An overview of herbal medicine for atopic dermatitis (아토피성 피부염의 한약치료 효과에 관한 고찰)

  • 목 적 : 한약 또는 한약제제의 아토피성 피부염에 대한 치료효과를 조사하고 향후 연구방향을 제시하기 위하여 고찰연구를 시행하였다. 방 법 : PubMed에 한약과 아토피성 피부염과 관련된 검색어의 조합을 넣어 포함기준에 맞는 무작위배정 대조군 임상연구만 포함하였다. 연구설계, 치료방법, 대조군, 평가지표, 결과, 부작용 관련 정보를 미리 정해놓은 자료 추출 형식에 맞추어 추출하고 방법론적 질 평가는 옥스포드 질 평가 척도와 그룹 할당 은닉(allocation concealment) 여부를 평가하였다. 연구들이 임상적 및 통계적으로 상이하여 메타분석은 이루어지지 않고 기술적 고찰만 실시하였다. 결 과 : 모두 8편의 연구가 고찰기준을 만족시켰다. 다양한 복합한약제제와 한약이 포함된 외용제가 평가되었는데 8편 가운데 5편에서 아토피성 피부염의 증상을 호전시키는 것으로 나타났다. 방법론적 질은 대체로 양호한 것으로 나타났으며 일부 효과적인 것으로 나타난 한약복합제제에서 간손상 등의 부작용도 보고되었다. 결 론 : 한약 또는 한약제제를 이용한 치료는 아토피성 피부염의 증상개선에 도움이 되는 것으로 보이나 현재 근거는 부족하다. 우리나라에서 많이 쓰이는 한약제제들 역시 엄정한 임상연구를 거쳐 그 효과를 평가하고 근거를 구축해야 할 것이다.

Effects for the New Formulation of Daesiho-tang on adipocyte development and differentiation in 3T3-L1 (대시호탕의 새로운 제형이 3T3-L1에서 지방세포 증식과 분화 과정에 미치는 영향)

  • Objectives : Daesiho-tang (DSHT) has been widely used in the treatment of cerebral infarct in traditional medicine. However, there was not report on the anti-obesity-related diseases efficacy of DSHT. In this study, we investigated the effects for the new formulation of DSHT, on the adipocyte differentiation cycle in 3T3-L1 cells. Methods : 3T3-L1 cells were treated with DSHT (50, 100, $200{\mu}g/m{\ell}$) during differentiation for 6 days. Also, the inhibitory effect of DSHT against 3T3-L1 adipogenesis was evaluated in various stage of adipogenesis such as early (0-2day), intermediate (2-4day), and terminal stage (4-6day). The accumulation of lipid droplets was determined by Oil Red O staining. and, the expressions of genes related to adipogenesis were measured by RT-PCR and Western blot analyses. Results : DSHT showed inhibitory activity on adipocyte differentiation at 3T3-L1 preadipocytes without affect cell toxicity as assessed by measuring fat accumulation and adipogenesis. In addition, DSHT significantly reduced the expression levels of several adipocyte marker genes including proliferator activated $receptor-{\gamma}$ ($PPAR-{\gamma}$) and CCAAT/ enhancer-binding $protein-{\alpha}$ ($C/EBP-{\alpha}$). Also, the anti-adipogenic effect of DSHT was strongly limited in the intermediate (2-4 day), terminal stage (4-6 day) of 3T3-L1 adipogenesis. In addition, the DSHT treatment down- regulated mRNA expression levels of $PPAR-{\gamma}$,, $C/EBP-{\alpha}$ in mature 3T3-L1 adipocytes. Conclusions : These results suggest that, the ability of DSHT has inhibited overall adipogenesis and lipid accumulation in the 3T3-L1 cells. The new formulation of DSHT may be a promising medicine for the treatment of obesity and related metabolic disorders.

Development of Microbial Inoculant Using By-product of Oriental Herbal Medicine (한약재박을 이용한 미생물제제의 개발)

  • The development of microbial inoculant was conducted using a by-product of oriental herbal medicine. The constituent of the by-product, which was high in organic matter, was 11.3% of crude protein, 5.1% of crude lipid, 49.7% of NDF (neutral detergent fiber), and 33.8% of ADF (acid detergent fiber). Microorganisms isolated from the by-product of oriental herbal medicine were 35 species. Among them, 6 bacterial species, 4 fungal species, 2 actnomycetes species, and 1 yeast species were effective in the utilization of the by-products. The 13 strains screened were tested for the plant growth-promoting effect in soybean seedling. BL-333 strain was found to increase the soybean yield by about 23% as compared with control. The strain BL-333 was identified as Paenibacillus marcerans. P. marcerans BL-333 showed high anti-fungal activities against virulent fungi, especially Fusarium sp. and Collectotrichum sp. Yields of plants which were inoculated with microbial inoculant prepared with P. marcerans BL-333 and by-product of oriental herbal medicine were found to be higher than control by $3{\sim}24%$. The yield was especially promoted in lettuce, radish, chinese cabbage and cucumber plants.

Screening for inhibitory effect on nine CYP isoforms by 20 herbal medications (고속 스크리닝 기법을 이용한 한약제제의 cytochrome P45O 저해능 탐색)

  • We evaluated the potential of 20 herbal medications (HMs), commonly used in Korea, to inhibit the catalytic activities of several cytochrome P450 (CYP) isoforms. The abilities of 500 ${\mu}g/ml$ of aqueous extracts of 20 HMs to inhibit phenacetin O-deethylation (CYP1A2), coumarin 6-hydroxylation (CYP2A6), bupropion hydroxylation (CYP2B6), rosiglitazone hydroxylation (CYP2C8), tolbutamide 4-methylhydroxylation (CYP2C9), S-mephenytoin 4'-hydroxylation (CYP2C19), dextromethorphan O-demethylation (CYP2D6), chlorzoxazone 6-hydroxylation (CYP2E1), and midazolam 1'-hydroxylation (CYP3A) were tested using human liver microsomes. The HMs Woohwangcheongsimwon suspension and Hwanglyeonhaedok-Tang strongly inhibited CYP2B6 and CYP2D6 isoform activity, respectively. These results suggest that some of the HMs used in Korea have potential to inhibit CYP isoforms in vitro. Although the plasma concentrations of the active constituents of the HMs were not determined, some herbs could cause clinically significant interactions because the usual doses of those individual herbs are several grams of freeze-dried extracts.

Evaluation on Pharmacological Effects and Compound Contents of Hwangryunhaedok-tang formulation for Tablet (황련해독탕을 함유하는 정제 개발과 성분함량 및 약리효과 평가)

  • Objectives : Hwangryunhaedok-tang (HRHDT) is one of the well-known prescription herbal drugs of Korean herbal medicine, which has been widely used for the treatment of various bacterial and inflammatory diseases. This study was conducted in order to develop the tablet formulations of HRHDT and compare its efficacy with the other commercial formulations. Methods : Corresponding herbal medicines comprising of HRHDT were extracted with water for 3 hr at $95{\sim}100^{\circ}C$ and then vacuum dried. Subsequently, some pharmaceutical excipients such as microcrystalline cellulose, croscarmellose sodium, magnesium stearate, etc were used to prepare the HRHDT tablets. The contents with characterizing components of HRHDT tablet was compared with the HRHDT decoction. The contents of characterizing components were analyzed with HPLC. Furthermore, we investigated the anti-inflammatory and anti-oxidative abilities of two different commercial HRHDT granules (HJP-1 and HJP-2) and were compared with that of the formulated HRHDT tablets. The anti-oxidant properties of HRHDR were studied using the 1,1-diphenyl-2-picryhydrazyl (DPPH) radical, contents of total flavonoid and polyphenol. In addition, based on this result the anti-inflammatory effects have verified by mechanism from LPS- treated Raw264.7 macrophages. Results : The results demonstrated that HRHDT tablets showed more anti-inflammatory and anti-oxidative effects than HJP-1, HJP-2. Moreover, it showed more superior effects in terms of dose, usability and stability than the granules. Conclusion : Hence, we concluded that in order to improve the quality and efficacy of the Korean herbal medicine, it is necessary to develop appropriate methods and establish standardized techniques for the development of good formulations.

Qunatitative analysis of liquiritin and glycyrrhizin in glycyrrhizae radix by HPLC-MS/MS (HPLC-MS/MS에 의한 감초의 liquiritin과 glycyrrhizin의 분석)

  • Licorice, Glycyrrhizae Radix is widely used as a herbal medicines and a dietary supplements in East Asia. We employed high performance liquid chromatography electrospray ionization tandem mass spectrometry to determine liquiritin and glycyrrhizin in the Glycyrrhizae Radix. Liquiritin and glycyrrhizin in Glycyrrhizae Radix were ionized by positive ion pneumatically assisted electrospray and detected by HPLC-MS/MS in the multiple-reaction monitoring (MRM) mode using precursor ${\rightarrow}$ product ion combinations at m/z $436.2{\rightarrow}257.0$ and $823.4{\rightarrow}453.4$, respectively. The assay had a calibration range from 10 to 3,000 ng/mL. The limits of detection (LOD) of the liquiritin and glycyrrhizin were 0.4 ng/mL and 0.01 ng/mL, respectively. The reproducibility and repeatability (relative standard deviation) at different analyte concentrations varied from 103 to 113 % and 0.95 to 1.8 %, respectively. According to the above results, HPLC-MS/MS method permits assignment of tentative structures such as liquiritin and glycyrrhizin in the Glycyrrhizae Radix.

  • The discussion of intellectual property protection with TK, GR in WIPO begin Herbal medicines get a important basis in a development of modern new medicine. For the protection of intellectual property, we set up this study. For the protection of intellectual property with herbal medicines, We analyze as follows. First, we analyze the specific theory and character of traditional herbal medicines. Second, we analyze the existing system on protecting them in Korea. The intellectual property protection with herbal medicines must maintain the specific character of that. We must select the basic traditional herbal medicines. The next, we must protect from intellectual property on add and subtract, combine, create of the traditional herbal medicines. In order that we must construct database on the Herbal medicines.

Safety Assessment of Oriental Medicines and Their Preparations (한약 및 한약제제의 안전성 평가)

  • This study was carried out to evaluate the safety of circulated oriental medicines and their related preparations in Gyeonggi-do. Total 366 samples (165 species) were analyzed about heavy metals, residual contents of sulfur dioxide ($SO_2$), and 68 samples were analyzed about preservatives. 17 samples (13 species, 4.6%) were exceeded the legal limit of heavy metal. The concentrations over the legal limit for Pb, Cd, As and Hg were 6.1~19.2 mg/kg, 0.4~0.7 mg/kg, 6.9 mg/kg and 0.7 mg/kg, respectively. In particular, the exceeding ratio (5.9%) of the legal limit of heavy metals in foreign products was 3.3 times more than domestic products (1.8%). 13 samples (10 species, 3.6%) exceeded the limit of residual sulfur dioxide and the concentration ranges were 105 to 428 mg/kg in domestic products, on the other hand foreign products were from 114 to 2,468 mg/kg. The mean concentration over the limit of residual sulfur dioxide of foreign products (804 mg/kg) was 2.4 times more than domestic products (338 mg/kg). In studying of the preservatives in oriental preparation, the contents of dehydroacetic acid (48.9~64.1%) in 3 samples of labeled solutions were under the labeled preservative contents and the contents of benzoic acid (139.9%) in 1 sample of labeled pill product was exceeded the labeled preservative contents. The numbers of the detected preservatives in unlabeled solutions, pills and granules were 4, 11 and 7, respectively.
