Inpatient Outcomes by Nurse Staffing Grade in Korea (간호관리료차등제 등급별 입원 환자의 건강 결과)

  • Objectives: This study investigates the relationship between nurse staffing levels and differences in patient outcomes in terms of average length of stay, in-hospital mortality rate and 30-day death rate in order to evaluate the effectiveness of a policy that differentiates fees for inpatients on the basis of nurse-to-bed ratios. Methods: We obtained information on inpatients from health insurance claims data published by the Health Insurance Review and Assessment Service(HIRA) in 2008, organizational factors(type of hospital, ownership) from the records of the hospital report system in 2008, and nurse staffing levels, which were graded on a scale of 1 to 7, from data compiled between December 15, 2007, and September 20, 2008. The data were segregated according to type of hospital and quarter and finally 3,517 records of 1,182 hospitals were analyzed using multi-level analysis. Results: The average length of stay in grade 1~6 hospitals was lower than that in grade 7 ones, but the difference was much below one day. No significant difference was found among different grades in tertiary hospitals. Further, variations in staffing levels did not result in any significant difference in the in-hospital mortality rate and 30-day death rate. Conclusions: High nurse staffing levels did not result in better patient outcomes compared with low staffing levels. We therefore recommend modifying the above nurse staffing policy so as to make it more effective in improving patient outcomes.

전자금융 침해사고 예방 및 대응 강화 방안

  • 편리한 금융거래 수단으로써 인터넷뱅킹을 포함한 전자금융 서비스가 생활화 되었으며 그 중요성 또한 갈수록 증가하고 있다. 이에 대한 부작용으로서 사용자의 실수나 금융기관, 쇼핑몰, 포털 등의 해킹을 통한 전자금융 접근매체의 유출, 비정상적인 지불결제나 인터넷뱅킹 이체 사고 등 침해사고 또한 함께 증가하고 있다. 금융권은 금융감독원을 중심으로 전자금융 종합보안 대책 수립(2005년) 및 전자금융거래법 시행(2007년) 등을 통해 고객 PC의 해킹방지를 위한 다양한 보안프로그램 제공 의무화, 보안등급에 따른 이체한도 차등화, 금융권 통합 OTP 인증체계 구축 등 전자금융 침해사고 예방을 위한 적극적인 노력을 기울여오고 있으나, 최근 들어 피싱/파밍 등 신종 사이버사기 기법이나 해외의 전문 해커에 의해 개발된 고도의 지능화된 해킹툴이 사용되어 보안프로그램을 무력화시킨 후 고객정보를 유출해가거나 일반 포털사이트, 웹하드, 웹메일 등의 해킹을 통해 인터넷 사이트에 등록된 고객의 인터넷뱅킹 접근 매체를 유출하여 인터넷뱅킹 침해 사고를 일으키는 등의 신종 침해사고를 완벽히 차단하지는 못하고 있어, 더욱 강력한 전자금융 침해사고 예방 통제 방안의 수립과 함께 침해사고 발생 시 원인 파악 및 범인 검거를 위한 역추적 시스템의 구축 등 기존 보안체계를 대폭 강화할 필요성이 발생하고 있다. 본 연구에서는 시중 은행의 인터넷뱅킹 침해사고 발생 현황 조사를 중심으로 최근 발생한 전자금융 침해사고의 추이분석, 침해사고 주요 원인과 기존 대응 체계의 현황, 한계점 등을 파악하였다. 그리고 전자금융 침해사고의 효과적인 예방 및 대응 강화 방안으로서 사용자 관점에서 공인인증서를 중심으로 한 전자금융 접근매체의 관리 강화 방안을 제안하였으며, 전자금융 서비스를 제공하는 금융 기관 관점에서 효과적인 전자금융거래 로깅 및 역추적 시스템의 구축 및 전체 금융기관과 감독기관 간의 유기적인 공조를 기반으로 한 침해사고 공동 대응체계의 구축 및 운영을 위한 시스템의 구성 방법, 운영 프로세스, 관련 법률의 검토 및 대응 방법 등을 제안하였다.

Consumer Perception, Purchase Behavior and Demand on Ham and Sausage Products (햄.소시지제품에 대한 소비자 의식 및 구매 실태)

  • Consumer survey(n=1,343) was carried out to investigate the consumer's perception, purchase behavior and demand on the processed meat products. Most consumers purchased the ham and sausage products once a month (40.73%). Consumers(47.19%) didn’t distinguish between ham and sausage products due to the similarity of meat contents and additives in products. Consumers(72.75%) considered the product label when they purchased whereas 27.25% of consumers were not interested in the product label. Consumers(47.65%) were not satisfied with ham and sausage products because there were too much additives other than meat, there were many similar processed products in kinds and appearance(27.70%) and they could hardly detect meat taste(11.62%). Consumers demanded to produce the ham and sausage products which contain high meat contents(64.04%). Consumers were favored with establishment of the price differential system depending on the quality based on meat contents in the product(83.54%). In conclusion, the establishment of the price differential system depending on quality as well as the quality grading system for processed meat products is needed not only to increase the processed meat market but also increase the pork consumption especially for non-preferred pork portions.

Tentative Suitability Criteria for the Consolidation of Cultivated Upland in Korea (밭 경지정리(耕地整理) 적합지역(適合地域) 선정기준(選定基準) 시안(試案))

  • Tentative consolidation criteria, a kind of land suitability classification, was established for cultivated upland soils. The criteria were composed of systematically quantified individual soil properties such as physiography(relief), slope, available soil depth etc., and local features such as soil complexity, mapping unit separation, width/length ratio etc. The criteria was applied to upland soils in granite area, and sedimentary rock area. It was clear that the comparison of local features among candidated areas, and classification of suitability classes for land consolidation of upland soils was satisfactory for selection. The status of the areas classified to "Well suitable" were distributed on alluvial plains or less dissected rollings with lower complexity of soils occurred and extended acreage in each mapping unit. On the other hand, the areas classified as "Unsuitable" were distributed on the elongated narrow valleys with quite divided mapping units by paddy fields on the valley bottom.

Environmental Assessment of the Shihwa Lake by using the Benthic Pollution Index (저서오염지수(BPI)를 이용한 시화호 환경평가)

  • In order to assess the ecological changes induced by organic pollutants of the Shihwa Lake, BPI (Benthic Pollution Index) based on the benthic faunal community was employed. It was modified from Infaunal Trophic Index (ITI), and recommended as a pollution detecting method for the environmental assessment. The BPI values were calculated from the benthos data, which were collected for three terms: in 1980, before the Shihwa Lake was built up; in 1994-1997, which the Shihwa Lake was completely isolated from the outer seawater; in 1997-1999, after inflow of the outer seawater. Since the Shihwa Dike was constructed in February 1994, the pollution intensity of the lake had been increased from the narrow and inner part of the former Gyeonggi Bay and spread fast along the coast line of the Shihwa Lake. Then, in 1996 it showed the very high BPI levels all around the Lake. This serious polluted condition had been lasted till 1997, when the inflow of the seawater was begun. In 1998, from the nearest part of the Shihwa Gate, the BPI levels gradually became low in most area of the Lake, except its inner and narrow part. These greatly lowered BPI levels mean that the seawater inflow could be assumed to affect positively in the lake. Furthermore, BPI gave the same results from the other environmental assessment based on the abundance and the species richness of macrobenthic community. It shows that BPI could be useful as an effective method to assess the marine environment and evaluate the status of environmental conditions.

A Comparative Study of Biotope Mapping between Korea and Germany (한국과 독일의 비오톱지도화에 대한 비교)

  • The purpose of this study is to derive political implications and to look into the establishment of new directions for the biotope mapping across the Korean territory so that such biotope mapping process could be used as a means of actual plan for natural environment preservation through the comparative study of biotope mapping methodology with that of the biotope mapping pioneer-Germany. As for research methodology, this study collected literature and data related to the biotope mapping of Korea and Germany and examined the general characteristics and inclinations about biotope mapping of both country, at a level of the national territory, federal government, provincial government, city and settlement areas. As a result of the examination, first, it was found that Germany drew up a landscape eco-type map and ecological landscape grade map throughout the nation in order to preserve and manage effectively by differential landscape eco-type unit. In contrast, Korea drew up a map on which Degree of Green Naturality and Ecological Nature Status are reflected for natural environment preservation at a nationwide unit. Secondly, the biotope mapping of German was worked centering on natural areas and their corresponding areas at German provincial government level and it drew up a Red-list by each province, while Korea has yet to carry out biotope mapping at a provincial level corresponding to Germany's. Thirdly, the biotope mapping on German cities and settlement areas includes not only big city areas but also medium & small cities and rural areas whereas Korean biotope mapping is conducted mainly centering on urban areas. In conclusion, this study suggests that the biotope mapping including natural areas and anti-natural area, not limited to city areas should be revitalized in order to be a real means of the all territory's balanced natural environment and biotope preservation & management. In addition, for this purpose, this study suggests that research on biotope pattern catalog and biotope red-list applicable to all territory should be preceded.

Evolution of Aviation Safety Regulations to cope with the concept of data-driven rulemaking - Safety Management System & Fatigue Risk Management System

  • Article 37 of the International Convention on Civil Aviation requires that rules should be adopted to keep in compliance with international standards and recommended practices established by ICAO. As SARPs are revised annually, each ICAO Member State needs to reflect the new content in its national aviation Acts in a timely manner. In recent years, data-driven international standards have been developed because of the important roles of aviation safety data and information-based legislation in accident prevention based on human factors. The Safety Management System and crew Fatigue Risk Management Systems were reviewed as examples of the result of data-driven rulemaking. The safety management system was adopted in 2013 with the introduction of Annex 19 and Chapter 5 of the relevant manual describes safety data collection and analysis systems. Through analysis of safety data and information, decision makers can make informed data-driven decisions. The Republic of Korea introduced Safety Management System in accordance with Article 58 of the Aviation Safety Act for all airlines, maintenance companies, and airport corporations. To support the SMS, both mandatory reporting and voluntary safety reporting systems need to be in place. Up until now, the standard of administrative penal dispensation for violations of the safety management system has been very weak. Various regulations have been developed and implemented in the United States and Europe for the proper legislation of the safety management system. In the wake of the crash of the Colgan aircraft, the US Aviation Safety Committee recommended the US Federal Aviation Administration to establish a system that can identify and manage pilot fatigue hazards. In 2010, a notice of proposed rulemaking was issued by the Federal Aviation Administration and in 2011, the final rule was passed. The legislation was applied to help differentiate risk based on flight according to factors such as the pilot's duty starting time, the availability of the auxiliary crew, and the class of the rest facility. Numerous amounts data and information were analyzed during the rulemaking process, and reflected in the resultant regulations. A cost-benefit analysis, based on the data of the previous 10 year period, was conducted before the final legislation was reached and it was concluded that the cost benefits are positive. The Republic of Korea also currently has a clause on aviation safety legislation related to crew fatigue risk, where an airline can choose either to conform to the traditional flight time limitation standard or fatigue risk management system. In the United States, specifically for the purpose of data-driven rulemaking, the Airline Rulemaking Committee was formed, and operates in this capacity. Considering the advantageous results of the ARC in the US, and the D4S in Europe, this is a system that should definitely be introduced in Korea as well. A cost-benefit analysis is necessary, and can serve to strengthen the resulting legislation. In order to improve the effectiveness of data-based legislation, it is necessary to have reinforcement of experts and through them prepare a more detailed checklist of relevant variables.
