Analysis of the Dental Implants MRI Artifacts by Using the ACR Phantom (ACR 팬텀을 이용한 치아 임플란트 자기공명영상 인공물 분석)

  • ACR phantom for quality control of magnetic resonance imaging equipment can evaluate magnetic resonance imaging picture quality through various structures within the phantom. In this study, percent Signal Ghosting and Slice position accuracy of imaging could be analyzed by attaching implant and the wire for correction of tooth using ACR phantom in Head coil of 3.0T equipment. In the T1 weighted imaging of the first slice and the eleventh slice of implant, the slice position accuracy appeared to be good in ingress bandwidth 300, and it was good in ingress bandwidth 130 when wire for correction was attached. Percent Signal Ghosting in the seventh slice of SE T1 weighted imaging, implant and wire for correction added all appeared to be good when ingress bandwidth was 230. It is thought that in case of implant dental prosthesis patients in brain exam using magnetic resonance imaging, optimum image can be obtained by changing ingress bandwidth.

  • This study proposed a new classification system for maxillary anterior alveolar ridge deformities based on CAT-scan implant simulation as a useful concept in order to more precisely predict treatment outcomes and the necessity for ridge augmentation prior to implant placement. The results indicate that a high number of cases in the maxillary anterior area would require augmentation procedures in order to achieve ideal implant placement and restoration.

Three dimensional analysis of tooth movement using different types of maxillary molar distalization appliances (간접골성 고정원을 이용한 상악 구치부 원심이동 장치 종류에 따른 치아 이동 양상 평가)

  • Objective: The purpose of this study was to compare the three dimensional changes of tooth movement using four different types of maxillary molar distalization appliances; pendulum appliance (PD), mini-implant supported pendulum appliance (MPD), stainless steel open coil spring (SP) and mini-implant supported stainless steel open coil spring (MSP). Methods: These experiments were performed using the Calorific $machine^{(R)}$ which can simulate dynamic tooth movement. Computed tomography (CT) images of the experimental model were taken before and after tooth movement in 1 mm thicknesses and reconstructed into a three dimensional model using V-works $4.0^{TM}$. These reconstructed images were superimposed using Rapidform $2004^{TM}$ and the direction and amount of tooth movement were measured. Results: The mean reciprocal anchor loss ratio at the first premolar was 17 - 19% for the PD and SP groups. The appliances using mini-implants (MPD or MSP) resulted in less anchorage loss (7 - 8%). On application of a pendulum appliance or MPD, distalization was obtained by tipping rather than by bodily movement. Furthermore, the maxillary second molar tipped distally and bucally. But on application of MSP, distalization was achieved almost by bodily movement. Conclusions: Regarding tooth movement patterns during molar distalization, stainless steel open coil spring with indirect skeletal anchorage was relatively superior to other methods.

  • When teeth are extracted, patient face social, psychological and aesthetic problems which can be minimized by fabricating a interim immediate denture. Interim immediate denture manufactured using digital technology can be completed with reduced number of patients' visits and simple laboratory process. Implant-supported removable partial denture (ISRPD) has been suggested as alternative treatment option when fixed implant prosthesis is not feasible. In this case, interim immediate dentures were fabricated using digital technology for patient after teeth extraction and treatment using ISRPD by installing implants and surveyed crowns is found to be successful with better support, stability and maintenance of removable partial dentures.

  • Purpose: The purpose of this study is to compare the accuracy of the CT guided implant template that was produced by using an intraoral scanner according to the edentulous distance. Materials and methods: Five maxillary casts were fabricated using radiopaque acrylic resin with the second premolars, first molars, and second molars missing. Then a virtual cast was acquired by scanning each resin cast. Implant treatment was planned on the missing sites by superimposing the presurgical CT DICOM file and the virtual cast. Then the implants were placed using a surgical template followed by postsurgical CT scan. The distance and angle of the platform and apex between the presurgical implant and postsurgical implant were measured using the X, Y, and Z axis of the superimposed presurgical CT and postsurgical CT via software followed by statistical analysis using Kruskall-Wallis test and Mann-Whitney test. Results: The implant placement angle error increased towards the second molars but there was no statistically significant difference. The implant placement distance error at the platform and apex also increased towards the second molars and there was a statistically significant error at the second molars. Conclusion: Although the placement angle had no statistically significant difference between the presurgical implant and postsurgical implant, the placement distance at the platform and apex showed a larger error and a statistically significant difference at the second molar implant.

The influence of magnet on tissue healing after immediate implantation in fresh extraction sites in dogs (성견에서 발치 후 즉시 식립 임플란트에 설치한 자석이 주위 조직에 미치는 영향)

  • Statement of problem: The clinical use of electric and electomagnetic fields for fracture healing applications began in the early 1970s. Since then, several technologies have been developed and shown to promote healing of fractures. Developments of these devices have been aided in recent years by basic research and several well controlled clinical trials not only in the medical field but in dentistry. Purpose: The purpose of this study was to compare alveolar bone reduction following immediate implantation using implants onto which magnets were attached in fresh extracted sockets. Material and methods: Four mongrel dogs were involved. Full buccal and lingual mucoperiosteal flaps were elevated and third and fourth premolars of the mandible were removed. Implants with magnets and implants without magnets were installed in the fresh extracted sockets and after 3 months of healing the animals were sacrificed. The mandibles were dissected and each implant sites were sampled and processed for histological examination. Results: The marginal gaps that were present between the implant and walls of the sockets at the implantation stage disappeared in both groups as a result of bone fill and resorption of the bone crest. The buccal bone crests were located apical of its lingual counterparts. At the 12 week interval the mean of marginal bone resorption in the control group was significantly higher than that of the magnet group. The majority of specimens in magnet group presented early bone formation and less resorption of the buccal marginal bone compared to the control group. Conclusion: Within the limitations of this study, it could be concluded that implants with magnets attached in the early stages of implantation may provide more favorable conditions for early bone formation and reduce resorption and remodeling of marginal bone.

  • Excessive wear causes many complications when untreated, so that accurate diagnosis, analysis and predictive treatment plan should be made, and through evaluation of vertical dimension and stepwise treatment, a stable inter-arch relationship can be set. For the long-term success of implant treatment, ideal position and angle of implant is important, and its importance increases especially in multiple implant cases. Therefore, thorough diagnosis and planning, accurate surgery and prosthodontic procedures are significant. In this case, a 68-year-old male patient with a loss of vertical dimension due to multiple tooth loss and overall tooth wear was planned with systematic analyses from the pre-treatment stage to rehabilitate vertical dimension. Full-mouth fixed rehabilitation with computer tomography guided implant surgery was performed to the newly set vertical dimension and attained satisfactory outcomes both functionally and esthetically.

Maxillary anterior implant restoration with appropriate anterior guidance using T-Scan in a patient with full fixed prostheses (전악 고정성 보철 수복 환자에서 T-Scan 분석을 이용해 전-측방유도를 부여한 상악 임플란트 보철 수복)

  • In implant restorations, it is difficult for the patient to percept any symptoms. In addition, they are absent of shock absorbers, which can lead to mechanical failure if stress distribution is not considered. Since maxillary anterior multiple-implant restorations play a significant role in guiding the functional movement of the mandible by distributing lateral force, it is crucial to form appropriate occlusion. The use of the T-scan system is more advantageous in assessing 'dynamic occlusion', such as the change of occlusion over time, the amount of tooth contact during functional movement, and assessing the occlusion in the less-visible posterior teeth. The case is reported as it has satisfactory results in harmonious anterior guidance of a maxillary anterior multiple-implant restoration using T-scan analysis.

  • Induction heating is the process in which an electrically conducting object (usually a metal) is heated by electromagnetic induction through heat generated in the object by eddy currents. The main advantage of an induction heating device is the generation of the heat inside the target object itself. Hence, non-contact and safe heating devices are widely used in many industrial and medical fields. Recently, a new dental implant system was developed using a shape-memory alloy, wherein an artificial tooth could be easily removed from the dental implant by heating. This paper discusses the development of an induction-heating device to remove the dental crown in the new implant system. First, the finite element simulation of electromagnetic and thermal coupling analysis was implemented to obtain the temperature distributions of the target object for various frequencies, input currents, and coil shapes. Based on the simulation results, experiments were conducted by using prototypes, and an induction heating device was developed to remove the dental crown from the implant.

  • Traditional removable partial denture is a successful treatment for partial edentulous patients. When the abutment is formed with unilateral minority teeth, satisfaction with the use of removable partial denture may be lowered due to patient's discomfort and damage of the abutment. Recently, it has been reported that the unilateral posterior extension partial denture is used as a bilateral posterior extension partial denture using implant fixed prostheses as abutments. In this case, by using implant surveyed crown prostheses, bilateral posterior extension partial denture is fabricated in site that is predicted to have a poor prognosis. This resulted in improved support, maintenance, and stabilization of the removable partial denture, which were economically beneficial to patient and satisfied with use.
