Digital imaging of film-based cephalograms using a digital camera (디지털 카메라를 이용한 필름 두부방사선사진의 디지털 이미지 전환)

  • As computer Programs for cephalometric analysis were developed in diagnosis & treatment planning, digital imaging of film-based cephalograms came to be needed. When a digital camera is used, a problem encountered the image distortion produced according to the focal length, which causes errors in indentifying landmarks. In addition, changes in the image size and compression ratio will inevitably produce a low quality image, causing errors in identifying landmarks. Hence. we have found the focal length producing the least image distortion when digital imaging the film-based cephalograms and the minimal digital camera setting which helps to identify the correct landmarks using the COOLPIX4500 digital camera (Nikon, Japan). The results were as follows The image distortion was minimized at a focal length of 16.4mm (79.4mm when converted into a 35mm film camera) when digital imaging the film-based cephalograms. When wide imaging, with a focal length of under IS.4mm, barrel distortion was found and when tole imaging. with a focal length of over 15.4mm pincushion distortion was found. The minimal digital camera setting was $2272{\times}1704$ pixel at normal (1/8) compression from which we can identify the correct landmarks at the same level as tracing the film-based cephalograms manually. As a result. when digital imaging the film-based cephalograms, using a COOLPIX4500 digital camera (Nikon, Japan), the focal length should be 16.4mm the pixel image size over $2272{\times}1704$, and the compression ratio over normal (1/8).

  • Analysis of lateral cephalometric radiograph (cephalogram) has been used routinely to evaluate skeletal and dental relationships, but analysis of the lateral facial photograph has not been used frequently for evaluation of skeletal relationships. As concerns about harm of X-ray irradiation increases, this study was planned to evaluate the possibility of substituting analysis of the lateral cephalogram with analysis of the lateral facial photograph by comparing these two analyses. According to the ANB values from cephalometric analysis, subjects were divided into three groups: Class I malocclusion group (n=32). Class II malocclusion group (n=32), and Class III malocclusion group (n=31). After measurements of angles indicating horizontal and vertical relationships of the maxilla and mandible on the lateral cephalograms and photographs, differences between Class I, II and III groups were evaluated. To evaluate the similarity between two similar values in the cephalograms and photographs, t-test using standardized variable Z and correlation analysis were performed in the Class I malocclusion group. The results showed that 1) SnN'Pg' on the photograph can be used to evaluate the antero-posterior relationship of the maxilla and mandible (ANB), 2) N'-Sn/Sn-Pg' on the photograph can be used to evaluate facial convexity (NA/APg), 3) Sn-Tra-Me' on the photograph can be used as a measurement similar to FMA. In conclusion, partly substituting lateral cephalogram analysis with lateral facial photograph analysis was possible in the evaluation of the maxilla and mandible.

  • It is important that the orthodontist accurately assess the degree to which facial asymmetry contributes to a given malocclusion before treatment planning. P-A, submentovertex and verticosubmental view have been used in the assessment of facial asymmetry. Among them, submentovertex view is rarely used because it has low reproducibility and is short of normal data and proper analysis method. The purpose of this study was to develop a submentovertex cephalomentrics and obtain normal data in Korean adults. The subjects consisted of 40 normal adults (male : 22, female 18) without the experience of orthodontic treatment. We find the 2 angular and 9 linear measurements. Though submentovertex cephalomentrics has the limitation in comparing the absolute length between right and left, it is useful to examine the relationship of skeletal and dental midline, the shape and location of condyle head and the shape of mandibular body in submentovertex view Therefore, if we understand the limitation of submentovertex cephalomentrics and use lateral , P-A and submentovertex cephalomentrics together, we will measure the location and amount of skeletal disharmony more exactly.

GEOMETRIC CORRECTION OF RADIOGRAPHIC IMAGES USING GENERAL PURPOSE IMAGE PROCESSING PROGRAM (일반 영상처리 프로그램을 이용한 방사선사진의 기하학적 보정에 관한 연구)

  • 본 연구는 비규격화시켜 촬영한 두장의 방사선 사진을 일반 영상처리 프로그램으로 기하학적 보정을 하여 디지탈 공제 촬영술을 시행한 결과를 개인별로 제작된 필름유지장치를 이용하여 규격화 시켜 촬영한 두장의 방사선사진을 디지탈 공제 촬영술을 시행한 결과와 비교하여 일반 영상처리 프로그램의 임상적 유용성을 평가해보고자 시행하였다. 19명의 자원자를 대상으로 하여, 각 환자에서 4매의 하악구치부 치근단 사진을 촬영하였다. 그중 2매는 XCP 필름 유지장치만으로 평행촬영법으로 촬영하였고, 나머지 2매는 교합제에 인상재를 부가하여 개인별로 제작된 XCP필름 유지장치를 이용하여 표준화시켜 역시 평행촬영법으로 촬영하였다. 기하학적 보정은 "Adobe Photoshop"과 "NIH Image" 프로그램으로 시행하였다. 특히 "Adobe Photoshop"의 임의영상회전 기능과 "IH Image"의 공제시술시 중첩된 사진을 투명하게 보여주는 기능, 병진기능을 활용하였다. 두 사진의 유사성을 측정하기 위해 공제된 사진의 계조도의 표준편차를 구하였다. 표준화군의 평균 표준편차가 기하학적 보정군의 평균 표준편차보다 약간 낮았으나, 통계적으로 유의성이 있는 차이를 보이지는 않았다. 위의 결과로 미루어보아, 하악구치부 에서 XCP필름 유지장치로 평행촬영한 비표준화방사선사진을 "Adobe Photoshop"과 "NIH Image" 프로그램을 이용하여 기하학적 보정을 할 수 있으리라 사료된다.

Assessment of the Object Detection Ability of Interproximal Caries on Primary Teeth in Periapical Radiographs Using Deep Learning Algorithms (유치의 치근단 방사선 사진에서 딥 러닝 알고리즘을 이용한 모델의 인접면 우식증 객체 탐지 능력의 평가)

  • The purpose of this study was to evaluate the performance of a model using You Only Look Once (YOLO) for object detection of proximal caries in periapical radiographs of children. A total of 2016 periapical radiographs in primary dentition were selected from the M6 database as a learning material group, of which 1143 were labeled as proximal caries by an experienced dentist using an annotation tool. After converting the annotations into a training dataset, YOLO was trained on the dataset using a single convolutional neural network (CNN) model. Accuracy, recall, specificity, precision, negative predictive value (NPV), F1-score, Precision-Recall curve, and AP (area under curve) were calculated for evaluation of the object detection model's performance in the 187 test datasets. The results showed that the CNN-based object detection model performed well in detecting proximal caries, with a diagnostic accuracy of 0.95, a recall of 0.94, a specificity of 0.97, a precision of 0.82, a NPV of 0.96, and an F1-score of 0.81. The AP was 0.83. This model could be a valuable tool for dentists in detecting carious lesions in periapical radiographs.

EVALUATION OF THE SKELETAL MATURITY USING THE CERVICAL VERTEBRAE AND HAND-WRIST RADIOGRAPHS (경추골과 수완부골 방사선 사진을 이용한 골성숙도 평가)

  • In clinical orthodontics, it is significant to understand the stage of growth in a growing patient. In order to assess the skeletal maturity of the patients, the hand-wrist radiograph and the cervical vertebrae (Cervical Vertebrae Maturation Indicators : CVMI) was evaluated from the lateral cephalograph and the skeletal maturity determined from the hand-wrist X-ray (Skeletal Maurity Indicators : SMI) taken on the same day in the same patients, and its interrelationship examined to come up with the following results: 1. The skeletal maturity evaluated from the hand-wrist radiograph and the maturation of the cervical vertebrae from the lateral cephalograph showed a significant interrelationship with each other. 2. In the evaluation of the skeletal maturity using the SMI and CVMI, the CVMI 1 showed a siginificant correlation with SMI 1, 2, the CVMI 2 with SMI 3, 4, CVMI 3 with SMI 6, 7, CVMI 4 with SMI 7, 8, CVMI 5 with 9, 10 and CVMI 6 with SMI 11. 3. When the morphological changes in the 2nd and 3rd cervical vertebrae were separately observed, it was seldom that the concavity appeared in the lower border of the 2nd cervical vertebra and at the same time not appear in the 3rd cervical vertebra (CVMI 2 : 10.38%, CVMI 3 : 6.56%) 4. In each of the skeletal maturation stage evaluated from the hand-wrist and the cervical vertebra, the average age and its standard deviation in male and female patients appeared to have large differences among individuals. Skeletal maturation seemed to appear earlier for the girls than for the boys, and its termination 24 months faster for girls.

Evaluation or potency or panoramic radiography for estimating the position of maxillary impacted canines using 3D CT (3D CT를 이용한 파노라마 방사선 사진상 상악 매복 견치 위치 정보의 유용성 평가)

  • Objective: The aim of this study was to evaluate the potency of panoramic radiography for the detection of maxillary impacted canines. Methods: Twenty-five patients were selected, comprised of 7 males (mean age: 10.9 years, range: 8.5 - 14.5 years) and 18 females (mean age: 10.9 years, range: 8.2 - 15.7 years). In total, thirty-five maxillary impacted canines were estimated. The position of the canine and root resorption of adjacent teeth were evaluated on panoramic radiography and 3D CT. Results: Except for angulation to the occlusal plane, the other parameters, such as tooth length, crown width, vertical distance and lateral shift showed larger values on panoramic radiography compared to 3D CT. In palatally impacted cases, the angulation of canine was smaller, and the vertical distance to the occlusal place was larger on panoramic radiography than 3D CT. For labially impacted canines, tooth length, crown width, and angulation to the occlusal plane were similar for the two methods. The sensitivity for detecting root resorption on panoramic radiography was calculated as being 33.3% of 3D CT. Conclusions: The position of labially impacted canines can be effectively estimated using panoramic radiography, but palatally impacted canines need further investigation such as 3D CT for proper diagnosis.

  • Three-dimensional approaches for the diagnosis and analysis of the dentofacial area are becoming more popular in accordance with the development of cone-beam CT (CBCT). The purposes of this study were to evaluate the reliability of cephalometric measurements of lateral cephalograms generated from a CBCT image by making comparisons with the traditional digital lateral cephalogram, and to evaluate the possibility of the clinical application of CBCT generated cephalogram images. Methods: Twenty patients whose external auditory meatus could be identified in the CBCT image were selected, and both CBCT and digital cephalograms were taken. Differences between the measurements of both cephalograms were tested by paired t-test. Results: Among the 22 measurements used, only U1-FH, Mx6 to PTV, and maxillomandibular difference showed statistically significant differences between the CBCT generated cephalogram and the digital cephalogram. Conclusions: The results suggest that the CBCT generated cephalogram can be used for some cephalometric measurements not requiring porion, PTV, condylion as a landmark (SNA, SNB, U1 to SN, IMPA, interincisal angle, etc.).

  • Periapical cemental dysplasia(PCD) is a condition most commonly seen in the mandibular incisor region. Radiographically it passes through the three phases(osteolytic stage, intermediate stage, and mature stage). At osteolytic stage, the lesion is similar to features associated with granuloma or cyst that arise following pulpal necrosis. So, it is important to confirm the vitality of the pulp to diagnosis. In this case, it is difficult to confirm the vitality of involved tooth because the tooth was covered with PFG bridge. And it is unusual that the PCD lesion at mandibular incisors has occurred at first and the lesion of mandibular canine and mandibular premolar were occurred afterward.

Change in tooth length and angulation on panoramic radiographs taken at different labiolingual and buccolingual inclinations (치아의 순(협)설 경사도 변화에 따라 파노라마 방사선 사진에 나타난 치아 길이 및 각도 변화)

  • Objective: The purpose of this study was to examine how the mesio-distal angulation and the length of each tooth changes on panoramic radiograph at different bucco-lingual inclinations. Methods: After constructing an acrylic model based on the mean arch of 30 adults with normal occlusion, the wire was placed in the center of the teeth on the acrylic model. First, the wire was implanted in normal angulation and inclination and a panoramic radiograph taken. After changing the inclination from $I-5^{\circ}\;to\;I+15^{\circ}\;by\;5^{\circ}$, a panoramic radiograph was taken again and the mesio-distal angle and wire length on the panoramic radiograph were assessed. Results: When the wire was implanted at the normal angulation and inclination, the length measured in the panoramic radiograph was magnified $111{\sim}117%$ from the original length in the anterior region and $121{\sim}125%$ in the posterior region. Only the central and lateral incisors showed significant length differences when the inclination was changed from $l-15^{\circ}\;to\;I+15^{\circ}$ at fixed angulation. When the inclination was changed from $l-15^{\circ}\;to\;I+15^{\circ}$, the angulation of most teeth on panoramic radiograph appeared to be more disto-angulated than in reality, and the lateral incisor and canine showed the largest difference. Only $l-15^{\circ}\;to\;I+15^{\circ}$ groups of premolars and $I+15^{\circ}$ group of molars showed more mesio-angulation than in reality. As the labio(bucco)lingual inclination of all teeth were decreased, tooth angulation in the panoramic radiograph appeared to be more disto-angulated. Conclusion: The labio-liugual inclination of teeth should be considered because it affects panoramic image of teeth, such as length of incisors and angulation of other teeth.
