A Study on the Utilization of Complementary and Alternative Medicine on Children with Atopic Dermatitis (아토피 피부염 환아의 보완대체요법 이용 실태)

  • Atopic dermatitis (AD) is prevalent in preschoolers and schoolchildren. The utilization of complementary and alternative medicine (CAM) is increasing in patients with AD as well as the general population. This study is based on the utilization of CAM for children with AD. Subjects were children suffering from AD who participated in the project of 'Seoul Atopy Asthma Free School'. In 6,800, the number of subject available data to final analysis was 4,985 for children with AD. Independent variables were age, monthly income of households, sleep disturbance, and dependent variables were medical treatment (MT), oriental medicine clinic (OMC), home remedy (HR) use experience during the past 12 months. As a result, MT was 76.5%, OMC was 24.6%, and HR was 62.3%, while multiple treatment from OMC and HR was 34.2%, and MT with OMC and HR was 18.8%. And variables influenced on CAM were age, monthly income of households, and sleep disturbance. However, some parents, tended to recognize that home remedy was effective and safe than other treatments. Therefore, it is recommended to counsel with physician about the merits and demerits of CAM.

Prevalence and Associated Factors of Snoring in School-Aged Children (학동기 아동에서 코골이의 유병률과 관련인자)

  • Purpose : We studied the prevalence of snoring and its association with diseases, obesity and environmental factors, as well as sleep disturbance arising from snoring, in school-aged children. Methods : The survey was performed by a special questionnaire on 1,707 children at elementary schools from 1 to 30 July 2001 in Gwangju City. The prevalence of snoring, associated factors, and sleep disturbance were evaluated. Results : 266(16.5%) of the children snored at least once a week. 73(4.3%) of the children snored almost every day. Regarding the associated factors of snoring : There was a statistically significant difference between snoring and nonsnoring groups due to gender, obesity, sinusitis, tonsillar hypertrophy, bronchial asthma and allergic rhinitis. Of them, male gender, tonsillar hypertrophy, and obesity were significant risk factors for development of snoring. Especially, the odds ratio increased to 1.048(CI, 1.004-1.659), 1.748(CI, 1.175-2.599), and 2.266(CI, 1.300-3.950) in children with an obesity index of 20-29%, 30-49%, ${\geq}50%$, respectively. There was a statistically significant association of snoring with decreased sleep duration, sleep talking, and drowsiness during the day as a result of sleep disturbance. Conclusion : 16.5% of school-aged children snored at least once a week. The association of snoring with tonsillar hypertrophy, obesity and gender as risk factors was demonstrated in this study.

A Convergence Study of Factors Affecting Life Satisfaction for Adolescents with Allergic Disease (알레르기 질환이 있는 청소년의 삶의 만족도 영향요인의 융합연구)

  • The aim of this study was to identify factors affecting life satisfaction for adolescents with asthma or atopic dermatitis. Korean Child and Youth Panel Survey (KCYPS) Data in 2016 was used. The data were analyzed by Chi-square test, t-test, one-way ANOVA and stepwise multiple linear regression. In multiple stepwise regression analysis, less depression, higher resilience, higher self-esteem, more affectionate parenting behavior, lower age enhances the life satisfaction of adolescents with allergic disease. Educational program is necessary to improve the life satisfaction of adolescents with asthma or atopic dermatitis which is reflecting the result of this study.

A study on the utilization of complementary and alternative medicine for elementary children (학동기 소아에서 보완대체요법의 이용실태)

  • Purpose: Recently, complementary and alternative medicine (CAM) has been increasingly used in children. Studies have shown that 34% of adults and 11% of children use CAM in the USA and Canada. The purpose of this study was to investigate the prevalence and patterns of CAM use in elementary children in Korea. Methods:From July to August 2007, parents of elementary children completed a questionnaire survey at Gwang-ju. In all, 794 questionnaires were analyzed. Results:Of the 794 respondents, 278 answered that their pupils (35%) had experienced CAM. The following types of CAM therapy were used: herbal medicine, 62.5% dietary supplements, 31.2% vitamins, 30.2% and acupuncture, 11.1%. CAM therapies were used for the following diseases: nutritional deficiency, 33.3% atopic dermatitis, 31.3% arthralgia, 31.3% allergic rhinitis, 28.8% obesity, 26.3% and asthma. The following were the motives to use CAM: prevention of diseases (33.5%), dissatisfaction with modern medicine (21.2%), and complementary therapy to modern medicine (20.5%). People gained information about CAM through neighbors (65%) and mass media (21%). Moreover, 83 parents (30%) were satisfied with CAM because of its effectiveness. Conclusion:Many parents have advocated the use of CAM in their children. However, most of them used CAM without any prescription or adequate knowledge. Further studies are required to determine the efficacy of CAM.

Psychometric Evaluation of a Six Dimension Scale of Nursing Performance and Student Nurse Stress Index Using an Objective Structured Clinical Examination - Modules for Asthma and Type 1 Diabetes (객관구조화 임상시험을 활용한 간호수행능력의 Six Dimension Scale과 간호학생 스트레스 평가지수의 도구 평가-천식 및 1형 당뇨 모듈을 중심으로)

  • Purpose: The study purposes were to describe the process of developing the Korean versions of the Six Dimension Scale of Nursing Performance (Six-D) and Student Nurse Stress Index (SNSI) and psychometric evaluation of the two measurements. Methods: This was a methodology study using a descriptive cross-sectional design with 51 nursing students in 4th year of university. Internal consistency reliability was assessed using Cronbach alphas. Construct validity was determined by exploring correlations among Six-D, SNSI, objective structured clinical examination (OSCE), self-efficacy and grade point average (GPA). Results: Internal consistency reliability of Six-D and SNSI was acceptable with Cronbach's ${\alpha}$ of .95 and .82. Correlation analysis to determine construct validity revealed that Six-D presented positive correlations with OSCE (r=.109~.272) and self-efficacy (r=.005~.161) and negative correlation with GPA (r=-.246~-.394), although all were not statistically significant. SNSI presented all negative correlations with OSCE (r= -.007~-.238), self-efficacy (r=-.246~-.394), and GPA (r=-.092~-.426) and were mostly statistically significant except OSCE. Conclusion: Six-D needs more evidence to confirm validity to predict observed clinical competency and theoretical relationships with self-efficacy and GPA. However, SNSI presented trends of expected relationships with relevant variables. Therefore, further research is recommended in testing validity of Six-D with other student populations.

Association between Pets and Asthma Symptoms in Korean Children - Focusing on Dogs and Cats - (반려동물과 아동의 천식증상과의 관계에 관한 연구 - 반려견 및 반려묘를 중심으로 -)

  • Objectives: The purpose of this study was to assess the association between pet ownership and asthma symptoms in Korean children. Methods: This study used data from the 8th Panel Study on Korean Children in 2015. For the final study, 1598 cases were used. Wheezing as an asthma symptom was self-reported as an outcome variable. Whether children had owned dogs or cats or not was used as an independent variable. Results: A total of 6.8% of children lived with dogs and 2.1% of children lived with cats in their homes. There were 4.0% of children who had asthma symptoms in the past year. When comparing 'children who had owned dogs for more than 12 months' with 'children who had not owned dogs', the adjusted odds ratio (OR) for 'asthma symptoms' were 1.20 (95% confidence interval (CI), 1.10-1.30) among male children and 2.19 (95% CI, 1.94-2.46) among female children. When comparing 'children who had owned cats for more than 12 months' with 'children who had not owned cats', the adjusted ORs for 'asthma symptoms' were 38.94 (95% CI, 35.55-42.65) among male children and 5.32 (95% CI, 4.60-6.16) among female children. When comparing 'children who had owned both dogs and cats for more than 12 months' with 'children who had not owned at all', the adjusted OR for 'asthma symptoms' was 13.22 (95% CI, 11.93-14.65), and when comparing 'children who had owned cats only' with 'children who had not owned at all', the adjusted OR for 'asthma symptoms' was 9.25 (95% CI, 8.50-10.08). Conclusions: As a result, children's asthma symptoms were higher in the case of cats than with dogs, and in the case of cats, boys were more at risk than girls. In the analysis for the risk of asthma symptoms when a dog and a cat are raised at the same time, it was statistically confirmed that the children in the home were more at risk than with single animal ownership. The results of this study can be considered to be of great importance in policy-making and for the use of educational materials in the field of environmental health.

  • 미국 캘리포니아 주의 건강문제로는 흡연, HIV/AIDS, 천식, 정신질환, 아동비만, 유방암, API, 건강한 생활양식 형성, 인종간의 건강격차 등을 들 수 있다. 이러한 건강문제를 해결하기 위해 캘리포니아주에서 진행되는 지역사회 중재전략은 지역사회 단체와의 협력이라는 특성을 지니고 있으며, 도시지역의 노인이나 다양한 인종들을 대상으로 하는 프로그램들의 경우는 교회를 통한 접근방법이 빈번하게 활용된다. 중재 프로그램들은 지역사회 조직모형과 역량강화의 개념을 활용하며, 문화적으로 다른 배경을 가진 인구집단을 대상으로는 이들의 요구에 맞는 프로그램을 진행하는 형태로 기존의 프로그램을 채택하는 것이 아닌 이들의 다양한 요구에 맞도록 수정된 프로그램을 제공하고 있다. 개인 간 의사소통을 활용하여 사회적 관계망의 활성화를 도모하며, 일반인이 건강지도자가 되는 교육모형도 활용하고 있다. 한국교민사회 자문위원회(Korean Community Advisory Board, KCAB)는 CDCPRC(CFCH)의 기금으로 진행되는 연구 프로젝트를 수행할 목적으로 1993년 설립되었다. 장기적인 지역사회 건강증진을 위하여 전문가와 일반인 지도자들이 지역사회에서 협력하는 방법을 활용하며 요구도 진단, 수행 평가, 건강옹호와 같은 다양한 사업을 진행한다. 회원들은 핵심 회원과 일반회원으로 구분되고 이들은 사업의 수행과 관련된 의사결정에 도움을 주게 된다. 한국교민사회 자문위원회가 수행한 프로그램은 'Health is Strength', 'Virtual Village Program'이 있으며, 현재는 'Quitting is Winning'이라는 미국거주 한국 남성을 대상으로 하는 인터넷 금연 프로그램을 수행하고 있다. 또한 캘리포니아 주 거주 한국인의 건강상태에 대한 조사를 진행하고 그 결과를 보고하는 프로젝트를 진행하고 있다. 이들이 하는 주요 업무는 지역에서 발생되는 건강문제에 대한 우선순위를 파악할 수 있도록 하여 한국교민들의 건강요구를 파악하고 사업의 우선순위를 설정하는데 도움을 주고 있다. 센터에서 진행되는 한국교민 대상 연구나 사업에 가장 효과적일 수 있는 방법에 대한 지침이나 정보를 제공한다. 또한 문화적으로 민감한 사항이나 적절성 여부에 대한 확인을 하며 연구결과를 보고하고 확산하는데 도움을 주고 있다. 캘리포니아 주는 다양한 인종들이 거주하는 지역이다. 따라서 문화적 민감성이나 문화적 대응전략, 문화적 적절성은 건강증진사업을 수행할 경우에 매우 중요한 이슈가 된다. 또한 이들은 소수민족인 경우가 많아 언어장벽으로 인하여 의료서비스 이용에 제한을 받게 된다. 따라서 보건의료전문가들은 이러한 문화적 요소들을 능숙하게 다루는 기술이 필요한데, 이러한 기술이란 문화가 다른 사람들 간에는 갈등이 존재한다는 점을 인식하고, 건강관련 믿음과 건강행동에 영향을 미치는 사회문화적 요인들을 파악하고, 대안적 해결책을 제시하며, 결과를 평가할 수 있는 능력을 의미한다. 이러한 영역에서 health care interpreter는 소수인종의 보건의료 서비스의 원활한 이용을 위하여 매우 활발한 역할을 수행하고 있다.
