체계적인 통증자가조절기에 대한 교육이 수술 후 통증자가조절기 사용에 대한 지식과 태도, 통증 및 진통제 사용량에 미치는 효과 -대장암 수술 환자를 중심으로- (Effects of a Structured Patient Controlled Analgesia (PCA) Education on Knowledge and Attitude Regarding PCA Usage, Pain, and Consumption of Analgesics in Colorectal Surgery Patients)

  • Purpose: The purpose of this study was to investigate the effects of a structured preoperative PCA education on the knowledge and attitude regarding PCA usage, level of pain, and the consumption of analgesics after operation for colorectal surgery patients. Methods: This study was conducted from 18 Feb to 2 May, 2008. Participants were 80 colorectal cancer patients who would use the IV-PCA after colorectal surgery in a cancer hospital in Gyeonggi-do, Korea. Two groups, experimental and control were consisted of 40 patients each. The 20-minute structured education regarding PCA usage was applied to each patient individually in the experimental group but only the routine anesthetic consultation was given to each patient in the control group the day before the surgery. The SPSS/PC 10.0 program was introduced to analyze the collected data on frequency, percentage, mean, standard deviation, $x^2$-test, t-test and paired t-test. Results: The experimental group with the structured preoperative PCA education showed higher knowledge and more positive attitudes regarding the PCA usage than the control group. Also the experimental group showed better pain control and lower consumption of analgesics at 4, 8 and 24 hours after than the control group. Conclusion: The structured preoperative PCA education is an effective nursing intervention for improving the knowledge and attitude of the colorectal surgery patients on the PCA usage, and enabling the patient to take the analgesic more effectively with lower consumption, while reducing the patients' pain after operation.

암환자용 통증관리 지침서 교육이 환자 및 가족의 암성 통증관리에 대한 지식과 태도에 미치는 효과 (The Effects of Patient and Family Education Guideline on Knowledge and Attitude toward Cancer Pain Control)

  • Purpose: This study was conducted to identify the effects of provision of a cancer pain guideline and education for patients with cancer and their families on their knowledge and attitude toward cancer pain control. Method: A nonequivalent control group pre-post test design was used. The participants were selected according to a selection criteria from patients and their families on the medical ward at K university hospital in D city. Both the experimental group for patients with cancer and control group consisted of 16 patients and their families. Results: The first hypothesis, which assumed that there was a difference between the patients in the experimental group and the control group on knowledge (t=-20.006, p=.000) and attitude (t=-13.492, p=.000), was accepted. The second hypothesis, which assumed there was a difference between families in the experimental group and the control group on knowledge (t=-16.087, p=.000) and attitude (t=-10.262, p=.000), was supported. Conclusion: Education for cancer related pain control with a patient guideline had a positive effect on knowledge and attitude of patients with cancer patients and their families suggesting that there is a need to developed this type of guidelines.

  • Purpose: This study is purposed to provide basic data needed in constructing the educational contents about cancer pain management for the health care professionals. Methods: This study surveyed the degree of knowledge in cancer pain management of clinical nurses and doctors. Subjects were clinical 143 nurses and 88 doctors in 3 cities. The tool used are 32-item scale for evaluation of health care professionals' knowledge modified by Kim(1997), which was originally developed by McCaffery and Ferrel(1995). Results: The level of the health care professionals' knowledge about and attitude toward pain management were insufficient. The level of the doctors' knowledge and attitude showed higher score than those of the nurses'. The knowledge of health professional who were not hesitated to administrate analgesia was showed more higher than who were hesitated to do. Conclusion: Nurses need more knowledge and effective attitude toward cancer pain management. Various and sufficient educational program about cancer pain management can be contribute to improve the nursing quality of cancer pain.

  • Purpose: This study investigated the sought to identify the pain management knowledge and attitude of nurses in internal medicine and surgery stream wards and intensive care units to obtain basic information needed for improved pain control. Method: Data was collected through self-reported questionnaires and analyzed by descriptive statistics, t-test and ANOVA using SPSS Win 12.0. Results: Pain management knowledge score averaged 51.1 out of 100, with significant differences evident depending on age, marital status, educational level, position, total work career, working department and cancer care-giving experience. Pain management attitude score averaged 2.9 out of 4, and significant differences were evident depending on age, marital status, educational level, position, total work career, working department, cancer care-giving experience, education experience and number of patients treated. Conclusion: Further studies are necessary to develop effective nursing knowledge and attitude education programs and evaluation such as discussion by cases, workshops, system of expert help for uncontrollable pain or a multi-disciplinary pain management task force. In addition, patient satisfaction level with pain management should be assessed and the responses reflected practically.

임상간호사의 통증관리에 대한 지식과 태도 (Clinical Nurses' Knowledge and Attitudes on Pain Management)

  • The purpose of this study was to understand clinical nurses' knowledge and attitudes on pain management. The subject of the study were 254 nurses working at two hospitals affiliated with a university in Seoul. The questionnaires included four areas: general knowledge on pain, knowledge on the use of analgesia, knowledge and implementation on the pain assessment scales and pain interventions and nurses' general characteristics. The data were analyzed with descriptive statistics, analysis of variance, LSD test and t-test using SPSS statistical package. The results were as follows. 1. The mean score of the general pain knowledge was 61.46 and that of knowledge on the use of analgesia was 52.19. 2. Most nurses(74%) answered with hesitation about injecting narcotic analgesia to patients. 3. The pain assessment scale which nurses knew (57.5%) and used(48.0%) extremely was a simple descriptive scale. 4. The pain intervention which nurses knew (94.5%) and implemented(92.1%) extremely was to inject analgesia. 5. The number of nurses who had learned about pain management was 49 of 254(19.3%). 6. Nurses' knowledge on the use of analgesia was of relevance to having learned pain management, but general pain knowledge was not so relevant. According to this research, I suggest the following. 1. It is necessary to develop an education program with actual practice and intervention which nurses can perform for themselves. 2. It is necessary to continuously educate about pain management in clinical wards.

  • 최근 만성 통증 환자가 증가하고 있다. 특히 정신과 외래환자에서는 40% 이상이 통증으로 고통을 받고 있다. 그런데 한국인의 질병관과 질병에 대한 태도와 관련되어 나타나는 비정상 질병행동은 통증 환자에게 만성화 경향을 재촉하고 치료에 많은 어려움을 야기시키고 있다. 본 심포지움은 근관절계, 신경계질환, 만성 내과질환과 관련된 만성 통증을 어떻게 효율적으로 치료 할 것인가를 조명해 보고자 한다. 이로서 자문-조정 정신의학에서 만성 통증환자에 대한 효과적인 대처 요령을 습득하고자 한다.

  • Purpose: The purpose of this study was to examine the effects of preoperative pain management education on postoperative pain control in patients with uterine tumor using patient controlled analgesia. Methods: This study used non-equivalent control group non-synchronized design. Data were collected from September, 2008 to March, 2009 at one university hospital in Daegu, Korea. There were 60 participants, 30 in both the experimental and control group. The experimental group was given preoperative pain education using videos, leaflets, and a PCA model. Postoperative pain intensity, frequency of the PCA button being pressed, and doses of additional analgesics were observed through 24 hours postoperative and knowledge of pain and attitude about the use of the pain medicine were measured at 3 days postoperative. Collected data were analyzed using t-test, ${\chi}^2$ test, repeat measured ANOVA, and Bonferroni methods. Results: There were no significant differences in the postoperative pain level between the experimental and control group. Postoperative frequency of the PCA button pressed, doses of additional analgesics, pain knowledge and attitude about the use of the pain medicine of the experimental group were significantly higher than those of the control group. Conclusion: Pain management education is an effective nursing intervention for pain control after surgery.

소아환자의 국소마취 시 통증을 줄이기 위한 2단계 주사바늘 자입법 (2-Step Needle Insertion Technique to Reduce Pain in Children during Local Anesthesia)

  • 이 연구의 목적은 어린이의 구강내 국소마취시 통증을 줄이기 위한 방법으로 고안된 2-단계 자입법(2-step needle insertion technique)의 효과를 평가하기 위하여, 어린이 환자의 구강 내에 양측성으로 국소마취를 시행하고 마취시 느끼는 통증의 정도를 pain scale을 이용, 측정하여 통상적인 자입법(Conventional needle insertion technique)과 비교하는 것이다. 통상적인 자입법과 2-단계 자입법에 의해 어린이가 느낀 통증 점수는 각각 5.82 ± 2.14 and 2.57 ± 2.09로 통계적으로 유의한 차이를 보였다(p < 0.001). 성별, 연령, 그리고 Frankl의 행동평가척도에 따른 두 방법 간의 주관적인 통증의 정도는 2-단계 자입법에서 훨씬 작았다(p < 0.001). 부위별 마취법에 따른 주관적인 통증의 정도는 상악과 하악, 전치부와 구치부 등 모든 부위에서 2-단계 자입법으로 마취하는 동안의 통증이 더 작게 나타났다(p < 0.001). 2-단계 자입법은 치과치료에 긍정적인 태도를 가지는 어린이 뿐만 아니라 부정적인 태도를 가진 어린이 환자들에서도 국소마취시 통증을 감소시킬 수 있는 간단하고 효과적인 방법이며, 도포마취제의 적용이나 주의분산 등과 함께 병용한다면 통증을 더 효과적으로 감소시킬 수 있을 것으로 사료된다.

탄력밴드를 이용한 운동이 운동에 대한 태도와 관절운동시 통증 정도에 미치는 효과 (The Effect of a Physical Exercise Program Using Elastic Bands in the Attitude of Physical Exercise and the Degree of Joint Pain)

  • Purpose: The purpose of this study was to identify the effects of a physical exercise program using elastic bands in rural area women. Method: This study adopted a pre-post test design. The subjects were 89 rural area women who were selected from four community health care centers. The physical exercise program was executed 30 times for 10 weeks (3 times a week) from August 23 to October 31, 2003. Results: There was a significant difference in the attitude of physical exercise (t=-5.517, p=.000). In the degree of joint pain, there were significant differences in the flextion and extension of the shoulder joint (t=2.557, p=.020; t=5.625, p=.000), and there was a significant difference in the flextion of the knee joint (t=4.747, 0=.000) but there was no significant difference in the extension of the knee joint (t=1.795, p=.083). Conclusion: Physical exercise programs need to be implemented and spread continuously.
