Measurement of Skin Dose from Using the Treatment Immobilization Devices (치료 보조기구 사용 시 후 방향 피부선량 측정)

  • The research was about the relation between the dorsal side dose measured by using the phantom body (Alderson Rando Phantom) and factors like contacted material of the patients, the size of the field, angle of incidence. Compared with mylar (tennis racket), the dose on $10{\times}10\;cm^2$ field size of cotton was increased by 2% and by 8% in the case of breast board. In the case of $15{\times}15\;cm^2$ field size, the dose was increased by 6% compared with $10{\times}10\;cm^2$ size. The field size of $20{\times}20\;cm^2$ resulted in 10% increase of dose, while $5{\times}5\;cm^2$ produced 13% decrease. Compared with incident angle $0^{\circ}$, the cases for the incident angle $5^{\circ}$ had 0.4% less dose for breast board, 0.5% for tennis racket, 1.1% for cotton. The cases for the incident angle $10^{\circ}$ had 1.5% less dose for breast board, 1.9% for tennis racket, 2.6% for cotton. For the incident angle $15^{\circ}$, breast board, tennis racket, cotton caused decrease of dose by 3.9%, 2.6%, 3.86% respectively. Resultantly carbon material can cause more skin dose in treatment field. By the results of this study, we recommend that one should avoid the contact between the carbon material and skin.

A Survey on the Present Situation and Satisfaction in Assistive Technology of Person with Physical Disabilities (지체장애인의 보조공학 이용 실태 및 만족도 조사)

  • Objective : This study was aimed to survey the present situation and satisfaction in assistive technology of the physically disabled person. Method : 110 patients, being diagnosed with spinal cord injury and stroke, were chosen and structured interview was carried out using by a questionnaire. Result : The followings are the results of this study. First, spinal cord injury(76.2%) and stroke(72.5%) patients used the most wheelchair and mobility devices in assistive technology device. Second, they were satisfied in effectiveness of assistive technology device. Third, spinal cord injury(39.4%) patients used the most repair service in assistive technology service. Fourth, stroke(28.2%) patients used the most application and training service. Fifth, they were satisfied in professionality while were dissatisfied in service delivery program. Conclusion : The results of this study would contribute to better assistive technology for the physically disabled person.

  • Since the endosaccular coil embolization technique was introduced as an alternative for treating selected patients with aneurysms, it has become a mainstay of treatment for cerebral aneurysms. In lesions with a neck larger than the aneurysmal body, an irregular shape, or arterial branches incorporated within the sac, endovascular treatment using detachable coils are traditionally contraindicated because of technical difficulties. Coil embolization has evolved as a result of both the development of related devices and the introduction of technical improvements using various devices. Use of various technical and device options can make endovascular treatment of cerebral aneurysms safer and can widen the treatment indications. Various technical options, including the technical modification of device-assisted techniques, will be presented, and the related practical points will be discussed in this issue.

Comparison with ABCHES and Abdomen Compression Device in Respirational Radiation Therapy on Patients in Hepatocellular Carcinoma (간세포암 환자에서 ABCHES와 복부압박장비의 적용한 호흡동조방사선 치료의 유용성 비교)

  • 4D-Radiation Therapy is the optimal treatment to track moving organs(tumor) and give the appropriate prescription dose to tumor and low radiation dose to normal tissue surrounding tumor volume. The ABCHES is a 4DRT devices maintaining shallow breathing to patients. It allows the tumor's movement was minimize. Meanwhile, Abdominal compression device is limited the breath compressing abdomen on patients. In this paper we will quantitative analysis the movement of tumor on only ABCHES versus ABCHES with Abdomal compression device and Analysis tumor dose and normal tissue's dose by Dose Volume Histogram on two parts. The result of Comparision ABCHES and ABCHES with Abdominal compression device, SI(Superior-Inferior) direction, AP(Anterior-Posterior) direction and LR(Left-Right) direction was limited 1.0 mm, 0.2 mm, 0.2 mm(average). and also reduction rate of voluume in HPTV was $16{\pm}2%$, and LPTV was $15.8{\pm}0.8%$ under only using ABCHES and ABCHES with compression. The analysis dose volume histogram was more radiation dose in ABCHES and abdominal compression device than only using ABCHES, and less normal tissue-ipsilateral lung, whole lung, kidney-dose in ABCHES and abdominal compression device than only using ABCHES. The overall analysis was ABCHES with abdominal compression better than only using ABCHES method. In hereafter it will be studies that limitation of ABCHES and abdomonal compression device. In other words, patient's discomfort on compression intensity, method of application on patient with inaccurate respiration cycle.

Case Study on Design, Manufacture and Application of Customized Assistive Device for the ADL of Person with Brain Lesions (뇌병변장애인의 일상생활을 위한 맞춤형 보조기구의 설계·제작·적용 사례 연구)

  • This paper is case study on design, manufacture and application of customized assistive device for the ADL of person with brain lesions. The study had carried out from October 2010 to November 2010 and 2 clients from Assistive device case management demonstration project which is performed by Daegu assistive and rehabilitation center had participated. Case A, a 47-year-old man with brain lesions, stays only on the floor and could not get close to the usual toilet by himself because of the poor low extremities muscle strength and function. The moveable-toilet and customized ramp had applied to Case A. He clambers up the ramp and reach to the toilet easily. Case B, a 8-year-old boy with brain lesions, had difficulties with self-feeding because of his clumsy hand. Several existing feeding aids were applied but not appropriated. A customized feeding aid was designed and manufactured out of regard of Case A's ROM and right hand size. And his occupational therapist works together continuously to make him feed himself in the near future. Already-made assistive devices were not appropriate to Case A and B and to solve this problem, customized assistive devices were applied. The importance and needs of design and manufacture of assistive device were recognized through these case study.

Development of Abdominal Compression Belt and Evaluation of the Efficiency for the Reduction of Respiratory Motion in SBRT (체부 정위방사선치료 시 호흡운동 감소를 위한 복부 압박기구 개발 및 유용성 평가)

  • Purpose: It's essential to minimize the tumor motion and identify the exact location of the lesions to achieve the improvement in radiation therapy efficiency during SBRT. In this study, we made the established compression belt to reduce respiratory motion and evaluated the usefulness of clinical application in SBRT. Materials and Methods: We analyzed the merits and demerits of the established compression belt to reduce the respiratory motion and improved the reproducibility and precision in use. To evaluate the usefulness of improved compression belt for respiratory motion reduction in SBRT, firstly, we reviewed the spiral CT images acquired in inspiration and expiration states of 8 lung cancer cases, respectively, and analyzed the three dimensional tumor motion related to respiration. To evaluate isodose distribution, secondly, we also made the special phantom using EBT2 film (Gafchronic, ISP, USA) and we prepared the robot (Cartesian Robot-2 Axis, FARARCM4H, Samsung Mechatronics, Korea) to reproduce three dimensional tumor motion. And analysis was made for isodose curves and two dimensional isodose profiles with reproducibility of respiratory motion on the basis of CT images. Results: A respiratory motion reduction compression belt (Velcro type) that has convenient use and good reproducibility was developed. The moving differences of three dimensional tumor motion of lung cancer cases analyzed by CT images were mean 3.2 mm, 4.3 mm and 13 mm each in LR, AP and CC directions. The result of characteristic change in dose distribution using the phantom and rectangular coordinates robot showed that the distortion of isodose has great differences, mean length was 4.2 mm; the differences were 8.0% and 16.8% each for cranio-caudal and 8.1% and 10.9% each for left-right directions in underdose below the prescribed dose. Conclusion: In this study, we could develop the convenient and efficient compression belt that can make the organs' motion minimize. With this compression belt, we confirmed that underdose due to respiration can be coped with when CTV-PTV margins of mean 6 mm would be used. And we conclude that the respiratory motion reduction compression belt we developed can be used for clinical effective aids along with the gating system.

The Effect of Sponge on Proprioception at Tilt Table Standing: A Pilot Study (경사침대에서 스폰지가 고유수용성 감각에 미치는 효과)

  • 본 연구는 건강인에 있어서 발의 세 가지 조건하에서 경사침대를 세울 때 완전히 섰다고 인지하는 각도를 측정하여 스폰지가 고유수용성 감각 변화에 미치는 효과를 알아보는 것이다. 34명의 지원자(남자 15명, 여자 19명)를 대상으로 하였다. 대상자의 연령 범위는 20세에서 31세(평균 $21.4{\pm}2.3$세)이었다. 실험 대상자는 3일에 걸쳐 무작위 방법에 의한 세 가지 조건 중 하루에 한 가지씩 실험하였다. 실험 과정은 6단계로 나뉘었다. 실험 기구는 전동 경사침대를 이용하였으며 맨발로, 낮은 스폰지 혹은 높은 스폰지위에 맨발로 경사침대를 세울 때 '정지'하는 각도를 측정하였다. 연구자 외에 2명의 보조 연구자가(기록자 2, 경사침대 조정자 1) 참여하였다. 맨발, 낮은 스폰지, 높은 스폰지 순으로 경사침대의 평균 각도가 높았으며 통계학적으로 유의한 차이를 보였다. 맨발과 낮은 스폰지, 맨발과 높은 스폰지, 그리고 낮은 스폰지와 높은 스폰지 사이에서 통계학적으로 유의한 차이를 보였다. 성별에 따른 차이는 없었다. 체중이나 키에 따른 차이도 없었다. 본 연구의 결과로 미루어볼 때, 스폰지가 완전히 섰다고 인지하는 고유수용성 감각을 둔화시켰음을 보였다. 앞으로의 환자치료에 있어서 고유수용성 감각 촉진을 위한 치료 프로그램이 필요하며, 환자를 대상으로 한 임상연구가 필요하다고 사료된다.

  • In this article, we introduce a new method of tooth positioner fabrication using modified T.A.R.G. to measure the inclination and angulation of individual teeth. In finishing stage of orthodontic treatment with a fixed appliance, we anticipate that tooth positioner fabricated using the described construction method provides the movement of individual tooth into a desired position; corrected inclination and angulation of teeth, extrusion, intrusion, rotation and so on.

Questionnaire-based analysis of growth-promoting attempts among children visiting a university growth clinic (대학병원 성장클리닉을 내원한 아동에서 설문 조사를 통한 키성장 관리 실태분석)

  • Purpose : Growth-promoting attempts are widespread in Korea, but little is known about their prevalence or associated factors. This study was designed to assess the prevalence of growth-promoting attempts among children visiting a university growth clinic. Methods : A questionnaire-based survey was carried out with 823 children (416 boys, 407 girls) who visited the growth clinic at Paik Hospital. Results : The mean age of the subjects was $10.4{\pm}2.6$ yr, and the height z-score was $-1.58{\pm}0.91$. Approximately 33.4% of the children had tried growth promotion. Among the height-gain methods, herbal medicine was the most frequently used (37.8%), followed by health-promoting supplements (37.1%), exercise or machine (3.0%), and growth hormone treatment (2.9 %). The mean age at which the parents began to worry about their children's height was 7.7 yr. The mean age at which they started height-gain methods was 8.9 yr for herbal medicine, 9.1 yr for health-promoting supplements, 9.4 yr for exercise or machine, and 9.9 yr for growth hormone treatment. Motivating factors included advice from relatives or friends (36.0%), advertisements in the Internet or newspaper (28.4%), advice from pharmacist (16.8%), and advice from their medical doctor (5.5%). The degree of satisfaction from the height-gain methods was 29.1% with growth hormone treatment, 6.6% with exercise or machine, 6.4% with herbal medicine, and 2.8% with growth-promoting supplements. Conclusion : Approximately one third of the children reported use of growth-promoting methods, but the satisfaction rate was not high. The benefits of growth-promoting methods should be carefully weighed against their costs and side effects.

The effect of muscle activity wristbands with mouse using (컴퓨터 마우스 보조기구 사용이 근활성도에 미치는 영향)

  • Purpose : This study is to know the effect on muscle activity when people use mouse aid or not in computer working environment. Method : 11 people of both sex in their twentieth were tested. As a independent variable, the condition without mouse aid and the condition with movable mouse aid were selected, and as a dependent variable, MVC(%) on four muscles and inconvenience degree(RPE scale) on three body part(wrist, forearm, shoulder) were measured. Result : Analyzing the research, MVC(%) of extensor muscle of finger had notably higher muscle activity than other muscles, and there wasn't notable difference in muscle activity before and after using mouse aid. In case of using mouse aid, we tested by giving RPE questionnaire which segmentalized mental fatigue degree into hand, forearm, and shoulder, and resultingly, there wasn't notable differences perspectively. However, people felt less tiredness subjectively after they use the mouse aid. Conclusion : The usage of mouse aid seems to give comfort to computer users.
