  • A free-floating ball thrombus in the left atrium that may cause fatal systemic emboli or left ventricular inflow obstruction, which often resulting in sudden death, is rarely seen. We describe a very unusual case of a patient with a large, free-floating left-atrial ball thrombus who underwent mechanical mitral valve replacement 7 years ago.

  • Report of right atrial thrombus complicating pulmonary embolism after cardiac surgery is rare. A 54-year-old woman operated on the atrial septal defect 10 months ago was admitted for left pleuritic pain and dyspnea. Multiple segmental perfusion defects were detected in lung perfusion scan. Transesophageal echocardiography showed a large mobile right atrial mass attached to the free wall of the right atrium with a stalk. Despite the intravenous heparinization for 13 days, follow-up echocardiography revealed the right atrial mass had not diminished in size. The mass which was confirmed as an organizing thrombus was excised under cardiopulmonary bypass. The patient recovered uneventfully and was discharged on warfarin therapy.

  • A right atrial thrombus in structurally normal heart is very rare. A 66-year-old woman was admitted with chest discomfort and dyspnea. She was diagnosed of right atrial myxoma on echocardiography and chest computed tomography, We peformed an excision of the mass attached to atrial septum, which was found to be an organized mural thrombus by pathologic examination. We report this rare case with a review of literature.

  • A rare syphilitic abdominal aortic aneurysm associated with pulmonary embolism and deep vein throbosis is reported. We have experienced a huge infrarenal syphilitic abdominal aortic aneurysm which caused venous compression at left common iliac vein. A 29 year-old female was admitted via emergency room due to several episodes of hemoptysis. Clinical evaluation for this patient revealed a couple of small ill-defined masss densities on the both lung field and abominal aortic anuerysm. Emergency wedge resections of left upper and lower lobes were performed because of a massive hemoptysis. Pathologic diagnosis showed pulmonary infarction. 20 days after thoracotomy an elective operation on abdominal aortic anuerysm was successfully carried out and post operative pathology finding showed syphilitic aortitis.

오가피 및 작약으로부터 혈소판 응집 억제작용 물질의 개발에 관한 연구

  • 혈관내에서의 비정상적인 혈소판의 활성화는 혈소판 응괴 또는 혈전 형성을 초래함으로서 동맥경화증, 심근 경색증, 혈전중등의 허혈성 질환의 발현에 직접 또는 간접적인 원인으로서 주목되고 있다. 본 연구자 등은 천연물로부터 혈소판 응집억제작용 물질의 개발을 목표로 하여 한약 또는 민간약의 형태로서 관련질환에 사용되어온 식물생약 약 40종을 검색하였으며 이들중 혈소판 응집억제작용을 갖는 식물로부터 작용 물질의 분리를 계속하여 오고있다. 본 실험에서는 작약을 메타놀로 추출하고 작용을 추적하면서 용매로 분획하였으며, 작용분획인 Et0Ac fr.으로부터 methyl gallate를 분리하였다. Methyl gallate는 오가피로부터 작용성분으로서 이미 분리 보고된 protocatechuic acid 및 artifact인 ethyl protocatechuate와 구조적으로 매우 유사하여 이들 analogs 수종에 대하여 작용을 비교 검색하였다.

  • Pulmonary arterial thrombosis develops during hypercoagulable states, intra-arterial tumorous conditions, and congenital heart disease accompanied by pulmonary hypertension. Thrombosis in the main pulmonary arterial stump after pneumonectomy can also occur. Herein, we report a very rare case of pulmonary arterial thrombosis in a patient with pulmonary hypertension and a lung destroyed by tuberculosis. He presented with aggravated dyspnea without fever or purulent sputum. His chest computerized tomography scan showed left main pulmonary arterial thrombosis as a convex shape, with the ipsilateral distal arteries and arterioles showing parenchymal destruction. After excluding pulmonary thromboembolism and hypercoagulable disorders, we diagnosed pulmonary arterial thrombosis and treated him with an anticoagulant.

  • Trousseau's syndrome comsists of migratory thrombophlebitis and thromboembolic disorders of the venous and arterial systems in a malignancy or occult cancer. The overall incidence has been reported to vary from 1 to 11%. Pancreatic, lung, prostate, and stomach cancer is associated with the greatest risk of thromboembolic events. We encountered a 49-year-old man who presented with painful swelling of his lower legs. The chest radiograph showed increased opacity of the Left middle lung fields and Doppler sonography showed a thrombus in the left superficial femoral vein. Chest Computed Tomography showed a 5cm sized left hilar mass invading the pericardium with lymphadenopathy. The bronchoscope biopsy demonstrated an adenocarcinoma of the lung. Platinum based chemotherapy and anticoagulant therapy with warfarin was carried out. The patient was later discharged with an improvement in the painful swelling of his lower legs.

Anti-thrombotic effect of artemisinin through regulation of cAMP production and Ca2+ mobilization in U46619-induced human platelets (U46619 유도의 사람 혈소판에서 cAMP 생성 및 Ca2+동원의 조절을 통한 Artemisinin의 항혈전 효과)

  • The regulation of platelet aggregation is crucial for maintaining normal hemostasis, but abnormal or excessive platelet aggregation can contribute to cardiovascular disorders such as stroke, atherosclerosis, and thrombosis. Therefore, identifying substances that can control or suppress platelet aggregation is a promising approach for the prevention and treatment of these conditions. Artemisinin, a compound derived from Artemisia or Scopolia plants, has shown potential in various areas such as anticancer and Alzheimer's disease research. However, the specific role and mechanisms by which artemisinin influences platelet activation and thrombus formation are not yet fully understood. This study investigated the effects of artemisinin on platelet activation and thrombus formation. As a result, cAMP production were increased significantly by artemisinin, as well as phosphorylated VASP and IP3R which are substrates to cAMP-dependent kinase by artemisinin in a significant manner. The Ca2+ normally mobilized from the dense tubular system was inhibited due to IP3R phosphorylation from artemisinin, and phosphorylated VASP by artemisinin aided in inhibiting platelet activity via αIIb/β3 platelet membrane inactivation and inhibiting fibrinogen binding. Finally, artemisinin inhibited thrombin-induced thrombus formation. Therefore, we suggest that artemisinin has importance with cardiovascular diseases stemming from the abnormal platelets activation and thrombus formation by acting as an effective prophylactic and therapeutic agent.

Biological Activities of Wild Sparassis crispa Extracts (야생 꽃송이버섯 추출물의 생리활성)

  • Physiological activities of solvent extracts of Sparassis crispa were investigated for fibrinolytic, thrombin inhibitory, anti-oxidative and anti-inflammatory activities. The fibrinolytic activity was the highest in ethyl acetate extract (2.03 plasmin units/mL) followed by butanol extract (0.70 plasmin units/mL). The ethyl acetate extract exhibited the highest anti-oxidative activity as assessed by the 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging rate with a value of 95.94%. The chloroform extract showed thrombin inhibitory activity up to 83.87%. The chloroform extract also showed the highest anti-inflammatory effects on the production of nitric oxide (NO) in lipopolysaccharide (LPS)-stimulated RAW264.7 cells. These findings suggest that Sparassis crispa may be a useful material for development of drugs and functional foods.

  • A fifteen-year-old boy was transported to our hospital emmergency room due to sudden onset of chest pain, hemoptysis and dyspnea. He was diagnosed as persistent ductus arteriosus that had developed acute bacterial endocarditis with pulmonic valve vegetation and pulmonary embolism with pulmonary infarction. After conservative antibiotic therapy (vancomycin + aminoglycoside), we operated this patient successfully - patch closer of the ductus and pulmonary valve valvuloplasty - under the cardiopulmonary bypass.
