Diffuse Dendriform Pulmonary Ossification with Spontaneous Pneumothorax (자발성 기흉을 동반한 미만성 수지상 폐 골화증)

  • Diffuse pulmonary ossification is a rare entity which was reported just a case in Korea, defined as "wide spread heterotropic bone formation within the lung parenchyme", and it's pathogenesis remains unclear. Generally, diffuse pulmonary ossification is diagnosed by autopsy because most of case is asymptomatic, and classified as either nodular and dendriform diagnosed pathologic examination. We reviewed a case of diffuse dendriform pulmonary ossification with spontaneous pneumothorax.

  • 본 논문은 임신의 시기가 내분비계의 변화로 인한 프로게스테론의 과잉분비가 발생하여 혈관의 투과성을 항진시켜 구강내의 잇몸 부종, 잇몸 출혈 등을 유발하고, 입덧으로 인한 메쓰꺼움, 몸의 무거움과 나른함, 음식 기호변화, 간식횟수의 증가 등을 겪게 되어 신체뿐 아니라 구강 환경 또한 변화하게 된다는 여러 관련 자료들을 근거로 임산부의 구강 건강상태와 구강보건인식을 파악하고자 2008년 12월 15일 ~ 2009년 1월 30일까지 180명의 임산부를 대상으로 구강건강상태와 구강보건인식에 대한 조사를 실시하였다. 설문의 결과 임산부의 생활수준, 분만경험, 임신시기에 따라 생활수준이 좋을수록, 분만경험이 없는 경우에서, 4-6개월 임신시기 시 구강건강상태에 대하여 유의미한 차이가 나타났다. 건강한 구강건강을 위한 우선 행위인 잇솔질에서의 교육 수준별, 생활 수준별, 임신 시기별로 식사 후 3회 정도의 잇솔질의 횟수가 유의미한 차이의 결과를 나타내었으며, 잇솔질시 오심과 구토로 잇솔질이 어려운 임산부들의 구강상태 청결을 위해 사용되어지는 보조구강위생용품으로는 치실, 구강 양치액, 혀 클리너, 치간 칫솔이 사용된다는 결과가 나타났다. 이러한 결과의 자료들은 임신 시기 중 가장 고민된다고 답하여준 구강건강 부분에서의 입덧으로 인한 잇솔질의 어려움, 잇몸출혈, 충치, 치석제거, 구강질환 치료의 적정시기, 구취 등의 부분을 임산부 대상의 구강보건교육 시 기존의 자료에 조금 더 중점적으로 다루어져야 하는 부분임을 확인할 수 있었다.

Triple Primary Cancer -A Case Report- (원발성 삼중암 수술치험 1례 보고)

  • Multiple primary cancer is a disease of more than two cancers occurring in an individual indepen- dently. We experienced a case of triple primary cancer, that is, lung cancer, malignant thymoma and bladder cancer which has not been reported in Korea. The patient was a 60 year old man with dyspnea and chest discomfort. He was rirst diagn sed bladder cancer and received Bricker's operation 3 months ago. At that time the chest roentgenography and computerized tomographic scan revealed as a preaortic, retrosternill medidstin;11, nlass and a lung mass at the posterior portion of the left lower lobe. On operation, there was An identillable lung mass in the left lower lobe and a thymoma already invaded the surrounding structures. So, left lower lobectomy and thymectomy were performed simul- taneously. He received one cycle of postoperative chemotherapy, but refused further management and self-discharged. He died, about one year later.

Video-Assisted Thoracoscopic Thymectomy -Report of Two Cases- (흉강내시경을 이용한 흉선 절제술)

  • We report two cases of the thoracoscopic thymectomies for patients of myasthenia gratis with tracheostomy (Osserman's Group-ll-C-1). The Irst case was 47-year-old male wlth generalized myas- thenia gravis who was under the mechani al ventilator therapy with tracheostomy. The second case was 60-year-old male with deteriorating generalized myasthenia gratis after the mechanical ventilator therapy. We decided to resect the thymus by vldeo-assisted thoracoscopy to prevent the ouurrence of postoperative complications, especially mediastinitis because all two cases were under tracheostomy state. We could stop the mechanical ventilator therapy on the postoperati'fe 16th day and 3rd day respect- ively and they were recovered without mediastinitis. So we concluded that video-assisted thoracoscopic thymectomy is a good alternative surgical method for myasthenia gratis patients with tracheostomy.

  • Totally implantable venous devices (TIVD)는 장기간 항암 화학 요법이나 총 정맥 영양, 수액치료가 필요한 환자에서 많이 사용된다. TIVD는 매우 유용하고 안전한 장치이지만, 이와 관련된 다양한 합병증이 보고 되어 왔다. 저자들은 카테터의 혈관 내 유착에 의해 TIVD의 제거가 불완전하게 된 2예를 경험하였기에 보고하는 바이다.

  • Anomalous systemic arterial supply to the lung is a rare congenital anomaly. The lung supplied by the anomalus systemic artery has a normal bronchial tree, which is usually in the basal segment of the lung, especially in the left lung. Most of patients are asymptomatic, but the main clinical symptoms of this disease are hemoptysis and exertional dyspnea. CT is useful for the diagnosis and showed a retrocardiac nodular shadow connected to the descending aorta branching into the basal segments of the relatively normal lower lobe. Surgery is indicated for all patients. Here we report a case of anomalous systemic arterial supply to normal basal segments of left lower lobe in a patient with hemoptysis with a review of the relevant literature.

  • Diffuse esophageal spasm(DES) is a rare disease seen in 4% of all patients studied in an esophageal motility laboratory, and its diagnosis and surgical management is still controversial. Recently, we treated two patients by extended esophageal myotomy for diffuse esophageal spasm which was diagnosed by the clinical symptoms of patients, esophagoscopy, esophagography, and esophageal manometry. The successful result of treatments was proved with subsidence of previous clinical symptoms(dysphagia and chest pain), postoperative esophagography and esophageal manometry. We present the results together with the review of literatures.

  • A 16 month old male infant was found with slip down state in a bath room without evidence of trauma to whole body. The infant was treated with several thoracentesis and closed drainage due to persistent right pleural effusion at other hospital and transferred to our hospital for further evaluation and treatment at July 2003. The pleural effusion was confirmed as chylothorax by chemical analysis. He was treated with parenteral feeding for 21 days. Because the amount of chest tube drainage was about 110∼210 cc/day, and could not be decreased with conservative treatment. patients underwent ligation of thoracic duct. Post-operative course was uneventful except post-op. empyema thoracis, The open drainage tube was removed at post operative 30 days, The patient was in very good condition with complete cure until post-operative 3 months.

  • Desmoplastic small round cell tumor (DSRCT) is rare and a recently described, poorly differentiated malignant tumor and it usually presents with widespread intra-abdominal involvement. We report a case of DSRCT arising form the mediastinum which was treated with multimodality anticancer therapy in 15 year-old girl.

  • Tracheoinnominate artery fistula (TIF) is a very rare complication that can happen after long standing tracheostomy or insertion of an endotracheal tube and its mortality is very high. This condition requires early diagnosis and urgent management because of the life threatening results from tracheal obstruction or hypovolemic shock that is caused by massive bleeding. We report here on a case of successful tracheoplasty with using the costal cartilage, including the perichondrium, in a patient with laryngeal stricture, and this was done to preserve the trachea for the following operation to relieve the laryngeal stricture.
