A Study on Industry-University Cooperative Customized Curriculum of Dental Hygiene Study (산학연계형 치위생학과 주문식 교육과정 연구)

  • A recent rapid change in society drives the colleges and universities into the unlimited competition for satisfying users' demand. The introduction of customized educational system is one of the survival strategies of university that reflected this characteristic. The result of a research on the customized curriculum of the dental hygiene study shows that the managers of dental hospitals and clinics think that the dental assistance field is dental hygienist's major task. On the other hand, though the dental hygienists think that the oral health education is the main task, they just comply with the intention of managers of the dental hospitals and clinics in reality. Hence, the reinforcement of the practicum in the industry field is required to meet the demand from the industry. And there is also a need to offer an opportunity of self-realization through work life by allowing dental hygienists to have stable status within dental hospitals and clinics with enhanced education and training on preventive dental treatment and oral health.

  • Anterior tooth spacing is observed by pathological tooth movement (PTM), which is common in periodontal patients. And various occlusal factors contribute to PTM, especially in the maxillary anterior region, when there is excessive occlusal force, flaring due to position problem easily occurs. Teeth with loss of periodontal support tissue can secure stability when expanding the support area through intentional splinting, and change the occlusion when restored as a fixed prosthesis. After confirming the stable occlusion through the provisional prosthesis, it can be transferred to the final prosthesis through CAD-CAM. In this case, we present a long-term stable case through accurate diagnosis and treatment of the maxillary anterior teeth that have lost interdental contact.

  • One of the fastest growing segments of implant dentistry is the utilization of computed tomography (CT) scan data and treatment planning software in conjunction with guided surgery for implant reconstruction cases. Computer assisted planning systems and associated surgical templates have established a predictable, esthetic, functional technique for placing and restoring implants. Especially, a philosophy of restoratively driven implant placement has been generally adopted. Recently, a variety of commercial dental fields have released their scanning and fabricating protocols and methods for restorations. This process is still being investigated and developed for the most precise and predictable outcome. This case report describes a female patient who wanted dental implants in fully edentulous areas. Restoratively driven implant placements were performed with surgical guide and the patient was fully satisfied with the clinical results, and at 5-year post restorative follow-up assessment, both implant and prosthesis were proved clinical success.

The Experience of Medical Conflict and the Educational Needs of Dental Hygienists (치과위생사의 의료분쟁과 관련한 경험 및 교육 요구도 조사)

  • The study was conducted to provide data for the development of effective medical conflict prevention programs for dental hygiene by analyzing the dental experience conflicts. A self-administered questionnaire survey was conducted targeting dental hygienists who were performing dental assistance at university hospital, dental hospital and dental clinic in Busan and Gyeongnam regions from April 1 to 30, 2014. Collected questionnaires of 212 dental hygienists were then analyzed with IBM SPSS Statistics ver. 20.0 program. A total of 59.4% had experienced complaints, discontents and medical disputes and 24% of these had experienced a legal trouble developed from such and 95.3% were anxious and doubtful about a possible future medical dispute to some degree. Patient complaints, complaints of non-medical issues raised by 24.3% was the most common, notices, maps and descriptions in relation to the issues raised by 14.4%, is related to the impression issue was raised in the order of 13.5%. Occurrence of disputes did not show a significant difference by place of work, however, a possibility of development of dispute into a legal proceeding showed a significant difference depending on service career and it was investigated that the experienced group feels more pressures with regard to a medical dispute according to their experience of such. All respondents said that education on a prevention and countermeasures of medical disputes is necessary. Understanding of dental hygienists on medical related laws regarding the scope of duty is required to be enhanced and, in order to do so, opportunities to receive an education with regard to a prevention and countermeasures of medical disputes are to be expanded.

  • 방대한 양의 다양한 데이터들이 발생되는 의료분야에서는 임상데이터를 기반으로 보다 정확하고 효율적으로 현상을 분석/판단하여 의사가 환자진료 시 정확한 진단과 치료를 수행할 수 있도록 보조해주는 적절한 의사결정지원시스템이 요구되고 있다. 따라서, 이와 같은 요구를 충족시키기 위해서는 다종 다양한 데이터로부터 간결하면서도 효과적으로 개념들을 추출하고 구조화하여 개념계층구조로 표현할 수 있어야 하며, 실세계의 데이터에 대한 구조화와 요약을 제공하고 필요한 정보를 수월하게 접근할 수 있어야 한다. 본 연구에서는, 도메인 내의 다양한 데이터들로부터 개념들을 추출하고, 개념들 사이의 상하위 관계를 파악하여 개념계층구조를 구축하기위한 정형화된 데이터분석기법으로서 형식개념분석기법(Formal Concept Analysis)을 소개하고, 이를 치과 교정학 분야의 환자 임상데이터 분석기법(Cephalometric Analysis)에 융합한 형태의 임상의사결정지원시스템 개발 및 향후 연구과제 등에 관해 설명한다.

A Study on Radiation Safety Management by Dental Hygienist (치과위생사의 방사선 안전관리에 대한 조사 연구)

  • In spite of relatively low level of radiation dose used at dental clinics, long term exposure may be harmful, so radiation workers at dental clinics must be well aware of its danger. This study was to analyze the factors to have an influence on safety management behavior in the radiography chamber by understanding the relationship among the knowledge, attitudes and behavior in regard with radiation safety management by dental hygienists in order to take preventive measures for dental hygienists and suggest ideas to develop radiation safety training programs. For this, we contacted dental hygienists working at the local dental clinics for 4 months from December of 2003 to march of 2004 and obtained the following findings. 1. Concering the knowledge level of radiation safety management, $8.59{\pm}2.36$ was average score with the highest of 13 and the lowest of 3 from 15-scale test. In addition, knowledge level of radiation safety management by general characteristics showed statistically significant difference according to working experience (p < 0.001), marital status (p < 0.001), attendance rate of radiation safety management training program (p < 0.001), and type of clinic (p < 0.001). 2. Concering the attitude level of radiation safety management, $4.08{\pm}0.50$ is average score with the highest of $4.31{\pm}0.73$ and the lowest of $3.82{\pm}0.89$ by item from 5-scale test. Besides, attitude level of radiation safety management by general characteristics showed statistically significant difference according to age (p < 0.001), working experience (p < 0.05), attendance rate of radiation safety management training program (p < 0.01), and type of clinic (p < 0.001). 3. Concering the behavior level of radiation safety management, $2.89{\pm}0.77$ is average score from 5-scale test, which was relatively low in comparison with the level of attitude and the highest score was $3.82{\pm}0.94$ and the lowest $2.37{\pm}1.04$ by item. Behavior level of radiation safety management by general characteristics showed statistically significant difference according to working experience (p < 0.001) and type of clinic (p < 0.001). 4. From the survey of relationship among knowledge, attitude and behavior of radiation safety management was, we found that the higher the knowledge level of radiation safety management, the higher the level of attitude and behavior, and the higher the attitude level was, the higher the level of behavior.

NON SURGICAL TREATMENT OF SOFT PALATE LACERATION (연구개 열창의 비외과적 처치술)

  • Laceration of soft palate and oropharynx is relatively common in children. Soft palate laceration has been reported patients of all ages, ranging from new born to geriatric patients. However, young children often place objects their mouth, they may fall on the object or receive a direct force on the object which then perforates the soft palate tissue. Most frequently affected site is the left supra-tonsillar area. Lesions are predominately in the soft palate with-out perforation. Linear and superficial wounds are frequent. A typical injury is the flat- U-, or V-shaped with apex directed anteriorly. In those cases without any through-and-through lacerations or any tissue loss, suture is not necessary. Furthermore, suture of the affected site may hinder wound healing. Healing of the wound should be complete by three weeks with minimal scarring. There have been reports of carotid artery injury due to soft palate laceration causing neurologic complications. For such reasons, traumatized child must be in close observation for 2-3days, and if symptoms of complication are noticed consultation with oromaxillofacial surgeons, E.N.T., or neurologist is required. These are two reports of boys 2 and 3 years of age who had soft palate laceration caused by a falling-down-in jury with an object in their mouth. They were treated non-surgically and neurologic evaluation was carried out for 1 week with complete healing.

DIFFERENT WAYS OF SURGICAL MANAGEMENT FOR CHILDREN WITH CONFOUND ODONTOMA IN THE MANDIBLE (아동의 하악골에 발생한 복합 치아종의 외과적 처치에 관한 증례보고)

  • The odontoma is relatively a common benign odontogenic tumor and caused by overgrowth of odontogenic tissues. The recent classification by World Health Organization divides odontoma into 2 groups such as compound odontoma and complex odontoma. Compound odontoma comprises dental tissues, resembling the morphology of a tooth and has predilection for the anterior maxilla. In contrast, complex odontoma has unorganized mass, not resembling the normal tooth and has predilection for the posterior mandible. The etiology of odontoma is unknown and almost asymptomatic. So, it usually is found in routine radiographic examination, and most common presenting symptom is impacted or unerupted permanent teeth and retained primary teeth. It can occurs almost anywhere in jaws. It is desirable that odontoma should be removed by surgical enucleation including follicle and surrounding soft tissues. Considering the age and behavioral cooperation of patient, the development of permanent dentition, the location of odontoma in jaw, the need for the concomitant operative dentistry, operation is performed in outpatient department with/without sedation or under general anesthesia with endotracheal intubation. In this case report, 2 patients with compound odontoma were treated by surgical enucleation including follicle and surrounding soft tissues. One patient, about 5 years old, was treated under general anesthesia and concomitant operative dentistry was performed. The other patient, about 11 years old, was treated under local anesthesia in outpatient department. In 2 cases, after 4 months, surgical defects were filled with new bone and normalization of eruption path of impacted permanent teeth was observed.

A Study on Job Stress and Eating Behavior and Habits of Dental Hygienists (치과위생사의 직무스트레스와 식행동 및 식습관에 관한 연구)

  • Study the effect of stress factors and increases in stress at work on the changes in eating behavior and habits and the influence of these changes on the health of dental hygienists. Use the responses of 273 dental hygienists who work in or around the Seoul area after surveying them by post or face to face. The biggest reason for skipping breakfast was having no time (60.4%) and the most desired change in eating habits was over eating (33.0%) while 51.6% responded that they over ate or binge ate due to stress. The largest stress factor was work load 39.6% and subjects responded that after stress the amount of food consumed increased very much for 12.1%, very for 49.8%, indicating increased food consumption than usual. The group with the most job stress had a high level of self stress awareness (p<0.001), nervousness (p<0.01), back pains (p<0.001), optical fatigue (p<0.01), headache (p<0.01) and indigestion (p<0.01). The higher the job stress, the poorer the eating behavior was (p<0.01) and job stress was high for dental hygienists working in dental clinics with less than three dental hygienists (p<0.05). Low eating behavior scores could be seen in groups that took 4 night shifts per week (p<0.05). Job stress scores for high for groups with no break time during work (p<0.001) and these groups showed low eating behavior scores (p<0.01). Furthermore, if high levels of self stress awareness was present, job stress was very high (p<0.001) and eating behavior scores were the lowest (p<0.05). Job stress of dental hygienists impede the eating behavior and habits therefore has a negative influence on their health. There is a need for more efforts to solve this problem and thus maintain the health of dental hygienists.

ROENTGENOCEPHALOMETRIC STUDY ON FACIAL HEIGHT AND OCCLUSAL PLANE INCLINATION IN CLASS II MALOCCLUSION GROUP (성인 II 급 환자의 안면 수직고경및 교합평면의 특징에 관한 두부방사선학적 연구)

  • This study was investigated to assess the difference of facial height and occlusal plane inclination between normal occlusion group and class II malocclusion group. The subjects consisted of 50 normal occlusion (male 25, female 25) and 50 class II(male 25, female 25) malocclusion patients. All subjects are adult. lateral cephalogram was taken with standard method traced, and digitized for each subjects. The computerized statiscal analysis was carried out with SPSS program. The results were as follows 1. In class II malocclusion group, variables significant different from normal occlusion group were as follows ; SN-FOP, FH-BOP, MP-BOP, AB-BOP, AB-FOP, Facial plane-BOP, FP-FOP 2. In class II malocclusion group, the posterior facial height -especially posterior lower facial height-was significantly smaller than normal occlusion group.(P<0.05) 3. In class II malocclusion group, the angles between occlusal plane and upper and lower incisor, the angle between upper molar and bisected occlusal plane were significantly larger than those of normal occlusion group. (P<0.05) 4. L1 to Mandibular plane (mm) was a unique factor of occlusal plane position that showed significant difference in class II malocclusion group. 5. The correlation between overbite and occlusal plane inclination existed in class II malocclusion group, but the correlation didn't exist in normal occlusion group.
