Pulmonary Infarction of Left Lower Lobe after Left Upper Lobe Lobectomy - 1 case report - (좌 상엽의 폐엽 절제 후 발생한 좌하엽의 폐 경색 치험 1례)

  • The remaining lung infarction is a rare but life-threatening complication after a thoracic operation and trauma. We report a case of this rare complication after the left upper lobectomy due to pulmonary aspergilloma. The infarction of the remaining left lower lobe occurred due to kinking of the pulmonary vessels after the left upper lobectomy and the completion pneumonectomy was performed in the post-operative second day. Therefore, prompt diagnosis and treatment may be necessary to prevent morbidity and mortality associated with pulmonary infarction from torsion of pulmonary artery and vein.

  • Infarction of the lung usually results from pulmonary arterial obstruction. Pulmonary infarcts often become infected from bronchial contamination and may become lung abscesses, empyema, or bronchopleural fistula causing sepsis. Diagnosis is important for intensive therapy, since infection is prone to spread. Resection of the infarcted lung should be considered early in an attempt to control infection. A sixty-seven-year-old man was hospitalized with dyspnea. A computed tomographic scan of the chest showed left lower lobe infiltration and mild pleural effusion with pleural thickening. There was a thrombus in the left pulmonary artery leading from the lower lobe to the upper lobe artery. At operation, the left lower lobe was found to have complete hemorrhagic infarction. The left lower lobectomy was performed. The remaining thrombus was removed after the left main pulmonary arteriotomy. He has been followed up for 15 months and has done well with no recurrence of thrombus and infarction of the lung.

  • An eight-year-old boy was referred to our hospital with cough and high fever. His past medical history included a small sized ventricular septal defect (VSD) at birth. Transthoracic echocardiography disclosed a 10 x 6 mm vegetation on tricuspid valve, a small VSD and the moderate tricuspid valve insufficiency were found. Blood cultures grew methicillin-resistant staphylococcus aureus. Despite proper antibiotic therapy, fever was not controlled and his course was complicated by pulmonary infarction. The patient simultaneously underwent pulmonary resection and open heart surgery. Through the median sternotomy we performed open thrombectomy and lobectomy (right lower lobe) at first, and then vegetectomy, tricuspid valve repair, and direct closure of VSD were done under cardiopulmonary bypass.

  • Pulmonary infarction is rarely diagnosed with certainty except at postmortem examination. Part of this uncertainty is because of the inability to distinguish between hemorrhage, congestive a electasis, and necrosis clinically and radiographically. The pathogenesis of pulmonary infarction is poorly understood. It is dif%cult to induce pulmonary infarction in animals by ligation of the arterial supply to the lung. Many factors seem to be important in its pathogenesis, in addition to congestive heart failure, malignant tumor, thrombophlebitis, chronic obstructive lung disease, nephrotic syndrome, and postopeiative state. However, pulmonary infarction have not been reported in association with chest trauma. We report a case of pulmonary infarction associated with fractures of right clavicle and multiple ribs. A 45-year-old male had admitted due to right chest pain and dyspnea, which developed after right chest trauma occurred at about 3 weeks ago. He was treated at local clinics under the diagnosis of fractures of right clavicle and ribs until the admission. Chest CT disclosed a huge mass with central low density in right upper lobe, and small masses were also seen on both l ng fields. Open lung biopsy resulted in negativity for the malignancy. Clinical symptoms and radiological findings were not improved by conservative treatment. Right upper lobectomy was done eventually. The final diagnosis was pulmonary infarction. And, the patient has been well after operation.

  • Background: Pulmonary thromboembolism(PTE) is a life threatening disease that needs early diagnosis. Spiral CT angiography depict thromboemboli in the central pulmonary vessels with greater than 90% sensitivity and specificity, which approaches the results of pulmonary angiography in the Prospective Investigation of Pulmonary Embolism Diagnosis(PIOPED) study. This study was performed to evaluate the findings and the diagnostic value(clinical utility) of the spiral CT angiography with 2D image(multiplanar reformation) and 3D images(Shaded surface display, Minimal intensity projection) in the pulmonary thromboembolism. Methods: We retrospectively analysed spiral CT angiography and pulmonary angiography, lung scan and clinical recordings of 20 patients who had PTE diagnosed by spiral CT angiography(n=19 cases) or pulmonary angiography(n=l case) from September 1997 to August 1998. Among 20 patients who had underwent spiral CT angiography, 14 patients could be performed lung perfusion scan at the same time. We analyzed the vascular and parenchymal change in spiral CT angiogram. Dispersion measurement technique based on a self-seeding laser oscillation of a Fabry-Perot laser (Fabry-Perot 레이저의 자기궤환 레이저 발진을 이용한 색분산 측정법)

  • A simple dispersion measurement technique has been demonstrated by using a self-seeding laser oscillation of a Fabry-Perot laser through a closed loop. The optical pulses of different wavelengths emitted from the Fabry-Perot laser travel down an optical fiber and the group velocity difference between the pulses due to the chromatic dispersion of the optical fiber is measured through the self-seeding laser oscillation process. The dispersion parameter of the optical fiber is calculated from the measured group velocity difference. The performance of the proposed technique has been confirmed experimentally and the accuracy of dispersion parameter measurement was comparable to that of commercial instruments with expensive equipment and components. The repeatability of the proposed method was better than 0.5%.

  • The complications associated with the use of pulmonary artery catheter include dysrhythmias, heart block, pulmonary artery rupture, pulmonary infarction, endocardial damage, balloon rupture, arterial puncture, thromboembolism, air embolism, infection, pneumothorax, and knotting of the catheter. Knotting of the catheter is a rare complication and it should be anticipated if there is an excessive advancement of the pulmonary artery catheter beyond the normally expected distance. We report a successful surgical removal of knotted pulmonary artery catheter by sternotomy and cardiopulmonary bypass.

Meta-Analysis for Effect of Dietary Isoflavones on Breast Density and Hot Flush Suppression (이소플라본의 유방암 항암효능에 대한 메타분석)

  • For establishing the efficacy against breast cancer occurrence, women's mammographic breast density and hot flush remission related to pre and post-menopausal symptoms were selected as biomarkers. Meta analysis applied the final selection of 40 papers from Medline to assess the efficacy of isoflavone-rich soy or supplements versus placebo under randomized controlled trials. Interestingly, the exposure to the purified isoflavones was more effective than the exposure to the crude extract of soy isoflavones for hot flush suppression. The length and the amount of dosage dependency is the most appropriately suggested to be one year long with 50~100 mg/day of isoflavone-rich soy or supplements. Funnel plots was used to interpret the results, overall effect of isoflavones on breast density in post-menopausal women was revealed to be less effect [effect size: 0.062, 95% confidence interval (CI): 0.005 to 0.12], than that of pre-menopausal women (effect size: 0.101, CI: -0.003 to 0.205). The reason why breast density was found higher among the pre-menopausal than the post-menopausal women is that the phytoestrogens of dietary isoflavones were antagonized against estrogen by the basis of estrogen receptor binding affinity. Overall, the Meta analysis reported that isoflavone had limited influence on breast density by suppressing the expansion by only 2%, while it was more effective to suppressing the hot flush, showing a dramatic decrease of 23%.

Growth and morphological characteristics of Polygonatum species indigenous to Korea (한국산 둥굴레속(Polygonatum) 수집종의 생육 및 형태적 특성)

  • Morphological characteristics and growth pattern of 10 Polygonatum collections indigenous to Korea were examined to select the promising medicinal, edible resources and horticultural crops. Plant heights of I0 collections ranged from 15 to 102cm. Stem type was ascending or erect, and node numbers per a stem was 6.2 to 23.2. Phyllotaxis type was alternate or verticillate, and leaf shape was elliptical or linear. Leave numbers per a stem was 5.2 to 63.4, and bract types were classified into bracteate and nonbracteate. Flowers bloomed from May 7 to May 30, and flowering period was 5 to 13 days. Inflorescence types were classified into racemose, corymbose, and umbellate. Flower numbers per a stem was 1.5 to 125.2, and flower length was 13.1 to 30.2㎜. Perianth shapes were classified into tubular, constrict and urceolate. Surface colors of rhizome were pale yellow, pale brown, brown, and dark brown. As a result of this experiment, P. sibiricum, P. odoratum var. pluriflorum and P. odoratum var. thunbergii were thought to be useful as the medicinal and edible resources plants. On the other hand, P. odoratum var. pluriflorum 'Variegata', and P. odoratum var. maximowiczii, P. lasianthum. P. involucratum, P. desoulavyi, P. humile, and P. inflatum were thought to be useful as horticultural plants.
