Comparative Studies on Absorbed Dose by Geant4-based Simulation Using DICOM File and Gafchromic EBT2 Film (DICOM 파일을 사용한 Geant4 시뮬레이션과 Gafchromic EBT2 필름에 의한 인체 내 흡수선량 비교 연구)

  • Monte Carlo method has been known as the most accurate method for calculating absorbed dose in the human body, and an anthropomorphic phantom has been mainly used as a method of simulating internal organs for using such a calculation method. However, various efforts are made to extract data on several internal organs in the human body directly from CT DICOM files in recent Monte Carlo calculation using Geant4 code and to use by converting them into the geometry necessary for simulation. Such a function makes it possible to calculate the internal absorbed dose accurately while duplicating the actual human anatomical structure. Thus, this study calculated the absorbed dose in the human body by using Geant4 associating with DICOM files, and aimed to confirm the usefulness by compare the result with the measured dose using a Gafchromic EBT2 film. This study compared the dose calculated using simulation and the measured dose in beam central axis using the EBT2 film. The results showed that the range of difference was an average of 3.75% except for a build-up region, in which the dose rapidly changed from skin surface to the depth of maximum dose. In addition, this study made it easy to confirm the target absorbed dose by internal organ and organ through the output of the calculated value of dose by CT slice and the dose value of each voxel in each slice. Thus, the method that outputs dose value by slice and voxel through the use of CT DICOM, which is actual image data of human body, instead of the anthropomorphic phantom enables accurate dose calculations of various regions. Therefore, it is considered that it will be useful for dose calculation of radiotherapy planning system in the future. Moreover, it is applicable for currently-used several energy ranges in current use, so it is considered that it will be effectively used in order to check the radiation absorbed dose in the human body.

Comparison of Film Measurements, Convolution$^{}$erposition Model and Monte Carlo Simulations for Small fields in Heterogeneous Phantoms (비균질 팬텀에서 소조사면에 대한 필름측정, 회선/중첩 모델과 몬테 카를로 모사의 비교 연구)

  • Intensity-modulated radiation therapy (IMRT) often uses small beam segments. The heterogeneity effect is well known for relatively large field sizes used in the conventional radiation treatments. However, this effect is not known in small fields such as the beamlets used in IMRT. There are many factors that can cause errors in the small field i.e. electronic disequilibrium and multiple electron scattering. This study prepared geometrically regular heterogeneous phantoms, and compared the measurements with the calculations using the Convolution/Superposition algorithm and Monte Carlo method for small beams. This study used the BEAM00/EGS4 code to simulate the head of a Varian 2300C/D. The commissioning of a 6MV photon beam were performed from two points of view, the beam profiles and depth doses. The calculated voxel size was 1${\times}$1${\times}$2$\textrm{cm}^2$ with field sizes of 1${\times}$1$\textrm{cm}^2$, 2${\times}$2$\textrm{cm}^2$, and 5${\times}$5$\textrm{cm}^2$. The XiOTM TPS (Treatment Planning System) was used for the calculation using the Convolution/Superposition algorithm. The 6MV photon beam was irradiated to homogeneous (water equivalent) and heterogeneous phantoms (water equivalent + air cavity, water equivalent + bone equivalent). The beam profiles were well matched within :t1 mm and the depth doses were within ${\pm}$2%. In conclusion, the dose calculations of the Convolution/Superposition and Monte Carlo simulations showed good agreement with the film measurements in the small field.

혈관내벽에 홀뮴-166 방사선 분할 조사시 흡수선량 분포

  • 경피적관상동맥성형술(CPTCA)이나 스텐트삽입술 후에 발생하는 재협착을 방지하기 위한 방사선을 조사하는 방법 중에 베타 입자를 방출하는 액체 선원을 catheter풍선 내에 넣어 일정 시간 방사선 조사 시키는 방법이 있다. 조사시킬 혈관의 길이가 길어 한번의 방사선 조사가 어려울 경우 영역을 분할하여 두 번에 나누어 조사할 경우가 있다. 조사영역의 겹치는 부근의 흡수선량이 고선량이나 저선량이 되는가를 알기 위하여 두 풍선간의 접근 거리에 따른 혈관내벽의 흡수선량 분포를 알아보았다. 풍선내의 액체 선원은 Ho-l66을 이용하였고 Ho-l66의 물리적 반감기는 26.8시간이고 최대에너지 1.85 MeV, 평균에너지 0.69 MeV와 최대에너지 1.77 MeV, 평균에너지 0.65 MeV를 갖는 베타 입자를 방출한다. Ho-l66 의 방사선 흡수선량을 측정하기 위하여 GafChromic 필름(Nuclear Associates, Carle Place, NY, USA)을 이용하였고, 방사선이 조사된 필름의 optical density는 videodensitometer(Wellhofer, Schwarzen-bruck, Germany)를 이용하여 값을 읽었다. Catheter 풍선은 직경이 3 mm 이고 길이가 20 mm인 것을 이용하였다. 혈관 내벽의 최대 흡수선량을 표준화하여 겹치는 부분의 흡수선량 분포를 접근 거리에 따라 구하였다. 또한 몬테카를로 시abf레이션으로 확인하였다. 두 풍선의 겹치는 부근의 선량 분포는 풍선 중앙에서 중앙사이의 거리가 21 mm 일 때 중앙에서 20% 증가하였고, 거리가 22 mm일 때와 23 mm일 때 각각 10%와 40%의 감소를 보였다. 풍선 도자의 풍선 안에 베타입자 방출 액체 선원을 넣어 혈관내벽에 방사선 조사하는 방법은 비정거리가 짧아 혈관 내벽 부근에만 방사선을 조사시키고 그 외 중요 장기에는 영향을 덜 미치는 장점이 있다. 그러나 혈관 내벽 표면으로 부터의 거리에 따라 흡수선량이 급격히 떨어지는 분포를 이루기 때문에 두 개의 풍선이 겹치는 부근의 흡수선량은 아주 작은 접근 거리에서도 급격한 변화를 보였다. 따라서 시술 중에 겹치는 부분을 아주 적게 분할하여 정확하게 차례차례로 조사시키기 위해서는 신중한 거리 조정을 하여야 한다.

Quality Assurance of CORVUS Planning System for Intensity Modulated Radiation Therapy (CORVUS Planning System을 사용한 세기조절방사선치료의 정도관리에 관한 연구)

  • The intensity modulated radiation therapy (IMRT) is believed to be on of the best treatment techniques for the goal of radiation therapy: to irradiate fatal dose to tumor region while minimizing dose to critical organs. It is essential to have comprehensive quality assurance program to assure the precision and the accuracy of the treatment due to the characteristic of the IMRT. The quality assurance technique for the Corvus treatment planning system was developed and its effectiveness was tested with the treatment planning of H&N region. Acrylic phantom, film and ionization chamber were used for this study, the discrepancy between the treatment planning and the film measurements showed 0.03 cm and 0.28 cm for the 90% of isodose line in each directions. Dose measurements showed 1% and 1.2% differences for ionization chamber and TLD, respectively. This concluded that the system can be used for clinic.

The Influence of Volume Effect in 2D-array Ion Chamber on the Measurement of IMRT Dose Distribution (2차원 배열형 이온함의 부피효과가 세기조절방사선치료의 선량분포 측정에 미치는 영향)

  • We evaluated the influence of volume effect on the measurement of IMRT dose distribution by comparing a 2D-array ion chamber and other dosimeters. Matrix phantom which is a 2D-array ion chamber having volume effect was compared with beam image system and film for the measurement of dose distribution. Five intensity-modulated radiation therapy plans were created using five fields in thevirtual phantom. The measured dose distribution was compared with the calculated one by radiation treatment planning system and analysis program. We evaluated the conformity of dose distribution by calculating correlation coefficients and gamma values. The highest error rate of 1.3% was associated with matrix phantom in which volume effect in small field sizes was substantial.

  • This paper described an improved technical method required for proper evaluation of personnel exposures by means of the photographic dosimeter developed by KAERI in lower gamma or X-ray energy regions, with which response of the dosimeter varies significantly. With calibration of the dosimeter in the energy range from 30 to 300 keV, the beam spectrum was carefully selected and specified it adequately. The absorber combinations and absorber thickness used to obtain the specified X-ray spectra from a constant potential X-ray machine were determined theoretically and also experimentally. A correlation between the density and exposure for the four separate energies, such as $49\;keV_{eff},\;154\;keV_{eff}\;250\;keV_{eff}\;and\;662\;keV$, is experimentally determined. As a result, it can be directly evaluated the exposure from the measured response of dosimeter.

Impact of Computed Tomography Slice Thickness on Intensity Modulated Radiation Therapy Plan (전산화단층촬영 슬라이스 두께가 세기변조방사선치료계획에 미치는 영향)

  • $\underline{Purpose}$: This study was to search the optimal slice thickness of computed tomography (CT) in an intensity modulated radiation therapy plan through changing the slice thickness and comparing the change of the calculated absorbed dose with measured absorbed dose. $\underline{Materials\;and\;Methods}$: An intensity modulated radiation therapy plan for a head and neck cancer patient was done, first of all. Then CT with various ranges of slice thickness ($0.125{\sim}1.0\;cm$) for a head and neck anthropomorphic phantom was done and the images were reconstructed. The plan parameters obtained from the plan of the head and neck cancer patient was applied into the reconstructed images of the phantom and then absorbed doses were calculated. Films were inserted into the phantom, and irradiated with 6 MV X-ray with the same beam data obtained from the head and neck cancer patient. Films were then scanned and isodoses were measured with the use of film measurement software and were compared with the calculated isodeses. $\underline{Results}$: As the slice thickness of CT decreased, the volume of the phantom and the maximum absorbed dose increased. As the slice thickness of CT changed from 0.125 to 1.0 cm, the maximum absorbed dose changed ${\sim}5%$. The difference between the measured and calculated volume of the phantom was small ($3.7{\sim}3.8%$) when the slice thickness of CT was 0.25 cm or less. The difference between the measured and calculated dose was small ($0.35{\sim}1.40%$) when the slice thickness of CT was 0.25 cm or less. $\underline{Conclusion}$: Because the difference between the measured and calculated dose in a head and neck phantom was small and the difference between the measured and calculated volume was small when the slice thickness of CT was 0.25 cm or less, we suggest that the slice thickness of CT should be 0.25 cm or less for an optimal intensity modulated radiation therapy plan.

The Verification of Dosimetric Characteristics of the 3-D Compensator with the Exit Beam Dose Profile (Exit Beam Dose Profile을 이용한 3차원 보상체의 성능확인)

  • Dose compensators have been widely used in radiotherapy fields. But, few reliable verification methods have been reported. We have developed the verification method for the evaluation of the effect of dose compensator using exit beam dose profile. The exit beam dose profiles were measured with and without dose compensator. For this purpose X-Omat V films and lead screened cassettes are used and dose distibutions are compared. Phantom data are collected using CT simulator(Picker, AcQ Sim) and compensator information can be obtained from Render Plan 3-D planning System. Aluminum Compensators are generated by computer controlled milling machine. The real dose distribution in the phantom and the exit beam dose profile can be obtained simultaneously with the films in the phantom and the opposite site of the beam. Dose compensations effects for oblique beam, parallel opposing beam and inhomogeneous human phantom can be obtained using above tools. And we could simate those effects with exit beam dose profile using the method that we have developed in this study.

  • This study is investigated dose change on intra-oral radiography when same conditions under the others unit and same unit under the different exposed conditions. Three different radiation devices were studied. Exposure to the upper anterior, premolar and molar on the variant time and dose measure was using semiconductor radiation dose meter. Obtained film density value was analyzed to the belong in the range of diagnosis. Results for dose of each region were less dissimilar between the maximum and minimum. Its value was different 10 times as many as 3 times. In addition, the range of film density was 2.10 ~ 2.95. These values were exceeded on the allow density of diagnostic value '0.25 ~ 2.0'. Even if the same device and the same condition, measured dose was considerable differance and film density was showed show the inappropriate density range. Those can be caused the patient's re-take and patient's diagnostic errors so patients has affected direct and indirect radiological harm. Therefore, dental radiography devices will be required periodical maintenance and also provided standard on the exposure and processing conditions.

Surface Dose Measurement of Electron Beam within the Magnetic Field Variation (자기장 내에서 전자선의 표면선량 변화 측정)

  • Purpose: This paper describes a electron field presence of magnetic field, intensity and shape surface dose variation to clinical application possibility. Materials and Methods: The using 6 MeV electron and $10{\times}10\;cm^2$ field size, 9 hole to shielding block make the by measure the film, when the magnetic field position inside and outside of the X-Omat film and parallel plate ionization chamber using the surface dose measured. Results: Present of 4 cm to the side at angle about 3 degree from beam center, use of ring type magnetic is 0.9% increase the surface dose, lens block located in the magnetic field the surface dose 1.58% increase, half magnetic field's position on the side of them at the field center of the 3.6% increase of the surface dose. Conclusion: Surface dose variation is with magnetic field about the mean electron beam of progress direction change, orbit region patient's is inconvenient without surface dose increase percentage case goodness will be used as a useful way.
