  • 육상의 U-health서비스와 달리 해상이라는 특수한 지리적 요건으로 인해 사용 가능한 통신기술이 제한적이며, 선박이라는 특수한 공간에서 발생하는 진동과 소음 등 환경적 요소와 의사 및 의료팀의 부재에 따른 의료지식이 현저히 부족한 의료 관리자 및 일반인이 쉽게 시스템을 사용가능하도록 사용성을 고려하여 해상의 선박 및 플랜트 등에서 원격으로 의료서비스 지원이 가능하도록 해양환경에 최적화된 원격의료시스템을 개발하였다.

  • 육상과 떨어져 근무하고 있는 해양근로자들은 의료접근도의 한계성으로 인해 건강관리에 많은 어려움을 호소하고 있다. 이와 같은 해양근로자들에게 U- health기반의 해양원격의료시스템 도입을 한다면, 질병예방 및 건강관리, 나아가 건강증진에 이르기까지 매우 효과적일 것이다. 따라서 이 연구는 향후 해양근로자의 원격 건강관리 서비스를 위한 원격의료 도입 및 운영의 기본 방향을 제공하는 것을 목적으로 한다.

  • 해상에서 운항되는 선박에서 인명사고가 발생하면 육상과 달리 적시에 의료지원을 받기 구조적으로 어렵다. 특히 연안선박의 경우 원양선에 비해 상대적으로 고령의 선원들이 승무하고 있기 때문에 인명사고가 빈번하며 인명사고에 취약하다. 본 연구는 AR과 VR과 같은 실감기술을 기반으로하는 연박선박의 원격의료지원 모델 개발하기 위한 방향성을 제시하고자 한다.

The Effects of Service Characteristics of Maritime Telemedicine on Seafarer's Satisfaction and Loyalty (해양원격의료의 서비스 특성이 선원만족도 및 충성도에 미치는 영향)

  • The objective of this study was to analyze the effect of characteristics of maritime telemedicine (service quality, recognition or not of service, health counselling service experience or not) on seafarer's satisfaction, and also analyze the effect of seafarer's satisfaction on loyalty. This study's data was collected from the Research Report of Survey of telemedicine service subjects in 2017 conducted in the maritime telemedicine pilot project. However, only 103 seafarers who received maritime telemedicine (Tele-Monitering) services were analyzed statistically. The main results of this study were as follows. ⅰ) Better service quality led to higher levels of satisfaction, ii) High levels of seafarer's satisfaction led to high levels of loyalty, iii) Seafarers whose age is under 50 than 30 years old are less satisfied, ⅳ) In total boarding career, Seafarers with more than 5 years but less than 10 years were more satisfied than seafarers with less than 5 years, ⅴ) Seafarers with more than 10 years but less than 15 years were more satisfied than seafarers with less than 5 years, ⅵ) Seafarers with more than 15 years but less than 20 years were more satisfied than seafarers with less than 5 years, ⅶ) Also, Seafarers in excess of 20 years were more satisfied than seafarers with less than 5 years, In conclusion, ⅰ) Better service quality led to higher levels of satisfaction and loyalty. Accordingly, the improvement of service quality is not only an important factor in improving satisfaction and loyalty, but also plays an important role in forming a positive image of maritime telemedicine. ⅱ) Seafarers with younger age and lower overall total boarding career were more satisfied. It is believed that the seafarer who are willing to use telemedicine and who are willing to refer to others are young.

Implementation of Maritime Telemedicine System Using Android (안드로이드를 이용한 해양원격진료시스템 구현)

  • The purpose of this study is to design and implement a maritime telemedicine system to provide the best medical service for patients who rescue and happens in sailing ship. For this purpose, INMARSAT and MVAST, which are necessary communication channels for maritime telemedicine, were examined and diversity in maritime communication path was suggested. It was implemented to enable maritime telemedicine by linking smart devices and medical devices for physical examination. The marine telemedicine system adopts the HTTP communication protocol to configure the web server and process the client request. The patient's chart information was entered using an application for AndroidPad and the patient was diagnosed using Bluetooth electronic stethoscope for diagnosis. This system enables appropriate medical treatment to emergency patients at maritime, which ultimately can improve medical services. It is expected to be a more effective medical support system through linkage with various medical equipment.

  • 본 연구는 해양원격의료서비스의 특성이 선원의 만족도 및 충성도에 미치는 영향을 규명하기 위해 서비스를 받은 선원 103명을 대상으로 설문조사를 실시하여 자료를 분석하였다. 본 연구의 주요 결과는 다음과 같다.ⅰ) 서비스 질이 높을수록 만족도는 높았다, ⅱ) 만족도가 높을수록 충성도는 높았다, ⅲ) 연령에서 30세미만 보다 40미만의 선원들이 만족도가 낮았다. ⅳ) 총 승선경력이 5-10년 미만인 선원이 5년 미만인 선원보다 만족도가 높았다, ⅴ) 승선경력이 10-15년 미만인 선원이 5년 미만인 선원보다 만족도가 높았다, ⅵ) 승선경력이 25년 이상인 선원이 5년 미만인 선원보다 만족도가 높았다.

  • This paper proposes a smart glass-based emergency tele-medical direction system. This system is designed for hospital specialists to provide remote medical guidance to on-site coast guards or emergency responders. To identify the requirements necessary for system development, relevant technological trends and case studies were analyzed. Based on this analysis, three system requirements were defined: 1) The system must be able to determine the necessity of patient transport, 2) It should assist in providing emergency medical care during transport to the hospital, and 3) It must be capable of transmitting patient information to medical facilities. A prototype that meets these requirements was developed and its usability was evaluated.

Perceived Usefulness and Attitude toward Smart-glass for First-aid Remote Support among Coast Guards in Korea (응급처치 원격지도용 스마트글래스 사용에 대한 한국 해양경찰의 인지된 유용성 및 태도)

  • This study was to investigate the types of emergencies transported by the Southwestern Coast Guard, the need for telemedicine guidance, and the perception and attitude of smart glasses as a communication method targeting 31 coast guards. A relatively high frequency and training requirement were confirmed for bleeding, abrasion, and abdominal pain. The demand for telemedicine guidance on medication and triage was higher, and the perceived usefulness and attitude scores for the use of smart glasses were 3.76±0.61 and 3.64±0.45, respectively. A moderate correlation between perceived usefulness and attitude toward smart glasses was confirmed (r=.630, p<.01). With the development of technology, it is time to actively introduce new devices such as smart glasses.

  • This descriptive research study aims to identify the importance and satisfaction of new advanced safety education and marine telemedicine service, and to identify the priority of educational needs for trainees who have received advanced safety reeducation. Data from 190 trainees who conducted advanced safety retraining at the MarineCom were used for the study. As a result of the study, the vital sign measurement method was selected as the top priority for the new advanced safety education, and the nervous system, circulatory system, and surgical system were selected as the top priority for the educational need by type of marine telemedicine guidance. These results suggest that there was a high demand for education on vital sign measurement methods in order to determine the appropriate first aid by grasping the overall condition of the patient in the event of an emergency. Working and monotonous daily life acted as a cause of mental and physical stress, so the incidence of patients in the nervous system, circulatory system, and surgical system was high, and compared to other systems, it was judged that the demand for education was high because the patient was related to life.

Clinical Analysis of Marine Telemedicine Cases for Ocean-Going Vessel Crew (원양선박 선원들의 해양원격의료 실태를 통한 임상분석)

  • The purpose of this study is to contribute to the study of the prevention of diseases and promotion of health of ocean going vessel crew members, through the medical diagnosis and disease classification efforts of this study. From the second half of 2016 to 2017, the diagnosis and health characteristics of 195 crew members were collected through counseling, treatment, and emergency care for about 1 year and 2 months. As a result, it is noted that the incidence of diseases was in the order of urticaria (5.6%), lumbar sprain (4.1%), acute gastroenteritis (3.1%) and anxiety (3.1%). In categorical review, the incidence of musculo-skeletal disease was the most common (25.1%) which was followed by skin disease (17.9%) and digestive disease (11.3%). In addition, the disease that was noted as was the most common in the under 30 years old category, and the incidence of the disease was high in the crew group. Finally, there was a difference between the pathogenesis (trauma vs disease, etc.) (p <.001) and the type of vessel (merchant ship and fishing vessel) (p <.005) as noted in this case.
