국가적 수준의 치과의사 역량을 위한 치과마취과학의 교육역량과 치과의사국가고시 제고

  • 치과마취학의 교육목표는 치과치료 시 적절한 환자평가와 함께 동통 및 불안을 관리하기 위한 약품선택과 치과치료를 위한 흡입법과 정맥주사를 이용한 의식하진정법과 언제, 어떻게, 어디로 전신마취를 의뢰하는지를 알아야 할 사항과 특히 의학적 병력을 가진 환자와 특별한 관리가 필요한 소아 노인 장애인의 전반적 이해와 치료 전 전신적 환자평가를 할 수 있는 능력과 치과진료 중 일어날 수 있는 의료적 응급 상황과 치과응급상황을 식별하고 인공소생술을 포함한 치과 치료 과정에서 발생할 수 있는 의학적 응급상황들에 적절한 관리를 할 수 있어야 한다. 치의학교육의 최소한의 질 보장을 위한 국가적 수준의 치과의사 역량체계를 구축함으로써 치의학교육의 질 향상을 위한 치과대학(원)의 변화를 자극하고, 치과의료와 의료인력의 질적 증진 및 치과의료행위의 최소수준을 보장하는데 있다. 하지만 현행 국가고시에 있어 치과마취과학은 구강외과 과목에 포함되어 위 기술한 치과마취과학의 교육목표를 달성하는 데 어려움이 있다. 그러므로 국가수준의 치의학교육의 질에 대해 제고하고 학교의 교육과정 변화와 개선에 적용하고 국시원의 새로운 과목의 도입과 실기시험을 통하여 치과마취과학의 교육목표를 달성하는 데 목적이 있다.

Analysis of Factors that Affect the Follow-up Pattern after Dental Treatment under Conscious Sedation in Children (어린이의 진정법 하 치과치료 후 정기검진에 영향을 미치는 요인 분석)

  • Children need regularly dental check-ups after dental treatment because there are a lot of changes of oral conditions such as tooth exfoliation and eruption, occlusion, and recurrence of dental caries. The purpose of this study is to determine the factors that affect the follow-up pattern after dental treatment under conscious sedation. Patients who were treated under sedation at the department of pediatric dentistry of Ewha Womans University Mokdong Hospital from 2009 to 2013 were included in this study. The final samples comprised of 562 patients that were classified into 4 groups according to the number of visits during 3 years. The chi-square test was used to determine the factors affecting regular follow-up. There were statistically significant differences between each treatment groups and factors such as the change of doctor, overall treatment time, the number of treated teeth under sedation, restorative treatment, minor operation, and preventive treatment (p < 0.05). This study identified factors that affect regular follow-up after dental treatment under conscious sedation. It is recommended to educate the importance of regular check-ups to parents who are less likely to return.

  • 약관절장애는 그 원인요소가 복잡다단하듯이 그 치료, 또한 간단하지 않다. 치료는 그병인요소를 조절하거나 제거하여 질병의 재발이나 발병을 방지하려는 것과 질병자체를 치료하는 것으로 대별할 수 있다. 즉 질병의 발생을 예방하고 치료후의 재발을 막기 위하여는 원인이 될 만한 요소를 환자에게 알려주고 다음은 환자가 그것을 피하도록 시키며, 또 환자자신이 계속적으로 피하고 있는지를 확인하는 과정을 거치는 환자교육(patient education)을 통하여 원인요소(contributing factors)를 조절하는 것이 치료에 앞서 우선되어야 하며, 이점은 아무리 강조해도 지나치지 않다고 본다. 일단 약관절장애라는 진단이 내려지고 환자의 상태와 원인요소에 대한 이해가 이루어진 후에, 앞에서 언급한 환자교육과 아울러 치료를 시작하게 된다. 「미네소타」치과대학 악관절 및 악안면동통 크리닉에서의 치료과정은 크게 신체요법(somatic therapy), 행동요법(behavioral therapy) 및 물리요법(physical therapy)으로 구분할 수 있는데, 환자에 따라서는 단시일내에 간단한 방법으로 치료가 가능할 수도 있고 단시일내에 간단한 방법으로 치료가 가능할 수도 있고 때로는 장기간에 걸쳐서 이상의 여러방법을 동원한 복잡한 치료를 수행해야 할 경우도 있다.

A Retrospective Study on the Effect of Pulp Treatment on the Exfoliation of Primary Teeth (유치 치수치료가 치아 탈락에 미치는 영향에 관한 후향적 연구)

  • The aim of this study was to retrospectively analyze the effects of pulp treatment on exfoliation of primary teeth and its related factors. In this study, 167 teeth of 97 patients aged 6 months to 12 years who were treated with pulp treatment at Dankook Dental Hospital were selected, and information related to pulp treatment and tooth loss was collected. The included subjects were 72 primary anterior teeth (43.1%) and 95 primary posterior teeth (56.9%), of which 56 were males (57.7%) and 41 females (42.3%). The mean follow-up period was 106.1 ± 38.7 months, and the mean age at pulp treatment was 34.8 ± 15.4 months for primary anterior teeth and 69.1 ± 25.1 months for primary posterior teeth. Unilaterally pulpectomized teeth were significantly exfoliated earlier than the same tooth on the opposite side (p < 0.05). Also, in the case of teeth with periapical lesions, despite pulp treatment, the probability of extraction due to infection has been increased on primary anterior teeth (p < 0.05), but not on posterior teeth (p > 0.05). Pulpectomized teeth were lost earlier, an average of 7.8 months for primary anterior teeth and 8.5 months for primary posterior teeth. Early loss of the primary tooth can lead to space loss and premature eruption of the successor, so this can be considered when planning or performing treatment of the primary tooth.

임상가를 위한 특집 2 - 유치 및 미성숙 영구치 치수치료의 최신경향

  • 최근 10여 년 동안 치수치료 분야는 새로운 재료와 약제의 개발 뿐 아니라 새로운 치료개념이 도입되어, 지속적인 발전이 이루어지고 있다. 특히 2007년 11월에는 미국 소아치과학회 (American Academy of Pediatric Dentistry)와 미국 근관치료학회 (American Association of Endodontists)가 공동 심포지움을 열어, 유치와 미성숙 영구치 치수치료분야에서 증거-바탕 임상 (evidence-based practice)의 구현을 위한 획기적인 공동의 노력이 있었다. 또한 치수치료 분야에 조직공학(tissue engineering)의 개념이 적용되어 치수조직의 재생에 대한 연구가 활발히 이루어지고 있다. 이번 원고에서는 유치 및 미성숙 영구치의 치수치료에서 개념에 변화가 있는 분야를 정리해 보고 새로운 술식에 대하여 다루고자 한다.

  • 과거 정신지체는 사회의 불가사의한 것이었으나 현재는 이들의 원인과 병리현상의 이해로 예방과 의학적 관리에 괄목할 만한 발전이 이루어져 왔다. 그러나 이들 정신지체인의 치과관리는 과거와 별다른 변화를 이루지 못한 것이 우리의 현실이다 이들에 관심을 갖고 치료하기 원하는 치과의사들도 아직 진단이나 치료방법에 이용이 제한되어있다 정신지체인 들에게도 정상화 개념을 도입해 보다 나은 건강관리 환경을 이루도록 시대가 바뀌어 가고 있다. 환자의 권리를 보장키 위해 수용시설 입소화. 물리적인 강제치료 등 기존의 일반적인 행위에서 탈피하여 이제는 행동조절의 차원에서 약물이용과 치과적용에 필요한 모든 지식의 습득이 필수적인 것으로 변화되고 있다.

Effect of Dental Treatment Fear and Distrust of Dentists with Dental Caries Experience among Middle and High School Students in Daegu City, Korea (일부 지역 중·고등학생들이 치과치료공포감 및 치과의사불신감이 치아우식증에 미치는 영향)

  • The purpose of this study is to analyze factors impacting on the middle & high school students' dental treatment fear and distrust of dentists with dental caries experience to provide empirical data for the development of students oral health. The subjects of this study consists of 720 middle & high school students in Daegu area. The data were collected from July 1, 2009 to September 30, 2009 by self-administrative questionnaires. Among sub-scales of Dental Fear Survey(DFS), stimulate-response factor was the highest ($3.42{\pm}0.98$), and patient distrust toward dintist factor was the highest score($3.07{\pm}0.912$) among sub-scales of Dentist Distrust Survey(DDS). Mean score of DFS were DDS were higher in female than male student. The students who have higher DFS and DDS score, the also have high DMFT index. The higher DDS, the higher DFS, and it was statistically significant(p<0.001). The results suggest that oral health status of adolescent can improve diminishing dental fear and dentist distrust if patient-specific medical service may be provided.

A Study on Treatment under General Anesthesia in Pediatric Dentistry at Seoul National University Dental Hospital (서울대학교치과병원 소아치과에서 시행된 전신마취 하 치료에 대한 연구)

  • Behavior management of the child constitutes an important part of pediatric dentistry. The majority of children can be treated with adequate behavior management. However, some children who have extensive dental problems or mental, physical disabilities cannot cooperate. So, the need for general anesthesia as a special method of behavior management has increased recently. This study analyzes the records of patients who received dental treatment under general anesthesia. The 1378 cases of 1322 patients under 18 years of age, who received dental treatment under general anesthesia in department of pediatric dentistry at the Seoul National University dental hospital, from October, 2011 to December, 2015 were included. In this study, male (60.9%) was more than female (39.1%). The mean age of patients was 5.9 years and age from 5 to 9 years was the most frequent (56.0%). The primary dental treatment which patients received was dental caries treatment (51.1%), followed by surgical treatment (42.2%). The dental treatment under general anesthesia has many advantages including of performing a lot of treatments at once, producing good quality of treatment, and reducing the number of visits. General anesthesia may be a useful method of behavior management.

Study of Utilization of Dental High School and according to the Pain Experienced Dental Fear (고등학생의 치과이용실태와 통증 경험에 따른 치과공포에 대한 연구)

  • The purpose of this study was to study of utilization of dental high school and according to the pain experienced dental fear and anxiety. This survey was conducted on 370 high school students in Suwon from November 21 to 23, 2011. A total of 352 questionnaires were collected and analyzed. The collected data was analyzed using the statistical package SPSS 15.0 using frequency, mean and standard deviation analysis, t-test, one-way ANOVA, Duncan's test correlation analysis and Stepwise multiple regression analysis. The results state that students feel fear and anxiety were feeling anesthetic needle ($3.19{\pm}1.43$), seeing anesthetic needle ($3.14{\pm}1.44$). We found that students feel more rear and anxiety from caries treatment than scaling. It influence that having dental fear with past dental pain experienced during dental treatment and also hearing dental treatment of pain from their family and friends. We found out that there are some influencing factors on dental fear and anxiety, gender, oral health condition, smoking, pain experienced during dental treatment. We need to care dental fear and anxiety continuously and have prevention program. We have to try understanding students have dental fear and anxiety. So it's better they have good experience visiting dental clinic. We should develop the system and specially treat well while they have dental treatment with anesthesia and some sharp instruments.
