Effective speech recognition system for patients with Parkinson's disease (파킨슨병 환자에 대한 효과적인 음성인식 시스템)

  • Since speech impairment is prevalent in patients with Parkinson's disease (PD), speech recognition systems suitable for these patients are needed. In this paper, we propose a speech recognition system that effectively recognizes the speech of patients with PD. The speech recognition system is firstly pre-trained with the Globalformer using the speech data from healthy people, and then fine-tuned using relatively small amount of speech data from the patient with PD. For this analysis, we used the speech dataset of healthy people built by AI hub and that of patients with PD collected at Inha University Hospital. As a result of the experiment, the proposed speech recognition system recognized the speech of patients with PD with Character Error Rate (CER) of 22.15 %, which was a better result compared to other methods.

Depressive Symptoms in Patients with Parkinson's Disease (파킨슨병 환자에서의 우울증상)

  • Objectives: The aim of this study was to investigate 1) the prevalence of depressive symptoms, 2) the severity of depressive symptoms, 3) the correlation of depressive symptoms with clinical variables, and 4) factors that contribute to depressive symptoms in patients with Parkinson's disease. Methods: One hundred eighteen patients with Parkinson's disease referred from the Parkinson's Disease Clinic of Dong-A University Hospital, Busan, Korea, completed a self-administered questionnaire package, which included basic demographic data, the Beck Depression Inventory, the Parkinson's disease quality of life questionnaire, the Symptom Checklist-90-Revision(SCL-90-R), and the Spielberger's State-Trait Anxiety Inventory. In addition, a structured interview and a complete neurological examination, including the Hoehn and Yahr stage, the motor part of the Unified Parkinson's Disease Rating Scale(some selected scales of UPDRS part III), the Schwab and England Activities of Daily Living scale(ADL), and the Korean version of Mini-Mental State Examination were performed. Results: 1) Based on BDI score, subjects were divided into four groups:severely(40.7%), moderately(13.6%) and mildly(12.7%) depressive and non-depressive(33.1%). 2) The severity of depressive symptom in Parkinson's disease was positively correlated with Hoehn and Yahr(H & Y) stage(r=0.34, p<0.0001), the severity of motor symptom(r=0.35, p<0.0001), and trait anxiety inventory(r=0.33, p<0.001). On the other hand, the severity of depressive symptom was negatively correlated with educational level(r=-0.34, p<0.001), ADL(r=-0.37, p<0.0001) and Parkinson's disease quality of life (PDQL)(r=-0.69, p<0.0001). Among several clinical variables, the PDQL was the most influential factor predicting whether the depressive symptom was present or not. Conclusion: This study suggests that depressive symptom is very prevalent among patients with Parkinson's disease. Data from this study indicate that medical staffs who take care of patients with Parkinson's disease should pay attention to finding and treating depressive symptom among their patients. With appropriate psychiatric intervention, patient's depressive symptom can be minimized or alleviated and thus, the quality of life in these patients is likely enhanced.

  • Purpose: The study was to assess I-123-N-(3-iodopropen-2-yl)-2[beta]-carbomethoxy-3[beta]-(4-cholorophenyl) tropane(IPT) SPECT in differential diagnosis among early stage of Parkinson's disease(PD) and essential tremor(ET) and normal control(NL) groups quantitatively. Materials and Methods: I-123 IPT brain SPECT of 50 NL, 20 early PD, 30 advanced PD, and 20 ET were performed at 20 minutes and 2 hours. Specific/nonspecific binding of striatum was calculated by using right and left striatal specific to occipital non-specific uptake ratio(striatum-OCC/OCC). Results: Mean value of specific/nonspecific binding ratio was significantly different between advanced PD group and NL group. However, significant overlap of striatal specific/nonspecific binding ratio was observed between PD group and ET group. Bilateral striatal specific/nonspecific binding ratios were decreased in advanced PD. Lateralized differences in the striatal uptake of I-123 IPT correlated with asymmetry in clinical findings in PD group. Conclusion: I-123 IPT SPECT may be a useful method for the diagnosis of PD and objective evaluation of progress of clinical stages. Care should be made in the differential diagnosis of early stage of PD and other motor disturbances mimicking PD such as ET in view of significant overlap in striatal I-123 specific/nonspecific binding ratio.

  • [ $[^{123}I]{\beta}$ ]-CIT [$2{\beta}$-carbomethoxy-$3{\beta}$-(4-iodophenyl)tropane]는 도파민 운반체 (dopamine transporter)에 특이결합하며 $[^{123}I]{\beta}$-CIT의 도파민 운반체 결합정도는 파킨슨병에서 도파민 뉴우런의 변성정도를 반영하는 것으로 제안되어 왔다. 이 연구의 주요 목적은 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT SPECT를 이용하여 측정된 $[^{123}I]{\beta}$-CIT의 선조체 결합지표들이 질병의 임상적 진행정도를 반영하는지를 검토하고, 간편화된 조직방사능비가 $[^{123}I]{\beta}$-CIT의 결합정도를 나타내는 정량적 지표로 이용될 수 있는지를 검증하는 것이었다. 파킨슨병 환자 30명($59{\pm}9$세, 평균${\pm}$표준편차: Hoehn-Yahr stage 1-3)과 정상인 6명 ($58{\pm}5$세)을 대상으로 $[^{123}I]{\beta}$-CIT SPECT 영상을 얻었다. $[^{123}I]{\beta}$-CIT 선조체 결합의 정량적 지표로서 (선조체 방사능-소뇌방사능)/소뇌방사능 비(specific binding ratio, SBR)와 추적자역학모델을 이용하여 측정한 선조체 결합능(binding potential)($k_3/k_4$)을 구하였다. 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT의 선조체 결합역학은 정상인에 비하여 현저하게 느렸으며 그 결합지표들은 정상인에 비하여 뚜렷하게 낮았다. 한편, 편측파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT 결합은 증상 반대쪽 선조체 뿐만 아니라 같은 쪽 선조체에서도 정상인에 비해 유의하게 감소되어 있었다. 파킨슨병 환자에서 $[^{123}I]{\beta}$-CIT 투여 후 24시간의 선조체 SBR 및 최대 SBR, 선조체 결합능은 모두, 유병기간, Hoehn-Yahr stage, UPDRS(Unified Parkinson's Disease Rating Scale) 총점, UPDRS 운동점수, UPDRS 일상활동점수와 유의한 상관관계를 나타내었다. 24시간 선조체 SBR과 최대 SBR은 선조체 결합능과 우수한 상관관계를 보였다. 이상의 결과로부터 $[^{123}I]{\beta}$-CIT의 선조체 결합은 파킨슨병의 진행정도를 나타내는 지표로 이용될 수 있다. 또 $[^{123}I]{\beta}$-CIT 투여 후 24시간 영상으로부터 얻은 간편화된 조직방사능 비는 $[^{123}I]{\beta}$-CIT의 결합정도를 정량적으로 반영한다. $[^{123}I]{\beta}$-CIT SPECT는 파킨슨병의 조기진단 및 진행 추적에 임상적으로 유용할 것으로 판단된다.

Influence of Depression, Anxiety and Social Support on Suicidal Ideation in Elderly Patients with Parkinson's Disease (우울, 불안, 사회적지지가 노인 파킨슨병 환자의 자살생각에 미치는 영향)

  • Purpose: This study is to identify how depression, anxiety and social support influence on suicidal ideation of Parkinson disease patients related to general characteristics. Methods: This descriptive correlative study was conducted through an organized and structured questionnaire and 120 sampled Parkinson disease patients. Collected data was analyzed by t-tests, ANOVA, and Preason's correlation coefficient and multiple regression analysis using SPSS 19.0. Results : Finding revealed 1) The degrees of suicidal ideation were significantly different upon the martial status(F=3.37, p=.021) among groups, drinking(F=4.97, p=.008) and smoking history(F=4.77, p=.010): 2) Pearson's correlation coefficient indicated significant association among the depression, anxiety, social support and suicidal ideation 3) Multiple regression analysis showed depression(${\beta}=.58$ t=7.77, p<.001), social support(${\beta}=-.21$, t=-2.69, p=.008) and alcohol drinking(${\beta}=.17$ t=2.61, p=.010). Depression was the major influencing factor and it has 54% of explanation power. Conclusion: Based on the findings of this study, health professionals should provide parkinson disease patients with comprehensive and appropriate management method to prevent suicide ideation upon factors: depression, anxiety and social support to prevent suicide ideation. Especially, implement of self-help group program to parkinson disease patients is very needed.

Effects of Dynamic Tubing Gait Training on Postural Alignment, Gait, and Quality of Life in Chronic Patients with Parkinson's Disease : Case Study (동적탄력튜빙 보행훈련 프로그램이 만성 파킨슨병 환자의 자세정렬과 보행능력과 삶의 질에 미치는 영향 : 사례연구)

  • The present study investigated the effects of dynamic tubing gait training(I and II) on the postural alignment, gait, and quality of life in chronic patients with Parkinson's disease. This study is based on the case study that recruited a total of 3 patients with chronic Parkinson's disease (Hoehn and Yahr Stage of 1 to 3 each one person). Dynamic tubing gait training (I and II) applied to chronic patients with Parkinson's disease for 25 sessions, 30 minutes a day, 5 days a week, over 5 weeks period. To investigate the effects of this study, evaluating using the postural alignment test, muscle activity tests, gait analysis, and quality of life scale for patient with Parkinson's disease. After the intervention of Dynamic tubing gait training (I and II), Trunk flexion was decreased. Also, during walking from initial contact (IC) to mid stance (Mst), muscle activity of Quadriceps, Hamstring, and Tibialis Anterior (TA) was increased and muscle activity of Gastrocnemius was decreased. The muscle activation of Erector Spinae (ES T12, L3) was increased in the H&Y I and III stages and decreased in the H&Y II stage. Length of gait line, single support line, ant/post position and lateral symmetry of center of pressure (COP) parameters improved. The spatio-temporal gait parameters including of step length, stride length, and velocity was increased, and cadence decreased. Further the quality of life of patients with Parkinson's disease was improved. Based on these findings, Dynamic tubing gait training (I and II) could be applied as a new approach to improve posture, gait, quality of life in chronic patients with Parkinson's disease for more than 5 years, whose drug resistance is halved.

  • Like Alzheimer's disease, Parkinson's Disease(PD) is one of the most common neurodegenerative brain disorders. PD results from the deterioration of dopaminergic neurons in the brain region called the substantia nigra. Currently there is no cure for PD, but diagnosing in its early stage is important to provide treatments for relieving the symptoms and maintaining quality of life. Unlike many diagnosis methods of PD which use a single biomarker, we developed a diagnosis method that uses both biochemical biomarkers and imaging biomarkers. Our method uses ${\alpha}$-synuclein protein levels in the cerebrospinal fluid and diffusion tensor images(DTI). It achieved an accuracy over 91.3% in the 10-fold cross validation, and the best accuracy of 72% in an independent testing, which suggests a possibility for early detection of PD. We also analyzed the characteristics of the brain fiber pathways of Parkinson's disease patients and normal elderly people.

  • 본 논문에서는 파킨슨병이 우리 사회에서 증가하고 있고 노인들 만이 아니라 젊은 사람들에게도 나타날 수 있기 때문에 환자들을 위해 근 기능 및 인지능력을 향상 시키는게 목표이다. 파킨슨병은 현재까지 원인이 알려지지 않았지만 도파민 세포가 손상되는 것으로 알려져 있기 때문에 아직은 약물치료와 식이요법 치료를 통해 질환의 증상을 중지시키거나 느리게 할 수 없다. 파킨슨병의 증강 중 가장 중심이 되는 근 기능 문제와 인지능력을 동시에 향상 시킬 수 있는 기대를 할 수 있으며 이것을 통해 다른 증상들도 느려 질 수 있도록 하는 기대를 할 수 있다. 프로그램을 통한 근 기능 능력이나 인지능력을 동시에 향상하고 대상자 개개인의 능력에 맞게 운동을 할 수 있는 프로그램을 개발하는데 목적이 있는 연구이다.

Parkinson's disease diagnosis using speech signal and deep residual gated recurrent neural network (음성 신호와 심층 잔류 순환 신경망을 이용한 파킨슨병 진단)

  • Parkinson's disease, one of the three major diseases in old age, has more than 70 % of patients with speech disorders, and recently, diagnostic methods of Parkinson's disease through speech signals have been devised. In this paper, we propose a method of diagnosis of Parkinson's disease based on deep residual gated recurrent neural network using speech features. In the proposed method, the speech features for diagnosing Parkinson's disease are selected and applied to the deep residual gated recurrent neural network to classify Parkinson's disease patients. The proposed deep residual gated recurrent neural network, an algorithm combining residual learning with deep gated recurrent neural network, has a higher recognition rate than the traditional method in Parkinson's disease diagnosis.

Full mouth rehabilitation for a Parkinson's diseases patient with chronic periodontitis: a case report (만성 치주염을 동반한 파킨슨병 환자의 전악 보철 수복 증례)

  • Parkinson's disease is a neurological disorder characterized by tremor, bradykinesia, akinesia, postural instability, and muscular rigidity, which is caused by the depletion of neurotransmitters such as dopamine. Cooperative dental treatment is more challenging because of tremor of Parkinson's disease. In this case, a 47-year-old Parkinson's disease patient with chronic periodontitis was treated with full-mouth rehabilitation using conventional fixed prostheses and implant fixed partial denture, which attained satisfactory outcomes functionally and esthetically. Short term periodic follow-ups will be needed with consideration for the characteristics of Parkinson's disease such as decreased manual dexterity.
