Quality of Life, Perceived Health Status, Pain, and Difficulty of Activity of Daily Living of Degenerative Arthritis Patient in Island Residents (일 도서지역 퇴행성관절염 환자의 삶의 질, 지각된 건강상태, 통증 및 일상생활동작 장애)

  • Objectives: The purpose of this study was to identify and relationships among quality of life, perceived health status, pain and difficulty of daily activity with degenerative arthritis living in island residents. Methods: The participants were 601 degenerative arthritis patients. Data were collected using structured questionnaires from June 23th to Oct. 08th, 2008 and analyzed using the SPSS 12.0 program. and it included descriptive statistics, t-test, $x^2$-test, Pearson correlation coefficient. Results: The mean quality of life score was 115.87, perceived health status was 4.40, pain was 6.31 and difficulty of daily activity was 77.62. On the demographic variables, age, living arrangement, level of economics showed significant difference for the quality of life and sex and age showed significant difference for the perceived health status, pain, difficulty of daily activity. On the disease-related variables, duration of disease, use of analgesic drug showed significant difference for the dependent variables. There was a significant positive correlation among the quality of life, perceived health status, significant negative correlation pain and difficulty of daily activity. Conclusions: These results suggested that nursing intervention program including pain reduction, enhancing the perceived health status, ADL abilities and quality of life were necessary for the osteoarthritis patients in island residents.

The Effects of Stretching Exercise to Musculoskeletal pain in the Community-Dwelling Farmers (일개 지역 농부에서 근골격계 통증에 대한 스트레칭 운동의 효과)

  • This study aimed to evaluate the effects of stretching management on musculoskeletal joint pain in community indwelling elderly farmers. The study included 28 residents who lived in a rural community. All participants were assigned randomly to either the stretching group (n=13, $59.67{\pm}4.77$ year-old) or the control group (n=15, $61.44{\pm}10.41$ year-old). Respondents were interviewed by means of a structured questionnaire. Pain severity of 6 body areas caused by symptoms of work-related musculoskeletal disorders and pain severity on day time variations were measured by the visual analogue scale on a self-assessed questionnaire. The stretching group was administered stretching therapy (45 min, 2 sets, warming up and cool down, main exercise; total 19 stretching kinds of subset 5 fields) for 12 times for 4 weeks. There were no significant difference between the two groups in general social and pain characteristics. After 4 weeks of stretching, the stretching group showed significant improvements at almost all joint pain scores except arm/elbow, and day time variation scores of pain compared to both baseline scores, and with control group scores. These results showed that stretching therapy is one of the most useful modalities to manage musculoskeletal pain in community-based elderly farmers.

Effect of Dental Hygienists' Treatment Posture in Part of the Body Pain according to Height and Body Weight (치과위생사의 진료자세가 신장과 체중에 따라 신체부위 통증에 미치는 영향)

  • The purposed of this study was to examine the relation between dental hygienists' treatment posture and body pain by surveying 120 dental hygienists working at dental hospitals and clinics in Busan and to suggest desirable treatment postures and pain prevention methods for dental hygienists' health and job performance. Data obtained from the questionnaire survey were analyzed using SPSS 18.0 for Windows. The taller the hygienists were the less pains they had in their 'back' and 'hips' (p<0.05). The shorter the hygienists were the more pains they had in their 'back,' 'waist,' 'hips,' 'legs' and 'ankles/feet'(p<0.05). There was no difference in pains in each body part according to weight. In the results of analyzing correlations among pains in different body parts, a very strong positive correlation was observed between the pains in the 'knees' and 'legs' (r=0.71)(p<0.01). The hygienists of 56.7% had experience in education on treatment posture, and 62.5% were not doing exercise regularly. To the question on what should be done for health welfare in their workplace, the hygienists of 47.9% mentioned improvement in the environment of the workplace.

The Effect of Self Thoracic Mobilization Exercise on Shoulder Pain and Function (자가 등 가동운동이 어깨 통증 및 기능에 미치는 효과)

  • This study evaluated the effect of self thoracic mobilization exercise (STME) on shoulder pain, disability and shoulder function in patients with shoulder dysfunction. Thirty-two patients with shoulder dysfunction were randomly assigned to the STME group and the control group. The visual analog scale, shoulder dysfunction, and shoulder range of motion were evaluated before and after intervention. Sixteen STME intervention patients used a foam roller and quadripedal position flexion and extension. Sixteen controls received placebo effects using electrotherapy pads. The two groups performed 20 minutes for three times per week for four weeks. The STME group showed significant improvement in pain and dysfunction compared to that of the control group (p <0.05), and their range of motion of the shoulder was significantly improved in flextion, abduction, and external rotation compared to that of the placebo group (p <0.05). Therefore, STME is effective in resolving shoulder pain and dysfunction, and will be able to educate patients with shoulder pain in an easier approach and mediate social treatment costs.

Anti-inflammatory Effects of Korean red ginseng Extract in formalin-induced Orofacial Pain in Rats (포르말린으로 유도된 안면통증에 대한 홍삼추출물의 항염증 효과)

  • The effects of korean red ginseng (KRG) extracts on orifacial pain control in terms of the systemic inflammatory response and pharmacological effects as health supplements were investigated. The experimental group were divided into three groups, the control group (n=6), formalin (5%, $50{\mu}{\ell}$) injection group (n=6), and formalin (5%, $50{\mu}{\ell}$) injection added KRG administrated group (4.5 ml/kg, n=6). The KRG administrated group prior to the formalin injection significantly attenuated the behavioral response compared to that of the control group. Pain reduction was suppressed mainly from 15 min to 30 min. The KRG administrated rats showed significantly reduced p38 MAPK, iNOS and Nrf2 expression in the brain and medulla oblongata according to Western blot analysis. These findings suggest that KRE may have a useful effect on orificial pain control functions by preventing the p38 MAPK pathway.

Postoperative Analgesic Effect of Intraarticular Bupivacaine or Morphine After Arthroscopic Knee Surgery (슬관절경 수술 후 관절강 내 주입한 Bupivacaine 및 Morphine이 통증에 미치는 영향)

  • Recent studies have shown that opiods can produce potent antinociceptive effects by interacting with opioid receptors in peripheral tissues. This study sougt to compare the effects of morphine with those of bupivacaine administered intraarticularly upon pain after arthroscopic knee surgery. In a ramdomized manner, 60 healthy patients received either morphine(3 mg in 20 ml NaCl ; n=20), bupivacaine(20 ml, 0.25% ; n=20) intraarticularly at the completion of surgery, and others were not administered(n=20) under general anesthesia after 1, 2, 4, 6, 12 and 24h of postoperative day, pain was assessed by a visual analogue pain scales, time to first analgesic use were recorded. Pain scores were significantly greater in the morphine group than two groups at 1h. From 4th until the end of the study period, pain scores were significantly greater in the bupivacaine group than in the other two group. Anagesic requirements were significantly greater in the morphine group than two groups at 1h but were significantly greater in the bupivacaine group than in the other groups throughout the remainder of the study period. The results suggest that intraarticular morphine produces an analgesic effect of delayed onset but of remarkably long duration.

Effect of Kinesio Taping on Isokinetic Muscular Function and Pain of Knee Joint in Elderly Women (키네시오 테이핑이 고령자의 슬관절 등속성 근 기능 및 통증에 미치는 영향)

  • The aim of this study were to evaluate the effect of kinesio taping on isokinetic muscular function and pain of knee joint. The subjects for this study were twelve elderly women($65{\sim}70$years) suffered by knee joint pain over more than three years. The taping methods performed on 3 times a week for 8 weeks for improvement of isokinetic muscular function. Also, the change of visual analogue scale(VSA) and varval rating scale(VRA) has been measured before and post taping 8 weeks. Paired t-test was performed for data analysis at the 0.05 level of significance. After 8 weeks, the results of the study are as follow; First, peak torque, peak torque % BW, total work and ipsilateral balance ratio were significantly improved. And, work fatigue, visual analogue scale and verval rating scale were decreased significantly after 8 weeks taping. The results suggest that applying kinesio taping on knee joint may improve isokinetic muscular function of elderly women. Therefore kinesio taping method was effective for pain relief and prevention of knee joint pain in the elderly women.

Effects of Doin Gigong Exercise on Recovery from Facial Paralysis, Pain and Anxiety of Bell's palsy Patients (도인기공체조(導引氣功體操)가 구안와사(口眼喎斜)환자의 안면마비 회복정도, 통증 및 불안에 미치는 효과)

  • Purpose: This study was done to evaluate the effects of Doin Gigong Exercise on the Recovery from Facial Paralysis, pain and anxiety in patients with Bell's palsy. Methods: A nonequivalent control group posttest-only non-synchronized design was used for this study. Participants were contacted at the oriental medicine ward of D hospital in Busan and assigned to either the experimental group (25) or control group (25). Collected data were analyzed for changes in recovery from facial paralysis, in pain and in anxiety between pre and post Doin Gigong Exercise. Data analysis was done using t-test, Fisher's exact test, $x^2-test$ with the SPSS 12.0 Win program. Results: There were significant decreases in the lip paralysis status pain and in anxiety between pre and post Doin Gigong Exercise. Conclusion: The results indicate that Doin Gigong Exercise is effective for patients with Bell's palsy to decrease lip paralysis, pain and anxiety and therefore, an effective intervention for use with patients with Bell's palsy.

The Effectiveness of Using a Spinal Column Thermal Massage Device on Muscle Pain, Depression and Stress (척주 온열 마사지 기기를 이용한 근육통, 우울감 및 스트레스 개선 효과)

  • This study aims to examine whether the use of a spinal column thermal massage device for patients with muscle pain is effective in improving muscle pain, and to verify whether it is also effective in improving affective depression and stress. To this end, 16 study participants (male 31.25%) were treated with a thermal massage device during 5 sessions a week for 4 weeks and 40 minutes per session. According to the study results, the subjective pain level changed by a rate of VAS -46.32% and PDI -44.86%; a significant decrease was observed in, both, VAS and PDI. The depression and stress levels changed by a rate of BDI -21.84% and SRI -11.48%; a significant decrease was observed in, both, BDI and SRI. Therefore, the use of a thermal massage device to treat patients with muscle pain is expected to have a positive effect in improving not only subjective muscle pain, but also depression and stress.

  • Cracked teeth are a common problem with incomplete fracture that typically shows pain on biting and temperature stimuli. The most common cause of crack is known to be the masticatory accident. Predisposing factors were aging, old restoration, and iatrogenic stress concentration, etc. Accurate diagnosis is needed for detecting the extent of crack and pulp vitality. There are many diagnostic tools like bite test, direct observation with transillumination, dye penetration, and radiographs. Immediate treatment for pain relief and occlusal adjustment would be needed. Composite resin and bonded amalgam showed favorable prognosis. Moreover, cusp protection is reliable permanent treatment for cracked teeth. The purpose of this article is to review the literature about the prevention, diagnosis, and treatment for cracked teeth.
