The Effect of a Participatory Rehabilitation Program on the Physical Activity of Adults with Developmental Disability (참여형 재활프로그램이 의료취약계층 성인발달장애인의 신체활동 능력에 미치는 영향)

  • This study was conducted to investigate the effects of a participatory rehabilitation program on sit-rise and rise-to-walk test performances, and perception and motor skills in adults with medically vulnerable individuals and, adults with developmental disabilities in particular. Seventeen adults with developmental disabilities participated in a participatory rehabilitation program using resistance bands and exercise balls, for 60 minutes once weekly over 13 weeks. Their performances were measured before and immediately after the intervention, and 12 weeks after. The findings were as follows. In the sit-rise test, the number of times rising from sitting posture increased after the intervention versus before, but the difference was not statistically significant. In the rise-to-walk test, the performance showed statistically significant difference over time, and the post-hoc test showed a significant effect after the intervention versus before. There was no significant difference in perception and motor skills. In sum, the participatory rehabilitation program positively influenced dynamic balancing related to functional activities but had no significant effect on perception and motor skills, which is related to motor control and motor learning. It is suggested that to increase the participation in community activities, reduce fall risk, and improve dynamic balancing abilities in adults with developmental disabilities, participatory rehabilitation programs should be utilized to promote the physical wellbeing.

Effects of Song Discussion on Depression and Rehabilitation Motivation in Stroke Patients (노래 가사 토의가 뇌졸중 환자의 우울 및 재활동기에 미치는 효과)

  • This study investigated the effects of song discussion on depression and rehabilitation motivation in stroke patients. Older adults with chronic stroke participated in this study: nine for the experimental group and eight for the control group. The experimental group was divided into three subgroups and participated in 12 sessions over 6 weeks. Target lyrics were selected by the investigator among popular songs from the participants' young adulthood. The song-based discussion was facilitated to address issues targeted at supportive, insight-focused, or reconstructive stage. The control group was provided with delayed intervention. At pre and posttest, the short form of Geriatric Depression Scale and the Rehabilitation Motivation Scale were measured. The experimental group showed significantly decreased depression and significantly increased rehabilitation motivation (p < .01), while the control group showed no significant changes. Positive changes were also observed in all subcategories of rehabilitation motivation in the experimental group, particularly in significantly increased task-oriented motivation and decreased amotivation. This study suggests that song discussion will be effectively applied in rehabilitative settings to address psychological issues of older adults with stroke.

Research trends in rehabilitation program for disabled applying virtual reality technology in Korea (가상현실기술을 적용한 국내 장애인재활프로그램 연구동향)

  • The purpose of this study was to investigate the trends in research on the effects of virtual reality technology applied to rehabilitation for disabled. I have collected 60 papers published in Korea(2000-2014), and analyzed it by the P.I.C.O. method. As a result, adults and the elderly are most common subjects, and projection type has been used frequently in researches. It were included mostly balance, upper extremity function, and gait that the dependent variables represents the intervention effects. I interpreted it as the ICF factors. So, research topics were the most common mobility, in contrast to educational, vocational, recreational area as dealing with the participation area is very small. In the future, we should focused on social aspects for useful intervention in real life of people with disabilities based on virtual reality technology, and virtual reality technology should be easy to apply in terms of cost and accessibility.

Development of a Mobile Game and Wearable Device for Upper Limb Rehabilitation after Brain Injury (뇌손상 환자의 상지 재활을 위한 웨어러블 장치와 모바일 게임 개발)

  • Conventional upper extremity rehabilitation paradigm after brain injury has several shortcomings that is monotonous, simple, and repetitive in exercises over a long period of time, thereby causing training efficiency to decline as a consequence of low interest and participation. To resolve this issue, this paper proposes a new rehabilitative program integrating a wearable device integrated with EMG and motion sensor and a mobile game for the upper limbs' rehabilitative training. The developed wearable device is manufactured in the form of band, making it easy to wear. The mobile game is designed to enable rehabilitative training through games reflective of flexion, extension, abduction, and adduction identified by motion sensors along with grasp motion recognized by EMG signals measured from the wearable device. It also provides a tailored rehabilitative environment suitable for individual patients based on difficulty adjustments. As a consequence of applying the developed program to 14 brain injury in need of the upper limb rehabilitation and taking surveys on the utility of the developed rehabilitative program, the responses indicated that the developed rehabilitative program is far much more interesting and fun than the conventional rehabilitative program, further to the desire of those surveyed to reuse the developed program in the future.

Effects of Visiting Prehabilitation Program against Functional Decline in the Frail Elderly: A Prospective Randomized Community Trial (허약노인을 위한 방문재활 프로그램의 장애발생예방 효과에 대한 연구)

  • This study is aimed to evaluate the effects of community-based prehabilitation program developed to prevent functional decline in the frail elderly and to provide a basis to practically operate this program in the public health care service. From March to August 2009, 110 frail elderly people were recruited among the registered participants of the home visit program in Korea to perform a prospective randomized community trial. We randomly assigned these people into two groups. One group (n=50) participated in the visiting prehabilitation program for 3 months focusing on improving their muscle strength of upper and lower limbs, walking ability, and balancing. The other group (n=60) underwent our visiting fall prevention program for control. To assess the effectiveness of prehabilitation program, physical functioning (PF) and short physical performance battery (SPPB) were measured for the primary outcomes and also some other indicators: exercise performance, nutritional status, emotional functioning, experience of admission, and events of fall. As a result, significant improvements of geriatric functional status were noticed among the participants. After 3 months, PF increased by 1.3 ± 3.8 points in prehabilitation group and decreased by 1.1 ± 5.4 points in controls (p=.020). SPPB improved by 2.4 ± 2.0 points in prehabilitation group and increased only 0.3 ± 1.5 points in controls (p<.001). Significant effects were also shown in their exercise performance tests and emotional status, the number of multiple falls, and the experience of functional decline after the fall (p .002-.038). Visiting prehabilitation program is safe and effective program for frail older adults. Thus, it is strongly recommended to universally adopt this program to prevent functional decline in the frail elderly.

Effects of the Need for Rehabilitation Services on the Daily Life Satisfaction of Returned-to-Work and Reemployed Injured Workers : Mediating Effects Self-Efficacy (원직장복귀 및 재취업 산재근로자의 재활서비스 욕구가 일상생활만족도에 미치는 영향 : 자기효능감의 매개효과)

  • This study aims at creating a daily life satisfaction improvement plan by using motivation for seeking need for rehabilitation services and self-efficacy in individuals who returned-to-work and reemployed injured workers. The subjects of this study were 2,642 returnees and 3,742 reemployed individuals who participated in the first to fifth year survey of the first cohort of the Panel Survey of Workers' Compensation Insurance. The analysis checked the mediating effect through the multiple regression analysis and bootstrapping. The result indicated the need for rehabilitation services had negative impact on self-efficacy and daily life satisfaction whereas self-efficacy had a positive effect on daily life satisfaction and played a mediating role between the need for rehabilitation services and daily life satisfaction. Based on these results, a need for the advancement of social psychological rehabilitation program and establishing an integrative rehabilitation process were identified.

A Meta-Analysis on Improvement in Locomotor Skills of Children with Disabilities by Physical Activity Programs (신체활동 프로그램 참여가 장애아동의 이동운동능력에 미치는 효과: 메타분석)

  • The purpose of this study was to identify improvement in locomotor skills by physical activity programs. Method of this study indicates that the current literature (2004-2015) were reviewed and the data from 24 studies with 518 disabled children were analyzed by using CMA3 (Comprehensive Meta-Analysis ver.3) program. Analyzing the data of the primary studies included gender, age, type of disabilities, duration of the physical activity program intervention(weeks, session per week, minutes per session), run, gallop, hop, leap, horizontal jump, and slide. For sensitivity analysis, publication bias and outlier were reviewed. Results of analysis indicates that the overall effect size of improvement in locomotor skills by physical activity programs was 1.143. There were large effect size in categorical analyses. Autistic spectrum among type of disabilities was 1.697 and run among 6 of locomotor skills was 1.019. 8~10 aged was 0.920 and the intervention of 100~120minutes(1.261)per session, 3sessions(1.078) per week, 16~20(1.587)weeks was found to be more larger than the others. In conclusion, improvement in locomotor skills by program participation showed that treated group was 37% more effective than control group.

Effect of Activity Constraint due to Disability on Daily Life Satisfaction among the Disabled : Focusing on the Mediating Effect of Subjective Health Status (장애인의 장애로 인한 활동제약이 일상생활 만족도에 미치는 영향 : 주관적 건강상태의 매개효과를 중심으로)

  • The purpose of this study was to examine the effects of the activity constraints due to disability on subjective health status and daily life satisfaction among disabled, and mediating effects of subjective health status on the relationship between activity constraints due to disability and daily life satisfaction. For this purpose, the response data of 3,809 people who participated in the 5th year of the 2nd Wave of the Panel Survey of Employment for the Disabled (PSED) were used and analyzed. As a result, all sub-factors except for the degree of difficulties in hearing even when using hearing aids among activity constraints due to disability has a statistically significant negative effect on subjective health status, and all sub-factors has a significant negative effect on daily life satisfaction. And, subjective health status has a significant partial mediating effect on the relationship between all sub-factors except for the degree of difficulties in hearing even when using hearing aids among activity constraints due to disability and daily life satisfaction. Therefore, the level of activity constraint for the disabled should be reduced by providing systematic and differentiated customized rehabilitation programs that take into account the disability characteristics of the disabled, thereby improving the health level and social independence ability of the disabled. Furthermore, in order to improve the daily life satisfaction of the disabled, it is necessary to provide policy support such as the establishment of a community-based integrated support system for the disabled by linked rehabilitation program and health promotion program.

Measuring Range of Motion and Muscle Activation of Flower Arrangement Tasks and Application for Improving Upper Limb Function (꽃꽂이 작업의 관절가동범위와 근육활성도 측정 및 상지기능 향상을 위한 적용사례)

  • The objectives of this study were to measure the range of motion for joints and muscle activation of upper limb for flower arrangement tasks for physical rehabilitation and to test horticultural therapy programs using flower arrangement tasks for improving upper limb function of the patients with stroke. Major flower arrangement tasks were classified with eight tasks (cutting 1, thick stem; cutting 2, thin stem; fixing 1, long stem; fixing 2, short stem; rolling a leaf; bending 1, thick stem; bending 2, thin stem; and winding, using a wire) based on the occupational analysis. When eight male university students (mean age $24.1{\pm}2.5$ years) conducted the eight flower arrangement tasks, range of motion for joints and muscle activation of upper limb were measured by a 3D motion analyzer and electromyography, respectively. Based on the results of the range of motion and muscle activation of upper limb, horticultural therapy programs using flower arrangement tasks (total 33 sessions) for improving upper limb function of the patients with stroke was conducted at a rehabilitation hospital, Seoul, South Korea and then the range of motion, grip strength, and upper limb function of the patients were tested. Among the eight flower arrangement tasks, cutting 1, winding, and bending 1 induced the highest value for the range of motion in joints of shoulder, elbow, and wrist, respectively (P < 0.001). In terms of muscle activation, eight flower arrangement tasks performed in this study showed various patterns of muscle activation and several muscles were simultaneously used for each task (P < 0.001). In addition, thickness and length of the materials used in a task had a prominent effect on the range of motion for joints and muscle activation of upper limb (P < 0.001). The stroke patients had positive effects for their range of motion of upper limb (shoulder, forearm, and elbow), grip strength, and overall upper limb function through the horticultural therapy program. Thus, this study suggested that flower arrangement tasks would be a potential horticultural activity for physical rehabilitation program. It would be interesting to apply a customized horticultural therapy program using flower arrangement tasks according to the symptoms of patient for physical rehabilitation.

The Effects of Multimodal Cognitive Intervention Focused on Instrumental Activities of Daily Living(IADL) for the elderly with High-risk of Dementia : a Pilot Study (도구적 일상생활에 초점을 둔 복합인지중재 프로그램이 치매고위험군 노인에게 미치는 영향 : 예비연구)

  • The purpose of this study is to investigate the effect of the multimodal cognitive intervention focusing on instrumental daily life on the cognitive function, depression and quality of life of the elderly with high-risk of dementia. This study was conducted on 24 elderly people with high-risk of dementia who participated in cognitive rehabilitation program from March to June, 2018 in Chungbuk A region. The intervention was applied to cognitive training and creative activities related to instrumental daily life. MMSE-DS, Subjective Memory Complaints Questionnaire, Short Geriatric Depression Scale-Korean version and Geriatric quality of life - Dementia were performed before and after the intervention. We confirmed that the subjects showed significant improvement in Subjective Memory Complaints and Quality of Life, but showed no significant changes in cognitive function and depression after the intervention program. Through this study, it was confirmed that this program which can affect the real life of the elderly can be usefully applied in the community. In the future, it will be necessary to develop a program that utilizes more diverse instrumental activities of daily living.
