Interleukin-8 and MCP(Monocyte Chemoattractant Protein)-1 expression by the Human Dental Pulps in cultures stimulated with Substance P (사람치수에서 Interleukin-8과 Monocyte chernoattractant protein-1의 분비에 대한 Substance P의 효과에 관한 연구)

  • The induction of the IL-8 and MCP-1 by the stimulation of Substance P and TNF-${\alpha}$ (IL-8 agonist) and the specificity for SP using Spantide (SP antagonist) in the dental pulp tissues was measured quantitatively. In addition, the secretion of the IL-8 in the human dental pulp tissue 36 hrs after the stimulation of SP was observed after the stimulation of SP qualitatively. According to this study the results were as follows: 1. There was the significant IL-8 induction at 36 h after SP (10$^{-4}$M) stimulation of the pulp tissue comparing with the unstimulated dental pulp tissues (p < 0.05) . IL-8 irnmunostaining was weakly detected along the periphery of the pulp tissue after Mock stimulation and IL-8 immunostaining was detected around the fibroblast in the pulp tissue 36h After SP (10$^{-4}$M) stimulation, 2. The secretion of MCP-1 from the dental pulp tissues comparing with Mock stimulation was induced at 36 hrs after TNF-$\alpha$ (40 ng/ml) stimulation, but no induction with SP(10$^{-4}$M) TNF-${\alpha}$ (40 ng/ml) did not induce the IL-8 secretion from the pulp tissue, weak IL-8 imrnunostaining was detected along the periphery of the pulp tissue 3. Spantide (10$^{-5}$M) inhibited IL-8 induction from the pulp tissues 36 h after SP (10$^{-4}$M) stimulation These results suggest that SP significantly induces IL-8 recruiting neutrophils in localized human dental pulp tissue MCP-1 appears to be less involved in the early establishment of pulpal inflammation in response to irritation such as mechanical insult of dentin. SP may have positive relation with the inflammation of the human dental pulp tissues.

Pulp and periodontal tissue changes following rapid tooth retraction by periodontal distraction in young adult dogs (유성견에서 periodontal distraction에 의한 급속 치아견인 시 치수 및 치주조직의 변화에 관한 연구)

  • The aim of this study was to evaluate pulp and periodontal changes following rapid tooth retraction by periodontal distraction after bone undermining surgery in young adult dogs. Methods: Alter extraction of second premolars, the interseptal bone mesial to the upper 3rd premolar was undermined. After activating the distraction appliance at 0.5 mm/day for six days, the dogs were sacrificed at 0, 1, 3, 5, 7, and 9 weeks during the consolidation period. Tissue changes of periodontium and pulp were evaluated radiologically, histologically, and immunohistochemically. Results: Digital subtraction radiography showed active bone formation in the stretched periodontal ligament from 0 - 4 weeks. Resorption of the alveolar bone, appearance of osteoclasts, and infiltration of inflammatory cells were observed just after the activation period at the pressure side, and distinctive bone formation was seen in the tension side of the periodontal ligament from 1 week. New bone formation was active at 1 - 3 weeks. The expression of calcitonin gene-related peptide in the experimental group was increased at the alveolar bone and pulp, and periodontal ligament at the pressure side from 0 - 1 week, and it decreased after 5 weeks to become similar to that of the control group. Conclusions: The results showed that rapid tooth movement using periodontal distraction can be new form of orthodontic tooth movement for accelerating normal bone formation.

A CLINICAL EVALUATION OF SAFETY OF AN OFFICE BLEACHING GEL CONTAINING 30% HYDROGEN PEROXIDE (30% 과산화수소를 함유한 전문가 미백제의 안전성 평가)

  • This study evaluated the safety of an office bleaching gel (RemeWhite, Remedent Inc., Deurle, Belgium) containing 30% hydrogen peroxide. 37 volunteers were received office bleaching with the RemeWhite for 3 times at one visit, total 2 visits. As control group, the same gel in which hydrogen peroxide was not included was applied to 34 volunteers with the same protocol. There was no difference between experimental group and control group using electric pulp test. In the result of gingival inflammation index and tooth sensitivity test, there was mild pain response in experimental group but it disappeared as time went by. Therefore, safety of the office bleaching gel containing 30% hydrogen peroxide was confirmed.

Effect of local anesthesia on pulpal blood flow in mechanically stimulated teeth (기계적으로 자극 받은 치아에서 국소마취가 치수혈류에 미치는 효과)

  • The aims of the study were to evaluate the effect of epinephrine-containing local anesthetics on pulpal blood flow (PBF) and to investigate its effect on cavity preparation-induced PBF change. PBF was recorded using a laser Doppler flowmeter (Perimed Co., Sweden) from canines of nine cats under general anesthesia before and after injection of local anesthetics and after cavity preparation. 2% lidocaine hydrochloride with 1 : 100,000 epinephrine was administered by local infiltration given apical to the mandibular canine at the vestibular area and the same volume of isotonic saline was injected on the contralateral tooth as a control. A round carbide bur was operated at slow speed with isotonic saline flushing to grind spherical cavities with increasing depth through the enamel and into the dentin on both teeth. The obtained data was analyzed with paired t-test. Cavity preparation caused significant increase of PBF (n = 9, p < 0.05). Local infiltration of lidocaine with epinephrine resulted in decreases of PBF (n = 9, p < 0.05), whereas there was no significant change of PBF with the physiologic saline as a control. Cavity preparation on tooth anesthetized with lidocaine with epinephrine caused significantly less increase of PBF than in control tooth (p < 0.05). Therefore, the result of the present study demonstrates that local infiltration of 2% lidocaine with 1 : 100,000 epinephrine effectively reduces PBF increase caused by cavity preparation.

Comparison of gene expression profiles of human dental pulp cells treated with mineral trioxide aggregate and calcium hydroxide (인간치수세포에 Mineral Trioxide Aggregate와 수산화칼슘 제재 적용 시 유전자 발현 양상 비교)

  • Objectives: This study investigated changes in gene expressions concerning of differentiation, proliferation, mineralization and inflammation using Human-8 expression bead arrays when white Mineral Trioxide Aggregate and calcium hydroxide-containing cement were applied in vitro to human dental pulp cells (HDPCs). Materials and Methods: wMTA (white ProRoot MTA, Dentsply) and Dycal (Dentsply Caulk) in a Teflon tube (inner diameter 10 mm, height 1 mm) were applied to HDPCs. Empty tube-applied HDPCs were used as negative control. Total RNA was extracted at 3, 6, 9 and 24 hr after wMTA and Dycal application. The results of microarray were confirmed by reverse transcriptase polymerase chain reaction. Results: Out of the 24,546 genes, 43 genes (e.g., BMP2, FOSB, THBS1, EDN1, IL11, COL10A1, TUFT1, HMOX1) were up-regulated greater than two-fold and 25 genes (e.g., SMAD6, TIMP2, DCN, SOCS2, CEBPD, KIAA1199) were down-regulated below 50% by wMTA. Two hundred thirty nine genes (e.g., BMP2, BMP6, SMAD6, IL11, FOS, VEGFA, PlGF, HMOX1, SOCS2, CEBPD, KIAA1199) were up-regulated greater than two-fold and 358 genes (e.g., EDN1, FGF) were down-regulated below 50% by Dycal. Conclusions: Both wMTA and Dycal induced changes in gene expressions related with differentiation and proliferation of pulp cells. wMTA induced changes in gene expressions related with mineralization, and Dycal induced those related with angiogenesis. The genes related with inflammation were more expressed by Dycal than by wMTA. It was confirmed that both wMTA and Dycal were able to induce gene expression changes concerned with the pulp repair in different ways.

DIRECT PULP CAPPING WITH BONDING RESIN (접착용 레진을 이용한 유치의 직접 치수복조술에 관한 연구)

  • A direct pulp capping with different medicaments has been attempted for a long time. The most commonly used among those is the calcium hydroxide. In primary teeth, however, a success rate of direct pulp capping with calcium hydroxide has been reported to be lower than that of pulpotomy. The disappointing results of calcium hydroxide have prompted the search for other capping materials. Lately, several researchers suggested an application of adhesive resin-based composite systems as a capping material. They claimed that when an exposed vital pulp is capped directly with bonding resin, the pulp tissue is free of inflammation or necrosis without clinical symptoms. The aim of this study was to compare short-term effects of the bonding resin which was applied on the mechanically exposed vital pulp tissue and those of direct pulp capping with calcium hydroxide. The second objective was to compare success rates of the primary teeth which already underwent physiologic root resorption and those of the teeth which had not undergone physiologic root resorption yet, in each capping material groups. The vital, healthy pulp of forty-one primary teeth were exposed mechanically during a cavity preparation. They were divided into two groups: Group 1(n=21) underwent capping with bonding resin, and group 2(n=20) underwent capping with calcium hydroxide. Then these two groups were subdivided into two groups in each : the teeth which show physiologic root resorption and the teeth without root resorption. All of the sample teeth were restored with composite resin. Clinical evaluations such as percussion test, ice test, EPT, were recorded and also before- and after- standard x-ray films were compared and evaluated to decide whether the case was successful or not. Evaluation was performed at least 3 months after the capping materials. The results were as follows 1. There was no difference in success rate between group 1 and group 2. 2. Success rate of the teeth with physiologic root resorption was higher than that of the teeth without physiologic root resorption in group 1 and group 2. 3. There was no difference in success rate between anterior teeth and posterior teeth.

CONSERVATIVE ENDODONTIC TREATMENT OF PERMANENT TEETH WITH PERIAPICAL LESIONS : CASE REPORTS (치근단 병소를 가진 영구치의 보존적 치수 치료)

  • Recently, undifferentiated stem cells which exist in dental papillae of immature permanent teeth were newly discovered and these stem cells appear to be the origin of ameloblasts associated with the formation of root dentin. When treating immature permanent teeth, the preservation of these stem cells induce the continuous formation of the root. Therefore, it is reported that minimal invasion to periapical region in immature permanent teeth with periapical inflammation resulted in good-healing pattern in clinical and radiographic examination. In this case, a 10 year-old boy(mandibular right premolar) and a 8 year-old girl(maxillary left premolar) who visited the department of pediatric dentistry at Chosun University Dental Hospital were diagnosed with pulp necrosis and periapical abscess in clinical and radiographic examination. Endodontic instrumentation to the periapical region was limited and MTA(Mineral Trioxide Aggregate) was applied into the pulp canal. The periodic checks showed healing of periapical abscess and the development and growth pattern of roots. In permanent teeth with pulp necrosis and periapical abscess, preservation of pulp and dental papillae in the periapical region showed good prognosis during the periodic examinations. Therefore, a lot of clinical examination and long-term evaluation of conservative pulp treatment in immature permanent teeth are expected to be necessary.

Comparative Histologic Study of 3-Root Canal Filling Materials for Dog's Teeth (성견 치아의 치수절제술 후 3가지 근관충전재에 따른 조직학적 반응의 비교)

  • Endodontic treatment of primary molars is a great challenge to the dentists because of their complex morphology. However, there have been limited controlled in vivo studies about filling materials for primary teeth. Hence, the aim of this study is to evaluate and compare histologic responses of different calcium hydroxide canal filling materials that are used in daily clinical practice. Pulpectomies were performed in 132 teeth of the dogs. The root canals were randomly filled with either $Vitapex^{(R)}$ (n = 44), $Metapex^{(R)}$ (n = 44), or $Metapaste^{(R)}$ (n = 44). After 4 or 13 weeks, respectively after operation, the dogs were sacrificed, and teeth were processed for histologic examinations. Inflammatory reaction was evaluated and scored in a blind manner. The Fisher's exact test and Kruskal-Wallis test were used to compare the tested groups. In 4-week-group, all inflammatory responses were in normal range except for fibrosis. In 13-week-group, increased response to various inflammation reaction was shown compared to the 4-week-group. However, there were no statically significant differences between the tested groups in all inflammatory reaction. Overall, there were no significant differences among three materials in responses of apical tissues and all of them showed favorable tissue responses.

DENS INVAGINATUS AND A VITAL MAXILLARY LATERAL INCISOR WITH LATERAL PERIODONTAL ABSCESS (생활력이 있는 상악측절치에서 치내치로 인한 측방치주농양이 형성된 증례)

  • Dens invaginatus is a developmental anomaly resulting from an invagination of the enamel organ. The incidence is highest with maxillary permanent lateral incisors. The reported occurrence ranges from 0.04 to 10%. This anomaly may involve the pulp and periapical tissues and cause pulpal inflammation, loss of vitality, apical and lateral periodontitis, periapical abscesses and cysts and stimulate internal resorption. Oehlers describes dens invaginatus as occurrence in three forms. In treating type 3 invaginatus, treatment strategy can be determined by considering the complexity and accessibility of invagination. In this case, showing simple invagination, it could be treated by simple endodontic treament confining to invagination without loss of vitality of tooth. After treatment of the present case, the results were as follows: 1. In type 3 dens invagiantus, if the tooth is vital and there is no evidence of communicating between invagination and pulp, we can save the vitality of the tooth and resolve the lesion by endodontic treament confining to the invagination. 2. In the invagination with opened apex, the closure of apex can be induced by apexification procedure doing this, we can avoid the neccessity of surgical intervention.

APEXOGENESIS OF A DENS EVAGINATUS (하악 소구치에 발생한 치외치의 치근단유도술)

  • Dens evaginatus is a dental anomaly most commonly seen in premolar teeth in which a tubercle or protuberance projects from either the center of the occlusal surface or the buccal triangular ridge. These tubercles are easily fractured from mastication as the tooth erupts and frequently leads to pulp necrosis as a common complication. To prevent these sequelae, prophylactic treatment soon after the tooth starts erupting is essential. These preventive treatments include, selective grinding and protection of the tubercle by pit and fissure sealant. When the tooth does shows signs of pulp necrosis and apical periodontitis, endodontic procedures are needed. Apexification and apexogenesis are usually the treatment of choice for the affected teeth which have immature apices. Apexogenesis is a vital pulp therapy procedure performed to encourage continued physiological development and formation of the root end. It involves removal of the inflamed pulp and the placement of calcium hydroxide on the remaining healthy pulp tissue. This case report describes an atypical apexogenesis of a mandibular premolar which showed to be a dens evaginatus. The tooth which was treated with calcium hydroxide shows good results and is planned for permanent root canal filling.
