THE EFFECT OF THE pH OF REMINERALIZED BUFFER SOLUTIONS ON DENTIN REMINERALIZXATION (재광화 완충용액의 pH 변화가 상아질의 재광화에 미치는 영향)

  • Dental caries is the most common disease in the oral cavity However, the mechanism and treatment of dental caries is not completely understood since many complex factors are involved. Especially the effect of pH on remineralization of early stage of dental caries is still controversial In this study, dental caries in dentin was induced by using lactic acidulated buffering solutions and the loss or inorganic substance was measured. Also decalcified specimens were remineralized by three groups of solution with different pH (group of pH 4.3, 5.0, and 5.5). Then, the amount and the area of inorganic substance precipitation was quantitatively analyzed with microradiograph. Also a qualitative comparison of the normal phase the demineralized phase, and the remineralized phase of hydroxyapatite crystal was made under SEM. The results were as follows, 1. In microradiograghic analysis, as the pH increased, the amount of remineralization in decalcified dentin tended to increase significantly As the pH decreaced, deeper decalcification, however, occurred along with remineralization. The group of pH 5.5 had a tendency to be remineralized without demineralization (p<0.05). 2. In SEM view, the remineralization in dentine caries occurred from the hydroxyapatite crystal surface surrounding the mesh of organic matrix, and eventually filled up the demineralized area. 3. 5 days after remineralization, hydroxyapatite crystal grew bigger with deposition of inorganic substance in pH 4.3 and 5.0 group, and the crystal in the remineralized area appeared to return to normal. After 10 days, the crystals in group of pH 4.3 and 5.0, which grew bigger after 5 days of remineralization, turned back to their normal size, but in group of pH 5.5, some crystals were found to double their size. In according to the results of this experiment, the decalcifying and remineralizing process of dentine is neither simple nor independent, but a dynamic process in which decalcification and remineralization occur simultaneously. The remineralization process occurred from the hydroxyapatite crystal surface.

FLUORIDE RELEASE AND RECHARGE OF GLASS IONOMER CEMENTS (글라스 아이오노머 수복재의 불소 유리 및 재흡수 양상)

  • The replacement of dental restorations due to secondary caries is a continuing problem in restorative dentistry. The secondary caries can be partly prevented by using fluoride containing dental materials such as glass ionomer cement, which releases and be recharged with fluoride ion acting as a fluoride reservoir. For the purpose of investigating the behaviors of fluoride release and recharge of conventional and high viscosity glass ionomer cements, the experiment was performed on the seven specimens each from 4 kinds of materials ; 1 kind of conventional glass ionomer cement, 2 kinds of high viscosity glass ionomer cements and 1 kind of composite resin. The amount of fluoride release was measured over 7 days with pH/ion meter and fluoride specific electrode. After measuring daily fluoride release, the specimens were recharged with 2% NaF solutions for 4 minutes and measured for 3 days with recharging repeated two consecutive times. The results were as follows : 1. Significantly more fluoride was released at first day after recharge in all materials except Z-100. 2. High viscosity glass ionomer cements released more or nearly equal amount of fluoride after recharge compared with the initial release(P<0.05). 3. The fluoride release after recharge with 2% NaF solution was in order of Fuji IX, Ketac Fil, Ketac Molar, Z-100.

THE ANALYSIS OF REMINERALIZATION EFFECT IN FLUORIDE VARNISH USING CONFOCAL LASER SCANNING MICROSCOPE (공초점 레이저 주사 현미경을 이용한 불소 바니쉬 재광화 효과의 분석)

  • It is well established that fluoride products play an important role in the prevention and remineralization of carious lesion. Fluoride varnish is a concentrated topical fluoride and varnishes adhere to tooth surface, permitting prolonged fluoride exposure and uptake. In this study, the artificial initial enamel caries was caused on the sound human enamel and divided 60 specimens into three groups. Group 1 and group 2 were treated with the topical application of fluoride varnish and stored in artificial saliva for 1 and 2 weeks. Group 3 was stored in artificial saliva for 2 weeks, which acted as control group. Changes in mineral contents were analysed with the confocal laser scanning microscope. The following results were obtained: 1. In group 1 and group 2, the total fluorescence of the lesion(TFL) was reduced in remineralized area compared to in demineralized area(p<0.05). 2. The total fluorescence of the lesion of remineralized area was more reduced in group 2 than in group 1(p<0.05). 3. The total fluorescence of the lesion was more reduced in group 2 than in control group(p<0.05). 4. Confocal laser scanning microscope can be used in quantitative analysis of remineralization by fluoride varnish.

DEVELOPING OF QLF-D FOR EARLY DETECTION OF DENTAL CARIES (치아 우식증의 조기 진단을 위한 QLF-D 개발)

  • QLF-D system composed with DSLR(digital single-lens reflex) camera, and the images of natural enamel caries and artificial caries was developed from 2 days to 14 days captured by QLF-D system. The correlation between lesion depth of the polarized microscope and luminosity ratio of QLF-D image was analyzed and the results were summarized as follows: 1. The Pearson correlation value between the lesion depth of polarized microscope images and luminosity ratio of QLF-D images was 0.969(p<0.01). 2. From Regression analysis of lesion depth from polarized image by demineralized period, the equation was y = 8.67x - 1.16(p<0.05). 3. From Regression analysis of luminosity ratio from QLF-D image by demineralized period, the equation was y = 3.53x + 6.42(p<0.05). From the results, QLF-D system can detect the enamel caries at the very early stage and can monitor the progression of demineralization and remineralization. For the convenient use of QLF-D system in the laboratory, the image analysing software was needed to analyze of interest site of enamel caries lesion.

CHARACTERISTICS OF FLUORIDE RELEASING RESTORATIVE MATERIALS AFTER TOOTHBRUSH-DENTIFRICE ABRASION (잇솔질 마모 처리한 불소방출성 수복재의 특성)

  • The objectives of this study were to evaluate the fluoride release and the change of surface roughness of fluoride releasing restorative materials after toothbrush-dentifrice abrasion. Seven commercially available fluoride releasing restorative materials (Fuji II LC Improved: FL, Compoglass F: CG, Dyract AP: DR, F2000: FT, Ariston: AT, Tetric: TR, and Gradia: GD) were selected as experimental materials. The results obtained were summarized as follows: 1. The AT, FL, and CF groups produce an initially large burst of fluoride release and comparatively low but continuous release of fluoride. But other groups produce no initial burst of fluoride and levels of release remain relatively constant. 2. The rate of fluoride release remains steady state after 40 days. The results of Tukey's test (P<.05), the fluoride release showed the order of decreasing rank as follows: AT > FL > compomers > TC and GD. 3. The surface roughness indicated that the highest value was observed in the FT group and the lowest value was observed in the CF group. The surface roughness of Tukey's test showed the significant differences between groups of FT, AT and FL and groups of CF, DR, GD and TC (P<0.05).

Remineralization effect according to application cycle of fluoride varnish: QLF-D analysis (불소바니쉬 도포주기에 따른 재광화 평가: QLF-D의 활용)

  • Objectives: The purpose of the study is to investigate remineralization effect according to application cycle of fluoride varnish in preschool children. Methods: The study subjects were fifty six children (92 teeth) having one or more early dental caries lesion. Fluoride varnish was applied to the primary area of maxillary and anterior mandibular teeth with the interval of one month, three months, and six months. Results: Dft index was 3.54 in male and 3.44 in female. The fluorescence loss(${\Delta}F$) value due to early caries lesions showed a significant difference in experiment groups I and II after the application of fluoride varnish, but there was no significant difference in experiment group III. After fluoride varnish application, there was a significant change in experimental group I and III. The maximum fluorescence loss(${\Delta}Fmax$) from early caries lesions showed a significant difference after application of the fluoride varnish in all three experiment groups of I, II, and III(p<0.001). Conclusions: Fluoride varnish should be recommended and applied to the children of high risk caries group in every three months.

  • The optimum culture conditions for an antibiotics from Actinomyces sp. were investigated. The optimum composition of medium for antibiotics production was 1% glucose, 1% soybean meal, 0.5% NaCl, 0.1% $CaCO_2$, and the optimum initial pH was 7.0. And the antibiotics showed highest activity when the strain isolated from soil was aerobically cultivated at $28^{\circ}C$ for 72hours under the optimum conditions. A production of the antibiotics from Actinomyces sp. begins at the 36th hours and then reached the maximum at the stationary phase developed at the 72th hours under the optimum conditions.

  • Dental Caries which has high prevalence rate, accounts for majority of dental diseases. Many treatment and preventive treatment has been developed, thereby reducing the prevalence rate, but in our country, fluoridization has not spread widely yet, so prevention has not been done satisfactorily. When dental caries progresses, irreversible damage of tooth structure occurs. In initial dental caries, demineralizing tooth structure can be remineralized, so restorative treatment is unnecessary. In this study, 20 teeth restored with composite resin without fluoride release were used and divided into two groups. Incipient dental caries were artificially made and demineralization procedure was done for 1 and 2 weeks, for each group. Changes in mineral contents around the margins were analysed with confocal laser scanning microscope. The results were as follow. 1. Both total fluorescence of the lesion and average fluorescence of the lesion of remineralized samples decreased compared to demineralizing state. (p<0.01) 2. Confocal laser scanning microscopy can be used in quantitative analysis of mineral change. In result, confocal laser scanning microscopy can be used in quantitative analysis of mineral change and it could be used in many different fields of dentistry in the future.

REMINERALIZATION EFFECT OF INTERPROXIMAL CARIES ADJACENT TO GLASS IONOMER RESTORATIONS: IN VITRO STUDY USING QLF (QLF(Quantitative light-induced fluorescence)를 이용한 글라스 아이오노머 수복물의 인접면 우식 재광화 효과에 관한 연구)

  • This in vitro study compared the remineralization of incipient interproximal caries in the presence of three glass ionomer cements(highly-filled glass ionomer cement, resin-modified glass ionomer cement, compomer) and a resin composite(control). Thirty-two extracted premolars were selected based upon the lack of any visible demineralization. The teeth were coated in a transparent acid resistant nail varnish leaving $3{\times}3$ mm square. The teeth were subjected to the demineralizing buffer for 3 days and quantitative light-induced fluorescence(QLF) images of the subjects were taken. Proximal restoration was simulated by placing tooth specimens and the various glass ionomer cements in closed containers with artificial saliva at $37^{\circ}C$ and pH 7.0 with constant circulation. Further QLF images were subsequently taken at 30, 60, and 90 days. The changes of mineral loss(${\Delta}Q$) were evaluated by QLF and the change of ${\Delta}Q$(${\Delta}{\Delta}Q$) were compared between groups in order to evaluate the effects of remineralization. All data were analyzed using ANOVA and the post-HOC Dunnett C multiple comparison test at p<0.05. While ${\Delta}Q$(changes of mineral loss) increased for all treatments, the increases for three glass ionomer groups were significantly higher than that for the resin group at first month period. As time went on, the amount of ${\Delta}{\Delta}Q$ decreased.

Evaluation of Remineralization Effects on Enamel Demineralization by Anti-cariogenic Agents using Quantitative Light-induced Fluorescence-digital (QLF-D) in vitro (유치 및 영구치에서 QLF를 이용한 항우식 제품의 재광화 효과 비교)

  • The purpose of this study was to evaluate the difference of remineralization effects of various anti-cariogenic toothpastes on artificial carious lesions in primary and permanent teeth using quantitative light-induced fluorescence-digital (QLF-D) system. Sound human primary (n = 48) and permanent teeth (n = 48) were randomly divided into following groups : control group (Group 1), fluoride toothpaste (Group 2), functionalized tricalcium phosphate (fTCP) + fluoride toothpaste (Group 3), and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) toothpaste (Group 4). Specimens were prepared by exposure in a demineralizing solution and then treated using the different toothpastes twice daily during 14 days. All specimens were analyzed with the QLF-D system. QLF data analysis indicated three different toothpastes showed significant remineralizing effects compared to Group 1 in both primary and permanent teeth. Also, the remineralizing effects in Group 3 and 4 were significantly higher than in Group 2. This study suggested that the toothpastes containing fTCP + fluoride and CPP-ACP have the significant anti-cariogenic effects on enamel demineralization in both primary and permanent teeth, and QLF-D is an useful device to assess the incipient carious lesion and remineralization effects of the anti-cariogenic materials quantitatively. Therefore, clinicians can consider the QLF-D system for the evaluation of demineralization and remineralization in primary and permanent teeth.
