Delayed Bilateral Common Femoral Pseudoaneurysm after Percutaneous Access with Interventional Management: A Case Report (경피적 접근을 통한 시술 후에 발생한 지연성 양측 총대퇴 가성동맥류 및 인터벤션 치료: 증례 보고)

  • Common femoral artery pseudoaneurysm is a potentially serious complication of peripheral angiography. There have been few prior reports of simultaneous pseudoaneurysm in both common femoral arteries after percutaneous access. Here we report the case of a 58-year-old male patient who presented with phlegmon or abscess a few days after bilateral femoral access, after which newly developed bilateral femoral pseudoaneurysm with wide neck was observed on CT angiography 2 months after infection treatment. Because the patient refused surgery for pseudoaneurysm, a stent-graft was inserted in the left side, and percutaneous thrombin injection under US guidance with balloon occlusion was performed for the right side. Most pseudoaneurysms occur immediately after the causative procedure. However, there have been some cases in which pseudoaneurysms may occur several weeks or months later; it is therefore necessary to check the risk factors and to carefully observe the hemostasis site.

Evaluation of Antimicrobial, Antioxidant, and Antithrombin Activities of the Rhizome of Various Dioscorea Species (마 품종별 메탄올 추출물의 항균, 항산화 및 항혈전 활성 평가)

  • Yams (Dioscorea spp.) have been used as medicinal and edible resources worldwide. In the present study, methanol extracts were prepared from various kinds of yam, including D. batatas (Gyeongbuk No. 1 and No. 4), D. alata L. (Gyeongbuk No. 5 and No. 6), and D. bulbifera and D. nipponica, and the antioxidant, antithrombin, and antimicrobial activities of these extracts were evaluated. The water content of the various yams was 54.7~83.2% (D. batatas> D. alata L.>D. nipponica>D. bulbifera) and the methanol extracts contained 0.59-1.59% (w/v) solid matter (D. alata L.>D. batatas >D. nipponica>D. bulbifera). The mean concentrations of total polyphenols, total flavonoids, and total sugars in the different types of yam were 49.82 mg/g, 9.79 mg/g, and 173.81 mg/g, respectively. Analysis of DPPH radical-scavenging activity showed that D. alata L. Gyeongbuk No. 6 had a strong antioxidant capacity (IC50=$142.30{\mu}g/mL$), whereas the other yams showed reduced levels of activity (IC50=371-$486{\mu}g/mL$). D. alata L. Gyeongbuk No. 6 showed a strong antithrombin activity against human thrombin. Upon treatment with a D. alata methanol extract (1.5 mg/mL), thrombin time was increased 7.35-fold compared with that when methanol alone was used. In an antimicrobial activity assay, the D. nipponica extract showed moderate antibacterial activity against Bacillus subtilis, Staphylococcus aureus, Proteus vulgaris, and Salmonella typhimurium. Our results indicate that different varieties of Dioscorea spp., including D. batatas, have useful biochemical attributes, including antioxidant, antithrombin, and antibacterial activities.

Antimicrobial, Antioxidant and Anticoagulation Activities of Korean Radish (Raphanus sativus L.) Leaves (무청의 항균, 항산화 및 항혈전 활성)

  • Radish (Raphanus sativus) is a common cruciferous vegetable, and its aerial parts, called Mu-chung in Korean, have plentiful nutritional components such as vitamins, minerals and dietary fibers. Mu-chung has been used as a kimchi, a traditional Korean fermented dish, and dried Mu-chung is an important component of soups commonly consumed during winter in Korea. Since the advent of the mass production of radish in Korea, with the segregation of farm areas and towns and changing diets, Mu-chung has mostly been discarded instead of utilized. In addition, studies concerning the efficient utilization and useful bioactivities of Mu-chung are still lacking worldwide. In this study, we prepared the ethanol extract of Mu-chung and its subsequent solvent fractions. Antimicrobial, antioxidation, and anticoagulation activities were then evaluated in the hopes of developing a functional biomaterial from Korean radishes' aerial parts. The ethanol extraction yield for hot-air dried Mu-chung was 5.6%, and the fraction yields of n-hexane (H), ethylacetate (EA), butanol (B) and water residue were 25.3, 3.6, 19.4, and 51.7%, respectively. Analysis of total polyphenol and total flavonoid contents showed that the EA fraction had the highest content (97.57 and 152.91 mg/g) amongst the fractions. In antimicrobial activity assays, the H and EA fractions were effective against gram positive bacteria (Staphylococcus aureus, Listeria monocytogenes, and Bacillus subtilis), but not effective against gram negative bacteria (Escherichia coli and Pseudomonas aeruginosa). The B fraction also exhibited moderate antibacterial activity, suggesting that the extract of Mu-chung has various antibacterial components. In antioxidation activity assays, the EA fraction showed strong DPPH, ABTS and nitrite scavenging activities ($69-222{\mu}g/ml$ of $IC_{50}$), including reducing power. In anticoagulation activity assays, the EA fraction demonstrated strong inhibition activity against human thrombin and prothrombin. Prominent anticoagulation activity was found in aPTT assays; the aPTT of the EA fraction was extended 15-fold compared than that of the solvent control. Our results suggest that Mu-chung is an attractive nutritional food material possessing useful bioactivities, and the EA fraction of Mu-chung could be developed as a functional food ingredient.

Optimal Time to Localize Bleeding Focus and the Usefulness of Flexible Bronchoscopy in Hemoptysis (객혈에서 굴곡성 기관지경의 출혈부위 결정을 위한 적절한 시행시기 및 그 유용성)

  • Background : Bronchoscopy is a useful diagnostic tool, for accurate localization of the bleeding site and the management of hemoptysis. However, there is some controversy about the optimal timing of bronchoscopy. Method : To determine the optimal timing of bronchoscopy in hemoptysis, we reviewed the medical records of 118 patients and analyzed the following relationships amongst simple chest PA findings, namely, the duration and amount of hemoptysis, and the timing of bronchoscopy retrospectively. Results : The major causes of hemoptysis were active tuberculosis(28.8%), inactive tuberculosis(10.2%), bronchiectasis(17.0%), lung cancer(7.6%), and aspergilloma(7.6%). Localization of the bleeding focus by bronchoscopy was possible in 87.5% (21/24 cases) during active bleeding, and it was possible in 40.4% after bleeding had stopped(p<0.05). The localization rate of bleeding focus was 59.8% when the chest PA showed certain abnormalities, but it decreased to 27.8% when the chest PA finding was normal(p<0.05). When chest PA showed diffuse abnormalities or its finding was normal, the localization rate of bleeding focus significantly increased if bronchoscopy was performed during bleeding or within 48 hours of the cessation of active bleeding. The localization rate was higher as the amount of hemoptysis became larger(p<0.05). The localization rate of early bronchoscopy(during bleeding or within 48 hours of the cessation of active bleeding) was significantly higher when the duration of hemoptysis was less than 1 week, but there was no advantage if the duration was 1 week or longer. Early bronchoscopy was also necessary to localize the bleeding focus for surgical resection in 4 patients, and the bronchoscopy itself was therapeutic in 1 patient whose bleeding was successfully managed with thrombin-application via bronchoscope. Conclusion : It is concluded that flexible bronchoscopy is useful at not only localizing the bleeding focus but also in preparing a therapeutic plan, and early bronchoscopy is more favorable in hemoptysis.

Changes in the in vitro Antioxidant and Antithrombosis Activities of Salicornia europaea According to Harvest Time (수확시기에 따른 함초의 항산화 및 항혈전 활성의 변화)

  • In Korea, the aerial parts of the halophyte Salicornia europaea, known as hamcho, are used in salads in April–June and in oriental medicine in September–October In this study, with the aim of developing functional foods to aid blood circulation, hot water extract (HWE) and ethanol extract (EE) were prepared using hamcho harvested from the fields of Shinan, Jeonnam, Korea on 5th April (HWE-04, EE-04), 5th July (HWE-06, EE-06), 5th August (HWE-08, EE-08), 5th September (HWE-09, EE-09), and 5th October (HWE-10, EE-10), and their antioxidant and antithrombosis activities were evaluated. Among the HWEs, HWE-10 showed the highest concentration of total polyphenols and total flavonoids (22.4 and 17.6 mg/ml, respectively), and EE-09 had the highest concentration among the EEs (20.1 and 19.3 mg/ml, respectively). Among the HWEs and EEs, HWE-08 and EE-08 had the highest total sugar and reducing sugar content. In the antioxidation assay, HWE-10 and EE-09 showed strong reducing power, as well as DPPH, ABTS, and nitrite scavenging activities. The calculated RC50s of EE-09 against DPPH, ABTS, and nitrite were 578, 277, and 68.8 μg/ml, respectively. The antithrombosis activity assay revealed that HWE-04, HWE-06, EE-04, and EE-06 had anticoagulation activity against coagulation factors and that HWE-08, HWE-09, EE-08, and EE-09 expressed strong thrombin inhibitory activity, which was comparable to the antithrombosis activity of aspirin. In addition, EE-06 and HWE-08 exhibited strong aggregation inhibitory activities against human platelets. The results suggest that extract from hamcho harvested in particular periods and prepared using a defined solvent has strong potential as a novel food ingredient and an antioxidant and antithrombosis agent.

Comparison of Anti-thrombosis, Antioxidant and Anti-diabetes Activities amongDifferent Cultivars of Hibiscus cannabinus L. Seeds (케나프 품종별 종자 추출물의 항혈전, 항산화, 항당뇨 활성의 비교)

  • Recently, kenaf (Hibiscus cannabinus L.) has been highlighted due to its rapid growth in dry, infertile land and its versatile applications in textiles, paper, feed, food, medicine, and phyto-remediation. In Korea, a new cultivar, 'Jangdae', which has the merit of feasible seed-gathering, was established in 2013. However, most of the kenaf seeds used are still imported, and a comparison study of useful bio-activities between Jangdae and imported seeds has not been conducted. In this study, anti-thrombosis, antioxidant and anti-diabetic activities of the ethanol extracts of the Jangdae seed (E-JS) and 8 imported cultivar seeds (E-ICS) were investigated. The E-JS has 14.4 mg/g of total polyphenol and 5.3 mg/g of total flavonoids, whereas the E-ICS has 5.9~8.7 mg/g and 0.7~1.5 mg/g, respectively. Among the 9 kenaf seed extracts, IT 181215 and IT 207877 seed extracts showed 1.92 and 1.88-folds extended thrombin time (TT), and IT 207886 seed extracts showed 3.04-folds extended activated partial thromboplastin time (aPTT). All extracts, except Hong-ma seed, showed 1.36~1.56-folds extended prothrombin time (PT). The E-JS showed strong anti-thrombosis activities with 1.46-folds (TT), 1.38-folds (PT) and 2.37-folds (aPTT) compared to those of solvent controls. The E-JS has the strongest antioxidant activities among the seed extracts, with 0.48 mg/ml of RC50 for ABTS cation and 0.27 mg/ml of RC50 for nitrite scavenging. Meanwhile, the E-JS and IT 181215 extracts showed strong inhibition against α-glucosidase with 10.3~10.6% inhibition at 0.5 mg/ml. Our results suggest the E-JS has potential as an anti-thrombosis, antioxidant and anti-diabetic agent.
