Nutritional Assessment and Dietary Management during Anti-Cancer Chemotherapy in Pediatric Oncology Patients (항암 화학요법 중인 소아암 환자의 영양 평가 및 식이 관리)

  • Purpose: We assessed the nutritional status and the alterations of oral diets during anti-cancer chemotherapy in pediatric oncology patients. Methods: Twenty children with malignancy were evaluated from day 0 until day 21 of post-chemotherapy. Nutritional status was assessed by body weight and biochemical parameters. The amount and calories of oral diets were assessed and food preference before and during chemotherapy were analysed by questionnelle. Results: 1) The underlying diseases of 20 patients were 11 acute lymphoblastic lekemia, 2 non-Hodgkin's lymphoma, 2 Langerhans cell histiocytosis, 2 Wilm's tumor, 2 brain tumor, 1 rhabdomyosarcoma. 2) There were weight loss during chemotherapy in 8 patients (40.0%), weigt gain in 5 patients (25.0%), and no significant changes in 7 patients (35.0%). 3) Biochemical parameters showed no significant interval changes during chemotherapy except elevation of serum ALT level. 4) The daily caloric intakes of oral diets during chemotherapy were 310~600 Kcal which was much lower than average of daily recommended calory for Korean children. 5) The most favorate food was altered by chemotherapy, from meats to carbonated beverages and unfavorate food was not altered as vegetables. Conclusion: The periodic assessment of nutritional status and dietary supplements according to preferred foods of patients will be required for the optimal nutrition care in cancer patients.

Experimental Studies on the Antitumor Effects of Jinryungtang Gagambang Extract (진령탕가감방의 항종양효과(抗腫瘍效果)에 관(關)한 실험적(實驗的) 연구(硏究))

  • The sprig of Jinryungtang Gagambang has been used for curing as a traditional medicine without any experimental evidence to support the rational basis for their clinical use. This experiment was carried out to evaluate the possible therapeutic or antitumoral effects of Jinryungtang Gagambang extract against cancer, and to study some mechanisms responsible for its effect. The cytotoxic and antitumor effects were evaluated on human cell liens (A549, hep3B, Caki-1, Sarcoma 180) after exposure to Jinryungtang Gagambang extract using in ILS, colony forming efficency and SRB assay which were regarded as a valuable method for cytotoxic and antitumor effects of unknown compound on tumor cell lines. The results obtained in this studies were as follows. 1. As a result of exposure to Jinryungtang Gagambang extract, the proliferation of A549, hep3B, Caki-1, good correlations were shown from the results of SRB assay and those of clogenetic assay. 2. The oral administration of Jinryungtang Gagambang extract showed significant effects of increase of MST(mean survival time) and ILS(increased life span) depending on the increasing concentration. 3. Against squamous cell carcinoma induced by MCA, Jinryungtang Gagambang decreased not only the frequency of tumor production but also the number and weight of tumors per tumor bearing mice(TBM). Jinryungtang Gagambang also significantly suppressed the development of 3LL cell-implanted tumors by frequency and their size, and some developed tumors were regressed by the continuous treatment of Jinryungtang Gagambang extract into TBM. 4. Jinryungtang Gagambang extract also increased NK cell activities. According to the above results, it could be suggested that Jinryungtang Gagambang extract has prominent antiutmor effect.

  • 위암 환자의 수술후 5년 생존율은 조기암의 경우 90%이상으로 매우 좋으나 진행암의 경우 20~30%를 넘지 못한다. 위암의 예후에 영향을 주는 요인 중 조기 발견보다 더 중요한 것은 없으며, 조기암의 진단율에 노력을 기울이는 것이 현실적 타개책이라 하겠다. 현재 우리나라의 조기암 진단율은 16~22%로 일본의 40~45%보다 극히 저조하므로 소화기계 증상이 있는 경우 위내시경 검사를 적극 권고함이 바람직하다. 위암은 외과적 절제술이 유일한 근치치료법임은 주지의 사실이며 근래에는 위내시경 및 복강경을 이용하여 위장관 기능을 보존하는 술식도 시도되고 있어 바람직한 발전이라 사료되나, 림프절 곽청의 원칙을 훼손하지 않는 범위내에서 이루어져야할 것이다. 근치적 절제술의 정의에 합당한 외과적 치료법만이 5년 생존율 향상을 기대할 수 있으며, 보조적 항암화학요법과 면역요법은 대상 환자의 엄선으로 부분적 치료효과를 기대할 수 있다. 절제불능 위암이나 국소진행암에서 절제율을 높이기 위한 술전치료방법들의 적극적 도입이 바람직하다. 그러나 대상 환자의 선정에 있어 객관적 타당성이 있는 병기 결정 방법과 치료효과를 판정하는 방법의 혁신이 있어야 할 것으로 사료되며 현재의 부정확한 검사방법들의 대안으로 복강경의 이용이 바람직하리라 사료된다.

Cost-Effectiveness of Paclitaxel plus Cisplatin as a Neoadjuvant Chemotherapy for Locally Advanced Head and Neck Cancer (국소진행성 두경부암에서 선행항암화학요법제로 사용되는 Paclitaxel과 Cisplatin 병용요법의 비용-효과성)

  • This study was conducted to analyze cost-effectiveness of neoadjuvant chemotherapy for locally advanced head and neck cancer in Korean healthcare setting. We constructed a decision analytical model to estimate total costs and outcomes of paclitaxel+cisplatin (PC) or docetaxel+cisplatin+5-FU (DCF) for 2 years time horizon in 100 patient cohort with locally advanced head and neck cancer. Base analysis showed that cost savings of PC regimen were 379 million Korean Won and 231 million Korean Won in societal and payer's perspectives, respectively, compared to DCF regimen, and life saved was 0.18. PC regimen as a dominant strategy was found to be robust through sensitivity analyses.

Effect of Auricular Acupressure on Fatigue and Depression in Patients Undergoing Chemotherapy for Breast Cancer (이압요법이 항암화학요법을 받은 유방암 환자의 피로 및 우울에 미치는 효과)

  • Objectives: This study aimed to investigate the effects of auricular acupressure on fatigue and depression among patients undergoing chemotherapy for breast cancer. Methods: A nonequivalent control group nonsynchronized design was employed. Data were collected from December 2017 to May 2018, in the cancer center at a hospital located in B city. The experimental group received auricular acupressure four times a day, for 5 days, followed by a 2-day rest period. The control group received routine nursing care. The data were analyzed with independent t-tests, using the SPSS 21.0 program. Results: The level of fatigue and depression was lower in the experimental group as compared to that in the control group(t=5.29, p<.001; t=2.21, p=.038, respectively). However, depression scores increased post-intervention in the experimental group, as compared to the pre-intervention scores. Conclusions: The findings confirmed that auricular acupressure decreased fatigue however, depression scores increased post-intervention in the experimental group, as compared to the pre-intervention.

In this paper, the HCT-116 cells thought to be a cancer cell to analyze the interaction of TRAIL and radiation. Experimental results, single use of TRAIL and radiation, results were compared with the control group, it was found to have no significant effect on each cell proliferation and apoptosis. Conversely treated with TRAIL, when treated in parallel radiation, it was possible to know that the HCT-116 cells significantly apoptosis occurs, The proportion of G1 ratio G0 also was found to have increased. TRAIL conclusion is increased apoptosis radiation defensive cells can know that increased radiosensitivity, also possible to alter the cell cycle, reduce cell proliferation ability stepwise it was possible. TRAIL is increased apoptosis, decreased cell proliferative capacity, it is considered to be possible to use as a radiation sensitizer.

A Clinical Therapeutic Results on Small Cell Lung Cancer (소세포 폐암의 치료성적에 대한 고찰)

  • Background : A clinical study was carried out on 153 new cases with small cell lung cancer registered at Presbyterian Medical Center, Chonju during the 7 years from 1986 to 1992. They were analyzed by sex and age distribution, symptoms and signs, classification of stage and site and its treatments. Especially, an effort was made to compare the overall survival time between limited stage and extensive stage. Methods : Among 806 lung cancer patients diagnosed by biopsy or cytologic evaluation for the 7 years, 153 patients was shown small cell lung cancer. These 153 cases was analyzed retrospectivery through patient's records, letters or telephones. Results : The results of evaluation of small cell lung cancer are as follows. Over 85 percent of the small cell lung cancer patients were over 50 years of age and prominent clinical features were cough(86.3%), sputum(75.8%) and dyspnea(54.9%). One hundred and five patients(68.7%) was staged to have limited stage. Mean survival time of the chemotherapy and chemoradiotherapy in limited stage has significant difference and its survivals are 5.3 months and 15.0 months. Patients whose disease was staged as limited, regardless of whether or not chemotherapy was administered, had a median survival time of 10.9 months, compared with 4.8 months for those with extensive stage. Conclusion : Lung cancer is one of the malignant diseases tend to increase gradually in Korea and proven to be the most common cancer next to the gastric cancer among various cancers in males found at the Presbyterian Medical Center in the past seven years. This report is a retrospective view of the clinical therapeutic results of the small cell lung cancer patients. Especially at the limited stage, the combined therapy revealed higher survival rate than the chemotherapy alone. For a more accurate evaluation. a prospective view, without any bias, of patients selected at random is needed.

Trajectories and related Factors of Fatigue in Patients Undergoing Chemotherapy for Lung Cancer (항암화학요법 주기에 따른 폐암환자의 피로 양상과 관련요인)

  • This study was conducted to explore the trajectory of fatigue and related factors on people with lung cancer during chemotherapy. A total of 23 patients with lung cancer participated in the beginning stage of the study. However, 11 dropped out because of death or discontinuity of the studys regimens. The data using the Pipers Fatigue Scale, were collected twice the first day of each treatment cycle and the last day of the completion of 6 cycles. Also, the scores of fatigue using the Visual Analogue Scale (VAS) were measured on the same day of each week during chemotherapy to explain the trajectory of fatigue. The data obtained were analyzed using the Wilcoxon signed ranks tests and Kendalls tau b correlation coefficient. The score of fatigue increased in the first two weeks after the administration of chemotherapy, while decreasing after the second week. During the 1st, 5th, and 6th cycles, the scores of fatigue were greater than 5.5 out of 10. Overall, except for the 6th cycle, the score of fatigue was the highest in the first week. This could be accounted for there only being a small number of patients included in the 6th cycle. In conclusion, fatigue was severe at the end of the first week of chemotherapy, and then increased to reach a plateau in the fifth and sixth stages. The results of this study will help oncology nurses to understand the process of fatigue during chemotherapy. It will be useful to create various intervention programs to decrease fatigue in people with cancer especially in the first week of the chemotherapy.

Relationships among Malnutrition, Depression and Quality of Life in Patients with Gynecologic Cancer receiving Chemotherapy (항암화학요법을 받는 부인암 환자의 영양장애, 우울 정도와 삶의 질과의 관계)

  • Purpose: The purpose of this study was to identify nutritional status, and relationships among malnutrition, depression and quality of life in patients with gynecologic cancer who were receiving chemotherapy. Methods: For this study a descriptive cross-sectional design was used. Participants were 111 women who were enrolled and agreed to undergo a face-to-face interviews including administration of the structured questionnaires: Patient-Generated Subjective Global Assessment (PG-SGA), Beck Depression Inventory (BDI), Simplified Nutritional Appetite Questionnaire (SNAQ), and Functional Assessment of Cancer Therapy-General (FACT-G). Results: Mean body mass index was 23.3 and mean body weight was 56.5 kg. Sixty-three (57%) of the 111 patients were malnourished according to the PG-SGA. The malnourished patients showed higher levels of depression and lower quality of life compared to the non-malnourished patients. In addition, malnutrition was associated with BMI level, depression, appetite and quality of life. Conclusion: The findings indicate that the prevalence of malnutrition is high and malnutrition in patients with gynecologic cancer influences depression and adversely affects the quality of life of these women. To improve the patient's quality of life, nutritional assessment and appropriate management is important to decrease malnutrition in patients with gynecologic cancer.

Experiences of Changes in Cognitive Function for Women treated with Chemotherapy for Breast Cancer (항암화학요법을 받은 유방암 여성의 인지기능변화 경험)

  • Purpose: The purpose of this study was to describe the experiences of the process of the change in cognitive function for women treated with chemotherapy for breast cancer. Methods: There were ten participants in total in this study. Data were gathered utilizing in-depth interviews over 3 times from September 2010 to January 2011. Data were analyzed by employing Strauss and Corbin's (1998) grounded theory methodology. Results: Findings indicate that causal conditions of these results were 'side effects of chemotherapy' and 'menopausal state', including contextual conditions as, 'mental fatigue' and 'anxiety about recurrence'. The core category was identified as 'confronting with unexpected chaos'. Intervening conditions were 'support from other people', 'lack of information on cognitive impairment'. Interaction strategies were 'changing the habit of life', 'making efforts for living life' and 'seeking for medical information'. Consequences of the process were 'physical restriction', 'difficulty in social life', 'disturbed working ability' and 'psychological distress'. Conclusion: The results of this study show that nurses should recognize there is a notable difference between individual patient's contextual conditions and interactive strategies. Furthermore educational information and individualized intervention should be provided to improve cognitive function for women with breast cancer.
