Purpose: The purpose of the study was to investigate symptom clusters experienced by patients on hemodialysis and to identify relations between symptom clusters and quality of life. Methods: Data were collected from 127 patients at 10 local hemodialysis clinics. Symptoms were measured using 10-item physical symptom checklist as well as the Hospital Anxiety depression Scale. Quality of life was measured with the Satisfaction with Life Scale. Data were analyzed using factor analysis, Pearson correlation, and stepwise multiple regression. Results: The most frequently reported symptoms included fatigue, itching, depression, numbness/tingling, and insomnia. Four distinct symptom clusters were identified: cluster 1 was comprised of dry mouth, headache, nausea (gastrointestinal); cluster 2 of decreased appetite, insomnia (basic need); cluster 3 of itching, numbness/tingling (sensory-comfort); and cluster 4 of fatigue, depression (mood-vitality). Among the clusters, the 'basic need' cluster and 'mood-vitality' cluster had a significant negative association with quality of life. The 'mood-vitality' cluster, explained 17.4% of the variance in quality of life. Conclusion: The results of the study indicate that comprehensive symptom assessment provides better symptom management for patients on hemodialysis. Further studies are needed to verify symptom clusters identified in this study.
Purpose : The purpose of this study was to compare the effect of recirculation rate according to cannulation direction and interval among hemodialysis patients with arteriovenous fistula (AVF). Method : The research used repeated measures design. This study was conducted among thirty patients who received hemodialysis three times a week for longer than a year through AVF at the I University hospital. Three different types of interventions were administered to the participants each week for three weeks. Needles were placed at a different distance and in a different direction each week: 7 cm apart from each other in antegrade direction during the first week, 5 cm apart in retrograde direction in the following week, and 7 cm apart in retrograde direction in the third week. Results : No significant differences in the recirculation rate were found due to any of the three tested methods (p = 1.00). Conclusion : This finding suggests that, if the patients have well-functioned AVF, we can choose an appropriate intervention from among the three methods in consideration of the patient's diverse needs.
Purpose: This study aimed to identify the effect of food craving, patient activation, and family support on dietary self-care performance in hemodialysis patients. The study will provide baseline data for developing nursing interventions to improve hemodialysis patients' dietary self-care. Methods: The data was collected from 132 patients who regularly underwent outpatient hemodialysis between October 26 and November 12, 2022. Frequency and percentage, mean and standard deviation, independent t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple regression analysis were used to analyze the data, and all analyses were performed using SPSS 27.0. Results: The average scores of the main variables were 39.55±7.42 for food craving, 77.37±10.50 for patient activation, 63.62±11.72 for family support, and 38.21±5.54 for dietary self-care. The factors affecting the dietary self-care were age (β=.36, p<.001), family support (β=.36, p<.001), and patient activation (β=.20 p=.007). The total explanatory power of all variables was 34.0% (F=18.20, p<.001). Conclusion: Based on these results, it is necessary to consider the characteristics of age and improve the level of family support and patient activation when developing programs to improve dietary self-care in hemodialysis patients.
1년동안 ACVIM C단계의 심부전 및 IRIS 4단계의 신부전으로 관리 중이던 13세의 중성화 잡종견과 급성으로 발병한 신부전으로 인한 요독증과 이에 속발한 형태의 폐부종이 동반된12세의 중성화 암컷 잡종 고양이가 본원에 내원하였다. 본 환자들에 체외초미세여과법을 이용한 투석요법을 적용하여 폐부종, 요독증 및 이에 동반된 전해질 및 산염기 불균형 등이 개선되었다. 본 증례는 초미세여과법을 이용한 투석요법이 이뇨제 저항성 및 심한 전해질 불균형이 있는 심장신장증후군 환자와 이뇨제를 이용한 폐부종 치료 도중 급성으로 신장기능 이상이 발생한 환자 등에서 생명을 구하는 치료옵션으로 고려될 수 있음을 보여준다.
Hemodialysis(HD)-associated hypotension is a frequent complication, but it is difficult to manage. Until now, several maneuvers have been tried to prevent the HD-associated hypotension. Of these, the sodium content of dialysate was regarded as an important factor for maintaining blood pressure during HD. In this study, we evaluated the effect of hypertonic dialysate on blood pressure, interdialytic weight gain and the incidence of thirst. The study was done for 6 weeks successively with 3 different groups. Each patient was dialysed with 3 different dialysates for 2 weeks: Group I(Conventional HD: sodium concentration: 137 mEq/L), Group II(Hypertonic HD: 147 mEq/L) and Group III (Sequential HD: from 147 to 140 mEq/L). Hemodynamic parameters(blood pressure, pulse rate and ultrafiltration rate), biochemical parameters(hematocrits, blood urea nitrogen, creatinine, osmolality, sodium, potassium, chloride, fasting blood sugar) and complications (interdialytic weight gain & thirsty sensation) were compared among 3 groups. The results were as follows: 1. Decline of systolic blood pressure and diastolic blood pressure at the time of a 3 hour check during hemodialysis was lower in the Group II than Group I and III (p=0.002; p=0.012). and decline of diastolic blood pressure at the time of a 4 hour check during hemodialysis was lower in the Group II and III than Group I (p=0.04). 2. Incidence of hypotensive episodes during dialysis was significantly lower in Group II than group I (p=0.0287). 3. The ultrafiltration in Group III at the time of 1 hour, 2 hour and 3 hour check during hemodialysis was higher than that in Group I and II at the time of 1 hour, 2 hour and 3 hour check during hemodialysis respectively (p=0.0001; p=0.0001; p=0.0004). 4. Interdialytic weight gain was higher in Group I($3.1{\pm}0.8$) than Group I($2.8{\pm}0.8$) and III ($2.9{\pm}0.9$) (p=0.0422). 5. Hematologic and biochemical results were not significantly different among 3 Groups. 6. Frequency of thirst was different in Group I, II and III, $0.05{\pm}0.12$, $0.41{\pm}0.24$and $0.22{\pm}0.29$ respectively (p=0.0259). The results suggest that hypertonic HD was effective in preventing HD-associated hypotension but interdialytic weight gain and thirst sensations were increased as compared with a conventional method. In this situation, sequential HD seems to be an alternative method to minimizes the side effect of hypertonic HD.
배경: 말기 신부전 환자에서 심장병, 특히 관상 동맥 질환의 이환이 늘어나면서 관상동맥 우회수술의 대상 환자가 지속적으로 증가하고 있다. 이들 환자군은 수술후 유병률과 사망률이 매우 높고 고위험군으로 알려져 있다. 대상 및 방법: 1996년 3월부터 2000년 5우러까지 서울 중앙 병원 흉부외과에서 술전 말기 신부전증으로 진단 받은 후 관상 동맥 우회수술을 시행받은 환자 25명을 대상으로 의무 기록을 중심의 후향적 분석을 하였다. 술전 위험 인자 및 술전 신기능, 수술 결과, 술후 경과, 수술후 합병증, 사망률 및 생존률 등을 분석하였다. 결과: 술전 평균 크레아티닌 청소율은 12.7$\pm$5ml/mim였고 술전 평균 혈중 크레아티닌 치는 6.2$\pm$3mg/dl(1.7-14.4)였다. 술전 투석을 시행중인 환자는 11례(44%)였고 술전 투석을 시행하지 않았던 14례(56%)중 8례(8/14, 57.1%)는 수술전후로 새로이 투석이 필요하였다. 술전 혈액 투석 중이었던 9례중 2례에서 수술후 복막 투석으로 전환하였다. 수술 사망률은 2례(8%)로 흡인성 폐렴과 종격동염으로 1례, 그리고 수술후 출형과 종격도염으로 1례가 사망하였다. 수술후 합병증은 14명(56%)의 환자에서 발생하여 매우 높은 발생율을 보였다. 만기사항은 2례(8%)에서 발생하였으며 사망원인은 카테터에 의한 복막염이었다. 생존 환자의 4년 생존률은 82$\pm$13% 였다. 결론: 말기 신부전 환자에서의 관상동맥 우회수술을 비교적 만족스러운 범위의 수술 사망률(8%)을 보였으나 합병증 발생률이 매우 높고 합병증 발생 시 사망률이 매우 높아 수술주위 감염 예방과 세심한 환자 관리가 필요하다.
Long-term hemodialysis(HD) patients manifest various signs of protein and caloric malutrition due to poor intake of nutrients and other causes. Poor nutritional status increases the mortality and morbidity rates in HD patients. Thus, mataintnance of adequate nutritional status has been a major task in taking care of patients receiving HD. This study was to evaluate the nutritional status of HD patients and to clarify the degree of nutritional deficit based on usual dietary intake, anthropometric and biochemical indicators. Sixty HD patients comprised a HD group, while the control group consisted of 60 healthy adults whose age and sex matched those of the HD group. Nutritional status was evaluated by dietrary intake using instant nutritional scale, anthropometric measures, serum protein concentrations and the number of lymphocytes. The data were analyzed by using Chi-square test and unpaired t-test. The results are as follows. 1. Regarding usual dietary intake of HD group. 1) Estimated caloric intake was significantly lower than the recommended daily allowance(RDA) and among them, 35% were taking calories less than 85% of the RDA. 2) Estimated protein intake was significantly higher than the RDA and among them 40% were taking protein more than 115% of the RDA. 3) Estimated fat intake was lower than the RDA. 4) Vitamin A, B, $B_1,\;B_2$, C and niacin in take was lower than the RDA respectively. 5) Estimated ferrous intake was within the normal limit the RDA while estimated calcium intake was higher than the RDA. 6) Both calorie and protein intake were higher for the 10 patients who had been under continuous ambulatory peritoneal dialysis than for the patients under HD from the beginning. 2. Regarding anthropometric measures : 1) Body mass index(BMI), midarm circumference(MAC), and triceps skinfold thickness(TSF) were lower in the HD group than in the control group. 2) Among HD group, 47.1% were within the normal limit of BMI, while 86.7% were within the same limit in the control group. 3) Among HD group, 35.0% were within the normal limit of MAC, while 83.3% were within the same limit in the control group. 4) Among HD group, only 8.3% were normal, 30.3% were mild deficit status of TSF, while 50% were normal and 48.3% were mild deficit status in the control group. 3. Regarding biochemical laboratory tests 1) Albumin, transferrin concentrations and the number of lymphocytes were lower in HD group than in the control group. 2) Among HD group, 98.3% were within the normal limit of albumin concentration and all were within the same limit in the control group. 3) Among HD group, only 11.7% were within the normal limit of transferrin concentration, while 81.7% were within the same limit in the control group. 4) Among HD group, 25% were within the normal limit, while 93.3% were within the same limit in the control group. The above findings suggest that HD patients were in nutritional deficit status. Adequate diet therapy and periodical evaluation of the nutritional status in HD patients are needed. Accordingly, it turned out that anthropometric measures were very reliable parameters and easy to use to evaluate nutritional status. So nurses are encouraged to adopt anthropometric measures to examine nutritional deficit status of HD patients.
신부전 환자의 신장기능을 유지하는 방법의 일환으로서 사용되는 혈액 투석 시 동정맥루 문합각도에 따라서 혈액 유동상태의 양호가 결정된다. 지금까지의 연구는 CFD에 의한 것이 대부분이었으나 실험적 보고는 없다. 본 연구에서는 동정맥루의 혈류역학적 특성을 분석하기 위하여 PIV기법을 이용하였다. PIV실험을 위한 혈관모델의 문합부 각도는 $30^{\circ}C$로 정하였다. 유체역학적 상사를 만족시키기 위하여 Re수는 같게 하였고, 동맥부분에 밸브를 달아 개폐 정도를 조절하면서 PIV실험을 수행하였다. PIV 실험결과 분지부 부분에서 재순환유동이 발생되는 것을 볼 수 있었고, 동맥의 개폐 정도에 따라 전체적인 혈류속도의 변화가 나타났다.
혈액 항응고 활성물질을 검색하기 위하여 73종의 한약재, 41종의 산채류 및 일반 채소류, 5종의 해조류들의 열수추출물들에 대해 혈액응고 지연시간(Tr)을 시험하였다 1차검색에서 비교적 활성이 높았던 택사, 삼칠근, 현호색, 운지, 마늘, 청각의 산성 수추출물에서 가장 높은 항응고 활성(대조군에 비해 4.3배)을 보였다. 청각(1.2KG)을 0.8% 염산 수용액(24l)으로 추출한 후 메탄을 환류, 에탄올 환류, 침전 및 투석하여 활성이 약 2배 증가된 조다당 CF-1을 얻었다. CF-1은 80.8%의 다당과, 8.7%의 단백질 및 13.3%의 황산기로 구성되어 있었으며, 다당류는 arabinose, galactose, 및 glucose가 주구성당으로 확인되었다. 이 조다당 CF-1의 항응고 활성 본체를 확인하기 위해 pronase처리와 periodate 산화를 행한 결과 pronase 처리시에는 활성의 변화가 없었으나 periodate 산화시에는 약 70%로 활성이 감소한다는 사실로부터 CF-1의 항응고 활성은 다당에 의해 기인됨을 알 수 있었다.
배경: 의학의 발전으로 인하여 만성신부전 환자의 생존율이 향상되고 2차적인 동정맥루술을 시행하여야 하는 경우가 증가하였다. 이 논문은 2차적인 동정맥루 조성술의 하나인 Revision이 다른 방법과 마찬가지의 성공률 및 개존율을 나타내는가를 알고자 작성하였다. 대상 및 방법: 1995년 1월부터 1998년 9월까지 총 365례의 동정맥루술을 시행하였으며 이중revision하여 동정맥루를 만든 62명의 환자를 대상으로 그 결과를 관찰하였다. 결과: Revision한 62명의 6개월 개존율이 78.8%, 12개월 개존율이 72%, 24개월 개존율이 63%, 36개월 개존율이 56.9%이었다. 결론: Revision에 의한 수술방법은 2차적 재수술시 우선적으로 고려해보아야할 수술방법이다.
