Factors influencing Health-Related Quality of Life in elderly who visited a senior center: with activity of daily living, quality of sleep and depression (복지관 이용 노인의 건강관련 삶의 질 영향요인: 일상생활 수행능력, 수면의 질 및 우울을 중심으로)

  • The purpose of this study was to understand the ability of the elderly to activity of daily living, assess their quality of sleep and level of depression at home and analyze how such relate health related quality of life. The subject was 223 senior over 65 years old who lived in B Metropolitan City. Descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation and stepwise multiple regression were performed using SPSS WIN 14.0. The major findings of this study are as follow: 1) 34.1% of the respondents experienced a degree of sleep disorder and 57.8% were vulnerable to depression. 2) Activity of daily living, quality of sleep and depression were related to health related quality of life to a statistically significant degree. quality of sleep, activity of daily living positively related to health related quality of life. level of depression related to health related quality of life. Stepwise multiple regression analysis revealed the most powerful predictor of health related quality of life was depression. While age, level of education and quality of sleep were also found as significant predictor variables. Based on these results, it is necessary to perform more studies on health related quality of life and related factors according to that in various settings. Especially, to develop a program intended to improve the health related quality of life of the elderly at home, we need to consider not only physical factors but also psychological factors.

Study on the Efficient Integration of Long-term Care Facilities and Geriatric Hospitals by Using NHIC Survey Data (실태조사를 통한 장기요양시설과 요양병원의 효율적 연계방안)

  • The purpose of this study is to identify how to efficiently integrate long-term care facilities into geriatric hospitals. We conducted a survey on the current operations of facilities and medical services of 2009 of 192 long-term facilities and 168 geriatric hospitals in Korea between October and November. Technical statistics and chi-square test were conducted on the collected data using the SPSS 13.0/Win program. There was a difference between the two facility types in terms of the co-payment levels of the food services. Both types selected the budget deficit as their major management problem. Ease of access and the surrounding environment were critical factors used to select the location of both types of facilities. Facility users benefited from the discounted co-payments of both facility types. However, facility users wanted more frequent visits and support from their family members during their stay at the facilities. It was discovered that users in the long-term care facilities stayed longer, that is until they died, compared to their counterparts in geriatric hospitals. The two types of facilities provided their services totally separately to users. Users of the two types of facilities are poorly supported and cared for by their families. This study suggests that setting reasonable service fees, paying caretakers, introducing an integrated facility, strengthening facility assessment standards, introducing the family doctor system, and introducing the handling of long-term care insurance by geriatric hospitals would allow the integration between long- term care facilities and geriatric hospitals to be beneficial.

Development and Validation of the Classification of Home-based Long-term Care Activities (노인장기요양보험 재가서비스 분류 틀 개발 및 타당도 검증)

  • The purpose of this study was to develop the classification of home-based long-term care activities and to test its validity. In this study, the taxonomy of long-term care activities was structured according to the service domain and process. Two expert groups participated in making a draft of the taxonomy that was composed of 7 service domains, 22 care needs, 22 service objectives, and 114 activities. Reliability and validity of the taxonomy was tested in a sample of 152 elderly subjects who used the home-based long-term care services. Based on the factor analysis of 114 activities, 21 factors were extracted from 114 activities. Internal consistency of the factors was high. Content validity was confirmed by the CVI. Long-term care insurance grade was used to assess the criterion validity. Among 21 care needs, 12 cares needs were significantly different from their grade. The classification of home-based long-term care activities demonstrated reliability and validity. In conclusion, the use of this classification is recommended while communicating with the elderly subjects, service providers, and the 3rd party payers.

Effect of Lifestyle Risk Factors on Daily Life and Cognitive Function of the Older Adults in the Community (지역사회 노인의 라이프스타일 위험요인이 일상생활 활동과 인지기능에 미치는 영향)

  • Objective : To identify the effect of lifestyle risk factors on the daily activities and cognition of the older adults in the community using the National Health Insurance Corporation 2015 geriatric cohort database. Methods : Lifestyle risk factors were defined as body mass index (BMI), smoking, drinking, vigorous exercise, moderate exercise, and walking, and basic and instrumental activities of daily living (ADL) and cognitive function variables were included in the analysis. ADL and cognitive function according to sex and age were analyzed using a t-test and one-way ANOVA. The correlation between lifestyle risk factors, ADL, and cognitive function was analyzed using Pearson's correlation analysis, and multiple regression analysis was performed to analyze their influence. Results : The factors affecting basic ADL (BADL) were sex and walking exercises, with an explanatory power of 1.7%. Instrumental ADL (IADL) included age, drinking, and walking exercises, with an explanatory power of 2.6%. Cognitive function included sex, age, BMI, vigorous exercise, and walking, with an explanatory power of 5.3%. Conclusion : Lifestyle risk factors partially affected BADLs/IADLs and cognitive function in community-dwelling older adults. This suggests the need to systematically manage lifestyle risk factors to improve and maintain the healthy lives of older adults facing biological aging.

Study on Discovery of Vulnerable Factors in Road Tunnels through AHP Analysis (AHP분석을 통한 도로터널의 취약요소 발굴에 관한 연구)

  • This study aims to identify vulnerability factors through comprehensive safety diagnosis and to seek improvement measures for the safety and maintenance of facilities. In this study, the results of road tunnel inspections and diagnostics were converted into a database (DB). Using this data, we explored to identify vulnerable elements (NATM, ASSM) based on structural types and to develop efficient improvement measures. In this study, we analyzed 76 detailed safety diagnosis reports covering 45 different types of road tunnel facilities. In the detailed guidelines for comprehensive safety diagnosis, the database (DB) items for identifying vulnerable factors were selected by categorizing the basic information, such as the year of completion and damage items. In addition, AHP analysis was conducted separately through experts in related fields to analyze the correlation between damages. As a result, the primary vulnerability factors for NATM and ASSM were identified as cracks, leaks, insufficient lining thickness, and joint rear. ASSM was identified as relatively more susceptible to network cracks and material separation compared to NATM. In contrast, flaking and rebar exposure were interpreted as more significant vulnerabilities for NATM than for ASSM. In addition, the correlation between elements in NATM was found to be low, whereas in ASSM, the correlation between elements was high, indicating a more organic relationship.

A Study on Oral Health Knowledge, Behavior and Education Needs of the Elderly (노인의 구강보건지식, 행위, 교육 요구도에 관한 연구)

  • The purpose of this study was to examine elderly people's oral health behaviors and education needs. The subjects in this study were 195 senior citizens who were users of senior cultural centers and senior welfare agencies in Seoul and Incheon. After a survey was conducted, the collected data were analyzed by the statistical package SPSSWIN 19.0. The findings of the study were as follows: 1. Gender, academic credential and monthly mean income were identified as the general characteristics to impact on their oral health attitude. 2. The senior citizens who ever received oral health education had a better knowledge(p<0.05) and took a better attitude(p<0.01). 3. As for a time for toothbrushing, many brushed their teeth after breakfast(74.9%) and dinner(71.8%). Utilized Oral hygiene devices were interdental brushes(21.5%), dental floss(13.8%). As many as 62.5% felt they had a dry mouth, and the most prevalent way for them to cope with it was drinking water often(68.2%). 79.5% didn't get their teeth cleaned on a regular scaling. 4. The rate of regular scaling was higher in the elderly groups that ever received oral health education and whose knowledge scores was above the average(p<0.05). 5. In relation to the necessity of oral health education, 87.2% felt the need for that, and as many as 79.0% intended to receive that education. Their favorite period of education was 6months(41.0%), and the greatest group hoped to receive that education for an hour(55.4%). The largest group wanted to learn about prevention of oral diseases, followed by toothbrushing, denture management, dry mouth. Given the findings of the study, senior welfare centers and senior cultural centers should offer oral health education programs as part of lifelong education to provide systematic and prolonged education for the elderly to improve their oral health care to promote their oral health.

Obstetric Complications by the Accessibility to Local Obstetric Service (지역별 분만서비스 접근도에 따른 산과적 합병증 비교)

  • Objectives: Pregnant women in rural areas do not have access to sufficient obstetric services in their own communities due to the shortage of obstetricians. Therefore, most of these women must seek obstetrician outside of their communities. The purpose of this study was to investigate the relationship between obstetric complications and accessibility to local obstetric care in Korea. Methods: This study was an ecological study in which the unit of analysis was an administrative district. Using Korea National Health Insurance Corporation data, the total number of deliveries and the delivery proportion within and outside of the community from 2001 to 2008 were calculated for 232 administrative districts nationwide. Three outflow levels were categorized based on each district's out-of-community delivery proportion: high outflow (upper one third), moderate outflow (middle one third), and low outflow (lower one third). In addition, three obstetric complication rates (the rate of complications following abortion, ectopic and molar pregnancy, the abortion rate, and the eclampsia rate) were calculated for the 232 districts. One-way ANOVA and multivariate linear regression were used to evaluate obstetric complications among the three outflow levels. Results: The high outflow districts had higher rates of eclampsia and complications following abortion, ectopic, and molar pregnancy compared to the other districts (ANOVA, p<0.05). However, there was no significant difference in the abortion rate among the three groups. Multiple linear regression analysis showed that high outflow districts were statistically significant in the rate of complications following abortion, ectopic and molar pregnancy and eclampsia rate after adjusting for local tax per capita (p<0.01). Conclusion: These results indicate that poor access to local obstetric care correlate with poor obstetric outcomes (delayed or excessive bleeding, embolism, genital tract or pelvic infection, shock or other complications following abortion and ectopic or molar pregnancy, or eclampsia).

Drug Resistance Rate of New Pulmonary Tuberculosis Patients Treated from the Private Sector in 2003~2005 (2003~2005년도 민간 병의원 신환자에서 분리된 결핵균의 항결핵약제 내성률)

  • Background: Surveillance of TB drug resistance (DR) is essential for providing information on the magnitude and trends in resistance, for developing treatment guidelines and for monitoring the effect of interventions. Up to now national surveys of drug resistance of M. tuberculosis have been conducted four times since 1994 among patients registered at health centers. The purpose of this study is to estimate the prevalence of primary drug resistance among new cases identified in private sector, and to compare it with the previous national drug resistance surveys. Methods: The study collected results of drug susceptibility testing (DST) performed at the Korean Institute of Tuberculosis by the request of private sector from January 2003 to December 2005, and then finally selected new cases for the analysis from the database of Korean TB Surveillance (KTBS) by matching patients' name and social identification numbers. Results: Of the 5,132 new patients included in the study, 689 (13.4%) patients were found to have drug resistance at least one drug, 530 patients (10.3%) were isoniazid resistant, 195 patients (3.8%) were multi-drug resistant (MDR), and 21 patients (0.4%) were extensively drug resistant (XDR). The rate of drug resistance tended to decrease annually but it was not statistically significant. When compared with previous national DR surveys in 2003 and in 2004 respectively, they were not significantly different. Conclusion: The prevalence of DR among new cases managed in the private sector did not show significant difference from that of new patients registered in the public sector in the same year.

The Analysis of Activities and Satisfaction of Volunteers for Hospice Care (호스피스 자원봉사자의 활동 및 만족도 분석)

  • Purpose : The purpose of this study was to analyze activities and satisfaction of volunteers for hospice care. Methods : We used 271 reports of hospice care and 20 questionnaire of activities and satisfaction of volunteers for hospice care from volunteers of in a community. Window SPSS-PC was used for the data analysis and the statistical method used were the descriptive statistics, t-test, ANOVA and Pearson's correlation coefficient. Results : 1) The highest score of activity was emotional area and the next activities was physical area. 2) The degree of emotional area (t=4.94, P<.05) and spiritual area (t=3.80, P<.05) was influence by religion of volunteers. 3) The mean score of satisfaction was 3.61. 4) There was correlation between duration of activity and death experience (r=.558, P<.05), emotional care (r=.698, P<.01), spiritual care (r=.474, P<.05), satisfaction (r=.651, P<.01), and the between physical care and family care (r=.559, P<.05), and the between spiritual cue and family care (r=.512, P<.05), and between emotional care and satisfaction (r=.536, P<.05). Conclution : The above result indicated that we must develope the management and education of volunteer of hospice care for various hospice care. Also, We should to encouraged the continuous education and efficient management.

Epidemiological Characteristics of Patients with Drug-Resistant Tuberculosis (약제 내성 결핵 환자의 역학적 특징)

  • Background : There is increasing concern in many countries about the problem of drug-resistant tuberculosis. Prevalence of primary drug-resistant tuberculosis is the optimal epidemiological indicator for long term monitoring of national tuberculosis control program. Our purpose was The purpose of our study is to assess clinical characteristics and socioeconomic status of patients with drug-resistant tuberculosis. Method : We studied 68 cases with drug-resistant Mycobacterium tuberculosis infection diagnosed at the Ewha Womans University Mokdong Hospital from March, 1995 to February, 2000. Results : Patients with primary drug-resistant tuberculosis(PDR) were younger (39.6$\pm$16.3 years vs. 48.2$\pm$16.5 years ; p<0.05), had more population of less than more were under the age of 40 years aged -group(62.9% vs. 36.4% ; p<0.05) and were more highly educated than those with acquired drug-resistant tuberculosis(ADR)(38.9% vs. 11.1% ; p<0.05). In patients with ADR, the rates of familial history of tuberculosis and living in a rented house residence in a rented house were increased higher than compared with to those of patients with PDR. Patients with ADR had more involved lobes(2.0$\pm$0.8 vs. 1.4$\pm$0.7 ; p<0.01) and longer treatment duration than those with PDR(18.3$\pm$7.2 months vs. 10.6$\pm$6.3 months ; p<0.05). Patients with ADR showed larger numbers of resistant were resistant to more number of drugs, lower hospitalization rate and higher rate of self-interruption of medication than those with PDR. In patients with PDR, mono-drug resistance was increased, whereas poly- and multi-drug resistances were decreased compared with those with ADR. Resistance to isoniazid was the highest among antituberculosis drugs, and resistance to isoniazid in patients with ADR was higher than that in patients with PDR(90.9% vs. 71.4% ; p<0.05). Conclusions : Patients with ADR were more likely to include more population be of lower socioeconomic class, and patients with PDR seem seemed to be young and socially active population. For control of drug-resistant Mycobacterium tuberculosis infection, proper isolation and prevention of patient with drug-resistant tuberculosis are needed.
