Relationship between Changes of Patient Safety & Medical Service Quality and Changes of Management Activity after Medical Institution Accreditation: Mental Hospitals and Geriatric Hospitals (의료기관 인증 후 환자안전 및 질 관리 변화와 경영활동 변화 간의 관계: 정신병원과 요양병원 대상)

  • This study was to investigate the relationship between safety & quality management changes of patient and changes in management activities based on hospital workers in five mental hospitals and five geriatric hospitals which should be required medical certifying authorities. As a result of the research study, participation whether or not of certification service of mental hospital & geriatric hospital workers was positive correlation to improve change of 'Performance level of Safety Activities for the patient' 'Provide the High Quality Medical Service for the patient' 'Respect the Rights and Responsibility of the patient' 'Performance level of Infection Control Activities' out of contents of Patient Safety & Medical Service Quality. Also developmental changes of Safety Activities for the patient Hospital Quality for the patient Rights and Responsibility of the patient out of contents of Patient Safety & Medical Service Quality need to the Capacity Management Activities through Education and training, and Medical System & Evaluation of Management Provide the High Quality Medical Service for the patient out of contents of Patient Safety & Medical Service Quality need to the need to the Customer Orientation Process.

Complex impact of Patient Safety and Medical Quality on Hospital Management Activities due to Healthcare Accreditation Adoption (의료기관인증제 도입에 따른 환자안전과 의료의 질이 병원경영활동에 미치는 복합영향)

  • The purpose of this study was to confirm the complex relationship between patient safety-medical quality and hospital management activities to observe the mediating effect of organizational culture in the relationship due to healthcare accreditation adoption. For this, we conducted a 4 weeks survey, from September 22, 2014 to October 17, 2014 to the employees of 6 hospitals that could accommodate less than 300 patients or more than 300 patients located in Daegu and Gyeongbuk. The study includes 377 participants who have worked for more than one year in the hospital. All of hospital with less than 300-beds or more than 300-beds, such as patient safety-medical quality, organizational culture, hospital management activities found a positive correlation among variables. The mediating effect of organizational culture in the relationship between patient safety-medical quality and hospital management activities was shown by the partial mediating effect and high causal effect. Therefore, these study suggest that patient safety-medical quality due to healthcare accreditation adoption has the potential to improve hospital management activities.

Change in patient safety, quality of medical care, organization culture and hospital management performance after healthcare accreditation system for convalescent hospital (요양병원 의료기관인증제 시행 후 환자안전, 의료의 질, 조직문화 및 병원경영성과의 변화)

  • This study was conducted to investigate the effects of implementation of the healthcare accreditation system on patient safety, quality of medical care, organization culture and management performance of convalescent hospitals. Ten hospitals were selected at random from among 36 convalescent hospitals in Daegu that had obtained accreditation until September 2015. To accomplish this, hospital staff were surveyed from 15 February to 25 March, 2016 using a questionnaire with 253 objects. We investigated how the healthcare accreditation system has influenced patient safety, service quality, organization culture and performance of hospital management. The change in the treatment support system showed the highest score of 3.93 among quality change and medical service items, while that of personnel source culture showed the highest score of 3.78 points among organization culture items, and that of internal process view, study and growth view showed the highest score of 3.71 among management performance items. Multiple regression analysis revealed that factors positively changing hospital management performance were nurses, employees and managers, basic value systems, organization culture, personnel source culture, open system culture, hierarchical culture, and production oriented culture. The quality of patient safety and medical care was positively influenced by changes in the quality of convalescent hospitals after implementation of the healthcare accreditation system. However, among the four types of organizational culture, the financial perspective was relatively low compared to other perspectives.

Effect of Leadership WalkRounds Convergence to Establish a Patient Safety Culture (환자안전문화 정착을 위한 리더십 워크라운드(Leadership WalkRounds)의 융복합적 적용 효과)

  • This study aims to study the change in the level of awareness of medical personnels regarding the patient safety culture(PSC) before and after the Leadership WalkRounds(LWR). The LWR in this study was based on the IHI and the models of the Patient Safety Rounds(PSRs) at University of Michigan, emphasizing the 5 steps of Preparation--Scheduling--Conducting--Reporting--Resolving. After the LWR the scores for the level of awareness showed a statistically significant increase from 2.63 to 3.36(p<.001). Among the participants, those who are pharmaceutists, women, 30.0~39.9 years old, or had work experience of a year or less showed particularly notable increase in awareness. The effect was significant across all categories of PSC, especially in Safety Accident Reporting(p<.001), then in Communication(p<.001). Therefore the LWR proved an convergent concept for applying new leadership skill and the concept of patient safety management as an method to elevate the frontline staffs' awareness of PSC.

  • 문제: 투약오류는 의료기관 전반에서 가장 많이 발생하는 오류의 하나이며, 환자에게 중대한 위해를 초래하기도 한다. 특히 고농축전해질은 문제발생의 가능성과 위험성이 높아 지속적인 관리 및 교육을 필요로 하고 있다. 목적: 발생한 투약오류 건에 대한 근본원인분석을 시행함으로써 유사사례가 발생하는 것을 예방하여 환자안전을 도모하고자 한다. 의료기관: 연세대학교 세브란스병원 질 향상 활동: 투약오류 건에 대해 근본원인분석 시행 후 고위험약물 관련 내규를 보완하였고, 고위험약물에 대한 Alert System 개발, 고위험약물 라벨 부착, 약 처방 관련 의료진 교육을 시행하였다. 개선효과: 고위험약물 투여와 관련된 시스템 개선 활동 이후 유사사례는 발생하지 않았고, 의료진 교육을 통하여 환자안전에 대한 인식과 중요성을 더욱 증가시켰다.

The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital (의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로-)

  • This study was conducted to examine the effects of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment. One group pretest posttest design was used. The study included 405 participants, who have worked for more than one year in a general hospital. The treatment was healthcare accreditation which was prepared from Apr. 2012 and tested by Korea Institution for Healthcare Accreditation on Nov. 2012. Data was analyzed using paired t-test and One-Way ANOVA. This study found patient safety and quality(t=7.28, p=.000), job satisfaction(t=7.13, p=.000) and organizational commitment(t=3.44, p=.001) increased significantly after healthcare accreditation. The results of this study suggest that healthcare accreditation has the potential to improve patient safety and quality, job satisfaction and organizational commitment.

  • The purpose of this study was to ascertain new policies fo the development of the Korean Hospital Accreditation Program (HAP) by comparing and reviewing the domestic HAP of Korea, the U. S. and Australia as well as the International Accreditation Program of the International Society for Health Care Quality. The results of this study are as follows. First, it is necessary to improve the standards of the HAP such as 'patient and community centered standards' and 'staffing related standards'. Second, it is necessary to strengthen patient safety through the HAP. Third, the linkage among the HAP, clinical indicators and patient experience evaluation should be strengthened. Fourth, the competencies of surveyors should be improved. Fifth, the HAP of small and medium hospitals also needs to be vitalized. Sixth, it is necessary to improve the disclosure of the healthcare quality evaluation results. The findings of this study can be used as a basis to improve the Korean HAP.

The Effects of Patient Safety Culture Perception and Patient Safety Competencies on Patient Safety Management Activities in Nurses (간호사의 환자안전문화 인식과 환자안전역량이 환자안전관리활동에 미치는 영향에 대한 융복합 연구)

  • The purpose of this study is to identify the impact on patient safety management activities. In this study, 134 nurses from three local hospitals were investigated for patient safety culture recognition, patient safety capacity, and patient safety management activities, and the impact on patient safety management activities was thus attempted. The patient safety culture recognition level was 4.25/5, the patient safety capacity was 4.37/5, the patient safety management activity was 4.26/5, and the patient safety awareness and patient safety capability(r=.765, p<.001) and Patient Safety Capacity and Patient Safety Management Activity (r=.837, p<.001) and Patient Safety Capacity and Recognition of Patient Safety Culture(r=.728, p<.001) had a significant amount of correlation. Factors affecting patient safety management activities(β=.582, p<.001), Patient Safety Culture Recognition(β=.3632, p<.001), position(β=-.132, p=.009) Patient safety requires strengthening the capabilities of nurses and systematic training and evaluation of continuous patient safety activities.

  • This study was conducted in order to examine the effects of patient safety culture perception and patient safety knowledge on patient safety activities of Chinese-Korean caregivers. A convenience sample of 102 Chinese-Korean caregivers were recruited. Factors influencing Chinese Korean caregivers' patient safety activities included patient safety culture perception and Korean speaking ability. These variables explained 45.8% of the variance in patient safety activities. These results suggest that it is necessary to strengthen patient safety culture perception and to develop some program to enhance their speaking skills in order to improve Chinese Korean caregivers' patient safety activities.

  • In this paper, we propose a secure and efficient RFID-based patient authentication protocol to not only authenticate patients' authenticity but also protect patients' personal medical informations for u-Healthcare environments. Since the proposed RFID-based patient authentication protocol provides strong security and efficiency, it can be used practically for patient authentication and personal medical information protection on the high technology medical environments such as u-Hospital and u-Healthcare.
