Prosthetic Treatment Strategies for Improving Denture Retention in Pediatric Patients with Hypohidrotic Ectodermal Dysplasia: a Report of Two Cases (저한성 외배엽 이형성증을 가진 소아 환자에서 의치 유지력 향상을 위한 보철 치료 전략: 두 가지 증례 보고)

  • This report presents two cases concerning strategies for improving denture retention in pediatric patients with oligodontia caused by hypohidrotic ectodermal dysplasia (HED). Both patients presented with multiple missing teeth, conical canines, alveolar bone atrophy, and a skeletal Class III tendency. In the first case, a modified form of conical-crown-retained denture was used to cover the canines. This approach was carried out without tooth extraction or coping. In the second case of severe alveolar bone resorption and a distally tilted lower left canine, the tooth was restored with a hybrid ceramic crown. It was subsequently converted into a clasp-retained removable partial denture, utilizing a suction mechanism. Both patients are currently receiving regular check-ups for the maintenance of their prosthetic appliances and the evaluation of their growth patterns. This study presents innovative prosthetic treatment methods for pediatric patients with HED who have inadequate denture retention.

  • 지적 재산권 보호 중에서 디지털 저작물 보호는 근래에 활발히 연구되고 있으며 법 과학 분야는 지문감식, 치아감정, DNA 등 많은 분야가 있다. 법과학 분야 중 법적용 컴퓨팅(Forensic Computing)에 관한 응용은 새로운 연구 과제이다. 그 중에서도 디지털 저작물에 대하여 증거를 보전 하고자 많은 연구가 진행되고 있지만 디지털 저작물에 관하여 네트워크를 통한 능동적 저작물 보호는 미약하다. 현재의 데이터 추출(Extraction), 발굴(Exploitation), 복구, 암호 해독, 패스워스 풀기(Defeat), 미러 이미징 등의 방법 가지고 해결 못하는 경우와 인터넷 상에서 온라인으로 이루어지는 불법 복제에서 결정적 기여(smoking gun)를 찾아내려고 하는 것이 본 논문에서 해결 하고자 하는 부분이다. 오프라인일 경우도 가능하며 분석된 결과는 변호사/대리인, 법인, 보험회사, 법집행관 등에게 온라인으로 제공한다. 진행 과정은 서버에서 파견시킨, 미션을 부여받은 에이전트가 저작물 불법 복제 상황을 트래킹 한 후, 네트워크를 통하여 정해진 시간별로 서버에 전달하면, 법 조항과 매핑시켜서 분석한 다음 서버의 지식베이스에 저장되어 사용자의 요구에 응하는 능동형 디지털 저작물 보호 관리 시스템이다.

MANAGEMENT OF CROWDED DENTITIONS IN YOUNG CHILDREN (어린이 총생의 조기치료)

  • Dental crowding is one of the most common type of malocclusions in the mixed dentition. During the period of transition from the primary to the permanent dentition, minor incisor crowding is often present in the normally developing dentitions, but severe crowding can be caused by arch length/tooth size discrepancy. To determine the need for and appropriate timing of treatment for arch-length discrepancies, clinicians must be knowledgeable about normal development. This paper reviewed the literature on normal dental arch development and proper management of dental crowding according to its severity. Due to variations in the timing and the sequence of permanent tooth eruption, management of dental crowding should be specific to the individual patient.

A Study on Oral Health Awareness, Oral Health Behavior and Dental Caries among low Socio-Economic Status Children: the cases of local children's center in Incheon (저소득층 아동의 구강보건인식과 행위 및 치아우식실태 조사 (인천광역시 지역아동센터를 중심으로))

  • The purpose of this study was to attempt to lay the foundation for the development of oral health programs geared toward promoting the oral health of low socioeconomic class children. The subjects in this study were 257 school children who used local children's centers. The findings of the study were as follows: 1. The children mean scored 5.74 on oral health knowledge. 2. In terms of oral health awareness, 47.1% viewed the right toothbrushing as the best way to stay away from dental caries. 3. 45% of the subjects reported toothbrushing at least three times daily. 21.4% visited dental institutions three or more times in the past year. 33.1% had never undergone application of fluoride. 30.4% had never received oral health education. 4. The mean level of caries was 4.61 dft index in 1-2th grade, 3.27 DMFT index in 5-6th grade, 1.47 DMFT index in the 3-4th grad and 1.19 DMFT index in the 1-2th grade. 5. The mean level of Patient Hygiene Performance (PHP index) was 3.59, and there was no significant association was pound between PHP index and grade. 6. Oral health behavior wasn't affected by their oral health awareness, and knowledge.

Comparison of Expectation and Satisfaction of Implant Patients in Pre-post Implant Therapy (임플란트 시술환자 시술 전·후의 기대와 만족도 비교)

  • This study was conducted to improve patients satisfaction by comparing patients expectation prior to the implant therapy to patient satisfaction after the implant therapy. The data of 158 patients with dental implant in Seoul, Gyeonggi-Do and Cheonan city was obtained. The results are as follows : 1. 40.5% of patients placed one implant and 36.1 % of patients answered that the average cost for each implant is from 1,500,000 to 2,000,000won. 23.4% of them answered they decided to have implant because no tooth preparation is required. 2. Overall, the expectation level for implant therapy in general properties, women showed higher level of expectation than men. Especially, 'Implant has a longer life than other denture' showed significant difference between women(4.29) and men(3.87). Expectation by age, 'Implant has similar color and shape' was found the highest in age 20(4.38). Satisfaction by general properties, 'No tooth preparation required' was found the highest in age 20(4.57) and was statistically significant(p<.002). 3. Comparing patients' expectation prior to implant therapy to satisfaction afterward, 'Implant prevents jawbone resorption and keeps facial appearance' showed 3.7 of expectation but satisfaction was found increased to 3.87(p=.020). No tooth preparation required' was found 3.95 in expectation prior to implant therapy but after therapy it was found 4.23 in satisfaction (p<.001). 4. Overall satisfaction in implant therapy was found 4.25, recommendation to other patients was found 4.18 and repurchase was found 4.17.

Evaluation of Caries Status among Adolescents in Jeonju City with WHO Basic Methods, International Caries Detection and Assessment System II (ICDAS-II) (WHO basic methods와 International Caries Detection and Assessment System II (ICDAS-II)를 사용한 전주시 청소년의 우식상태 조사)

  • Since it is favorable to include the incipient caries lesion in the diagnostic criteria in adolescence, this study had surveyed dental caries status of adolescents at ages of 13 and 16 by using WHO basic methods and ICDAS-II codes. In this study, mean DMFT index was 3.71, and mean DT index was 1.94. For both indices, the age 16 group showed higher values than the age 13 group. By groups of teeth, DMFT index and DT index exhibited highest to the lowest values in molar, premolar, and anterior teeth, respectively. 77.46% of total numbers of teeth were classified as code 0 in ICDAS-II. Compared to anterior teeth, numbers of decayed teeth were increased in posterior teeth. All caries lesions in anterior teeth and premolars were limited to enamel. ICDAS-II code is an useful method to detect the incipient caries lesion, allowing preventive control on caries management.

Inpatient Dental Consultations to Pediatric Dentistry in the Yonsei University Severance Hospital (연세대학교 세브란스 병원 내 입원한 환자의 소아치과 의뢰 현황)

  • The goal of this study was to describe dental consultation of pediatric inpatients to the department of pediatric dentistry at Yonsei University Severance Hospital. 391 dental consultations at Yonsei University Severance Hospital referred to pediatric dentistry in the year 2012 were included in this study. Consultations were categorized according to patients' gender, age, chief complaint, referred department and diagnosis. 288 patients (166 males and 122 females) with an average age of 5.9 were referred to the Department of Pediatric Dentistry. 129 cases (33.1%) from Department of Rehabilitation Medicine, 80 cases (20.5%) from Pediatric Hematology- Oncology, 51 cases (13.0%) from Pediatric Cardiology, and 44 cases (11.3%) from Pediatric Neurology. Chief complaints were ranked from oral examination (39.7%), dental caries (14.0%), pre-operative evaluation (12.8%) and others (33.5%); including oral pain, trauma, tooth mobility, orthodontic treatment, self-injury, fabrication of obturator and etc. Dental consultations should be encouraged as dental care and treatment could affect the control of systemic diseases of admitted patients. Pediatric inpatients have been referred to pediatric dentistry for not only comprehensive oral exam but also various chief complaints. The most frequent dental diagnosis made and treatment performed were dental caries and non-invasive/preventive care respectively.

Retrospective Study of Infants who Visited Pediatric Dentistry (소아치과에 내원한 영아들에 대한 후향적 연구)

  • The first dental visit is recommended at the time of the eruption of the first tooth and no later than 12 months of age. However, even before the age of 1, children can visit the dental hospital for various reasons. The purpose of this study was to analyze the reasons for the dental visit of infant. From January 2006 to December 2015, medical records of infants who visited the Department of Pediatric Dentistry of Kyung Hee University were analyzed. The total number of patients was 419 (238 males and 181 females). The reasons for the dental visits were trauma (47.5%), natal/neonatal tooth (19.8%), dental caries (8.1%), teething problem (4.3%), abnormal frenum (3.6%), soft tissue swelling (3.6%), Bohn's nodule (3.3%), cleft lip and palate (2.9%), gingival neoplasm (1.9%), tongue ulceration (1.7%), oral examination (1.4%), enamel hypoplasia (1.2%) and abnormal temporomandibular joint sound (0.7%). According to this study, there were various oral diseases that could occur in infants. Since infants are usually cared by caregivers, pediatricians, and obstetricians, education of oral diseases of infants is needed to manage the oral symptoms properly.

DEVELOPMENTAL DISTURBANCE OF PERMANENT TEETH AFTER RADIOTHERAPY FOR TREATMENT OF MALIGNANT TUMOR : REPORT OF CASES (소아암 환자에서 방사선 치료 후 영구치 치근발육장애 : 증례보고)

  • Multimodal cancer therapy including surgery, chemotherapy, and radiotherapy could not only improve the prognosis of malignancy but also reduce the dosage and toxicity of cancer drug for treatment of malignant tumor. The effects of radiotherapy are generally localized, additive, and accumulative, and depend on dosage, site and cell sensitivity. However, in growing individuals, the dental and skeletal sequelae to radiotherapy result in dental or facial abnormalities that are irreversible : arrested root development, disturbances in enamel formation, microdontia, anodontia, altered tooth eruption and mandibular or maxillary hypoplasia. Especially, the teeth which are developing is affected according to the stage. We report three cases of developmental disturbance of permanent teeth after radiotherapy. These children had received radiotherapy for malignant tumor at the age of 3 to 4 years, in which root hypoplasia, short tapered root and early apex closure were observed. For the management of radiation caries and radiotherapy-related teeth, periodic recall check and oral hygiene instruction are required.

The Anti-inflammatory Effect of Green Tea Extract Against Prevotella intermedia (녹차추출물의 잇몸 질환 원인균에 대한 항염증 효능 연구)

  • Dental bacteria can cause gum diseases, i.e. gingivitis and periodontitis, by inducing inflammation in human gingiva. Therefore, the most effective way to prevent and treat gum diseases is the control of the inflammatory reactions induced by dental bacteria. Almost all present dental care products contain anti-bacterial agents to eliminate dental bacteria. However, recent studies report that even heat-killed dental bacteria can induce the inflammation responses in oral cells. Therefore, the method using anti-bacterial agents should be improved for better anti-inflammatory effect and the effective natural anti-inflammatory substances need to be found. In addition, the mechanisms of gingival inflammation should be elucidated. In this study, we tried to find out the mechanism of the gingival inflammation and effective natural anti-inflammatory substances with human gingival epithelial cells and Prevotella intermedia which is well known as a typical dental bacteria inducing gingivitis and periodontitis. In results, Prevotell intermedia initiated the gingival inflammation response by stimulating gingival epithelial cells to release an inflammatory cytokine, IL-8. Furthermore, the inflammation by Prevotella intermedia is related to COX-2, AP-1, and TNF-${\alpha}$ pathways. Green tea extract could effectively suppress the inflammatory responses induced by Prevotella intermedia. We find out the effective natural substance for the improvement of gum diseases by studying the mechanism of the gingival inflammation induced by dental bacteria.
