Quality of Life in Children and Adolescents with Cancer (소아청소년 암환자의 삶의 질)

  • Purpose: This study was done to provide basic data for developing nursing interventions to enhance quality of life of pediatric patients with cancer (children and adolescents) by examining the quality of life and related factors. Methods: Participants were 134; 67 pediatric patients and 67 parents. The PedsQL$^{TM}$ 3.0 Cancer Module was employed to measure quality of life in the participants. The related factors included general and clinical characteristics of the participants. Results: Mean score for quality of life in the patients was 75.07, and mean score for patient quality of life as perceived by their parents was 64.40. Among the quality of life subscales, treatment anxiety had the highest score whereas nausea had the lowest score. Mean score in adolescent patients (13-18 years of age) was 71.62, lower than the 78.04 for child patients (8-12 years of age). Regarding general and clinical characteristics of the participants, there were no significant differences in the scores. Conclusion: The results indicate that there is difference in perception of quality of life between patients and their parents, and between children and adolescents and these differences should be taken into account when planning and providing nursing care.

Review of Psychiatric Adolescent Inpatient with Dermatologic Consultations (청소년 정신과 입원 환자들의 피부과 자문 의뢰에 관한 행태 분석 및 고찰)

  • Objective:To review the patterns of the dermatologic consultations of psychiatric adolescent inpatient and to explore the relationship between the dermatologic disorders and psychiatric disorders. Methods:We retrospectively studied the data from 22 cases referred by psychiatric adolescent for a dermatologic consultation over 10 years in Daegu Catholic University Medical Center and compared with the data from 108 cases referred by the other department adolescent patients. Results:The mean age of patients was 15.9. The male to female ratio was 1:1.44. The most common psychiatric and dermatologic disorder was major depressive disorder and acne, respectively. The most frequent reason for consultation was to ask for dermatologic disease or condition(54.5%) followed by to perform cosmetic procedure of patients need(40.9%) and to perform dermatologic test(4.6%). Conclusions:More than just a cosmetic disfigurement, dermatologic disorders are associated with a variety of psychopathologic problems that can affect the patient. Increased understanding of biopsychosocial approaches and liaison among psychiatrists and dermatologists could be beneficial.

Relationship between locus of control and treatment compliance in adult orthodontic patients (성인교정환자의 내외통제소재와 치료협조도에 대한 연구)

  • It could be suggested that adult orthodontic patients may have peculiar psychological features as well as different periodontal tissue conditions compared with adolescent patients. The aims of the present investigation were to explore the relationships between psychological characteristics and treatment compliance of patient to orthodontist and assistants (dental hygienists). Two types of locus of control data (I-score, Internal locus of control score ; E-score, External locus of control score) were obtained for 312 adult patients and 765 adolescents and then analyzed in relation to their sex and treatment compliance. In general, orthodontic patients showed higher I-score trends than ordinary persons and adult patients manifested higher I-score than adolescent patients. Unlike adult patients, female adolescent patients showed higher I-score than male adolescent patients. Adult patients with higher E-score and adolescent patients with higher I-score than their counter-part subgroup were classified into more compliant group. Orthodontist performed more generous decision about patients' compliance than assistants. The results of this study also suggest that psychological survey in orthodontic field could comprise useful diagnostic criteria.

  • By observing the psychotherapy process expressed by an adolescent with obssessive compulsive disorder, the author suggests that the main problem of OCD is not merely a product of an unconscious process as traditionally thought, but a learned problem solving process as a method surviving the more realistic conflicts, and therefore the treatment should be directed to more or less conscious and realistic issues and conflicts, family therapy should be offered too.

Obesity Prevalence in Pediatric Headaches (소아청소년 두통 환자에서 비만 유병률)

  • Purpose: Obesity and headache are two highly prevalent diseases both in childhood and adolescent. In this study, we assessed the prevalence of obesity in pediatric headaches patients in a single institution in Korea, and differences according to age, sex, headache type, frequency, intensity, and disability. Methods: We retrospectively reviewed the medical records of 340 subjects (6-18 years of age) who visited the Pediatric Headache Clinic of Bucheon St. Mary's Hospital during the period from January 2015 through March 2018. Data on age, sex, height and weight, as well as headache type, frequency, intensity and disability, were collected. Body Mass Index (BMI) percentile was calculated based on the 2017 Korean Children Adolescence Growth Chart. Results: 17.6% of the pediatric headache patients were obese. The prevalence of obesity in male patients was higher than females (Male 23.8% VS Female 11.6%, P=0.002). There were no significant differences in obesity rate according to age, headache type, frequency, intensity, and disability. Conclusion: The prevalence of obesity in the pediatric headache population was 17.6% which is higher than 10.1% in general population (Korea National Health and Nutrition Examination Survey, KNHANES, 2013).

  • Anemia commonly occurs in kidney transplantation (KT) recipients. Many factors such as viral infection, bleeding, erythropoietin deficiency, and immunosuppressants are the causes of chronic anemia in KT recipients. We report 2 cases who developed severe anemia caused by parvovirus B19 infection and were successfully treated with intravenous immunoglobulin in pediatric KT recipients.

Influence of Clinical Characteristics and Restriction Factors on Cooperation for Orthodontic Treatment in Adolescent Orthodontic Patients (청소년 교정환자의 임상적 특성과 통제소재가 교정치료 협조도에 미치는 영향)

  • Cooperation of the patients in orthodontic treatment is an essential element. This study examines the internal and external restriction factors and level of cooperation for orthodontic treatment of adolescent orthodontic treatment patients, and the factors that impart influence on the level of cooperation for orthodontic treatment. 'Form for students' among the indices of internal and external restrictions for orthodontics, which is an adaption of 'Orthodontic Locus of Control Scale' was used as survey tool, and 'Reliability and Stability of the Orthodontic Patient Cooperation Scale' was used as survey tool for the level of cooperation of orthodontic patients. As the result of examination of the effect of internal and external restriction factors on level of cooperation for orthodontic treatment by compensating the general characteristics in the multiple regression analysis, it was found that the level of cooperation for orthodontic treatment is higher for the patients who believe there is strong influence of external restrictions-patents-on the events that determines orthodontic treatment. In order to enhance the level of cooperation for orthodontic treatment of adolescent orthodontic patients, there is a need to assess the relationship between the patient and parents as well as the attitude or psychological state of the patients on orthodontic treatment.

THE PRELIMINARY STUDY ON THE ADOLESCENT PSYCHIATRIC PATIENTS THAT SHOW THE CHRACTERISTICS OF PEER GROUP NEGLECT OR REJECTION (청소년 정신과 환자 중 외톨이 혹은 왕따 특성을 보이는 환자에 대한 예비 연구)

  • This study was a preliminary study on the adolescent psychiatric patients that show the characteristics of peer group neglect or rejection. We divided the adolescent psychiatric patients(n=25) into neglect-reject group(n=15) and non-neglect group(n=10). Each patient answered 8 scales and several questions about general relationships with friends. And, both the score of each 8 scales and answer about qusetions were examined. Lastly, records of 5 cases from neglect-reject group were analyzed. The depression dimension score of SCL-90-R, anxiety dimension score of SCL-9O-R, paranoid dimension score of SCL-90-R, interaction anxiousness scale(IAS) score were significantly different between 2 groups. On the result of case analysis about 5 patients of neglect-reject group, the adolescent psychiatric patients had a tendency to be rejected or neglected in case of having developmental problems(for example, language development or cognitive impairment).

Restless Legs Syndrome in Children and Adolescents with Type 1 Diabetes (제 1형 당뇨병 소아 청소년의 하지불안증후군)

  • Objectives: Restless legs syndrome (RLS) is a common sleep disorder in adults with diabetes. This study investigated the frequency of RLS and clinical correlations in children and adolescents with type 1 diabetes. Methods: This study included 55 consecutive patients (21 males, age $12.6{\pm}3.4years$) with type I diabetes that were regularly treated at the Department of Pediatric Endocrinology. RLS was diagnosed by intensive interviews which also included the Epworth Sleepiness Scale (ESS) and International RLS Rating Scale (IRLSRS). Patients also received neurological examinations and laboratory tests for diabetes, iron metabolism and renal function. Results: Thirteen patients (23.6%, 6 males) were compatible for the diagnostic criteria of RLS. None of the RLS patients showed abnormal findings in neurological evaluations and 7 patients had familial history of RLS. Demographic and laboratory findings were not different between the patients with or without RLS. The RLS group showed significantly increased ESS and IRLSRS scores. Conclusion: RLS was prevalent in children and adolescents with type 1 diabetes. The association between RLS and diabetes-related laboratory findings requires further investigation.

  • Coronavirus disease 2019 (COVID-19) presents as a mild-to-moderate respiratory illness in most children. However, a small proportion of children with COVID-19 develop severe or critical illnesses. Although pediatric clinical trials for the treatment of COVID-19 are sparse, some drugs are available for children and adolescents with severe COVID-19. This review summarizes clinical data focusing on antiviral agents and immunomodulators for use in treating COVID-19. In addition, current recommendations for therapeutics for children and adolescents with COVID-19 are discussed.
