis was infrequent in both arms. The radiotherapy was interrupted more frequently in the concurrent arm than in the sequential arm due to hematologic toxicities (p=0.001). Conclusion: This study suggests that concurrent TRT with etoposide plus cisplatin is more effective for the treatment of LS-SCLC than sequential TRT. However, there is a significant increase in the risk of toxicities, and radiotherapy was frequently interrupted in the concurrent arm due to hematologic toxicities.
We wanted to evaluate the usefulness of three-dimensional reconstructive images using computed tomography for rib fracture patients. The reconstruction used in clinical multi planar reformation(MPR), volume rendering technique(VRT), and image data using quantitative methods and qualitative methods were compared. Much more, the artifact shadow was minimized to reconstruct with 3D volumetric image by using an law data in the analysis of the reconstructive image and chest CT scan of the evaluation result fractures of the thoracic patient. And we could know that the fractures of the thoracic determination and three dimension volume image reconstruction time were reduced.
At OPD follow-up in December 1998, chest x-ray of a 42-year-old female showed a solit ary pulmonary nodule at a superior basal segment in the right lower lobe. After percut aneous transthoracic needle aspiration failure, wedge resection of the superior basal segment of lower lobe in right lung was performed for diagnosis and therapy. Three years ago, she had received surgery to remove a mass in the left buttock. The mass was pathologically diagnosed as malignant fibrous histiocytoma. She subsequently received 4500 rad radiotherapy for 35 days. Pathology confirmed metastatic malignant fibrous histiocytoma of the lung.
Chordomas are slowly growing and rare malignant tumors arising from the remnants of the notochord. Furthermore, intrathoracic chordomas presenting as a posterior mediastinal tumor account for only 1∼2% of all reported chordomas. Incomplete resection of these tumors can lead to local recurrence, distant metastasis and result in a poor outcome, but complete remission can be expected with complete resection and adjuvant radiotherapy. We report a case of thoracic chordoma presenting as a posterior mediastinal tumor which was successfully treated with complete resection and adjuvant radiotherapy without recurrence and distant metastasis for 2 years.
The patient was a 42-year-old female with breast cancer who had an implantable central venous catheter inserted percutaneously into left subclavian vein for chemotherapy. The postinsertion chest x ray revealed that there was no compressive sigh of catheter. Three months after insertion of the catheter, the patient was admitted to the hospital for 4th chemotherapy. The port was accessed but blood could not be aspirated and the catheter could not be flushed. A chest x ray revealed that the catheter was completely transected at the point where the catheter passed under the clavicle. Percutaneous removal of the distal fragment of the catheter was accomplished. The patient was discharged after successful removal of fragment of catheter.
Unilateral diaphragmatic paralysis due to a phrenic nerve injury is not rare after cardiothoracic surgery and may range from an asymptomatic radiographic abnormality to severe pulmonary dysfunction and even mortality in patients with reduced lung function. The most effective treatment for symptomatic unilateral diaphragmatic paralysis has been known to be a plication of the paralyzed hemidiaphragm. A 38 year-old male patient with asthma received a phrenic nerve reconstruction with a sural nerve for right phrenic nerve injury during resection of the anterior mediastinal tumor. Ten months later, chest PA showed good result and we report this case with literature.
