An Expert Survey for Developing the Pattern Diagnosis Instrument of Acne (여드름 변증 유형 도구 개발을 위한 전문가 설문조사 연구)

  • Objective : The purpose of this study was to develop a Pattern Diagnosis Instrument of Acne in traditional Korean Medicine. Methods : Data was collected by structured survey papers from 20 professors of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society(Delphi method). We investigated importance rates of the symptoms of each Pattern in the survey papers, then calculated Weighted Value of the symptoms. Results : Nine objective symptoms and nine subjective symptoms were selected by investigated importance rate in the survey from 20 professors of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society. In Wind-Heat pattern(WHP, 風熱型), inflammatory lesion of acne was the most important symptom. Oily skin, nodule and effect of menstruation are the most important symptoms in Dampness-Heat pattern(DHP, 濕熱型), Phlegm-Stasis pattern(PSP, 痰瘀型) and Disharmony of the thoroughfare and conception vessels pattern(DTCVP, 衝任不調型) each. Conclusions : The results might provide backgrounds and methods for clinical care of acne and follow-up study.

The Study on the Korean Traditional Medical Treatment and System of Collaborative Practice Between Korean Traditional Medicine and Western Medicine for Dementia: Based on Analysis of Questionnaire Survey in Professional Group (치매의 한방치료와 한양방 협진치료에 관한 전문가 집단 설문연구)

  • Objectives : Recently, the elderly population and dementia prevalence have been rapidly increasing in Korea. Dementia is a concept of syndrome composed of characteristic symptoms. However, there have been no approved treatments for dementia with proven efficacy. Therefore, we investigated Korean traditional treatment being used in clinic and collaborative practice for dementia in the professional group. Methods : We conducted a survey using questionnaires to professors of oriental neuropsychiatry for a month. Items of the questionnaire consisted of overall Korean traditional medical treatment for dementia, which included herbal medicine, acupuncture, moxibustion, cupping, psychotherapy and others. Results : 1. It was shown that Yukmijihwangtanggamibang, Jowiseungcheongtang, Seonghyangjeonggisan have been used the most in herbal medicine. 2. Chechim, Saamchimbeop, and scalp acupuncture have been used the most in acupuncture treatment. 3. In Korean traditional psychotherapy, Ijeongbyeongi, Jieongoronyobeop, Gigongyobeop have been used, and effective in anxiety, agitation, improvement of comprehension for caregiver and environment. 4. Most professors answered that collaborative practice between Korean traditional medicine and western medicine has been needed for dementia. They suggested that herbal medicine, followed by acupuncture, and moxibustion can be used with western medicine simultaneously. Conclusions : The survey revealed that professors use herbal medicine and acupuncture as main treatments for dementia. They share perspective regarding the need of long period to treat dementia patients, and there are needs of collaborative practice for dementia.

Developing Pattern Identification Questionnaire of Allergic Rhinitis (알레르기 비염의 변증을 위한 설문문항 개발)

  • Objectives : The prevalence of allergic rhinitis (AR) is 10-40% worldwide and there are many demands for Traditionl Medicine (TM) treatment for AR. Pattern identification (PI) is essential process in diagnosis and treatment of TM, however, objective assessment measurement for PI of AR in TM is lacked. Methods : PI questionnaire for AR was developed in 2008 by specialists in the department of Otolaryngology of TM, based on systemic and nasal symptoms and signs. However, that questionnaire had many problems in application to clinical practice and has since been revised based on several studies and delphi method. Consequently, the PI questionnaire for AR version 3.0 has been developed. Results & Conclusions : This questionnaire classifies patients with AR as possessing lung-heat, lung-cold, or spleen qi deficiency based on nasal symptoms and general conditions of AR patients. This is first questionnaire for pattern identification of AR. We plan to conduct a validation and reliability study, and revise the questionnaire based on the results of this study.

A Study on the Development of the Questionnaire for Pattern Identification of Liver-qi Stagnation Infertility (간울형 불임 변증 설문지 개발 연구)

  • Objectives: The aim of this study is the development of the questionnaire for pattern identification of Liver-qi stagnation infertility. Methods: We conducted a literature review and retrieved the symptoms and diagnosis from Korean and Chinese literatures which mentioned pattern identification of Liver-qi stagnation infertility. Based on the findings, We conducted three times expert Delphi surveys on selection of question items and determination of weight. Results: 12 questionnaire items for subjects and 4 questionnaire items for researchers were selected from 19 references. From expert delphi surveys, we finally determined 14 questionnaire items (10 items for subjects, 4 items for researchers) which are clinically significant and obtained weight of question items. Conclusions: Questionnaire for pattern identification of Liver-qi stagnation infertility was developed through experts' discussion. Further study is required to identify the validity and reliability of this pattern identification instrument for Liver-qi stagnation infertility.

Study about Correlation between BMI, Diet-situation and Psycological Factors: Self-esteem, Depression, Restraint Eating, Disinhibition, and Hunger, using Internet Survey (인터넷 설문을 통하여 조사한 여성의 비만도와 자존감, 섭식요인, 우울감의 상관관계와 다이어트진행에 따른 섭식요인의 변화)

  • Objectives : This study examined the association between body mass index(BMI) and psychological factors; self-esteem, depression, restraint eating, disinhibition, and hunger. The difference of TFEQ(restraint eating, disinhibition and hunger) was analyzed in variety of current diet situations. Methods : The research was based on a sample of people who visited our website and submit the result of self test(self-esteem n=3183; TFEQ n=5167; BDI n=2457). Results : Self-esteem and BDI was slightly correlated with BMI. There is negative correlation between restraint eating and BMI and positive correlations were observed between hunger and BMI, and disinhibition and BMI. We studied diet situation and eating habits. We divided the sample into 4 groups in accordance with self-submitted result: non-diet group(n=2806), diet-failed group within 3 months group(n=371), maintaining group after successful-diet(n=233), on-going diet group(n=1757) As to restraint eating, non-diet group showed the lowest score in restraint eating and maintaining group after successful-diet showed the highest. The score in disinhibition and hunger was significantly lower in maintaining group after successful-diet group than one in diet-failed group. Conclusion : These results suggest that it would be essential to treat these psychological factors such as depression and self-esteem in weight loss program. Considering the result in diet-situation and TFEQ, we insist that restraint-eating and disinhibition should be controlled. If these psychological factors were not treated in place, even the successful diet might turn out to be a worse situation like binge-eating

A Survey on Korean Medicine Treatment for Traffic Injuries: A Second On-line In-depth Survey (교통사고 상해 증후군의 한의 임상진료 현황 조사: 2차 온라인 심층 설문 조사)

  • Objectives To understand current status of clinical practice patterns on Korean Medicine treatment practice of traffic injury patients by 2nd on-line survey for developing Korean Medicine treatment clinical pratice guideline (CPG) for traffic injuries. Methods A revised questionnaire about current practice patterns of Korean Medicine treatments for traffic injuries was distributed by e-mail survey to 19,385 respondents of Korean Medicine doctors from May 21th in 2018 to June 21th in 2018. All data were statistically analysed. Results The response rate was 3.5%. Most of the respondents felt necessity of the development of Korean Medicine CPG for traffic injuries. The results showed the detailed usage and trend of current Korean Medicine treatment such as acupuncture, pharmacopuncture, moxibustion, Chuna manual therapy, cupping, Korean Medicine physiotherpy for traffic injury patients. Conclusions This survey helps to determine current Korea Medicine treatments' clinical practice patterns for traffic injury patients. All data from the survey will provide basic data for future clinical studies and adaptation for the revision of Korean Medicine CPG for traffic injuries.

A Study on Correlation of Transvaginal Sonography Findings, CA-125 and Blood Stasis Pattern for 15 Adenomyosis Patients (자궁선근증 환자 15명의 초음파 검사 소견, CA-125와 어혈증(瘀血證) 설문지 점수와의 상관성 규명)

  • Objectives: The purpose of this study is to know the correlation of transvaginal sonography findings, CA-125 and blood stasis pattern for 15 adenomyosis patients. Methods: We got questionnaires from 15 patients among 243 patients who had hysterectomy related with adenomyosis in the department of obstetrics and gynecology, OO medical center from April 11th 2010 to November 15th 2010, and analyzed them. We conducted correlation analysis between age, pain(VAS), duration of pain and blood stasis score, and between difference and sum of uterine walls, CA-125 and blood stasis score. For statistics, we used Spearman correlation coefficient and SPSS version 17.0 for windows. Results: The correlation between sum of uterine walls and CA-125 showed statistical significance. The correlation between CA-125 and blood stasis score showed statistical significance. There is no statistically correlation between age, VAS, duration of the pain and blood stasis score. There is no statistically correlation between difference of uterine walls and CA-125, and between difference and sum of uterine walls and blood stasis score. Conclusion: The result showed that sum of uterine walls is related with CA-125. We need a further study to develop blood stasis indicators with a validated questionnaire, and more subjects involved at multiple institutions.

A Study on Relations between MRS, MENQOL, HRV, Yin-Deficiency Questionnaire in Menopausal Woman with Hot Flush (안면홍조 증상을 호소하는 갱년기 여성의 MRS, MENQOL, HRV, 음허(陰虛)설문의 상관성 연구)

  • Purpose: To evaluate the correlation of MRS, MENQOL, HRV, Yin-Deficiency Questionnaire and the clinical use of Yin-Deficiency Questionnaire by comparison with MRS, MENQOL, and HRV. Methods: The participants were 21 women(44-58yr) with hot flush who were not currently on hormone therapy. The evaluating index of this trial are MRS, MENQOL, HRV, Yin-Deficiency Questionnaire. Results: The MRS and the MENQOL were significantly correlated with each other. The MRS and the HRV(LF/HF ratio) were significantly correlated with each other. The MENQOL and the HRV(LF/HF ratio) were significantly correlated with each other. The MRS somatic subscale and the Yin-Deficiency Questionnaire were significantly correlated with each other. The MENQOL and the Yin-Deficiency Questionnaire were significantly correlated with each other. The Yin-Deficiency Questionnaire and the HRV(LF/HF ratio) were significantly correlated with each other. Conclusions: The Yin-Deficiency Questionnaire is a valuable tool of oriental medicine for assessment of symptoms. The Yin-Deficiency Questionnaire were significantly correlated with the MRS somatic subscale, the MENQOL and the HRV(LF/HF ratio). Yin-Deficiency Questionnaire could serve as an adequate diagnostic instrument of oriental medicine for menopausal syndrome. Further large-scale study is needed for evaluation of the correlation of these measurements related with Menopausal syndrome and the clinical use of Yin-Deficiency Questionnaire.

A Survey on Treatment Trend for Allergic Rhinitis in Korean Medicine Clinic (알레르기 비염의 한의 진료 현황 설문조사)

  • Objectives : The purpose of this study was to investigate the treatment trend for allergic rhinitis in Korean Medicine. Methods : We conducted an online survey for Korean Medicine Doctors who were registered in the association of Korean Medicine. The questionnaire was consisted of patient characteristics, diagnosis status, treatment status, and future research needs. Results : Data from total of 396 respondents were analyzed. More than 70% of the patient came to the Korean Medicine Clinic after visiting the Western Medicine Clinic in 43.9% of the respondents. 55.6% of the respondents performed combination therapy. History taking, nasal examination, x-ray, and Korean Medicine diagnostic test were used for diagnosis. The mean duration of treatment ranged from $4.9{\pm}2.91$ to $15.2{\pm}8.45$ for pediatric and early childhood patients and from $17.5{\pm}16.15$ to $5.3{\pm}3.85$ weeks for adolescents and adult patients. The mean number of treatment times was from $9.8{\pm}7.00$ to $33.5{\pm}24.45$ for pediatric and early childhood patients and from $10.8{\pm}11.55$ to $40.4{\pm}48.18$ times for adolescents and adult patients. 64.5%, 48.0%, and 91.2% of the respondents used herbal medication in national health insurance coverage, herbal medication uninsured in health insurance and herbal prescription filled at each medical institution, respectively for pediatric and early childhood patients and 67.6%, 42.8% and 86.1% for adolescents and adult patients. 36.9% and 36.4% of respondents answered that the study of acupuncture and herbal medicine are needed preferentially. Conclusions : The results of this survey will be used to develop clinical practice guideline that reflect actual clinical practice.

A Study on the Validity of DSOM According to Alcohol Intake (알코올 섭취량에 따른 한의변증설문(DSOM)의 타당성에 대한 연구)

  • Purpose : This study was conducted to find the possibility of DSOM (Diagnosis System of Oriental Medicine) as a diagnostic method according to alcohol intake. Method : 49 men who drink alcohol over 40g per day and whose AUDIT scores were over 12 were allocated to the drinker group. 30 men who do not drink alcohol at all were allocated to the control (non-drinker) group. The study period was from June 15, 2006 to September 30, 2008. All of both groups were analyzed using DSOM. Result : There were some differences between the drinker group and the non-drinker group in stagnation of Ki(氣滯), deficiency of Yin(陰虛), insufficiency of Yang(陽虛), heat syndrome(熱), dryness (燥), and lung(肺). A group whose gamma-GTP is over twice the normal condition shows meaningful difference in stagnation of Ki(氣滯), heat syndrome(熱), dryness(燥), and insufficiency of Yang(陽虛). Conclusion : We found out that DSOM can be a diagnostic method on alcoholic liver disease patients. However, other studies to supplement it should be continued.
