Influencing factors of using Korean Medicine services - focusing on the 2017 Korean Medicine Utilization Survey (한방의료이용 선택 요인에 관한 연구 - 2017 한방의료이용실태조사를 중심으로)

  • Objectives: The aim of this study was to investigate influencing factors of using Korean medicine services (KMS) using the 2017 Korean Medicine Utilization Survey (KMUS). Methods: Demographic statistics of the survey were summarized and influencing factors of the KMS experience and the intention to visit KMS were analyzed using logistic regression model with complex sample design. Influencing factors were specified based on Andersen's behavioral model of health care utilization and factors associated with individual recognitions of KMS. Additionally, using the ordinary logistic regression model without complex sample design, the survey data were analyzed to compare the results. Results: In the logistic regression analysis, sex, age, health condition, presence of chronic disease, a degree of knowledge about Korean Medicine, and a view about herbal medicine safety were statistically significant both in the KMS experience, and the intention to visit KMS. Marital status was statistically significant in the KMS experience, while family income, a view about the cost of KMS were statistically significant in the intention to visit KMS. Conclusion: Individual recognitions of KMS and enabling components should be considered when establishing KMS policies. In addition, future studies analyzing KMUS need to take into account the complex sample design features of the survey to avoid statistically misleading results.

A Case-Study of Taeeumin Patient with Atypical Parkinsonian Disorders Having Dizziness and Gait Disturbance (비정형파킨슨증후군 태음인의 어지럼증과 보행장애 치험례)

  • A 52-year-old female diagnosed with Atypical Parkinsonian Disorders (APD) about one year ago suffered from dizziness and gait disturbance. The patient was identified as Taeeumin esophagus-cold lung-dry symptomatology pattern and was treated with Jowisengcheong-tang. The patient's symptoms of dizziness and night sweats were assessed using Global Assessment Scale (GAS). And, the Unified Multiple System Atrophy Rating Scale (UMSARS) and the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) were used to assess the overall function of the patient. The patient's symptoms of dizziness and night sweats were significantly improved, and UMSARS and MDS-UPDRS scores were decreased. This case showed that Sasang constitutional medicine treatment can improve APD symptoms and improve the quality of life of the patient.

Anti-inflammatory Effects of Naetakbaekryeom-san (내탁백렴산 추출물의 항염증 효능 연구)

  • Objectives This study was conducted to confirm the anti-inflammatory effect of Naetakbaekryeom-san (NTB), and whether it could be another treatment for inflammatory diseases. Methods The NTB water extract was extracted with hot water at 100℃ for 2 hours, concentrated at 80℃ under reduced pressure, and used. After 2 hours of pretreatment with NTB and positive control Bay11-7082, nitric oxide (NO), inducible NO synthase (iNOS), interleukin (IL)-6, IL-1𝛽, tumor necrosis factor alpha (TNF-𝛼) were measured in RAW264.7 cells activated with lipopolysaccharides (LPS) 500 ng/mL. After 2 hours of pretreatment with NTB, the anti-inflammatory effect of NTB was evaluated by measuring nuclear factor kappa-light-chain-enhancer of activated B cells (NF-𝜅B) in RAW264.7 cells and 293T cells activated with phorbol 12-myristate 13-acetic acid (PMA) 30 ng/mL. Results In RAW264.7 cells activated with LPS, NTB at concentrations of 0.1, 0.3, and 1.0 mg/mL showed no cytotoxicity, significantly inhibited NO production and inhibition of iNOS expression. TNF-𝛼 cytokine levels was not regulated, but NTB at each concentration inhibited the production of IL-1𝛽 and IL-6, and the effect was higher than that of the positive control Bay11-7082 (20 𝜇M). In PMA-activated RAW264.7 cells and 293T cells, each concentration of NBT decreased the NF-𝜅B transcriptional activity, with the greatest decrease at 1 mg/mL. Conclusions These results demonstrated the anti-inflammatory effect of NTB water extracts, but further studies such as comparison of anti-inflammatory effects and antioxidant effects by NTB component, comparison of effects according to extraction solvents, and clinical studies are needed.

The Efficacy of Low Frequency Electroacupuncture Therapy for Weight Loss in Obese Korean Women (저주파 전침 위주의 한방비만치료의 체중감량 효과 및 관련 인자에 관한 연구)

  • Purpose: We investigated the efficacy of electroacupuncture therapy in obese Korean women in order to make basic guidelines for the use of oriental medicine in treating obesity. Methods: Forty women were divided into two groups and each was treated 12 times: 1) one group was treated using electroacupuncture (EA) involving auricular acupuncture and physical treatment, without exercise (EA group, n=10), and 2) the other group was treated using electroacupuncture and exercise (EA plus exercise group, n=30). The EA was applied to subcutaneous fat tissue manually. Body weight was evaluated every visit and body composition was checked at the $1^{st}$ and $12^{th}$ visits. Statistical comparisons were made using SPSS13.0. We compared the weight loss, body mass index (BMI), and body composition before and after treatment using the pairedt-test. The correlations between weight loss and BMI and age were examined. Results: After the $12^{th}$ treatment, both groups showed significant reductions in body weight, BMI, body fat, and body fat percentage. The weight loss averaged 4.58${\pm}$3.87% in the EA group and 4.69${\pm}$3.10% in the EA plus Exercise group. The reduction in skeletal muscle was significant in the EA group, but not significant in the EA plus Exercise group. The speed of the weight loss was correlated with age using Pearson's correlation coefficient. Based on BMI, normal and obese subjects in the EA plus Exercise group had significant reductions in body weight, BMI, body fat, and body fat percentage; the obese subjects also had a significant reduction in skeletal muscle. Conclusion: Combined EA therapy may be effective for weight reduction. Age and BMI were related to weight loss using EA therapy.

Experimental study about the effect of several herbs on collagen synthesis (수종(數種) 한약재의 콜라겐 생합성 효과에 대한 실험적 연구)

  • Background & Objective : This study was performed to find a new anti-aging, anti-wrinkle agent. Ten kinds of hers were evaluated by proliferation, cytotoxicity, collagen synthesis tests in cultured human dermal fibroblast. Methods : MTT assay was carried out to check cell proliferation, cytotoxic effect of each herbal medicine. Collagen synthesis was investigated by the amount of propeptide using procollagen type-Ⅰ C peptide(PIP) ELISA kit(Takara). Results and Conclusions : There are no cytotoxicity in all group herbs by MTT assay. Collagen synthetic ability was greatest in LYCII FRUCTUS(LF) and was excellent in order of Rehmanniae Radix Preparat(RRP) > Nelumbinis Semen(NS) > Asparai Radix(AR) > Schizandrae Frucus(SF).

  • Paranasal sinusitis is a common disease in the otorhinolaryngology area. It is the change of inflammation at the mucous membrane which surrounds paranasal sinus. Chronic paranasal sinusitis is a chronic inflammation disease with purulent, mucous rhinorrhea, postnasal drip and cough. The attacked period continues above three months and it repeats exacerbation and remission. We diagnosed and treated an outpatient who complains postnasal drip of chronic paranasal drip of chronic paranasal sinusitis based on the Oriental medical theories and had good result.

Four Cases of Stroke Patients with Defective Vision Treated with Venesection on Palpebral Conjunctiva (眼瞼結膜 刺絡療法(棘鍼療法)을 시술한 中風 환자의 眼昏 치험 4例)

  • Objective: This study was performed to evaluate the effects of venesection on palpebral conjunctiva, on the stoke patients complaint of defective vision. Methods: We studied four selected stroke patients with defective vision which start with stroke. Venesection was performed with syringe needleonce or several times on upper or lower palpebral conjunctiva. After every venesection we washed the eye(s) with normal saline. We evaluated the effects by VAS. Results & conclusion: Venesection on palpebral conjunctiva was efficacious against defective vision occured on the stoke patients. But more clinical & scienific trials are expected to follow this study.

Significant Reperfusion on $^{99m}Tc-HMPAO$ SPECT in a Case of Subacute MCA Infarction (한의학적 치료로 $^{99m}Tc-HMPAO$ SPECT상 현저한 재관류를 보인 아급성기 중대뇌동맥경색 환자 1례)

  • For ischemic cerebrovascular disease, it is well known that early luxury perfusion is related to a good clinical outcome and single photon emission computed tomography(SPECT) has the potential for providing useful information about regional cerebral blood flow. We report one case of Rt. MCA infarction mainly treated by oriental medicine and revealed luxury perfusion without thrombolysis. In acute stage, neurological deficits of the patient were very severe. 99mTc-HMPAO SPECT images obtained 10days after the attack showed large perfusion defect in the Rt. MCA territory. We followed up 99mTc-HMPAO SPECT 40days after the ictus. Despite of the poor early perfusion, we found considerably improved perfusion and neurological improvement.

  • Objectives: The purpose of this study is to evaluate the effects of Gamitonggyung-tang on Primary Dysmenorrhea. Methods: We treated 33 cases of primary dysmenorrhea patients with Gamitonggyung-tang for three menstrual cycles. We estimated the effects by Visual Analog Scale (VAS), Menstrual Distress Questionnaire (MDQ) and checked amount of the analgesics taken during menstrual phase. Results: After treatment, the mean of VAS, MDQ Score and amount of the analgesics taken were decreased. Conclusions: This case shows that Gamitonggyung-tang is effective in treating primary dysmenorrhea patients.

A Case Report of Sweating and Palpitation due to Chemotherapy for Cancer in a Soeumin Patient with Primary Central Nervous System Lymphoma (원발성 중추신경계 림프종으로 항암화학요법 후 발생한 자한, 심계정충을 호소하는 소음인 치험 1례)

  • Objectives The purpose of this case study was to report improvements of a Soeumin patient with sweating and palpitation after chemotherapy for primary central nervous system lymphoma by diagnosing and treating as Yang Depletion Symptomatology. Methods The Soeumin patient was administered Bojungikgi-tang and treated with acupuncture. Global assessment scale (GAS) was used to assess the improvements of symptoms. Results Sweating was improved to GAS 20 and palpitation was gone away after using Sasang constitutional medicine treatment. Conclusions This study shows that Sasang constitutional medicine can be effective treatment for side effects caused by chemotherapy for cancer.
