Evaluation of the Cell-Mediated Immunity in Treatment Failure Pulmonary Tuberculosis (치료실패 폐결핵 환자의 세포성면역반응에 관한 연구)

  • Background: Ineffective cell-mediated immune response in human tuberculosis is associated with a depressed Thl cytokine response and reduced production of IFN-$\gamma$. Most persons infected with Mycobacterium tuberculosis are healthy tuberculin reactors with protective immunity, but a minority with ineffective immunity develop extensive pulmonary tuberculosis. The cell-mediated immune response is an important aspect of host resistance to mycobacterial infection and is believed to be tightly regulated by a balance between Th1 cytokines including IFN-$\gamma$, IL-12, IL-18, regulated on activation, normal T cell expressed and secreted (RANTES) and Th2 counterparts such as IL-4, monocyte chemoattractant protein-l (MCP-l). Methods: Proliferation and mRNA expression of IFN-$\gamma$, RANTES and MCP-l by RT-PCR in peripheral blood mononuclear cells (PBMCs) in response to in vitro stimulation with mycobacterial antigens were compared in pulmonary tuberculosis patients with cured and treatment failure and in tuberculin-positive and tuberculin-negative healthy subjects. Results: Defective proliferative responsiveness to aqueous TSP antigen was involved with treatment failure tuberculosis patients. Aqueous TSP antigen-induced IFN-$\gamma$ and RANTES mRNA expression was decreased in treatment failure tuberculosis patients compared with healthy tuberculin reactors and cured tuberculosis patients (23.1 % versus 90.0% for IFN-$\gamma$ and 46.2% versus 70.0% versus 46.2% for RANTES). The frequency of MCP-l mRNA expression to aqueous TSP antigen in treatment failure tuberculosis patients was greater than in healthy tuberculin reactors and cured tuberculosis patients (76.9% versus 40.0%). Conclusion: The increasing expression of MCP-1 mRNA in response to aqueous TSP antigen might be predicted to favor Th1 responses and restricted Th1 responses in treatment failure of pulmonary tuberculosis.

The Effects of Electrical Stimulation Method on Muscle Strength, Pain, Range of Motion, Fear Avoidance-Belief in Patients with Anterior Cruciate Ligament Reconstruction (전기자극방법이 앞십자인대 재건술환자의 근력과 통증, 관절가동범위 및 공포-회피반응에 미치는 영향)

  • The purpose of this study was to investigate the effect of neuromuscular electrical stimulation (NMES) and transcutaneous electrical nerve stimulation (TENS) on muscular strength, pain, range of motion and fear-avoidance in anterior cruciate ligament reconstruction (ACLR) patients. The subjects of this study who received ACLR after arthroscopic injury were divided into two groups: NMES (n=14) and TENS (n=14). Each group received intervention after routine physical therapy. Each electrotherapy intervention was applied regularly for a total of 12 times, 3 times a day (30 minutes) for 4 weeks. The evaluation by intervention method included isokinetic muscle strength in flexion and extension of the knee joint, pain, range of motion of the knee joint, and fear-avoidance beliefs. The results showed significant improvement in pain, range of motion, and fear-avoidance beliefs in both groups (p<0.05). However, there was a significant improvement in isokinetic knee flexion and extension strength only in the NMES group (p<0.05). There was no significant difference between the two electrotherapy methods in muscle strength, pain, range of motion, and fear-avoidance beliefs. These study results suggest that NMES and TENS can be effective intervention methods for pain, range of motion, and fear-avoidance response in patients with ACLR, but to improve muscle strength NMES may be more effective.

Automatic Nerve Activity to Physiologic Response in Adult With Psychiatric Disorder: A Systemic Review (성인 정신장애의 청각자극에 따른 생리학적 반응 측정에 대한 체계적 고찰)

  • Objective : The aim of this study was to provide the method to measure physiologic response using equipments and auditory stimulation, and the physiologic response features of adult psychiatric disorders through a systemic review. Methods : The systemic review was executed using PubMed. The key words for search were "auditory stimulation, auditory startle, electromyograph, skin conductance, heart rate, psychiatric disorder, anxiety disorder, schizophrenia, depression". 8 studies were used for data analysis, and all of levels of evidence were level II. The substances of the review were subject(population), auditory stimulation, measure equipments and physiologic response features of psychiatric disorder. Results : 1. The subjects for the studies were anxiety disorder(4) and schizophrenia(4). 2. Auditory stimulation was used in 8 studies and visual stimulation with auditory stimulation was used in 2 studies to induce physiologic response. 3. Every study used electromyograph, and skin conductance was used in 2 studies and heart rate was used in 2 studies with electromyograph to measure physiologic response. 4. The subjects for the studies, schizophrenia and anxiety disorder(PTSD, OCD) have different physiologic response features with the normal control group. Conclusion : All studies used simple tones as an auditory stimulation and electromyograph to measure physiologic response. Psychiatric disorders indicated larger response, shorter and delayed habituation. The objective and systematic study using physiologic measure to investigate the sensory feature of psychologic disorders.

Response Properties of Meridians for focused variable electromagnetic stimulus (접속형 가변 전자계 자극에 대한 경락반응특성)

  • It is a method to treat pain using medication, neurotomy, and surgery. And acupuncture, ultrasound, electric treatment, and magnetic treatment are applied as a alternative physiotherapy. Electronic therapy is useful but it can be affected by impedance of skin or subcutaneous tissue. So, percutaneous stimulation is leading therapy, that is very randomly. We developed the system which can stimulate parts of acupuncture point noninvasively using the focused magnetic field. And we designed the magnetic stimulation electrode which is considered efficiency of the magnetic stimulation. It can make similar stimulation with manual acupuncture. To confirm the availability and reliability we compared Meridian Electronic Potential(MEP) change between manual acupuncture and magnetic stimulation. From this result, we found out the MEP changes of manual acupuncture and magnetic stimulation were similar. And there were various response properties as changes of stimulation method, intensity, and frequency. Also, the MEP change can be induced by electromagnetic stimulation. We confirmed that it is possible to use electromagnetic stimulation as a acupoint stimulation or pain treatment instrument.

Comparison of meridians electric response property for laser and acupuncture stimulation (레이저자극과 수기자극에 대한 경락전기반응 특성비교)

  • Laser stimulation has been widely studied and used in clinic. However, electrical response by laser stimulation on meridians has not been investigated. In this study, we compared electric potential of laser and acupuncture stimulation on meridians. We measured electric potential variation at acupoints(Samgan(LI3) and Hapgok(LI4)) on Large Intestine Meridian. In laser stimulation results, average peak electric potential is very low($7.53{\pm}3.44{\mu}V$) for before and after stimulation. However, acupuncture stimulation was performed in ground connection condition and resulted in huge variation of average peak electric potential($2.65{\pm}1.53mV$). That is, the intensity and pattern of electric potential were dependent on the ground connection condition and individual. Also, the electric potential pattern was very similar to the pattern of electric charge and discharge of capacitor. The acupuncture stimulation using a insulating needle resulted in lower average peak electric potential variation($0.25{\pm}0.16mV$) than that of acupuncture stimulation. It might present little electrical response of acupuncture stimulation using insulating needles. In point of electrical response, the laser stimulation was determined to be no acupuncture effect at meridian. Acupuncture stimulation seems to be most effective method to induce electrical response at meridians. The procedure and effect of acupuncture might be considered as energy consensus phenomenon by transportation of bio-ion charge between a practitioner and patient.

  • In this paper, the electrical values of resistance, current, and voltage condition of reactivity is analyzed by applying the direct current (DC) voltage in medium for stem cell regeneration treatment. The voltage response by medium is related to electrical stimulation in the process of induction of differentiation for stem cell and the differentiation condition can be checked depending on the response of voltage condition. If the voltage level is lower in reacting response of a medium, the stem cell stimulation condition is stable, and if the voltage changing level is higher, the stem cell stimulation condition is unstable and a considerable loss will be resulted in the differentiation process. In this research, the optimization of electrical stimulation condition is expected for possible stem cell regeneration treatment.

Evaluation of Electric Pulp Test (EPT) as a Tool for Measurement of Dentinal Hypersensitivity (상아질 지각과민 측정 도구로서의 전기치수검사에 대한 평가)

  • In this study, 40 hypersensitive teeth of 19 patients were investigated. The procedures performed were as follows: Before desensitization, EPT at occlusal third of buccal surface was done for the evaluation of pulp vitality and the EPT value was recorded for the reference value. And mechanical and thermal test was executed for the test of hypersensitivity. If the tooth responded to the above tests, we did EPT at the exposed surface, using toothpaste as a electrolite medium and recorded the EPT value at patient's response. After the tests had been done, desensitization procedures with Gluma(R) Desensitizer were performed according to the manufacturer's instructions. After desensitization, the same tests except EPT at occlusal third were repeated. All the 40 teeth responded positive before desensitization and negative after desensitization procedures. The EPT value at occlusal third ranged from 31 to 65 (48.9${\pm}$7.2). Before desensitization 34 teeth responded at EPT value of 2 and the remaining 6 teeth was in the range of 17 to 25. After desensitization all 40 teeth responded from 12 to 27 (19.6${\pm}$3.5). The 6 teeth responded at greater number than 2 before desensitization was in the range of 18 to 23. Within the limitations of this study we can conclude that: When a tooth with dentinal hypersensitivity responds to mechanical and thermal stimulation, the tooth shows very low resistance to electricity at the exposed surface while when a tooth is desensitized and doesn't show respond to mechanical and thermal stimuli, the tooth shows increased level of resistance to electric stimulation at the exposed surface. EPT can be used for the diagnosis of dentinal hypersensitivity. Furthermore EPT will be useful to evaluate the outcome of desensitization procedures. However, EPT is not a valid tool for measuring dentinal hypersensitivity.

The Effect of Low-Frequency Repetitive Transcranial Magnetic Stimulation on Treatment-Resistant Tourette's Syndrome among Children and Adolescents : An Open-Label Study (치료저항성 뚜렛장애를 가진 소아청소년에서 저빈도 반복적 경두개자기자극술의 효과 : 오픈 라벨 연구)

  • 목 적 : 본 연구의 목적은 치료저항성 뚜렛장애를 가진 소아 청소년들에 대해서 보조운동영역을 표적으로 한 저빈도 반복적 경두개자기자극술의 효과를 평가하는 것이다. 방 법 : 이 연구는 미국정신의학회 진단기준 및 통계편람, 제 4판을 기준으로 뚜렛장애로 진단받은 10명의 소아 청소년(평균연령 $12.56{\pm}1.04$세)을 대상으로 하였다. 모든 참가자들은 1년 이상 지속적으로 충분한 용량의 약물치료를 받았음에도 불구하고 한국판 예일 틱 증상 평가척도(YGTSS) 20점 이상, 임상인상척도(CGI-TS) 4점 이상을 받았다. 참가자들은 보조운동영역을 표적으로 1 Hz 반복적 경두개자기자극 치료를 매일 20분간 받았으며, 총 20회 치료를 받았다. YGTSS, CGI-TS 및 이상반응 체크리스트는 연구 시작 전과 연구 시작 후 4, 8, 12주에 시행되었다. 결 과 : 틱 증상은 연구 12주차까지 호전된 상태로 지속되었으며, YGTSS와 CGI-TS가 통계적으로 유의하게 감소하였다. 특히 음성 틱 점수의 감소가 운동 틱 점수의 변화보다 전체 YGTSS 점수의 감소에 큰 역할을 한 것으로 나타났다. 그리고 전체 10명의 참가자 중 9명이 심각한 부작용 없이 연구를 끝까지 완료하였다. 결 론 : 본 연구는 보조운동영역을 표적으로 한 저빈도 반복적 경두개자기자극술이 치료 저항성 뚜렛장애를 가진 소아 청소년들에게 효과적이고 안전한 치료도구가 될 수 있음을 시사한다. 이러한 반복적 경두개자기자극술의 치료효과를 확정하기 위해서는 보다 많은 환자를 대상으로 한 잘 통제된 연구가 필요할 것이다.

A Study on Verification of Acupuncture Therapy Effect by Bio-potential Analysis (생체전위 분석에 의한 침구치료 효과 규명에 관한 연구)

  • In oriental medicine, meridian is pathway of bio-energy, and basic an object of diagnosis/therapy. But, in western medicine, meridian has been accepted as action pathway of nerve-endocrine-immune system. Acupuncture effect has been regarded as not effect by transmission of bio-energy but action or response by stimulation. In this paper, when an acupuncturist's bio-energy is passed on(non-insulation) and when is not(insulation), we analysed electric potential on meridian/non-meridian to verify effect of acupuncture therapy. In the results, when bio-energy isn't passed on, bio-potential is reacted to only the first of 4 times acupuncture operating. But when bio-energy is passed on, it is reacted to 4 times, and potential on meridian is higher than that of on non-meridian. Electric potential for same acupuncture operating is different on non/meridians, which implies that physiological construction and bio-energy transmission material is different respectively. When is extracting acupuncture method, potential is higher than that of injection one. It implies that extracting method is more effect than injecting one, in aspect of acupuncture therapy effect. Therefore, acupuncture therapy effect is verified as effect of response by acupuncturist's bio-energy transmission rather than that by only acupuncture stimulus.

Analysis of Joint Attention Behaviors in Children With Autism Spectrum Disorder Depending on the Type of Attentional Cue and Reinforcing Stimulus (음악적 단서 및 후속 자극에 따른 자폐스펙트럼장애 아동의 공동주의 반응 행동 비교)

  • This study investigated whether joint attention response behaviors in children with autism spectrum disorder (ASD) change in response to musical cues and reinforcing stimulus, and compared them with neurotypically developing (NT) children. The participants included 13 children with ASD and 14 NT children aged between 3 to 5 years. The study established six task conditions, involving cues (verbal vs. musical) for responding to joint attention (RJA) behaviors and reinforcing stimulus (verbal vs. sound vs. musical) for social referencing behaviors. These tasks were presented 12 times with two repetitions each. The results of the study showed that providing musical cues during the RJA phase increased levels of RJA in children with ASD, consistent with prior studies. Subsequently, musical reinforcing stimuli increased the frequency of social referencing behaviors in these children. This indicates that musical stimuli can extend beyond mere sensory cues, helping individuals to understand and respond to social and emotional cues from others. Moreover, these musical stimuli could serve as effective social reinforcement factors for this population.
