Ultrasonic cavitation is a physical phenomenon that generates and collapses microbubbles in media (mainly fluids) under conditions of strong ultrasonic irradiation. In this study, changes in the ultrasonic acoustic characteristics of bubble clouds in relation to ultrasonic irradiation were observed by the quantitative evaluation of cavitation yields. Concave-type single ultrasonic transducers with center frequencies of 500 kHz and 1.1 MHz were used to produce cavitation, and 2.25 MHz interference ultrasonic waves that would traverse any bubble clouds generated were used to analyze the cavitation. The parameters used for the evaluation of cavitation yields (changes in the center frequency, attenuation characteristics, and the propagation time of penetrating waves) were analyzed in relation to the cavitation-generating conditions (irradiation intensity, excitation signal, and center frequency). On the basis of these results, correlations between the changes in the center frequency and irradiation intensity were identified. Although the correlation coefficient was low, notable changes were observed in the center frequency under certain irradiation conditions. Attenuation trends in the interference ultrasonic waves showed high correlations with all the irradiation conditions, and it was noted that these trends were not affected by the forms of cavitation generated. No differences in the propagation time were observed among different irradiation conditions. These findings suggest that bubble yields can be quantitatively evaluated effectively by evaluating the diverse irradiation conditions and that such a quantitative evaluation could be used to study the basic cavitation phenomenon occurring in high-intensity ultrasonic wave treatment.
Purpose : This study was done to analyze the changes in the clinical conditions and the diagnosis of hypertrophic pyloric stenosis. Methods : We report a retrospective clinical analysis of 39 patients with hypertrophic pyloric stenosis from Jan. 1992 to Aug. 2001. The age and sex distribution, family and birth history, clinical symptoms, the ultrasonographic and the operative sizes of pyloric canals were compared. Results : The body weight was below the 3 percentile at admission in eight cases(20.5%). "Olive like mass" in right upper quadrant was palpated during physical examination in 23 cases(59%) and gastric peristaltic wave observed in six cases(15%). The ultrasonographic measurements showed that the pyloric muscle thickness to be $4.95{\pm}0.99mm$($mean{\pm}SD$), pyloric diameter $14.42{\pm}2.64mm$, and pyloric length $20.17{\pm}3.92mm$. Fredet-Ramstedt pyloromyotomy was employed in all cases. The operative measurements of the pyloric muscle thickness was $5.11{\pm}1.01mm$, pyloric diameter $15.01{\pm}2.47mm$, and pyloric length $22.32{\pm}3.43mm$. Conclusion : There was no significant difference between the ultrasonographic and operative measurements. Currently, the hypertrophic pyloric stenosis patients showed lesser clinical hallmarks of the disease. The earlier diagnosis using imaging studies before development of significant metabolic abnormalities is becoming an important factor that change the future outcomes of hypertrophic pyloric stenosis.

Purpose: The purpose of this study was to analyze the effectiveness of suprascapular nerve block using platelet-rich-plasma (PRP) under ultrasonographic guidance in patients treated with arthroscopic rotator cuff repair. Material and Methods: 50 cases of patients, from March 2013 to March 2014, treated with arthroscopic rotator cuff repair were retrospectively analyzed. We performed ultrasonography-guided suprascapular nerve block using platelet-rich-plasma (PRP) to these patients in the outpatient clinics at the 6 weeks follow-up after operation. We evalulated results for visual analogue score (VAS) for pain, range of motion (ROM), Constant Shoulder Score (CSS) for these patients before arthroscopic operation, following 6 weeks and 3 months after operation. Results: There was clinically significant improvement in VAS, ROM, CSS after ultrasonography-guided suprascapular nerve block using platelet-rich-plasma (PRP). Conclusion: Ultrasonography-guided suprascapular nerve block using platelet-rich-plasma (PRP) is an effective treatment method not only for around shoulder pain but also postoperative residual shoulder pain and limitation of shoulder motion.
In this paper, we investigate the thermal response of skin tissue to high-intensity focused ultrasound (HIFU) by means of infrared (IR) thermal imaging. For skin tightening, a 7-MHz ultrasound transducer is used to induce irreversible tissue coagulation in porcine skin. An IR camera is employed to monitor spatiotemporal changes of the temperature in the tissue. The maximum temperature in the tissue increased linearly with applied energy, up to $90^{\circ}C$. The extent of irreversible tissue coagulation (up to 3.2 mm in width) corresponds well to the spatial distribution of the temperature during HIFU sonication. Histological analysis confirms that the temperature beyond the coagulation threshold (${\sim}65^{\circ}C$) delineates the margin of collagen denaturation in the tissue. IR thermal imaging can be a feasible method for quantifying the degree of thermal coagulation in HIFU-induced skin treatment.

Purpose: We retrospectively studied the outcomes of the shoulder impingement syndrome for the treatment of the ultrasound-guided subacromial bursal steroid injection. Materials and Methods: Sixty-six shoulders of sixty-two patients with shoulder impingement syndrome treated from March, 2006 to April, 2009 were involved in this study. All cases underwent standardized, nonoperative treatment protocol consisting of 5~12 MHz high resolution ultrasound-guided local steroid injection into the subacromial bursa in modified Crass position. The shoulder range of motion, VAS score and impingement signs were evaluated during the initial and 1year visits. After injection, shoulder elevation exercise was encouraged. Statistical analysis with ANOVA model and Tukey's post-hoc test with the significance level at 5% were performed using SAS program. Results: All cases showed improved range of motion without limitation of shoulder function at immediate post-injection, 6-week, 3-month and 1year visits. The average VAS score at one year follow-up decreased to 2.85 from 6.47 before injection. In all cases the impingement signs became negative immediate after injection. However, 6 cases showed positive impingement signs after 6-week, which became negative after reinjection. The range of motion and VAS score were improved at one-year follow-up compared to initial visit (p<0.0001). No complication was noted at all follow-up period. Conclusion: Ultrasound-guided subacromial steroid injection alleviated the need of surgery, because it was successful in all our cases to improve pain and function of the shoulders until one year follow-up period.

A 20-year-old patient who got injured on his right big toe and complained pain visited to our hospital 9 months ago. Before the patient transferred to our hospital, plain radiographs were obtained at a private hospital and the patient underwent conservative treatment with uncertain diagnosis. However, the symptom was not improved, and he continued to complain intra-articular pain on his interphalangeal joint of big toe during plantarflexion or dorsiflexion. On plain radiographs of our hospital, we observed small bone fragment in his interphalangeal joint of the big toe. On the ultrasonographic images, two loose bodies in the interphalangeal joint were found. Then, we removed the loose bodies as surgical treatment, and the symptom was subsided completely. The purpose of this study is to report the ultrasonographic finding of the loose bodies of interphalangeal joint of the big toe.
The authors, over the last 6 months, have treated 2 patients with perforated typhlitis complicating acute lymphocytic leukemia (ALL) with good outcome. The first patient was a 13-year-old male who developed intermittent high fever, abdominal pain, abdominal distention and diarrhea during the course of maintenance chemotherapy. The peripheral leukocyte ranged from 230-470/$mm^3$. Serial ultra sonograms and CT scans demonstrated irregular thickening of the cecal and ascending colonic walls and subsequent ragged perforation of the posterior wall of the cecum. He survived after treatment by right hemicolectomy and aggressive supportive measures. The patient case was a 3 year-old female who developed intermittent high fever, right lower abdominal pain, a mass, and watery diarrhea during the course of maintenance chemotherapy. Serial ultra sonograms and CT scans demonstrated irregular thickening of the cecal wall (6-15mm in thickness) and subsequent small perforation of the posterior wall of the cecum with thick-walled localized abscess. She has recovered completely after aggressive medical management. We learned two lessons from our experience treating these patients:1) early diagnosis provided by a high index of suspicion and the use of ultra sonogram or CT scan is essential. And 2) although perforation is one of the surgical indications for the treatment of typhlitis, it is possible to manage the perforation nonoperatively in selected cases with localized abscess.
The drug therapy for rheumatoid arthritis must be maintained constantly or for the whole life but is apt to induce the adverse effects in gastrointestinal or renal system. Therefor, newer methods are paid attention to reduce adverse effects. The thirty-two rat female separated into seven groups depending on the therapy or Freund's adjuvant applied: Normal group(n=8) not received anything, Positive control group(n=8) only received adjuvant, USC group(n=8) received continuous US, USP group(n=8) received pulsed US. The physical and radiological findings by thermal and non-thermal effects of ultrasound were evaluated in groups with continuous mode at 1 MHz, $0.5\;W/cm^2$ for 6 minutes and with pulsed mode (duty cycle 1:9) at 1 MHz, $0.5\;W/cm^2$ for 6 minutes. The result summarized followings. 1. Swelling of forepaw and hindpaw was significantly reduced in USP. 2. Arthritis indices in USP group were significantly reduced than those in PCG. 3. In naked eye and radiologic findings, swelling was significantly prevented in USP group but not in PCG. The change of swelling, arthritis index, gross feature in naked eye, radiologic finding were significantly improved in all groups except for USC group, taking care for using its continuous mode.
Background: The purpose of this study was to investigate the effects of pulsed-ultrasound intervention and continued-ultrasound on the MVIC (maximal voluntary isometric contraction) and active ROM (range of motion) recovery of before EIMD (exercise-induced muscle damage). Design: Randomized controlled trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I (n=10) and experiment group II (n=10). The subjects in experimental group were intervened by pulsed-ultrasound and continued-ultrasound, while ones on control group weren't by any intervention after induced EIMD. Results: First, In comparison of the MVIC, in the among group comparison, the MVIC of continued-ultrasound group was significantly larger than those of other groups (p<.005). Second, In the among group comparison, the active extension angle of continued-ultrasound group was significantly smaller than those of other groups (p<.005). Third, In the among group comparison, the active flexion angle of continued-ultrasound group was significantly lager than those of other groups (p<.05). Conclusion: The above results revealed that the continued-ultrasound intervention before an exercise had a significantly improve of muscle function after EIMD. Therefore we can consider the continued ultrasound as a considerable intervention method to prevent or reduce an exercise injury.
