Predictable Treatment in Dental Esthetics (예측 가능한 심미치과 치료)

  • 아름다움이라는 것은 완전히 객관적이기도 하지만 극히 주관적인 표현의 결과일 수 있다. 심미치의학은 얼굴에서 표현되는 이러한 주관적인 면과 객관적인 면을 조화시킬 수 있는 학문이기에 21세기에도 여전히 치과 치료에 있어서 중요한 화두 중의 하나임에 틀림이 없다. 과거 심미적인 치료로 행하였던 많은 치료 술식들(open faced crown, resin faced crown,3/4 crown,7/8crown등)이 치과 치료 술식의 발달과 함께 재료와 치아 보철물 제작술 등의 발달로 인해 점차 사라지거나 변화하고 있는 것이 사실이다. 지금 소개하는 증례들은 현재 치과계에서 심미적인 치료 술식으로 인정하고 있기에 이러한 치료 술식에 대한 이해가 필요할 것이다. 우리 임상가들의 최종목표는 환자들에게 자연스러운 얼굴과 웃음을 회복시켜주는 것이다. 각 증례에 대한 한계에 대해 미리 알 수 있는 방법을 모색해 보고, 예측 가능한 치료를 시행하기위한 과정들을 살펴보고자 한다.

Comparison of Related Influencing Factors on Medical Services Satisfaction between General Dental Patients and Orthodontic Patients (치과 교정치료환자와 일반치료환자의 의료서비스 만족도 관련요인)

  • This study was carried out to examine comparison of relating influencing factors on medical service satisfaction between general dental patients and orthodontic patients and to improve quality of the medical services by the treatment type of the visit. The data were collected from 421 patients who visited dental clinics and hospitals in Daegu and Gyeongbuk region, using self-administered questionnaire for the period from Sep 1 to Sep 30, 2015. The multiple regression showed that the medical staff and medical fee were common statistically significant independent variables to influence service satisfaction in both groups, however, fear in the treatment was added to statistically significant variables in only the orthodontic patients. Therefore, based on the provision of high - quality medical services and adequate medical care for patients' satisfaction, comprehensive plans including tailor-made counseling management and reducing the fear of dental treatment for general dental patients should be needed.

Status of dental physical therapy work and hygienists' educational needs (치과위생사의 치과 물리치료 업무 현황 및 교육 요구도)

  • Objectives: Although the number of clinics offering temporomandibular joint (TMJ) physical therapy has been increasing to help with its treatment, the scope of dental physiotherapy reflected in the dental hygienist curriculum is very limited. This study aims to survey the status of dental physiotherapy work and the need for educating dental hygienists. Methods: An online community platform was used to survey 140 dental hygienists working in clinical dental hospitals. The survey included questions about general characteristics, the experience of dental physiotherapy work, the importance of physiotherapy work performance, and the need for physical therapy education for dental hygienists. Results: Half of the study participants had prior experience working in dental physiotherapy. Out of 140 participants, 88.6% agreed with the opinion that 'physical therapy work is important or that it will become important'. Furthermore, 84.3% agreed that 'physical therapy education is necessary for dental hygienists'. Even dental hygienists without physical therapy experience (50.7%) believed that related education would be necessary because of the importance of physical therapy work in dentistry. Conclusions: This study suggests the need for education in dental physiotherapy to help dental hygienists effectively perform clinical tasks.

  • 늙는다는 것, 즉 노화는 정상적인 생활과정이며 노령자체가 반드시 치과치료의 금기가 아니라는 것은 알고 있는 사실이다. 그래서 우리 전문가들에게 주어진 임무는 수명을 연장시키는데 기여함은 물론이고 노후를 생산적이고 유쾌하게 하여 생활의 질을 높이고 일상생활의 평범한 행위를 지속하도록 도와 주는데 있다 하겠다. 그러나 불행하게도 노인들은 참고 참다가 견딜수 없어 치료를 제공받기 힘든 연령에 치과치료를 요구하게 되는 경향이 있는데 결국 이것이 문제가 된다. 퇴행성 변화와 이로 인하여 야기되는 만성질병 때문에 위험이 보다 커지고 총의치를 하기에 아주 불량한 상태에 있게 된다. 오늘날 불소치료와 그 외의 예방처치로서 치아상실에 영향을 미치고 있는 것은 주지의 사실이다. 그러나 현재 이미 노인이된 환자에 대한 혜택은 무시할 정도며 이러한 영향을 최소한 다음 세대까지 유지 될 것으로 보고 있다. 그러므로 앞으로는 어린이에 대한 관심과 예방에 대한 열의 속에서도 노인에 대한 불공정한 고정관념을 버리고 관심과 동정으로 노인의 치료를 우선으로 해서 치과의사는 상품의 공급자가 아니고 주된 목적과 임무가 인도주의자 라는 것을 보여줘야 하겠다.

The Distribution and Treatment of Outpatients with General Anesthesia in Chonbuk National University Dental Hospital for 9 Years (최근 9년간 전북대학교 치과 병원 외래 전신마취 환자 분포 및 치료)

  • General anesthesia (GA) for dental care in handicapped patients is necessary to facilitate the provision of safe, efficient, and effective quality treatment. The aims of this study were to determine the anesthetic characteristics of handicapped patients in need of dental treatment in these day care units, and to establish for plan to provide better services. 325 patients who had outpatient general anesthesia from January 2005 to March 2014 were assessed for this study. Patients' distribution and treatment patterns were examined. The proportion of male patients (202, 62.2%) was higher than female patients (123, 37.8%) and the largest group of patients were 5 to 10 years old (85, 26.2%). The reasons for general anesthesia included mental and physical disabilities (207, 63.7%), behavior management (84, 25.8%), parent needs (14, 4.3%), and so on. Restorative treatment was the most common procedure with the average of 4.2 teeth treated per one patient and 43 (13.2%) patients underwent general anesthesia for dental treatment more than once. To expand and improve access to the dental care of the disabled, improvement of the health care system, enhancement of their training for dental care by professionals, and enlarging caregivers'understanding of the importance of oral care in the early stages are required.

DENTAL CARE FOR HANDICAPPED PATIENTS AT DAY SURGERY CENTER:A CASE REPORT (통원수술센터를 이용한 장애아동의 치과치료에 대한 증례보고)

  • The dentists often encouter a child who can not be treated with traditional behavior management technique. Mentally or physically handicapped child could receive proper dental care under general anesthesia. General anesthesia for dental care can be done in an outpatient surgery or inpatient surgery depending on the patient. In outpatient surgery, the patient is admitted and discharged the same day. Postoperative care and discharge procedure are abbreviated. Advantages of outpatient surgery are much less emotional disturbance of patients, reduced risk of nosocomial infection and less cost of treatment. A case of a physically handicapped patient treated under general anesthesia at Day Surgery Center is presented and the procedure and patient selection of outpatient surgery as well as its limitation is reviewed in this paper.

Preoperative Factors of Immature First Permanent Molars Treated with Vital Pulp Therapy (생활치수치료를 시행한 미성숙 제1대구치의 술전 상태)

  • This study aimed to analyze the preoperative factors of immature first molars treated with vital pulp therapy and to find out their correlation in pediatric patients. From May 2014 to January 2020, 523 patients and 1,242 immature first molars were investigated. Factors including age, sex, tooth location, Molar-incisor hypomineralization (MIH), caries cavity location, and history of previous restoration were evaluated. As a result of the study, the vital pulp therapy group had 5.56 times more MIH, 3.39 times more mesial cavities, and 8.73 times more distal cavities. In order to avoid vital pulp therapy in immature first molar, early diagnosis and active management of MIH and preventive treatment of mesial and distal caries are necessary after its immediate eruption.

  • Autism is characterized by pervasive impairments in social interaction, verbal and non-verbal communication and restricted and stereotyped behavior. It is difficult for autism patients to receive dental treatment as lack of cooperation, so various behavior management method have been tried for dental treatment in clinic. The patients with mild autism can be managed by conventional behavior modification method and phamaco-logical conscious sedation. But the patients with severe autism and multiple dental caries can be treated by deep sedation or general anesthesia. The benefits of children's dental care in general anesthesia are full-mouth rehabilitation in one single appointment. We reported three cases of autism patients who were treated by various behavior management method as to severity of autism.

A STUDY ON THE DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA (서울대학교치과병원 장애인진료실에서의 치과치료 분석)

  • Seoul National University Dental Hospital opened The Clinic for The Disabled to provide dental treatment under outpatient general anesthesia. 432 patients underwent general anesthesia for dental treatment. The informations such as the pattern of dental treatment, the reasons of general anesthesia, the characteristics of the patients were analyzed. Followings are concluded. 1. The patients below 15 years old made up 50.60%. 2. The reasons for general anesthesia included mental and physical disabilities, systemic disease, dental phobia, and so on. 3. Restorative treatment was the most common procedure with the average of 9.7 teeth treated per one patient. 4. 62(14%) patients underwent general anesthesia for dental treatment more than once. 5. The efforts for better multidisciplinary system to provide more comprehensive and effective dental treatment including periodic recall check, preventive treatment for handicapped patients are necessary.
