A comparative study was made about health resources, medical care service statistics and public health service statistics by health subcenters at Jangdong and Jangpyung townships, Jangheung County, Chollanamdo before and after the unification of two health subcenter to improve their function. 1. While two general physicians, one dentist, 4 nurse aids arid one oral hygienist were working at two health subcenters with simple facility with examination room and public health office in 1997 prior to the unification, in 1999 after the unification of two health subcenters 14 staff including a specialist physician, a general physician, a dentist, a herb hygienist, a radiology technician and a physical therapist were working in the new health subcenters equipped with appropriate facilities in two storey building. 2. In 1997 before the unification the yearly total income of two health subcenters was 78,815 thousand won(about 14,000 won per capita) and the amount was 140,376 thousand won(about 25,000 won per capita) in 1999 after the unification. And the income was used for operation of health subcenters excluding personnel expense. 3. While 90.5% of visitors to the health subcenters came for general medical care, and 91.6% came for the revisit before the unification, after the unification 71.2% came for general medical care, 10.8% for dental care, 16.5% for oriental physician's care, 29.7% for the first visit and 70.3% for revisit. Most common problem cared for was musculoskeletal disorder like arthralgia. Average treatment cost per person per month was 9,363 won before the unification and 8,309 won after the unification. 4. Through the comparison of execution rate of public health services before and after the unification. the practice rate of most health service among target population including visiting service for chronic illness, maternal and child health service and immunization service increased after the unification. The practice rate of tuberculosis control service, hypertension control and diabetes management was a little decreased. In conclusion, continuous effort to satisfy all persons in two townships and evaluation are necessary to coincide with the spirit of unification of two health subcenters.

A u-healthcare remote diagnosis system is proposed for chronic disease and medical vulnerable groups check the health systematically and support for the most optimal environment to improve the quality of life. The u-healthcare remote diagnosis system using wireless measure the thoracic sound in the chest. And this is demonstrated that the system using radio frequency is not be affected by the electromagnetic wave with the use of an experiment and by confirming that this u-healthcare remote diagnosis system can not affect the doctors and the patients.

본 논문에서는 환자가 실내 측위 기술과 스마트폰을 이용하여 대형 병원의 내부에서 진료 동선을 스스로 찾아갈 수 있도록 하는데 도움을 주는 네비게이션 모델을 제안한다. 정보기술의 비약적인 발전과 함께 실내 측위 기술을 결합한 스마트폰의 위치기반 어플리케이션들이 각광 받게 되었다. 또한, 실내 측위와 관련된 응용 서비스도 새로이 창출되어 점차 이에 대한 관심과 그 정확성을 높이기 위한 연구들이 활발하게 이루어지고 있다. 기존의 실내측위 기법들 중 가장 보편적인 것은 Wi-Fi 신호를 이용 하는 삼각측량 기법으로 초기 구축비용이 저렴하며 서비스 제공 가능 범위가 넓어 본 논문에서 다루고자 하는 장소인 병원의 특성에 알맞다. 따라서 본 모델은 Wi-Fi를 이용하여 사용자의 정확한 위치를 추정하고 진료 프로세스에 따라 경로를 안내를 해주는 네비게이션 서비스를 제공한다. 이를 통하여 환자에게는 진료를 위한 효율적인 동선을 제공함과 동시에 대형 병원에서는 인적, 물적 낭비를 줄이는데 도움이 되고자 한다.

컴퓨터 보급의 급속한 발전과 멀티미디어의 등장으로 기존의 텍스트와 이미지, 음성, 오디오, 동영상 등의 정보를 디지털화하고, 컴퓨터를 이용하여 저장, 처리, 전송하게 되면서 의료 분야에서도 상당한 업무의 변화를 요구하게 되었다. 의료 분야에서의 이러한 급격한 개방과 더불어 초고속 정보 통신의 발달은 원격진료라는 또 다른 요구를 대두시키고 있다. 이를 위해서는, 멀티미디어 기술, 대용량의 정보를 저장하는 데이터베이스 기술, 초속의 광 대역 기술 등을 통합하여 종합적인 멀티미디어 의료 정보 시스템을 구축하는 것이 시급하다. 이러한 이유들로 본 논문에서는 병원/의원의 의료진들로 하여금 의료영상이나 자료를 상호 전송하여 환자의 진료 또는 검진결과를 확인하고 전문가의 조언 등을 구하는 원격 진료용 의료영상 시스템의 요구사항을 분석, 설계하고 구현하였다. 본 시스템은 클라이언트/서버 구조로써 영상 획득 및 출력, 의료영상 국제 표준 포맷인 DICOM 포맷으로의 영상 저장, MCA(Multi Channel Analyzer), ROI(Region Of Interest) 등의 영상 분석, 필터링 및 영상 확대/축소/회전 등의 각종 영상 처리의 주요 기능을 갖으며, 사용자가 편리하고 쉽게 사용할 수 있도록 아이콘(icon) 중심의 직관적인 인터페이스를 갖는다.

건강과 복지에 대한 사람들의 관심이 증대함에 따라 고령층 인구를 중심으로 병원에서 건강 점검 및 진료가 이루어졌던 기존의 Health산업 패러다임에서 연령층에 관계없이 언제, 어디서나 진료를 받을 수 있는 u-Health산업 패러다임으로 변화하고 있다. 이러한 흐름에 발맞추어 유비쿼터스 컴퓨팅 환경에서 혈압, 맥박, 심전도 등과 같은 의료정보뿐만 아니라 일상생활에서 획득한 정보를 통해 건강증진, 질병예방, 진료를 가능하게 하는 u-LifeCare 의료체계가 필요하다. u-LifeCare 를 지원하는 미들웨어는 유비쿼터스 환경에서 필수적인 기술 요소인 센서 네트워크 미들웨어와 상황인지 미들웨어를 통합하는 미들웨어이며, 라이프케어의 특성을 고려한 '초경량(LightWeight)의, 확장성(Scalability)있는, 보안성(Security)을 갖춘' 미들웨어이다.

진료 기록 문서(CDA)가 의사들에 의해 작성되기 때문에 많은 전문용어, 약어, 숫자, 기호 등을 포함하고 있다. 본 논문에서는 이러한 특성을 고려하여 문서 내에서 여러 의미로 해석될 수 있는 약어, 중의어 등의 단어 모호성을 해소하고자 의미적 등가 부류를 이용하여 모호성을 해소하였다. 특히 의료문서가 많은 비율의 숫자, 기호를 사용하고 있고 문서 내에서 많은 의미적 유의성을 포함하고 있기 때문에 이들을 불용어로 처리하지 않고 의미적 등가 부류에 포함시킴으로써 진료문서 특성을 반영하였다.

최근 국내의 모든 병원에서 PACS(Picture Atchiving and Communication System) 도입에 관한 관심을 보이고 있다. PACS가 구축이 되면 병원 내 모든 진료 과에서 디지털 데이터의 전송으로 정보를 공유할 수 있고, 진료가 자동화 되는 장점이 있다. 하지만, 환자가 다른 병원으로 이송될 경우 과거 진료 내역을 다른 병원으로 함께 전송하여야 되는데, 다른 병원의 시스템과 연계할 방법이 현재로는 존재하지 않는다. HIES 시스템은 의료 데이터 전송의 표준문서로 XML(eXtensible Markup Language)을 제안하고 있다. XML은 문서를 정의하는 메타 마크업(meta-markup) 언어로써 DICOM 프로토콜을 통하여 산출된 의료 데이터를 표현하기에 적당하다. 또한 병원 간 이질 데이터베이스 시스템 통합을 위하여 일관된 스키마 정보를 유지하는 정보 공유 관리자를 설계, 구현하였다.

Depressive disorder is one of well-known major public health problems in Korea. The socioeconomic burden of depression is growing bigger and bigger. Therefore, the Korean government decided to establish the clinical research center for depression for better care of patients with depression. One of the major task of the center is the development of Korean clinical practice guideline for depressive disorders. This guideline should reflect the clinical situation in Korea and meet Korean physicians' needs. In addition, it should be feasible and easy to use. We reviewed the present status of the guideline development in Korea and other countries, and introduces the scope, method, and goals of Korean clinical practice guideline for depression.

현재 핸드폰과 PDA(Personal Digital Assitants) 등과 같은 모바일PC에서는 작은 화면과 처리속도 및 전송속도등을 이용한 WAP을 통해 인터넷 서비스를 제공하고자 하는 많은 연구가 진행되고 있고 일부는 실용화되고 있다. 그러나 빠른 전송속도를 자랑하는 제 3세대 이동통신인 IMT-2000이 시장에 나왔음에도 불구하고 산업 기반에 기여할 만한 어플리케이션이 많지 않은 것이 현실이다. 이에 반에 휴대용 정보 단말기인 PDA는 CIS, 전화번호부 등과 같이 스케일이 큰 프로그램이 사용되고 있다. 따라서 본 논문에서는 PDA와 핸드폰 등 다양한 모바일 환경에서 신속하고 효율적으로 실시간에 환자검색 및 처방을 할 수 있는 원격진료시스템을 제안하고 구현한다.

전 세계적으로 공공데이터를 민간에 개방하고자 하는 노력으로 오픈데이터(Open Data) 정책을 도입하고 있다. 최근 보건의료에서는 삶의 방식과 환경의 변화로 인해 다양한 질병에 노출될 가능성이 증가하면서 예방과 진료를 목적으로 의료기관을 이용하려는 고객의 수 또한 지속적으로 증가하고 있는 추세이다. 이에 이 논문에서는 공공데이터 심사평가원에서 제공하는 보건의료 공공데이터를 활용하여 의료기관 정보에 대한 대용량 데이터를 활용하여 앱을 개발하였다. 개발된 내용은 심사평가원의 의료기관별 정보와 구글 맵 Open API를 통해 병원 위치를 지도를 통해 제공하며, 추가적으로 각 병원별 진료과목, 시설 정보 등의 상세정보를 제공한다.
