Image Evaluation according to Tube Current of X-ray Device for Intraoral Imaging (구내 촬영용 엑스선 장치의 관전류에 따른 영상 평가)

  • The radiation dose received by the patient varies according to the tube current and time used during dental intraoral imaging. A large amount of tube current is required for image quality, but the radiation dose to the patient increases accordingly. Therefore, in this study, the optimal amount of tube current that can reduce the radiation dose received by the patient while securing the image quality was calculated through the evaluation of the image quality according to the tube current used during intraoral imaging through simulation. The average tube current, time, and tube voltage presented in the Guidelines for Diagnostic Reference Level for intraoral radiography were used as basic imaging conditions, and images were obtained when only the tube current was changed, and then the optimal tube current was compared and analyzed with the basic image quantity was calculated. Images were obtained by changing the tube current to 0.1, 0.5, 1, 2, 3, 4 and 5 mA under the basic conditions of 63 kV, 6 mA, and 0.29 s. The obtained image was evaluated for structural similarity index with the image taken under the condition of 6 mA using the ICY program. As a result, even under the condition of 0.5 mA tube current, the index of structural similarity with the image of 6 mA was evaluated to be high. Based on these results, it is considered that the radiation dose given to the patient can be greatly reduced if imaging is performed at 0.5 mA instead of 6 mA during dental intraoral imaging.

Studies of Automatic Dental Cavity Detection System as an Auxiliary Tool for Diagnosis of Dental Caries in Digital X-ray Image (디지털 X-선 영상을 통한 치아우식증 진단 보조 시스템으로써 치아 와동 자동 검출 프로그램 연구)

  • The automated dental cavity detection program for a new concept intra-oral dental x-ray imaging device, an auxiliary diagnosis system, which is able to assist a dentist to identify dental caries in an early stage and to make an accurate diagnosis, was to be developed. The primary theory of the automatic dental cavity detection program is divided into two algorithms; one is an image segmentation skill to discriminate between a dental cavity and a normal tooth and the other is a computational method to analyze feature of an tooth image and take an advantage of it for detection of dental cavities. In the present study, it is, first, evaluated how accurately the DRLSE (Direct Regularized Level Set Evolution) method extracts demarcation surrounding the dental cavity. In order to evaluate the ability of the developed algorithm to automatically detect dental cavities, 7 tooth phantoms from incisor to molar were fabricated which contained a various form of cavities. Then, dental cavities in the tooth phantom images were analyzed with the developed algorithm. Except for two cavities whose contours were identified partially, the contours of 12 cavities were correctly discriminated by the automated dental caries detection program, which, consequently, proved the practical feasibility of the automatic dental lesion detection algorithm. However, an efficient and enhanced algorithm is required for its application to the actual dental diagnosis since shapes or conditions of the dental caries are different between individuals and complicated. In the future, the automatic dental cavity detection system will be improved adding pattern recognition or machine learning based algorithm which can deal with information of tooth status.

THE STUDY OF TECHNICAL FAILURE OF INTRAORAL ROENTGENOGRAPHY (국내 X-선 촬영술에 있어서 실패의 원인분석에 관한 연구)

  • To insure accuracy in dental roentgenographs, it is important that the patient's head position and angulation of the x-ray tube. In 1907 Cieszynski applied the rule of isometry to dental radiography and established that in the production of an accurate image of a tooth, the central ray must be porjected perpendicularly to a plane bisecting angle formed by the longitudinal axis of the tooth and the film plane. Proper exposure of the film is a part of the production of a good dental radiograph, and correct processing also makes an essential contribution to the quality of the radiograph. The author analysed the failure of exposure and processing results, and recommended followings. 1. The patient head must be positioned that occlusal plane are parallel with the horizontal floor. 2. Central ray must be projected to the objective tooth and supporting structure and projected as perpendicularly to tooth axis and film surface as possible. 3. In processing, the temperature of the solution and the processing time must be correct.

A RADIOLOGIC STUDY OF DEVELOPMENTAL SALIVARY GLAND DEFECTS (발육성타액선결손의 방사선학적 연구)

  • Forty-four cases of developmental salivary gland defects in 43 patients were analysed radiologically. The obtained results were as follows ; 1. It occurred more frequently in males(86 %) than in females(14 %). The age distribution of patients ranged from 20 to 74 years with the average being 52.7 years. 2. All but one were located between the angle of the mandible and the molar tooth. Only one patient showed bilateral occurrence. 3. The shape of the defect was ovoid(15 cases), round(10 cases), or half-ovoid(5 cases) 4. Their size ranged from 5 x 6 mm to 16 x 30 mm with the average around 9 x 14 mm. 5. Twenty-eight cases appeared to be surrounded partially(1/3 - 2/3) by a hyperostotic border. The thickness of the hyperostotic border varied ; thick(14 cases), moderate(l1 cases), thin(9 cases), or mixed(6 cases) 6. Thirteen cases were in contact with the superior border of the mandibular inferior cortex, 11 cases partially eroded the cortical plate, and 7 cases showed discontinuity of the mandibular inferior cortex. 7. Twenty-three cases were located below the mandibular canal, 13 cases overlapped the mandibular canal, and 2 cases showed slight deviation of the mandibular canal.

The Characteristic of Temperature and Dose Distribution of intra oral X-ray Tube (강내형 X선 튜브 온도 및 선량 분포 특성)

  • A new concept of periapical radiography, intra oral x-ray tube and detector system is introduced. It is new system that a miniature x-ray tube is inserted into mouse and it acquired digital image using external detector. In this study, we have investigated temperature and dose distribution of insertional x-ray tube for periapical radiography. To analyze temperature characteristic of x-ray tube, we attached the thermocouple to surface of x-ray tube and we measured the temperature according to distance. Also, we measured the dose distribution of a miniature x-ray tube according to distance. As a result, temperature was constant to $27^{\circ}C$ over 2mm without cooling system, dose distribution of x-ray tube was 3.14 and 1.84mGy in 3 and 5cm, respectively. Therefore, the proposed x-ray system works in lower dose than conventional dental x-ray system. Thus, it is considered that new concept of system will have a significant effect on medical imaging technology.

Settling time of dental x-ray tube head after positioning (치과용 X-선 관구의 조정시간)

  • Purpose: The aim of this study was to introduce a method of obtaining the oscillation graphs of the dental x-ray tube heads relative to time using an accelerometer. Materials and Methods: An Accelerometer, Piezotron type 8704B25 (Kistler Instrument Co., Amherst, NY, USA) was utilized to measure the horizontal oscillation of the x-ray tube head immediately after positioning the tube head for an intraoral radiograph. The signal from the sensor was transferred to a dynamic signal analyzer, which displayed the magnitude of the acceleration on the Y-axis and time lapse on the X -axis. The horizontal oscillation of the tube head was measured relative to time, and the settling time was also determined on the basis of the acceleration graphs for 6 wall type, 5 floor-fixed type, and 4 mobile type dental x-ray machines. Results : The oscillation graphs showed that tube head movement decreased rapidly over time. The settling time varied with x-ray machine types. Wall-type x-ray machines had a settling time of up to 6 seconds, 5 seconds for fixed floor-types, and 1 I seconds for the mobile-types. Conclusion: Using an accelerometer, we obtained the oscillation graphs of the dental x-ray tube head relative to time. The oscillation graph with time can guide the operator to decide upon the optimum exposure moment after x-ray tube head positioning for better radiographic resolution.

  • 생체의료 및 보안검색 응용을 위한 테라헤르츠파 기술에 대하여 조사하였다. 테라헤르츠파는 기존의 X-선이나 초음파, 적외선, 밀리미터파 등에 비하여 독특한 특성을 가지고 있어 새로운 분광 및 영상기술을 확립할 가능성을 가지고 있다. 테라헤르츠 기술의 발전과 더불어 생체의료분야에서는 암진단, 치과질환 및 바이오칩 등에서 큰 시장을 형성할 것으로 전망되며, 보안검색분야는 공항, 항만, 지하철, 우편물 취급 등의 공공부문에서부터 군사목적에 이르기까지 응용이 가능하다. 최근 수 년 동안 소형, 경량 및 저가격의 테라헤르츠 시스템이 개발되면서 시장전망에 크게 높아졌다. 초기에는 공공시설 및 대형 병원에서 시작하여, 기술적 안정성 및 시장 활성화에 의한 경제성이 확보되면 민간 부문의 시장으로 크게 파급될 것으로 가대된다.

STUDIES ON THE POSITION OF CANINES, PREMOLARS AND MOLARS BY 45° OBLIQUE LATERAL CEPHALOGRAPHY (두부 X-선 규격촬영법(사위)에 의한 견치 소구치 대구치의 위치에 관한 연구)

  • This study was done using the 45° oblique lateral old, 18 males and 27 females, with normal occlusion, premolars and molars on upper and lower jaws. Axial inclination to nasal floor, occlusal plane and inter-axial inclination were examined. In addition the position of each tooth was examined in height and depth in upper and lower jaws. The results were obtained as follows; 1. The inclination of long axis of upper 1st premolar was most nearly perpendicular, upper canine was tilted mesially, and 2nd premolar and molars were tilted distally. 2. The inclination of long axis of lowers molar were tilted mesially. 3. There were no severe variation on the inter-axial inclination of canine to mandibular plane, and 2nd molar.

A Comparative Study of Radiographic Images on Normal Anatomical Structures (정상 해부학적 구조물에 대한 X-선 영상의 비교 연구)

  • Purpose: To compare radiographic images of Digora/sup (R)/ system and Ektaspeed Plus film obtained from normal adults. Materials and methods: Storage phosphor plate(SPP) was placed in a film holder behind Ektaspeed Plus film package without lead foil. The effect of film on SPP was studied in a separate in vitro experiment. Forty-seven sets of images were prepared for the evaluaton. The regions of interest(ROI) for evaluation were designated at seven sites including normal anatomical structures. The image quality for each ROI was evaluated on enhanced and unenhanced storage phosphor(SP) images and Ektaspeed Plus film. Results: Two film-SPP configurations showed significantly different gray levels at each step of the aluminum step wedge(p<0.05). The contrasts were comparable. Enhanced SP images were significantly superior to unenhaned images and film in all anatomical sturctures(p<0.01). The differences between unenhanced SP images and film were significant(p<0.05) except root canal and cortical bone on alveolar crest. For anatomical items. there were statistically significant difference among five observers(p<0.05). Conclusions: The image quality of enhanced SP images were superior to Ektaspeed Plus film. and Digora system is potentially applicable to clinical diagnosis.

The Actual State and the Utilization for Dental Radiography in Korea (국내 치과방사선의 현황 및 이용 실태)

  • The purpose of this study was first to analyze the utilization of dental examination through questionnaire to develop a diagnostic reference level of patient doses for dental radiography in korea. 77 dental institutions were classified into three groups: A group for the dental hospitals of the college of dentistry (11 institutions), B group for dental hospitals (30 institutions) and C group for dental clinics (36 institutions). The results were as follows : The mean numbers of unit chairs and medical staffs were 140.2, 15.3 and 5.8 sets, 112.6, 7.3 and 1.7 dentists, 3.1, 0.5 and no one radiologic technologists, and 19.7, 12.5 and 3.3 dental hygienists in A, B and C groups, respectively. The mean numbers of dental X-ray equipments were 14.64, 3.21 and 2.19 in A, B and C groups, respectively. Intraoral dental X-ray unit was used the most, the following equipments were panoramic, cephalometric, and cone-beam CT units. The most used X-ray imaging system was also digital system (above 50%) in all three groups. Insight dental film (Kodak, USA) having high sensitivity was routinely used for periapical radiography. The automatic processor was not used in many dental institutions, but the film-holding device was used in many dental institutions. The utilization rates of PACS in A, B and C groups were 90.9%, 83.3% and 16.7% respectively, and the PACS software program was used the most PiView STAR (Infinitt, Korea). The annual mean number of radiographic cases in one dental institution in 2008 for A group was 6.8 times and 21.2 times more than those for B and C groups, and periapical and panoramic radiographs were taken mostly. Tube voltage (kVp) and tube current (mA) for periapical radiography were similar in all three groups, but exposure time in C group was 12.0 times and 3.5 times longer than those in B and C groups. The amount of radiation exposure in C group, in which dental hygienists take dental radiographs, was more than those in other groups. The exposure parameters for panoramic radiography were similar in all three groups. In conclusion, the exposure parameters in dental radiography should be determined with reference level, not past experiences. Use of automatic processor and film-holding devices reduces the radiation exposure in film system. The quality assurance of dental equipments are necessary for the reduction of the patient dose and the improvement of X-ray image quality.
