A Study of Clinical Model for Radiation Therapy in Lung Cancer Patients of Busan and South Gyeongnam Province (부산, 경남지역 폐암 환자의 방사선치료 이용에 대한 임상 결정 모델 연구)

  • Radiation therapy for lung cancer is an effective treatment during monotherapy or combination therapy. Studies have reported that the optimum utilization rate of radiation therapy is estimated at 61% to 74%. Radiation therapy in Korea has been investigated to be low; further studies are needed. This study was intended to assess the appropriateness of the use of radiation and to reveal the use of radiation therapy-related factors by examining radiation therapy in lung cancer patients of Busan and South Gyeongnam Province. This study was aimed at the population diagnosed with lung cancer in Busan and South Gyeongnam Province. To conduct the study, 1036 patients enrolled in two hospitals were collected and 897 appropriate as subjects were selected. We compared the optimum utilization rate and actual rate of radiation therapy, and revealed the adequacy and related factors for use of radiotherapy. Of 897 patients, 503 (56%) were treated with medical therapy and 394 (44%) were given radiotherapy. The radiotherapy utilization rate of all lung cancer patients was 42%. The proportion of non-small cell lung cancer by histologic type was 33% and that of small cell lung cancer was 90%. Factors related to radiation therapy used in cancer were age, histological type, clinical stage, doctor refereed to, and clinical examination. Compared to radiation utilization by region (site), curative chest therapy was 42%; palliative treatment was 26%. In the comparison of histologic types, utilization of small-cell lung cancer is lower; the lowest especially in the stage III. Utilization of radiation therapy in Busan and South Gyeongnam Province was lower than the reasonable one. Utilization difference could be explained by patient factors, tumor factors, and health service factors. To improve utilization,development ofoutreach service programs and activation of the multidisciplinary team are required.

Analysis of the Screening Rates and Examination Results of the National Dental Screening for Infants and Children (국가 영유아 구강 검진의 수검률과 수검 결과에 대한 분석)

  • The purpose of this study was to analyze the screening rates and examination results of the national dental screening program for infants and children since its initiation. Data for this study were obtained from the annual health screening statistical yearbook of the National Health Insurance Service. The investigation focused on the screening rate according to year, age, and region, as well as examination results from 2008 to 2021. The study revealed a general increase in screening rates over the years, with a simultaneous decrease observed as the age of the subjects increased. Regional variations in screening rates were observed, with the highest and lowest rates in Sejong and Jeonbuk, respectively. Analysis of examination results revealed an increasing proportion of subjects requiring treatment as they aged. From 2008 to 2013, less than 15% of subjects aged 18 - 24 and 18 - 29 months required treatment, whereas over 30% required treatment in the remaining age groups. Between 2014 and 2021, the proportion of subjects in the 18 - 29 month age group requiring treatment remained under 15%, but it exceeded 40% in the other age groups. In conclusion, active participation in national dental screening for infants and children is crucial to improving oral health outcomes.

Interregional Variant Factor Analysis of Hypertension Treatment Rate in COVID-19 (코로나19에서 고혈압 치료율의 지역 간 변이요인 분석)

  • The purpose of this study is to analyze regional variation factors of hypertension treatment rate in COVID-19 based on the analysis results based on ecological methodology. To this end, data suitable for ecological analysis were collected from the Korea Centers for Disease Control and Prevention's regional health statistics, local government COVID-19 confirmed cases, National Health Insurance Corporation, Health Insurance Review and Assessment Service's welfare statistics, and Korea Transport Institute's traffic access index. Descriptive statistics and correlation analysis were conducted using SPSS Statistics 23 for regional variation and related factors in hypertension treatment rate, and geographical weighted regression analysis was conducted using Arc GIS for regional variation factors. As a result of the study, the overall explanatory power of the calculated geo-weighted regression model was 27.6%, distributed from 23.1% to 33.4% by region. As factors affecting the treatment rate of hypertension, the higher the rate of basic living security medical benefits, diabetes treatment rate, and health institutions per 100,000 population, the higher the rate of hypertension treatment, the lower the number of COVID-19 confirmed patients, the lower the rate of physical activity, and the alcohol consumption. Percentage of alcohol consumption decreased due to COVID-19 pandemic. It was analyzed that the lower the ratio, the higher the treatment rate for hypertension. Based on these results, the analysis of regional variables in the treatment rate of hypertension in COVID-19 can be expected to be effective in managing the treatment rate of hypertension, and furthermore, it is expected to be used to establish community-centered health promotion policies.

DENTAL ANXIETY OF SECOND GRADE ELEMENTARY SCHOOL CHILDREN PROJECTED IN THE DRAWING OF DENTAL TREATMENT SITUATION (초등학교 2학년 어린이의 치과치료장면 그림에 투사된 치과불안도)

  • The purpose of this study was to observe dental anxiety of second grade elementary school children projected in the drawing of dental treatment situation. The subjects of study were 213 school children. Questionnaire survey was done regarding their experience of pain and fear during the dental treatment and children's attitude toward the dental treatment and dentist. Drawings were analyzed using criteria modified from Dental Anxiety Scale(DAS) by Sheskin et al. The mean anxiety score was 2.00 and the range was from 0 to 6. The mean anxiety score was 2.82 in untreated children and 1.92 in treated children (P<0.05). The mean anxiety score was 2.00 in boys and 1.99 in girls. The rates of children who showed severe fear and pain of the subjects were 8.2% and 5.6%. The rates of children who showed slightly or definitely negative attitude for dental treatment and dentists of the subjects were 19.9% and 4.6%.

Analysis of the major factors of influence on the conditions of the Intensity Modulated Radiation Therapy planning optimization in Head and Neck (두경부 세기견조방사선치료계획 최적화 조건에서 주요 인자들의 영향 분석)

  • Purpose : To derive the most appropriate factors by considering the effects of the major factors when applied to the optimization algorithm, thereby aiding the effective designing of a ideal treatment plan. Materials and Methods : The eclipse treatment planning system(Eclipse 10.0, Varian, USA) was used in this study. The PBC (Pencil Beam Convolution) algorithm was used for dose calculation, and the DVO (Dose Volume Optimizer 10.0.28) Optimization algorithm was used for intensity modulated radiation therapy. The experimental group consists of patients receiving intensity modulated radiation therapy for the head and neck cancer and dose prescription to two planned target volume was 2.2 Gy and 2.0 Gy simultaneously. Treatment plan was done with inverse dose calculation methods utilizing 6 MV beam and 7 fields. The optimal algorithm parameter of the established plan was selected based on volume dose-priority(Constrain), dose fluence smooth value and the impact of the treatment plan was analyzed according to the variation of each factors. Volume dose-priority determines the reference conditions and the optimization process was carried out under the condition using same ratio, but different absolute values. We evaluated the surrounding normal organs of treatment volume according to the changing conditions of the absolute values of the volume dose-priority. Dose fluence smooth value was applied by simply changing the reference conditions (absolute value) and by changing the related volume dose-priority. The treatment plan was evaluated using Conformal Index, Paddick's Conformal Index, Homogeneity Index and the average dose of each organs. Results : When the volume dose-priority values were directly proportioned by changing the absolute values, the CI values were found to be different. However PCI was $1.299{\pm}0.006$ and HI was $1.095{\pm}0.004$ while D5%/D95% was $1.090{\pm}1.011$. The impact on the prescribed dose were similar. The average dose of parotid gland decreased to 67.4, 50.3, 51.2, 47.1 Gy when the absolute values of the volume dose-priority increased by 40,60,70,90. When the dose smooth strength from each treatment plan was increased, PCI value increased to $1.338{\pm}0.006$. Conclusion : The optimization algorithm was more influenced by the ratio of each condition than the absolute value of volume dose-priority. If the same ratio was maintained, similar treatment plan was established even if the absolute values were different. Volume dose-priority of the treatment volume should be more than 50% of the normal organ volume dose-priority in order to achieve a successful treatment plan. Dose fluence smooth value should increase or decrease proportional to the volume dose-priority. Volume dose-priority is not enough to satisfy the conditions when the absolute value are applied solely.

  • The hypotheses of difference, ratio and odds ratio between two proportions are used for the non-inferiority trial. The approximate unconditional test suggested by Kang and Chen (2000) based on difference and ratio have the potential problem against the failure rate. When the sample size is small, the type I errors of the asymptotic test using the normal approximation suggested by Chen et al. (2000) tends to exceed the nominal level. Therefore, we propose the approximate unconditional test based on odds ratio and compare the test with the asymptotic test. And we compare the three hypotheses used in the approximate unconditional tests of two proportions with respect to the type I errors and power.

  • 운동은 모든 질환을 가진 사라에게 유용하다. 특히 만성질환을 예방하고 치료하는데 효과적이란 것이 잘 알려졌으며, 또한 각종 암의 예방과 치료에도 운동이 필요하다는 과학적 증거들이 밝혀지고 있다. 운동은 인체의 면역기능에도 영향을 미쳐, 격렬한 훈련을 받고 있는 운동선수들이 감기가 잘 걸린다거나 마라톤을 하고 난 직후에 면역기능이 떨어져 여러 가지 질환에 이환되는 비율이 높다. 그러나 적당한 운동은 면역기능을 향상시키고 질병에 대한 저항력을 강화시키며, 암의 예방과 치료에 도움이 된다는 것이 알려지면서 많은 운동프로그램이 나오고 있다.

Impact of nonsurgical periodontal therapy on oral malodor and microbial load of saliva (구취와 타액내의 미생물 수에 대한 비외과적 치주치료의 영향)

  • 연구 목적 : 구취의 주된 화학적 요소는 휘발성 황 화합물 (VSCs)들이다. 이들은 치태, 설태, 타액과 혈액, 상피세포의 혐기성 그람 음성 세균에 의한 단백분해 변성으로 만들어 진다. 또한 구취는 치주질환과 타액내의 세균의 증가를 야기하는 불량한 구강 위생상태에서 기인한다고 보고되고 있다. 이 연구의 목적은 VSCs를 측정함으로 구취에 대한 비외과적 치주 치료의 영향을 평가하고, 타액내의 세균 수를 측정함으로써 VSCs와의 상관관계를 살펴보고자 하는 것이다. 연구 대상 및 방법 : 중등도의 만성치주염을 가지는 23명의 참가자들이 대조군과 실험군으로 분류되었다. 대조군은 구강위생교육 (치간청결기구와 혀세정 포함)만을 시행하였고 실험군은 구강위생교육과 함께 24시간 내에 전악 치석제거술과 치근활택술을 시행하였다. 치료 전과 치료 4주 후에 VSCs 농도를 $Halimeter^{(R)}$(Interscan Corp., CA, USA)를 사용하여 측정하였고 타액 시료는 혐기성과 호기성 조건에서 배양되었다. 결과 : 4주 후에 두 군 모두에서 휘발성 황화합물의 농도가 유의하게 감소하였고, 실험군에서의 감소량이 통계적으로 유의하게 더 크게 나타났다. 타액 내 세균의 배양에서 총세균수에 대한 혐기성 세균의 비율이 실험군에서만 통계적으로 유의하게 감소하였다. 그러나 세균의 비율과 휘발성 황화합물의 농도와의 상관관계는 낮게 나타났다. 즉 구취의 감소는 타액 요인 보다는 치주치료에 의한 다른 복합적인 인자의 작용에 의한 것으로 추론할 수 있었다. 결론 : 본 연구는 중등도의 치주염환자에서 구강위생교육만으로도 구취의 주요 화학적 요인인 휘발성 황화합물의 농도를 감소시킬 수 있으나 비외과적 치주치료를 동반하였을 때 더욱 큰 효과를 얻을 수 있는 것으로 해석된다. 그러나 실험 참가자의 수가 적었고 장기간의 분석이 이루어지지 않았으므로 앞으로 더 많은 연구가 구취에 대한 치주치료의 영향을 이해하기 위해서 필요할 것으로 사료된다.

The evaluation of image guide system in case of rectal cancer (직장암 치료시 영상유도 시스템의 유용성 평가)

  • purpose: Image Guide System offers therapy precise, especially Intensity Modulated Radiation Therapy. However, organs at pelvis have variation and uncertainties each therapy. it brings IG system for verifying patient's position. In this study, analysis the variation at pelvis during rectal cancer radiation therapy. Moreover design the patient re-setup technique and apply to patients. Material and Method: 40 rectal cancer patient who have radiation therapy. The 530 image which acquired from IG system are analyzed. The bone structure, bladder, gas in the rectum, small bowel, soft tissue, weigh loss are evaluated by the criterion. The criterion are classified by best, good, bad and figure out the ratio with count. The re-setup proceed in case of one or over the two get the bad criterion and figure out the ratio of re-setup results: The ideal of therapy ratio is 19.2 % each criterion. And the good for therapy ratio is 54.9 %, the cases of bad for therapy is 25.8 %. The bad cases are have therapy after re-setup with post process. conclusion: Each pre-treatment image that acquired IG system has different results despite of same patients. The 25.8 % need to re-setup in order to unsuitable therapy. It is implies that the IG system is necessary establishing precise treatment plan like IMRT especially rectal cancer.

Effects and adverse-effects of growth hormone therapy in children with Prader-Willi syndrome: A two year study (Prader-Willi 증후군 소아에서 성장호르몬 치료의 효과와 부작용)

  • Purpose : The objective of this study was to evaluate the effects and adverse side-effects of growth hormone (GH) therapy in children with Prader-Willi syndrome (PWS). Methods : Forty-one patients who had been treated with GH for more than two years (24 boys and 17 girls, mean age $7.3{\pm}3.3$ years during treatment initiation) were enrolled for this study. Results : After 2 years of GH therapy, the height and weight standard deviation scores (SDS) increased significantly ($-1.19{\pm}1.37$ vs. $-0.02{\pm}1.45$, and $1.02{\pm}2.42$ vs. $1.63{\pm}2.22$, P<0.002); however the percentage body fat decreased ($44.6{\pm}9.9%$ vs. $38.1{\pm}10.5%$, P<0.001). Further, no change was observed in the thyroid and serum glucose levels, but the total cholesterol level decreased. GH therapy did not impact glucose control in the patients with diabetes. The most common adverse effects of GH therapy were the progression of scoliosis and adenoid hypertrophy. Conclusion : GH therapy improved the height SDS and body composition in patients with PWS. However, GH should be used with caution in patients with scoliosis and adenoid hypertrophy.
