Purpose: The purpose of this study was to establish the foundations for the application of appropriate programs for pain reduction in patients with rheumatoid arthritis. Methods: The literature on patients with rheumatoid arthritis were found on an electronic search site for South Korean academic papers. The search terms "rheumatism" and "rheumatoid arthritis" were used independently. The combined terms "rheumatism and pain" and "rheumatoid arthritis and pain" were also used. For the meta-analysis, the R version 3.5.1 program was utilized. Results: The meta-analysis of eight papers showed a large effect size of -4.11. The programs were most effective in the order of aquatic exercise, aromatherapy, self-help education, muscle strength exercises, and tai chi. Conclusion: This study could provide the basis for presenting appropriate programs for pain management in patients with rheumatoid arthritis.

과거와 다르게 최근의 병원들은 정보화로 인해서 상당한 양의 의료 데이터가 저장되어 있어서 이의 효과적인 이용에 관심을 가지고 있다. 그러나 기존 통합병원정보시스템(Integrated Hospital Information System)은 아직까지 일반관리와 원무관리 중심에서 벗어나지 못하고 있다. 품질 좋은 의료 서비스를 제공하기 위해서 환자 중심의 진료 및 진료지원, 임상연구 등을 종합적으로 지원하기 위한 데이터 웨어하우스의 필요성이 대두되기 시작했다 .이 글에서는 최근 임상 데이터 웨어하우스(Clinical Data Warehouse)에 초점을 둔 J병원 사례와 연구를 중심으로 기술한다.

최근 국내의 모든 병원에서 PACS(Picture Atchiving and Communication System) 도입에 관한 관심을 보이고 있다. PACS가 구축이 되면 병원 내 모든 진료 과에서 디지털 데이터의 전송으로 정보를 공유할 수 있고, 진료가 자동화 되는 장점이 있다. 하지만, 환자가 다른 병원으로 이송될 경우 과거 진료 내역을 다른 병원으로 함께 전송하여야 되는데, 다른 병원의 시스템과 연계할 방법이 현재로는 존재하지 않는다. HIES 시스템은 의료 데이터 전송의 표준문서로 XML(eXtensible Markup Language)을 제안하고 있다. XML은 문서를 정의하는 메타 마크업(meta-markup) 언어로써 DICOM 프로토콜을 통하여 산출된 의료 데이터를 표현하기에 적당하다. 또한 병원 간 이질 데이터베이스 시스템 통합을 위하여 일관된 스키마 정보를 유지하는 정보 공유 관리자를 설계, 구현하였다.

Objectives: To examine the effect of long-term Korean medicine treatment on mild cognitive impairment (MCI). Methods: This study was conducted on two patients who were diagnosed with mild cognitive impairment and received long-term Korean medicine treatment. In order to evaluate the degree of cognitive decline, SNSB and K-MMSE were performed 2~3 times at the first visit and during the treatment process. Results: Both cases 1 and 2 were diagnosed with MCI with a high possibility of developing dementia. In case 1, MCI progressed to early dementia at one year after the diagnosis of MCI. However, in case 2, that MCI status was maintained without progressing to dementia for 4 years after the diagnosis of MCI. The cause of such difference in the course of MCI might be attributed to the fact that Case 1 had lower K-IADL level but higher SGDS level than Case 2 at the time of initial diagnosis, with weak family support and irregular herbal medicine intake. Conclusions: Korean medicine treatment could improve and manage symptoms of cognitive decline due to MCI.
